目的:评估雷珠单抗反应欠佳患者治疗早期转为阿柏西普治疗疗效。方法:该研究包括38例湿性年龄相关性黄斑变性(W-AmD)患者。18例反应欠佳患者改用玻璃体内阿柏西普(IVA)每月3次疗法,其余20例反应欠佳患者持续进行玻璃体内雷珠单抗(IVR)治疗,每月增加3次使用。所有改变均行荧光素血管造影术(FA)和光学相干断层扫描成像术(OCT)评估。结果:患者术前平均视力(VA)和黄斑中心厚度(CmT)分别为0.84±0.47 logm AR和360±84μm。经过6次IVR或3次IVR联合3次IVA治疗1mo后,两组患者视力分别为1.1±0.14(P=0.11)logm AR和0.48±0.37(P=0.019)logm AR,黄斑中心厚度分别为300±79μm(P=0.002)和271±51μm(P=0.002)。结论:对于雷珠单抗治疗反应欠佳患者,减少重复性治疗早期转为使用阿柏西普疗法对于视力康复是更好的选择。

Introduction: Common bile duct stone (CBDS) is a common clinical problem that can cause serious complications, such as acute cholangitis and pancreatitis. It is important to have an accurate, safe, and reliable method for the definitive diagnosis of CBDS before proceeding to therapeutic endoscopic retrograde cholangiopancrea-tography (ERCP). Objective: To compare the accuracy of trans-abdominal ultra-sound (TAUS) as a diagnostic tool at our institution—Kurdistan Centre for Gastroen-terology & Hepatology (KCGH)—with invasive tool like ERCP in the diagnosis of bile duct stones, using specificity, sensitivity, and positive and negative predictive values. Patient and method: After obtaining ethical committee approval & informed consent from every patient. This was a prospective study conducted on 71 patients (24 male patients and 47 females patients) where suspected to have CBDS depending on history, clinical suspicion and blood tests. Their ages range between (2 - 88 years). Both TAUS and ERCP were performed. Final diagnosis was confirmed de-pending on ERCP as it served as a diagnostic standard in diagnosing CBDS. Result: In 71 patients suspected to have CBDS by TAUS, only 46 patients had stone (65%), and 55 patients had stone by ERCP (77%). In our result, sensitivity, specificity, pos-itive predictive value and negative predictive value for TAUS were 80%, 87.5%, 65.5% and 56%, respectively. Conclusion: TAUS can play an important role as an initial screening procedure for CBDS detection because of the various advantages like easy availability, cost effectiveness, no requirement of contrast material and lack of ionizing radiation but should done with other imaging modality to avoid se-rious complication of ERCP.

目的:比较单用贝伐单抗与贝伐单抗联合曲安奈德治疗糖尿病黄斑水肿疗效。方法:回顾性比较随机研究。对21例42眼黄斑水肿的糖尿病患者进行了评估。单眼内注射1. 25 mg贝伐单抗(IVB组),在同组的眼玻璃体内注射1. 25 mg贝伐单抗和1 mg曲安奈德(IVTA-IVB组)。使用光学相干断层扫描(OCT)、ETDRS视力和眼压测量黄斑中心厚度(CmT)。结果:平均随访时间为4. 7±1. 5mo。在IVB组和IVTAIVB组中,注射前平均CmT分别为494. 7±114. 4μm,546. 8±165. 6μm;第一个月分别为430. 4±133. 2μm,363. 7±105. 3μm;第三个月分别为484. 8±167. 4μm,407. 3±108. 7μm;六个月后分别为550. 4±191. 5μm,516. 8±158μm。第一个月和第三个月存在显著差异(P <0. 05)。在IVB组和IVTA-IVB组中,注射前平均ETDRS视力数分别为57. 1±13. 5,48. 9±13. 9;第一个月分别为2. 2±14,58. 8±12. 1;第三个月分别为59±13. 7,59. 3±13. 6;第六个月分别为55. 6±14. 9,55. 5±8. 7。第三个月和第六个月存在显著差异(P<0. 05)。IOP无差异。第一次注射后IVTA-IVB组在第三个月时视力最佳并持续6个月,而IVB组在第一个月时视力最佳但仅持续了3个月。结论:6个月后发现,IVTA-IVB组较IVB组疗效显著,且无激素依赖性并发症。

The distributed blocking flowshop scheduling problem（DBFSP） that is an important generalization of the traditional blocking flowshop scheduling problem（BFSP）, in which the blocking constraint has to be considered. The NEH heuristic is regarded as the best constructive heuristic for the permutation flowshop scheduling problem. Naderi and Ruiz proposed the NEH2 for the distributed permutation flowshop scheduling problem by adding a factory assignment rule to the Nawaz-Enscore-Ham（NEH） heuristic. In this paper, a priority rule is first presented for generating initial sequence for the NEH2. Then, the proposed rescheduling operator is applied in NEH insertion procedure. Additionally, we generated a sequence containing all job twice, and then the solution for the problem is constructed by applying the NEH insertion procedure on each element of the sequence. Based on the above ideas we proposed three NEH-based heuristics, namely NEH2 E, NEH2 Een and NEH2 EE. Computational results demonstrate that the proposed heuristics perform significantly better than the original NEH2.

Targeting UDP-α-D-glucose 6-dehydrogenase alters the CNS tumor immune microenvironment and inhibits glioblastoma growth

In this paper, we study a distributed blocking flowshop scheduling problem（DBFSP） that is an extension of the traditional blocking flowshop scheduling problem（BFSP）, in which an additional decision of which factory to process each job. We propose an iterated greedy（IG） algorithm to minimize makespan among all the factories. First of all, an effective initialization method based on the PW algorithm is used in order to make better use the problem-specific characteristics. Then, an enhanced construction method is developed to further improve the solution obtained at each iteration. At last, after calibration of algorithm parameters, comparison of algorithms is carried out using the well-known 720 instances from the literature. The results demonstrate the effectiveness of the proposed IG algorithm for solving the DBFSP with makespan criterion.

In this paper, we present a new heuristic to divide a batch of printed circuit boards(PCBs) into subgroups to save the setup time for loading and unloading components from the assembly machine. In the heuristic, we propose several concepts about similarity to make the number of groups as few as possible. To better show the relationship between the PCB types and the component types of a group, we introduce a new solution representation. In addition, considering the characteristics of the PCBs grouping problem(PGP), a method for pairing PCBs is presented. With the PCB pairs, an iterative scheme is applied to start a new group. We try the rest PCBs one by one according to the similarity between it and the PCB group. Finally, the experiments and comparisons show the good performance of the proposed heuristic.

Objective:To determine the levels of some heavy metals in the liver and edible tissues of four fish species[mugil cephalus L.,1758(***),Cyprinus carpio L.,1758(***),Perca fluviatilis L.,1758(***)and Stizostedion lucioperca L.,1758(***)]collected from Lake Balik,Kizilirmak Delta(Samsun)for analysis of Cu,Co,Pb,Zn,Cd,mn,Ni and *** metals were chosen because at higher concentrations there might be toxic to the fish and by extension humans that depends on such fish as ***:The concentrations of the metals were carried out by the flame atomic absorption spectrophotometer(UNICAm 929).Results:Large differences in heavy metal levels were observed between liver and muscle tissues within each *** results showed that the Fe concentrations were the highest and Cd concentrations were the lowest in livers and edible tissues of the four *** muscles of *** accumulated significant higher levels of Co,Zn and Ni than other *** was higher in the muscles of *** than those in other species,while *** showed more of mn and Fe *** muscles of *** accumulated significant higher levels of Pb and Cd than other ***,the liver of *** accumulated significant higher levels of Co,mn and Fe than other *** level of Cd was the highest in the liver of ***,while *** showed more of Cu and Pb,and the higher levels of Zn and Ni was measured in the liver of ***:The metal levels obtained were compared with the maximum permitted levels in food of the Turkish Food Codex and Commission Regulation(EC).The results of this study showed that estimated daily and weekly intakes of selected metals via consumption of fish were below the permissible tolerable daily intake and provisional tolerable weekly intake values established by Food and Agriculture Organization/World Health Organization.

Overview of flooding damages and its destructions: a case study of Zonguldak-Bartin basin in Turkey

A number of devastating flood events have oc-curred in the various river basins of Turkey in the last decade. Because floods caused deaths, suffering and extensive damages to both public and private properties in the flood areas, the government had to most of the damage in addi-tion to losing significant revenues due to the consequences of costly social and economic disruption. On the other hand, some social structures such as socioeconomic activities, land-use patterns and hydro-morphological processes are destroyed. Whereas flood control structures are considered as one of the basic strategies that can reduce flood damages and in this context flood protection planning should consider the full range of the hazard mitigation activities. In Turkey, between 1945 and 1990, 737 flooding events were occurred and at least 830 people were killed. In 1998, there was a major flooding in Zonguldak-Bartin region located on north of Turkey. Due to this devastated flooding, people lost their life and numbers of engineer-ing structures built on the river and surrounding area were totally destroyed or heavily damaged. Both side of the canal were covered with muddy soil having 0.10-0.15 m thickness. Cleaning up process took sometimes in the region. In this paper, all these subjects have been investigated in the basin and some engineering proposals have been presented.
