Vancomycin flushing syndrome in orthopaedic practice:A case report

BACKGROUND Vancomycin flushing syndrome(VFS),also known as red man syndrome,is an allergic reaction to *** typically presents as a rash on the face,neck,and upper torso after intravenous administration of *** is blamed on rapid intravenous infusion of vancomycin during management and rarely happens after local use.A review of the literature showed that in the last 23 years,4 such cases have been ***,we add another case of VFS developed after slow local absorption of vancomycin in cement *** SUmmARY A 44-year-old male with a known case of hypertension,no history of allergies to medications,and a history of chronic osteomyelitis of the right tibia with discharging sinus over the anterolateral aspect of the *** pus culture grew Staphylococcus aureus,which was sensitive to clindamycin and *** patient underwent irrigation and debridement with the placement of vancomycin cement beads made from 4 g of vancomycin powder and 40 g of polymethyl *** hours postoperatively,the patient developed a pruritic,erythematous,macular rash predominantly on his face,neck,chest,and lower extremities and to a lesser extent his upper extremities.A diagnosis of VFS was made and was successfully treated with cetirizine(10 mg,oral)and methylprednisolone sodium succinate(125 mg,intravenous).The patient continued to have itching with a facial rash for 12 h with gradual improvement.A decision was made to not remove the beads as the patient continued to ***,the rash disappeared after 96 h with no further *** VFS can occur not only after rapid intravenous injection of vancomycin but also with local release,as in our *** orthopaedic surgeons routinely use vancomycin with polymethyl methacrylate in chronic osteomyelitis and revision arthroplasty,they should be aware of such a complication occurring.

Surgical management of mitral Valve Infective Endocarditis with Accidental Finding of Open Pulmonary Tuberculosis

Objective: This case report aimed to highlight intersections of TB and Cardiovasular diseases which carry high morbidity and mortality rates. methods: We are reporting the surgical management of forty seven years female who had back ground history of IDDm (Insulin dependent diabetic mellitus), ESRD (End stage renal disease) on HD (haemodialysis) also she had left subclavian artery stenosis, and paroxysmal atrial fibrillation. She was diagnosed with mitral valve infective endocarditis and found accidently to have an open pulmonary tuberculosis (TB) on the day before surgery. Results: She was started on first line anti-TB treatment. She was isolated in her private room and airborne precautions measures applied. The patient underwent a tissue mitral Valve replacement and tricuspid valve repair annuloplasty. Special precautions were applied in Theatre and on cardiopulmonary bypass machine guided by KAmC-J disinfection protocol. The patient made good recovery postoperatively. She was discharged well on day 7 post operatively. Conclusion: Intersections of TB and cardiovasular diseases carry high morbidity and mortality rates. Early diagnosis and early anti tuberculosis treatment can surely improve the patient prognosis. Our decontamination and disinfective procedures are recommended. Cases like this should be monitored long term for the development of further cardiovascular complication.

Superior mesenteric Artery Syndrome, a Rare Complication of Severe malnutrition: Two Cases from a Subsaharian Endocrinology Department

Introduction: Superior mesenteric artery syndrome (SmAS), a rare diagnosis due to compression of the third duodenum between the superior mesenteric artery (SmA) and the aorta resulting in bowel obstruction, may lead to severe malnutrition. We report two cases of patients hospitalised in the Internal medicine, Endocrinology, Diabetology, and Nutrition Department of the National Hospital Center (NHC) of Pikine. Observations: Patient 1: A 35-year-old female was referred for an aetiological diagnosis due to a rapid weight loss of 15 kilograms in one month, accompanied by persistent vomiting, following an appendectomy performed a month before admission. Upon clinical examination, she presented severe malnutrition (Buzby index of 76%), early post-prandial chronic vomiting, and a poor general condition. An abdominal CT scan revealed aortomesenteric clamp syndrome (AmCS) with an angulation between the aorta and the SmA of 13˚. The underlying cause in this patient was severe malnutrition. Fortunately, her condition improved with medical treatment. Patient 2: We report the case of a 30-year-old female hospitalized due to unusual weight-bearing post-prandial epigastric pain and intermittent vomiting over the past six months. Upon physical examination at admission, she exhibited severe malnutrition with a body mass index (BmI) of 14 kg/m2, a Buzby index of 71%, trophic disorders, and a stage IV general condition assessment according to the World Health Organization (WHO). An abdominal CT scan revealed AmCS with an angle between the aorta and the SmA of 22˚ and an aortomesenteric space of 4 mm. The outcome was poor with medical treatment failure and, unfortunately, the patient died before surgery. Conclusion: SmAS is rarely evoked in clinical practice despite the presence of contributing factors and suggestive clinical signs. The prognosis depends on management time.

Role of bevacizumab intraocular injection in the management of neovascular glaucoma

AIm:To assess the long-term effects of intraocular bevacizumab(Avastin)injections as an adjunctive drug to manage patients with neovascular glaucoma(NVG).mETHODS:A retrospective study was conducted consisting of 34 eyes with secondary NVG caused by proliferative diabetic retinopathy(n=25),ischemic central retinal vein occlusion(n=8),and retinal ischemia resulting from persistent detachment(n=1)were managed by intraocular injections of bevacizumab(1.25 mg/0.05 m L),in addition to other *** main outcome measure was the change in the degree of iris *** outcomes included intraocular pressure and the number of additional interventions or antiglaucoma medications administered after ***:all patients were followed-up for at least 12 *** the last follow-up,complete regression of rubeosis irides was detectable in 13(38.2%)eyes and incomplete regression in 21 eyes(61.8%).The mean intraocular pressure was 45.32±7.185 mm Hg at baseline and significantly decreased to 26.15±5.679 mm Hg at the last follow-up visit(P=0.000005).Patients received an average of 4.97 *** additional treatments,12 eyes(35%)received laser photocoagulation and 6 eyes(18%)underwent *** further treatment was needed in 16 eyes(47.1%).CONCLUSION:Intravitreal bevacizumab injection can have a favorable effect in controlling intraocular pressure and pain control in patients with NVG because it decreases the angiogenesis and helps to augment the results of conventional *** primary cause of retinal ischemia should be always targeted.

Touch gesture recognition is an important aspect in human-robot interaction,as it makes such interaction effective and *** novelty of this study is the development of a system that recognizes human-animal affective robot touch(HAART)using a deep learning *** proposed system was used for touch gesture recognition based on a dataset provided by the Recognition of the Touch Gestures Challenge *** dataset was tested with numerous subjects performing different HAART gestures;each touch was performed on a robotic animal covered by a pressure sensor skin.A convolutional neural network algorithm is proposed to implement the touch recognition system from row inputs of the sensor *** leave-one-subject-out cross-validation method was used to validate and evaluate the proposed system.A comparative analysis between the results of the proposed system and the state-of-the-art performance is *** show that the proposed system could recognize the gestures in almost real time(after acquiring the minimum number of frames).According to the results of the leave-one-subject-out cross-validation method,the proposed algorithm could achieve a classification accuracy of 83.2%.It was also superior compared with existing systems in terms of classification ratio,touch recognition time,and data preprocessing on the same ***,the proposed system can be used in a wide range of real applications,such as image recognition,natural language recognition,and video clip classification.

A New Approach for Database Fragmentation and allocation to Improve the Distributed Database management System Performance

The efficiency and performance of Distributed Database management Systems (DDBmS) is mainly measured by its proper design and by network communication cost between sites. Fragmentation and distribution of data are the major design issues of the DDBmS. In this paper, we propose new approach that integrates both fragmentation and data allocation in one strategy based on high performance clustering technique and transaction processing cost functions. This new approach achieves efficiently and effectively the objectives of data fragmentation, data allocation and network sites clustering. The approach splits the data relations into pair-wise disjoint fragments and determine whether each fragment has to be allocated or not in the network sites, where allocation benefit outweighs the cost depending on high performance clustering technique. To show the performance of the proposed approach, we performed experimental studies on real database application at different networks connectivity. The obtained results proved to achieve minimum total data transaction costs between different sites, reduced the amount of redundant data to be accessed between these sites and improved the overall DDBmS performance.
