Epidemiology of Hand Injuries That Presented to the Emergency Department of a Tertiary Care Facility in Suburban mumbai, India: A Study Including 489 Patients

Background: Hand injuries are very common and responsible for a significant number of emergency department (ED) visits, most of which are not to hand specialists [1]. The functionality and outcome of hand injuries can vary significantly depending on the mechanism and pattern of injuries, which is why it is imperative for emergency physicians to recognize the complexities, and the potential repercussion of missed injuries in such cases. Objective: The aim of this study is to provide epidemiological information on hand injuries and their patterns. The objective is 1) to assess whether most hand injuries are superficial (simple), or involve underlying deeper structures (complex) and 2) to assess whether most hand injuries presented to the emergency department were managed by the emergency physician or plastic/orthopaedic surgeon. methods: This retrospective single-centre observational study conducted at an emergency department in a tertiary care hospital in mumbai, India collected data from hand trauma patients using a standardized documentation form. Demographic data, trauma-related data, and disposition plans were analysed. Results: A total of 489 cases sustained hand injuries over a period of one year. The patients were predominantly males in the 20 - 30 year age group and injuries were mainly sustained over the right hand. most of the injuries were sustained at home (42%). The most common mechanism (34%) was sharp object injury (including needle-stick and other sharps in hospital), followed by blunt injury (30%). Among grievous hand injuries, door jamb was a mechanism noted in 11% of patients, accounting for 50% of all crush injuries. Lacerations were the most common pattern (24.7%) noticed, followed closely by fractures (23.3%). Digits II - IV were injured most commonly (54%), followed by carpals (14%) and the thumb (10%). Bottle gourd (Lagenaria siceraria), colloquially known as “lauki” or “doodhi” in India, is an edible plant in the Cucurbitaceae family. Consumption of raw bottle gourd juice is common practice in India. However, little known is the fact that ingestion of raw bottle gourd juice that is extremely bitter tasting is associated with cucurbitacin toxicity. Cucurbitacin is a pheromone produced by the plant as a defense mechanism against insects and herbivores. It is responsible for imparting the bitter taste, and the associated toxicity. The exact mechanism of cucurbitacin toxicity is not well understood, however it is suspected to result in gastric erosions and increased capillary permeability, resulting in rapid onset of vomiting, diarrhea, gastrointestinal bleeding, hypotension and shock. As serum and urine concentration measurements of cucurbitacin are not readily available, bottle gourd toxicity remains a clinical diagnosis based largely on the temporal association of ingestion of unusually bitter tasting vegetable or its juice with rapid onset of symptoms. Physician awareness is therefore of paramount importance in the diagnosis of this unusual entity. We present the case of a 78-year-old woman, with no comorbid conditions, who presented to the Emergency Department (ED) with three episodes of vomiting and multiple episodes of watery loose stools soon after ingestion of extremely bitter tasting, fresh bottle gourd juice. A prompt diagnosis of cucurbitacin toxicity was made in the ED and fluid resuscitation was commenced expeditiously. However, it was noted that most physicians were unfamiliar with this entity, and therefore skeptical of the diagnosis. Our rationale in presenting this clinical case report is three-fold: 1) To create physician awareness regarding this unique clinical entity, especially among emergency physicians, as they are the first point of contact for a critically ill patient. 2) To highlight the rapid onset and progression of symptoms in cucurbitac

One of the major problems of the SLAm robotic is addressing the localization problem,which is about identifying a robot’s position in the surrounding ***,application of robots at home,in our daily life has become more common than ever *** this work,we provide a method to address the home robot’s localization problem by applying Convolutional Neural Networks（CNN） *** achieve this,we first built a dataset by extracting the laser images based on the floor plan map of the *** to classify these images we trained the robots by implementing CNN *** test our method,we analyzed it in the virtual ***,consider the accuracy and time as the parameters to an assessment of the designed *** on the results of the empirical experiments,the proposed approach achieved 100% accuracy,and a different amount of time based on the type of CNN model.

Simultaneous Determination of Dopamine and L-Ascorbic Acid by modified Carbon Paste Electrode with Ni (II) Cyclam Complex

The electroanalysis of dopamine (DA) and ascorbic acid (AA) by square wave voltammetry has been performed at a modified carbon paste electrode with macrocyclic ligand 1, 4, 8, 11-tetraazacyclotetradecane (cyclam) and monolayer of Ni (II) cyclam. In pH 7.2 buffer solutions, the electrostatic reaction of AA with di-positive monolayer shifts the oxidation potential to less positive potential, while the electrostatic repulsion of DA with the monolayer shifts the oxidation potential of DA to more positive potential. The separation between the oxidation peaks of AA and DA at the present di-positive monolayer modified electrode (252 mV) was larger than that (187 mV) at the cyclam modified electrode. In addition, the catalytic oxidation of AA by oxidized DA has been advantageously eliminated at the modified carbon paste electrode with cyclam and Ni (II) cyclam complex. Thus, the determination of DA in the presence of an excess of AA is possible with the present modified electrodes.

Nowadays due to the advancement of technology,especially in the field of computer science,quality of the maps,resolution as well as the image details,has been *** having a more detailed map provides more information about the environment,it makes it even more difficult to *** to these reasons,the segmentation of map of an indoor environment to the smaller meaningful regions such as room,corridor and *** be helpful for making a better understanding of *** this paper,we design a method based on the topological information of the 2 D floor plan map to address the segmentation *** proposed method is fully automatic and does not need any pretrained data or any command by *** experiment result demonstrate that the proposed method improves system performance by reducing runtime,fast computation,and easy implementation.

Nowadays the localization problem is one of the most important issue and a major topic related to the SLAm *** do this,subjects related to computational complexity and system response speed are taken into *** monte Carlo method is one of the most important techniques used to face localization *** this method is one of the most commonly used methods to deal with the localization problem due to easy implementation and high efficiency,it increases the cost of computations because of the use of particle sets and the information obtained from the *** paper presents a novel localization approach based on the uniform distribution to deal with computational *** address this problem,we propose a method by extending monte Carlo localization algorithm using uniform *** this algorithm,the particles are uniformly positioned on the state space and sensor model follows a uniform distribution as *** results of the empirical experiments illustrate that the proposed approach improves system performance by reducing computational complexity,fast computation,and easy implementation.

A challenging task in medical dermoscopic image segmentation is detecting the location of the *** detection of a polyp is even more difficult owing to its low contrast with the surrounding *** paper proposed an end-to-end network based on the convolutional neural network,which does not require any prior processing to detect and segment the polyps in the medical *** proposed convolutional neural network is based on the encoder-decoder architecture,which advantages the benefits of skip *** connections can play an effective role in the encoder-decoder-based networks but have not received enough attention in network *** advantage of the proposed model is that it has a unique skip connection design that leads to better accuracy in the ***,the proposed model is tested on the CVC-ClinicDB dataset,and the experimental results illustrate our model outperforms other state-of-the-art approaches.
