Antioxidant and antihyperlipidemic activity of Costus speciosus against atherogenic diet-induced hyperlipidemia in rabbits

Objective: An extract of Costus speciosus(CSE), a herb widely used in folk medicine, was evaluated for its antioxidant, antihyperlipidemic and ameliorating effects on histopathological changes in atherogenic ***: Twenty-four male rabbits(Oryctolagus cuniculus) were divided into 4 groups. Three groups were fed a diet containing 3% saturated fat and 1.3% cholesterol for 40 d. One of these was sacrificed on the 40 th day and was called the pathogenic(P) group; the other two groups received treatment for another 30 d as follows: one received 0.8 g/(kgád) of CSE and the other was given 0.01 g/(kgád) of simvastatin. The normal group was sacrificed on the 70 th day and used as a ***: CSE showed radical-scavenging ability. Administration of CSE for a 30-day period resulted in a significant decrease in total cholesterol, triacylglycerol, low-density lipoprotein and aspartate aminotransferase compared to the P group, while levels of hemoglobin, packed corpuscular volume and red blood cells were elevated. With respect to studies performed on the heart, a decrease in malondialdehyde and an increase in reduced glutathione were noted. Total protein increased in the liver, heart and aorta after treatment with CSE and also a marked improvement in histopathological parameters was ***: The present findings indicate that the C. speciosus rhizome possesses antiatherogenic and antioxidant properties which may provide protective effects against oxidative stress in atherosclerotic rabbits.

Occupant-centric controls(OcC)is an indoor climate control approach whereby occupant feedback is used in the sequence of operation of building energy *** OcC has been used in a wide range of building applications,an OcC category that has received considerable research interest is learning occupants'thermal preferences through their thermostat interactions and adapting temperature setpoints *** recent studies used reinforcement learning(RL)as an agent for OcC to optimize energy use and occupant *** studies depended on predicted mean vote(PMV)models or constant comfort ranges to represent comfort,while only few of them used thermostat *** paper addresses this gap by introducing a new off-policy reinforcement learning(RL)algorithm that imitates the occupant behaviour by utilizing unsolicited occupant thermostat *** algorithm is tested with a number of synthetically generated occupant behaviour models implemented via the Python APl of *** simulation results indicate that the RL algorithm could rapidly learn preferences for all tested occupant behaviour scenarios with minimal exploration *** substantial energy savings were observed with most occupant scenarios,the impact on the energy savings varied depending on occupants'preferences and thermostat use behaviour stochasticity.

Evaluation of the versatility of perforators-enhanced rhomboid flap technique for ischial sores reconstruction

Background: Prolonged pressure on the skin known as pressure sores, can cause frequent injuries to the skin and underlying tissues. However, several prevention approaches, including non-surgical and surgical management,are available. In flap surgery for pressure sores, a variety of flap types may be used; each offers certain benefits and some incidences of complications and recurrence. This study evaluated the versatility of the perforatorenhanced rhomboid flap technique for ischial sore ***: This prospective study was conducted in the Plastic Surgery Department of Fayoum University *** perforator-enhanced rhomboid flap technique was employed in 20 patients with ischial pressure sores who failed to respond to conservative treatment. A proper history was obtained and a preoperative arterial duplex was performed to determine the site of the prominent perforators for their preservation at the base of the *** follow-ups and evaluations were ***: The mean age of the study group was 43.7 ± 13.1, ranging from 19 to 65 years. Moreover, 80% of the participants were males while 20% were female. The main etiology of the sores was paraplegia in 45% of *** mean surface area of sores was 24.8 ± 6.3 cm2. In 75% of cases, the color match of the flap was rated as excellent. Similarly, 70% of the patients rated flap thickness as excellent. In 55% of the cases, the scar appearance was deemed excellent. Overall satisfaction was reported as excellent by 40% of the patients, while only 5% reported poor satisfaction. Effect of Single- versus Double-Layer Uterine Closure during Caesarean Section on Niche Formation and Menstrual Irregularity

Background: The myometrium at the location of the CS (caesarean section) scars, also known as residual myometrium thickness (RMT), is larger after a double-layer uterine closure procedure than following a single-layer one. It may lessen the formation of a niche that is the myometrium’s disruption at the location of the scar of the uterus. Gynecological manifestations, obstetric problems in a future pregnancy and birth, and maybe subfertility are linked to thin RMT and a niche. Objective: To ascertain if double-layer unlocked closure of the uterus is better than single-layer one in terms of post-menstrual spotting and niche development following a first CS. Patients and Methods: In this randomized clinical study, 287 patients were evaluated for qualifying. Of all eligible individuals, 57 patients were excluded from the study based on the inclusion criteria. Results: The variation in ages, gestational age, body mass index (BMI), and cesarean section indications between the two assigned groups is statistically insignificant. However, postmenstrual spotting was statistically significantly more common in single-layer group compared to in double-group. The current study revealed ultrasound findings suggestive of niche formation was statistically significantly more common in single-layer group compared to in double-layer group. Conclusion: As evident from the current study, it demonstrates the advantages of double-layer unlocked closure of the uterus over single-layer one in terms of post-menstrual spotting and niche development following first-time cs. Thus, we deduced that fewer niches are formed, and fewer menstrual spotting occurs in the presence of double unlocked layers closure. To ascertain the impact of uterus closure method on post-operative niche development and the risk of obstetrics and gynaecological problems, further prospective trials with extended follow-up periods are required.

Water, Sanitation and Hygiene in Lower-Level Health Care Facilities of Dar es Salaam Region in Tanzania: Status towards Achieving the Sustainable Development Goals and Way Forward

Background: Improving Water, Sanitation and Hygiene in health care settings is a critical prerequisite for achieving national health goals and Sustainable Development Goals (SDGs). The World Health Organization (WHO) has set a target for each United Nations member state to reach by 2030. Each member state is required to reach by 2022, 2025 and 2030 at least 60%, 80% and 100%, respectively of basic level of service of the five elements which are water, sanitation, hygiene, waste management and environmental cleaning. Methods: This study aimed to evaluate and document the current state of basic water, sanitation, and hygiene services in all lower-level health care facilities in the Dar es Salaam region of Tanzania as of July 2022. A cross-sectional study was conducted in 99 public dispensaries in the Dar es Salaam region’s five councils: Ubungo, Kigamboni, Kinondoni and Temeke Municipalities, and Ilala City. The interviewee form and observational checklists were both digitalized using the Kobo tool software. The respondents were health care facility in-charges or nurse in-charges. Data were downloaded, validated, and imported to Stata version 15 for analysis. Results: The basic WASH level per JMP is far below the target in 2022. Each member state by 2022 is required to reach at least 60% of the basic level of service of each element. We found a low coverage of basic WASH in the 99 dispensaries included in this study. The basic WASH coverage was met in only 10 (10.1%) of the dispensaries, while the remaining 89 (89.9%) dispensaries fall on limited WASH services. Conclusion: This study revealed lower coverage of basic WASH services in dispensaries. An urgent need is required to improve the status of WASH in all the dispensaries and facilitate the provision of quality health care services, patient safety and reduce health care associated infections.

Efficient Computation Offloading of IoT-Based Workflows Using Discrete Teaching Learning-Based Optimization

As the Internet of Things(IoT)and mobile devices have rapidly proliferated,their computationally intensive applications have developed into complex,concurrent IoT-based workflows involving multiple interdependent *** exploiting its low latency and high bandwidth,mobile edge computing(MEC)has emerged to achieve the high-performance computation offloading of these applications to satisfy the quality-of-service requirements of workflows and *** this study,we propose an offloading strategy for IoT-based workflows in a high-performance MEC *** proposed task-based offloading strategy consists of an optimization problem that includes task dependency,communication costs,workflow constraints,device energy consumption,and the heterogeneous characteristics of the edge *** addition,the optimal placement of workflow tasks is optimized using a discrete teaching learning-based optimization(DTLBO)*** experimental evaluations demonstrate that the proposed offloading strategy is effective at minimizing the energy consumption of mobile devices and reducing the execution times of workflows compared to offloading strategies using different metaheuristics,including particle swarm optimization and ant colony optimization.

Sex Predictive Value of Frontal Sinus Index Compared to Maxillary Sinus Index:Lateral Cephalometric Study

Sex estimation is a crucial step to outline the biological profile of unknown human *** frontal and maxillary sinuses have unique anatomical features and can be utilized efficiently in individual *** studies investigated the frontal and maxillary sinus indexes separately for sex ***,comparing the predictive value of the frontal sinus index(FSI)to the maxillary sinus index(MSI)has not been investigated *** study aimed to evaluate the sex predicting value of FSI compared to MSI assessed on digital lateral *** retrospective cross-sectional study included 88 digital lateral cephalometric images of known sex(44 males and 44 females)and age(20-50 years).Two radiologists measured the maximum anteroposterior sinus width and height of the frontal sinus and maxillary ***,the sinus index was calculated for each *** obtained frontal and maxillary sinuses parameters were tabulated,along with the patient’s sex and age,and subjected to statistical *** results revealed significant differences between males and females regarding frontal sinus width,FSI,and *** discriminant function analysis obtained in this study accurately predicted sex with an overall accuracy of 71.6%(81.8%of males and 61.4%of females)using FSI,and 60.2%(63.6%in females and 56.8%in males)using *** and maxillary sinus indexes can be useful tools for sex *** function equations were developed for both the frontal and maxillary sinus indexes with higher accuracy rates for FSI compared to MSI.

Assessment of necessity of neuronavigation in localization of calvarial extra-axial lesions in the setting of limited resources

Background:Neuronavigation is a very beneficial tool in modern neurosurgical ***,the neuronavigation is not available in most of the hospitals in our country raising the question about its importance in localizing the calvarial extra-axial lesions and to what extent it is safe to operate without ***:We studied twenty patients with calvarial extra-axial lesions who underwent surgical *** lesions were preoperatively located with both neuronavigation and the usual linear *** methods were compared regarding the time consumed to localize the tumor and the accuracy of each method to anticipate the actual center of the ***:The mean error of distance between the planned center of the tumor and the actual was 6.50±1.762 mm in conventional method,whereas the error was 3.85±1.309 mm in IGS *** more time was consumed during the neuronavigation method including booting,registration,and positioning.A statistically significant difference was found between the mean time passed in the conventional method and IGS method(2.05±0.826,24.90±1.334,respectively),P-value<***:In the setting of limited resources,the linear measurement localization method seems to have an accepted accuracy in the localization of calvarial extra-axial lesions and it saves more time than neuronavigation method.
