Long-Term Outcomes after Coronary Artery Bypass Grafting with Risk Stratification

Background: Risk stratification of long-term outcomes for patients undergoing Coronary artery bypass grafting has enormous potential clinical importance. Aim: To develop risk stratification models for predicting long-term outcomes following coronary artery bypass graft (CABG) surgery. methods: We retrospectively revised the electronic medical records of 2330 patients who underwent adult Cardiac surgery between August 2016 and December 2022 at madinah Cardiac Center, Saudi Arabia. Three hundred patients fulfilled the eligibility criteria of CABG operations with a complete follow-up period of at least 24 months, and data reporting. The collected data included patient demographics, comorbidities, laboratory data, pharmacotherapy, echocardiographic parameters, procedural details, postoperative data, in-hospital outcomes, and follow-up data. Our follow-up was depending on the clinical status (NYHA class), chest pain recurrence, medication dependence and echo follow-up. A univariate analysis was performed between each patient risk factor and the long-term outcome to determine the preoperative, operative, and postoperative factors significantly associated with each long-term outcome. Then a multivariable logistic regression analysis was performed with a stepwise, forward selection procedure. Significant (p ment of a multivariable risk prediction model. Results: The incidence of all-cause mortality during hospital admission or follow-up period was 2.3%. Other long-term outcomes included all-cause recurrent hospitalization (9.8%), recurrent chest pain (2.4%), and the need for revascularization by using a stent in 5 (3.0%) patients. Thirteen (4.4%) patients suffered heart failure and they were on the maximum anti-failure medications. Vitamin D and its association with cardiovascular disease (CVD) are currently a topic of investigation. The objective of this study was to explore the association between vitamin D status (serum 25-hydroxyvitamin D (25(OH)D)) and a cardiometabolic risk score (CmRS) derived from markers of cardiac autonomic nervous system activity, vascular dynamics, and body composition, using an innovative non-invasive technology. We found that individuals who were vitamin D sufficient ((25(OH)D) ≥ 30 ng/ml, n = 51), compared to those who were vitamin D insufficient (ml, n = 44), had significantly higher heart rate variability (as measured by time and frequency domain variables) and lower photoplethysmography analysis markers and CmRS. These outcomes show that vitamin D insufficient subjects had reduced cardio protective parasympathetic nervous system activity, increased endothelial dysfunction, and hence were at greater cardiometabolic risk, implying vitamin D may play a meaningful role in CVD.

AIm:To assess the effectiveness of hepatitis B virus(HBV) vaccination program among fully vaccinated ***:A national community based crosssectional study was carried out in 6 governorates representing Egypt. A total of 3600 children aged from 9 mo to 16 years who were fully vaccinated with HBV vaccine during infancy were recruited. Face to face interviews were carried out and sera were evaluated for hepatitis B surface antigen(HBsA g),anti-HBV core antibodies(total) and quantitative detection of hepatitis B surface antibody using enzyme linked immunoassays techniques. Samples positive to HBs Ag/anti-HBV core antibodies were subjected to quantitative HBV-DNA detection by real time polymerase chain reaction with 3.8 IU/L detection limit. RESULTS:Sero-protection was detected among 2059 children(57.2%) with geometric mean titers 75.4 ± 3.6 IU/L compared to 3.1 ± 2.1 IU/L among nonseroprotected children. multivariate logistic analysis revealed that older age and female gender were the significant predicting variables for having non seroprotective level,with adjusted odds ratio 3.3,9.1and 14.2 among children aged 5 to mpared to those mong girls compared to boys with P mong children aged ≥ 7 years(0.59%) compared to 0.07% among younger children with odds ratio equal to 8.4(95%CI:1.1-64.2) and P mong girls(0.48%) than boys(0.29%) with P > 0.05. C ON C LU S I ON :T he E gy pt ian c ompuls or y H B V vaccination program provides adequate protection. Occult HBV infection exists among apparently healthy vaccinated children. Adherence to infection control measures is mandatory.

A multi-Stage Security Solution for medical Color Images in Healthcare Applications

This paper presents a robust multi-stage security solution based on fusion,encryption,and watermarking processes to transmit color healthcare images,*** presented solution depends on the features of discrete cosine transform(DCT),lifting wavelet transform(LWT),and singular value decomposition(SVD).The primary objective of this proposed solution is to ensure robustness for the color medical watermarked images against transmission *** watermark embedding,the host color medical image is transformed into four sub-bands by employing three stages of *** resulting low-frequency sub-band is then transformed by employing three stages of DCT followed by SVD ***,a fusion process is used for combining different watermarks into a single watermark *** single fused image is then ciphered using Deoxyribose Nucleic Acid(DNA)encryption to strengthen the ***,the DNA-ciphered fused watermark is embedded in the host medical image by applying the suggested watermarking technique to obtain the watermarked *** main contribution of this work is embedding multiple watermarks to prevent identity *** the presence of different multimedia attacks,several simulation tests on different colormedical images have been *** results prove that the proposed security solution achieves a decent imperceptibility quality with high Peak Signal-to-Noise Ratio(PSNR)values and high correlation between the extracted and original watermark ***,the watermark image extraction process succeeds in achieving high efficiency in the presence of attacks compared with related works.

326 species of spiders belonging to 26 families are recorded from the Bolshekhekhtsyrski State Nature Reserve,of them 70 are new records for the reserve and six are new to the fauna of Russia:Asperthorax borealis Ono et Saito,2001; Cyclosa kumadai Tanikawa,1992; Cyclosa okumae Tanikawa,1992(earlier it was identified as C. argenteoalba Bosenberg et Strand,1906); Haplodrassus taepaikensis Paik,1992; Hypselistes fossilobus Fei et Zhu,1993; and Pachygnatha gaoi Zhu et al.,2003. The name Pronous minutus (S. Saito,1939) is synonymized with Pronoides brunneus Schenkel,1936. The male of H. taepaikensis is illustrated for the first time. Composition of the fauna is briefly discussed; 41% of the recorded species have their ranges confined to the SE Palaearctics. By its species diversity,the reserve's fauna is the second largest local fauna eastward of the Urals. An expected spider diversity of this reserve is likely to be over 400 species.

The Jiangmen Underground Neutrino Observatory(JUNO)features a 20 kt multi-purpose underground liquid scintillator sphere as its main *** of JUNO's features make it an excellent location for^8B solar neutrino measurements,such as its low-energy threshold,high energy resolution compared with water Cherenkov detectors,and much larger target mass compared with previous liquid scintillator *** this paper,we present a comprehensive assessment of JUNO's potential for detecting^8B solar neutrinos via the neutrino-electron elastic scattering process.A reduced 2 meV threshold for the recoil electron energy is found to be achievable,assuming that the intrinsic radioactive background^(238)U and^(232)Th in the liquid scintillator can be controlled to 10^(-17)g/*** ten years of data acquisition,approximately 60,000 signal and 30,000 background events are *** large sample will enable an examination of the distortion of the recoil electron spectrum that is dominated by the neutrino flavor transformation in the dense solar matter,which will shed new light on the inconsistency between the measured electron spectra and the predictions of the standard three-flavor neutrino oscillation *** m^(2)_(21)=4.8times10^(-5);(7.5times10^(-5))eV^(2),JUNO can provide evidence of neutrino oscillation in the Earth at approximately the 3sigma(2sigma)level by measuring the non-zero signal rate variation with respect to the solar zenith ***,JUNO can simultaneously measureDelta m^2_(21)using^8B solar neutrinos to a precision of 20% or better,depending on the central value,and to sub-percent precision using reactor antineutrinos.A comparison of these two measurements from the same detector will help understand the current mild inconsistency between the value of Delta m^2_(21)reported by solar neutrino experiments and the KamLAND experiment.

The Jiangmen Underground Neutrino Observatory(JUNO)is a large liquid scintillator detector designed to explore many topics in fundamental *** this study,the potential of searching for proton decay in the p→νK^(+)mode with JUNO is *** kaon and its decay particles feature a clear three-fold coincidence signature that results in a high efficiency for ***,the excellent energy resolution of JUNO permits suppression of the sizable background caused by other delayed *** on these advantages,the detection efficiency for the proton decay via p→νK^(+)is 36.9%±4.9%with a background level of 0.2±0.05(syst)±0.2(stat)events after 10 years of data *** estimated sensitivity based on 200 kton-years of exposure is 9.6×1033 years,which is competitive with the current best limits on the proton lifetime in this channel and complements the use of different detection technologies.
