Heterotopic gastric mucosa in the anus and rectum:first case report of endoscopic submucosal dissection and systematic review

Background:Heterotopic gastric mucosa(HGM)is the most reported epithelial heterotopia,but it is very rare in the rectum and ***:The first case of an asymptomatic adult male with a large nonpolypoid HGM in the low rectum underwent complete resection by endoscopic submucosal dissection(ESD)is *** systematic review was based on a comprehensive search of MEDLINE,EMBASE and Google *** on humans were identified with the term‘heterotopic gastric mucosa in the rectum and/or anus.’Results:The search identified 79 citations,and 72 cases were evaluated comprising the present *** malformations were observed in 17(24%)patients;rectal duplication accounted for most of the *** HGM was located in the anus and perineal rectum in 25 cases(41%)and low,middle and proximal pelvic rectum in 20(33%),five(8%)and 11 cases(18%),*** was nonpolypoid in 37 cases(51%),polypoid in 26 cases(36%)and ulcerated in nine cases(13%).Specific anorectal symptoms were reported by 50(69%)patients of the whole study population,and by 33(97%)of 34 patients≤18 *** were observed in 23 cases(32%).The HGM was excised in 50 cases(83%).Endoscopic resection was performed in 17 cases(34%);resection was piecemeal in five of 12 lesions≥5mm,required argon plasma coagulation in two cases and was associated with residual tissue in two(17%).Intestinal metaplasia and an adenoma with lowgrade dysplasia were described in three adults(4%).Discussion:This systematic review shows that the HGM in the rectum and anus may be associated with specific rectal symptoms and serious complications,mainly in the pediatric population,and a risk of malignancy in *** complete excision should be recommended,and the ESD can overcome the technical limits of conventional endoscopic snare resection and avoid unnecessary surgery.

Over-the-scope clip closure of two chronic fistulas after gastric band penetration

Gastrointestinal perforations are conservatively managed at endoscopy by through-the-scope endoclips and covered self expandable stents, according to the size and tissue features of the perforation. This is believed to be the first report of successful closure of two gastrocutaneous fistulas with over-the-scope clips (OTSCs). After laparoscopic gastric banding, a 45-year old woman presented with band erosion and penetration. Despite surgical band removal and gastric wall suturing, external drainage of enteric material persisted for 2 wk, and esophagogastroduodenoscopy demonstrated two adjacent 10-mm and 15-mm fistulous orifices at the esophagogastric junction. After cauterization of the margins, the 10-mm fistulous tract was grasped by the OTSC anchor, invaginated into the applicator cap, and closed by a traumatic OTSC. The other 15-mm fistula was too large to be firmly grasped, and a fully-covered metal stent was temporarily placed. No leak occurred during the following 6 wk. At stent removal: the OTSC was completely embedded in hyperplastic overgrowth; the 15-mm fistula significantly reduced in diameter, and it was closed by another traumatic OTSC. After the procedure, no external fistula recurred and both OTSCs were lost spontaneously after 4 wk. The use of the anchor and the OTSC seem highly effective for successful closure of small chronic perforations.

People with advanced chronic liver disease(ACLD)have an enhanced risk of malnutrition,which has multifactorial etiology and is mainly linked to a reduced energy and protein intake;malnutrition is critical for patients with cirrhosis since it is often associated with sarcopenia,a skeletal muscle depletion with a loss of muscle mass and ***-evening snacks have been extensively studied,and guidelines are recommended to counteract the effects of prolonged fasting at night in patients with ***,it has not been fully explored whether late evening snacking is clarified as a milestone to address the nutritional needs of people with ACLD or whether it has a potential role in improving body *** this randomised control trial,Yu et al demonstrated that long-term nocturnal snacks have the potential to significantly improve body composition by body fat mass,visceral fat area and body cell mass in patients with *** the improvement in skeletal muscle mass was minor,the promising results in other compositions provide hope for future research and patient care.

Perception of healthcare professionals on patient safety culture and associated factors:a qualitative study using MAXQDA software

Objective: Patient safety culture is a concern in every healthcare organization, therefore, the healthcare leadership is encountering issues related to patient safety across the globe. In India, there is limited research and information about patient safety culture among healthcare stakeholders and there is relatively little qualitative research available that captures the factors of patient safety culture. Hence, this study aims to explore the perception of healthcare professionals on patient safety culture. Methods: An exploratory qualitative study design was adopted in a tertiary care hospital. Structured focus group discussion (FGD) (n = 4) among healthcare professionals and two in-depth interview focus groups were audio-recorded and transcribed. Two coders reviewed transcripts using the editing approach and organized codes into themes. The data were analyzed through MAXQDA 2022 (VERBI Software GmbH, Berlin, Germany), qualitative data analysis software, and descriptive analysis technique. The main codes and themes were generated using inductive and deductive method and smart coding was done. Results: Overall, there were 190 unique mentions of codes related to patient safety culture from 4 FGDs. They were categorized into 6 major themes and subcodes were derived via smart coding using the MAXQDA software. “Resources and constraints” was the most prominent code, followed by management support, manpower shortage, burnout, and lack of personnel commitment. Conclusions: The study highlights significant gaps in patient safety culture within the healthcare setting, with resource constraints, management support, and manpower shortages emerging as critical challenges. Burnout and lack of personnel commitment further exacerbate these issues, underscoring the need for targeted interventions.

Effect of alpha-lipoic acid and Silybum marianum supplementation with a Mediterranean diet on metabolic dysfunction-associated steatosis

BACKGROUND The treatment of metabolic dysfunction-associated steatotic liver disease(MA-SLD)has focused on the control of *** marianum(SM)and alpha-lipoic acid(ALA)have shown antioxidant and adjuvant effects on the control of metabolic *** To evaluate whether the SM-ALA formulation(LUDLEV®),in combination with the Mediterranean diet(MD),could improve MASLD-related liver *** A randomized,double-blind clinical trial was conducted on patients with *** of SM-ALA plus MD(group A)vs placebo plus MD(group B)was compared for 24 *** baseline and weeks 12 and 24,anthropometric measurements,metabolic parameters,and liver function were *** effectiveness was evaluated through transient *** Fifty patients aged 54±10 years were included,and the majority(74%)were *** visceral fat and umbilical circumference were reported in both groups,with significance in group A(P=0.045 and 0.003,respectively).The de-crease in controlled attenuation parameter was gradual and maintained at 12 and 24 weeks in group A(P=0.026),whereas in group B the decrease was greater at week 12 and remained unchanged at week 24(∆controlled attenuation parameter:-27 dB/m).Mild adverse effects were reported in 4 patients in group A(16%)and 4 patients in group B(16%),with no significant differences between groups(P=0.641).CONCLUSION SM-ALA(LUDLEV®)combined with the MD can promote the improvement of metabolic parameters,reducing visceral fat and hepatic steatosis in Mexican patients with MASLD.
