Comparison of Sexual Function in Primiparous Women Pre-Pregnancy and Postpartum: Difference of the Sexual Function after the Normal Vaginal Delivery and the Cesarean Section

Introduction: Sexual function is undoubtedly an important dimension of adult life. Due to all the conflicting results with regard to sexual function after the normal vaginal delivery (NVD) or the Cesarean Section (CS), in the present study, we aimed to compare the sexual function in women pre-pregnancy and postpartum and also after the NVD and CS. Materials and Methods: In this cohort study, two groups of healthy women, with antenatal normal pregnancies, who underwent NVD (n = 90) and CS (n = 113), were prospectively studied. The sexual function of the participants was assessed through a Female Sexual Function Index (FSFI) questionnaire in two stages: once before pregnancy and then within 3 to 6 months after delivery, which lasted from June 2011 to September 2012. The data were analyzed by descriptive and inferential statistics. Data were analyzed using chi-square test, Mann-Whitney test, and T Test. Results: Based on the data gathered from 206 women who completed the FSFI questionnaire in two stages, the mean (±SD) self-reported timing of the resumption of sexual activity was 8.9 ± 1.3. There was no significant statistical difference found between the two groups by timing of the resumption of sexual activity in NVD and CS groups. There was also no statistically significant difference found in the overall sexual function scores between the two groups (NVD vs. CS). The average score for female sexual function in desire, arousal, orgasm, and satisfaction within 3 to 6 months after delivery was significantly lower than that of their pre-pregnancy period (p < 0.004). The mean coitus in postpartum period was 1.84 ± 1.20 per week. Conclusion: Based on the findings of this study, there was no significant relationship between the mode of delivery and changes in sexual function. Therefore, it can be claimed that CS is not preferred to NVD with regard to preserving normal sexual functioning.

Test anxiety in objective structured clinical examinations (OSCEs) compared with traditional assessment methods in undergraduate midwifery students

BACKGROUND: Final comprehensive exam is the most important examination for midwifery students to evaluate their professional ability and Test anxiety is a common phenomenon among college students. Because test anxiety is one of the problems of educational systems, this study was performed to compare test anxiety in objective structured clinical examinations (OS-CEs) and traditional assessment methods (TAM) among Undergraduate Midwifery Students. Methods: In this descriptive-analytical study, 52 students of Babol Midwifery College were participated in the study. 20 students were evaluated using TAM in November 2010, and 32 students were evaluated using the OSCE method in July 2011. Data were collected via a two-component questionnaire including demographic data and the test anxiety inventory (TAI). Results: All of midwifery students were women;the mean age of students, ages of father and mother’s were 23.1 ± 0.7, 52.3 ± 3.5 and 47.8 ± 4.9 years, respectively. Most of the students were single (61.5%). The mean education levels of father and mother’s were 11.6 ± 4.5 and 9.9 ± 4.0 years, respectively. The mean score of test anxiety in students was 42.51 ± 13.16. The most of participants did have moderate test anxiety (56.9%), two present not had any test anxiety, 37.3% had low test anxiety, and 3.9% had severe test anxiety. In sum, 98% had some degrees of test anxiety. There were the statistical differences in the mean score of text anxiety in OSCEs Compared with TAM in Undergraduate Midwifery Students (39.38 ± 13.81 vs. 47.35 ± 10.67, P = 0.033). Also, the mean severity of anxiety was different in two groups. The mean of moderate/severe test anxiety was more in TAM compared with OSCEs (52.57% vs. 49.56%, p = 0.000). The test anxiety had a positive correlation with father’s education, mother’s education (0.286, p = 0.042), father’s age, mother’s age, marital status, residency (0.292, p = 0.042). Optimization plays an effective role in various disciplines of science and *** problems should either be optimized using the appropriate method(i.e.,minimization or maximization).Optimization algorithms are one of the efficient and effective methods in providing quasioptimal solutions for these type of *** this study,a new algorithm called the Mutated Leader Algorithm(MLA)is *** main idea in the proposed MLA is to update the members of the algorithm population in the search space based on the guidance of a mutated *** addition to information about the best member of the population,themutated leader also contains information about the worst member of the population,as well as other normal members of the *** proposed MLA is mathematically modeled for implementation on optimization problems.A standard set consisting of twenty-three objective functions of different types of unimodal,fixed-dimensional multimodal,and high-dimensional multimodal is used to evaluate the ability of the proposed algorithm in ***,the results obtained from theMLA are compared with eight well-known *** results of optimization of objective functions show that the proposed MLA has a high ability to solve various optimization ***,the analysis and comparison of the performance of the proposed MLA against the eight compared algorithms indicates the superiority of the proposed algorithm and ability to provide more suitable quasi-optimal solutions.

There are many optimization problems in different branches of science that should be solved using an appropriate *** optimization algorithms are one of the most efficient approaches to solve this type of *** this paper,a new optimization algorithm called All Members-Based Optimizer(AMBO)is introduced to solve various optimization *** main idea in designing the proposedAMBOalgorithm is to use more information from the population members of the algorithm instead of just a few specific members(such as best member and worst member)to update the population ***,in AMBO,any member of the population can play a role in updating the population *** theory of AMBO is described and then mathematically modeled for implementation on optimization *** performance of the proposed algorithm is evaluated on a set of twenty-three standard objective functions,which belong to three different categories:unimodal,high-dimensional multimodal,and fixed-dimensional multimodal *** order to analyze and compare the optimization results for the mentioned objective functions obtained by AMBO,eight other well-known algorithms have been also *** optimization results demonstrate the ability of AMBO to solve various optimization ***,comparison and analysis of the results show that AMBO is superior andmore competitive than the other mentioned algorithms in providing suitable solution.

·AIM: To investigate the impact of cirrhosis on retinal morphology and to evaluate the role of endogenous opioids as a mediator in cirrhosis induced retinal change.·METHODS: Thirty-six male rats were divided into 3main groups; the common bile duct ligated(BDL) group,the sham-operated(Sham) group and the unoperated(Unop) group. Then each of these three main groups was divided into two subgroups; the first subgroup received daily injection of naltrexone hydrochloride(NTX) and the second group was injected with normal saline(Saline)daily. After 28 d, rats were anesthetized and their right eyes were enucleated and assessed for histological changes. The thickness of the rod and cons layer, outer nuclear layer, outer plexiform layer, inner nuclear layer,inner plexiform layer and ganglion cell layer for each eye were measured in micrometers by light microscope.· RESULTS: Ganglion cell layer showed significant increase in thickness in the BDL group(P <0.05). This increase was eliminated in the group where BDL rats received daily intraperitoneal injection of naltrexone hydrochloride(20 mg/kg). No other histological changes were detected in the other 5 layers we measured·CONCLUSION: The morphological change we detected in the retina of cirrhotic rats is probably due to opioids increased tone in cirrhosis since the increase in thickness in the ganglion cell layer was almost eliminated when naltrexone hydrochloride was *** results suggest a possible role for endogenous opioids in the morphological retinal changes detected in cirrhotic rats.

Purpose:Abdominal CT scan using oral and intravenous(IV)contrast is helpful in the diagnosis of intra-abdominal ***,the use of oral and IV contrast delays the process of diagnosis and increases the risk of *** has also been shown that CT scan with IV contrast alone is as helpful as CT scan with oral and IV contrast and rectal CT scan in detecting abdominal ***,the present study aims to prospectively compare the diagnostic value of CT scan with oral and IV contrast versus CT scan with IV contrast alone in the diagnosis of blunt abdominal trauma(BAT).Methods:Altogether 123 BAT patients,60(48.8%)women and 63(51.2%)men with the mean age of(40.4±18.7)years who referred to the emergency department of Imam Khomeini Educational and Medical Center in Sari,Iran(a tertiary trauma center in north of Iran)from November 2014 to March 2017 and underwent abdominal CT scans+laparotomy were *** with penetrating trauma or hemodynamically unstable patients were *** participants were randomly allocated to two groups:abdominal CT scan with oral and IV contrast(n=63)and CT scan with IV contrast alone(n=60).No statistically significant difference was found between two groups regarding the hemodynamic parameters,age,gender,injury mechanisms(all p>0.05).The results of CT scan were compared with that of laparotomy *** collected data were recorded in SPSS version 22.0 for *** data were presented as mean and ***:The sensitivity and specificity of CT scan using oral and IV contrast in the diagnosis of BAT were estimated at 96.48(95%CI:90.73-99.92)and 92.67(95% CI:89.65-94.88),respectively;while CT scan with IV contrast alone achieved a comparable sensitivity and specificity of 96.6(95% CI:87.45-99),42 and 92.84(95% CI:89.88-95.00),***:CT scan with IV contrast alone can be used to assess visceral injuries in BAT patients with normal hemodynamics to avoid diagnostic delay.
