Fatty liver index vs waist circumference for predicting non-alcoholic fatty liver disease

AIM: To determine the discriminatory performance of fatty liver index(FLI) for non-alcoholic fatty liver disease(NAFLD).METHODS: The data of 5052 subjects aged over 18 years were analyzed. FLI was calculated from body mass index, waist circumference(WC), triglyceride, and gamma glutamyl transferase data. Logistic regression analysis was conducted to determine the association between FLI and NAFLD. The discriminatory performance of FLI in the diagnosis of NAFLD was evaluated by receiver operating characteristic analysis. Area under the curves(AUCs) and related confidence intervals were estimated. Optimal cutoff points of FLI in the diagnosis of NAFLD were determined based on the maximum values of Youden's ***: The mean age of men and women in the study population were 44.8 ± 16.8 and 43.78 ± 15.43, respectively(P = 0.0216). The prevalence of NAFLD was 40.1% in men and 44.2% in women(P < 0.0017). FLI was strongly associated with NAFLD, so that even a one unit increase in FLI increased the chance of developing NAFLD by 5.8%(OR = 1.058, 95%CI: 1.054-1.063, P < 0.0001). Although FLI showed good performance in the diagnosis of NAFLD(AUC = 0.8656(95%CI: 0.8548-0.8764), there was no significant difference with regards to WC(AUC = 0.8533, 95%CI: 0.8419-0.8646). The performance of FLI was not significantly different between men(AUC = 0.8648, 95%CI: 0.8505-0.8791) and women(AUC = 0.8682, 95%CI: 0.8513-0.8851). The highest performance with regards to age was related to the 18-39 age group(AUC = 0.8930, 95%CI: 0.8766-0.9093). The optimal cutoff points of FLI were 46.9 in men(sensitivity = 0.8242, specificity = 0.7687, Youden's index = 0.5929) and 53.8 in women(sensitivity = 0.8233, specificity = 0.7655, Youden's index = 0.5888).CONCLUSION: Although FLI had acceptable discriminatory power in the diagnosis of NAFLD, WC was a simpler and more accessible index with a similar performance.

Time series modeling of animal bites

Objective:To explore the modeling of time series of animal bite occurrence in northwest ***:In this study,we analyzed surveillance time series data for animal bite cases in the northwest Iran province of Iran from 2011 to *** used decomposition methods to explore seasonality and long-term trends and applied the Autoregressive Integrated Moving Average(ARIMA)model to fit a univariate time series of animal bite *** ARIMA modeling process involved selecting the time series,transforming the series,selecting the appropriate model,estimating parameters,and ***:Our results using the Box Jenkins model showed a significant seasonal trend and an overall increase in animal bite incidents during the study *** best-fitting model for the available data was a seasonal ARIMA model with drift in the form of ARIMA(2,0,0)(1,1,1).This model can be used to forecast the frequency of animal attacks in northwest Iran over the next two years,suggesting that the incidence of animal attacks in the region would continue to increase during this time frame(2018-2019).Conclusion:Our findings suggest that time series analysis is a useful method for investigating animal bite cases and predicting future *** existence of a seasonal trend in animal bites can also aid in planning healthcare services during different seasons of the ***,our study highlights the importance of implementing proactive measures to address the growing issue of animal bites in Iran.

Stromal vascular fraction combined with silicone rubber chamber improves sciatic nerve regeneration in diabetes

Purpose:To study the effects of transplantation of characterized uncultured stromal vascular fraction (SVF) on sciatic nerve regeneration.Methods:A 10-mm sciatic nerve defect was bridged using a silicone conduit filled with SVF. In control group, silicone conduit was filled with phosphate-buffered saline alone. In sham-operated group, the sciatic nerve was only exposed and manipulated. The regenerated nerve fibers were studied 8 and 12 weeks after surgery.Results:Behavioral and functional studies confirmed faster recovery of regenerated axons in SVF transplanted animals than in control group ( p < 0.05). Gastrocnemius muscle mass in SVF transplanted animal was found to be significantly more than that in control group. Morphometric indices of the regenerated fibers showed the number and diameter of the myelinated fibers to be significantly higher in SVF transplanted animals than in control group. In immunohistochemistry, the location of reactions to S-100 in SVF transplanted animals was clearly more positive than that in control group. Conclusion:SVF transplantation combined with silicone conduit could be considered as a readily accessible source of stromal cells that improves functional recovery of sciatic nerve. It may have clinical implications for the surgical management of acute diabetic patients after facial nerve transection.

Identifying Association Rules among Drugs in Prescription of a Single Drugstore Using Apriori Method

These days, health care systems such as pharmacies and drugstores normally produce high volumes of data. Consequently, utilizing data mining methods in health care systems has become a conventional process. In this research, Apriori algorithm has been applied to perform data mining using the data obtained from the prescriptions ordered within a pharmacy. Ten association rules were achieved from the assigned pharmaceutical drugs in those prescriptions using the aforementioned Apriori algorithm. The accuracy of these rules is also manually studied and reviewed by a physician. Among these association rules, Vitamin D and Calcium pills are the most interrelated medications, and Omeprazole and Metronidazole rankd second in terms of association. The results of this study provide useful feedback information about associations among drugs.

What do Iranian general practitioners expect from family physician contracts?

background Family medicine has become a main prerequisite of providing primary healthcare and a main reforming strategy to ensure the delivery of efficient and high-quality health *** This study aimed to investigate general practitioners’(GP)preferences regarding family physician *** and setting Cross-sectional study was conducted among GPs who registered in Ministry of Health and Medical Education(MoHME)family physician plan and were working in the health network of moderately developed regions in *** sample size was calculated to be 150 GPs who were randomly selected from MoHME *** Developed questionnaire was distributed to *** were analysed by ordinal regression *** Study results confirmed that‘type of employer’had the most significant effect on GPs’preferences(β=0.86).Then attributes including‘allocating quota for being accepted in medical specialty’(β=0.78),‘increased length of contract’(β=0.00.42)and‘capitation payment+15%bonus’had respectively the great effects on participants’*** also revealed that a scenario of contracting with medical council was 2.4 times more likely to be chosen by GPs compared with a scenario of contracting with a medical ***,a scenario that allocated a quota for admission to medical specialty courses was 2.18 times more probable to be preferred by them(p<0.001).Conclusion Successful implementation of family medicine requires development of suitable solutions for attracting and attaining GPs in the *** seems that using a variety of incentives and applying them in physicians’work contract would be helpful in this regard.
