Sclerosing Mucoepidermoid Cancer: A Unique Entity

Sclerosing mucoepidermoid thyroid cancer (SMECE) is a rare entity with less than 100 cases reported in the literature. Previously considered to have an indolent course, however, recent evidence has reported an aggressive nature ranging from local invasion to distant metastasis. We present a 66-year-old Caucasian female with SMECE who initially presented neck compressive symptoms. A thyroid ultrasound (US) revealed a solid hypoechoic mass replacing the left thyroid lobe. Fine needle aspiration cytology (FNAC) of the nodule resulted in suspicion of Papillary Thyroid Cancer, Bethesda category 5. The patient underwent total thyroidectomy and surgical pathology showed SMECE. Post-therapy whole-body scan following treatment with 150 mCi I-131 showed no residual or metastatic disease. SMECE is more common in females, between the third to eighth decade of life. Preoperative diagnosis may not be accurate given variable cytopathologic features. Differential diagnoses include primary squamous cell carcinoma of the thyroid, squamous differentiation of other thyroid malignancies, anaplastic thyroid cancer and nodular sclerosing variety of Hodgkin’s lymphoma. Due to its rarity, treatment of SMECE has ranged from thyroid surgery without or with radioactive iodine therapy, to surgery and external beam radiation and even chemotherapy.

Reliability and sensitivity analysis of wedge stability in the abutments of an arch dam using artificial neural network

In this study,the seismic stability of arch dam abutments is investigated within the framework of the probabilistic method.A large concrete arch dam is considered with six wedges for each abutment.The seismic safety of the dam abutments is studied with quasi-static analysis for different hazard levels.The Londe limit equilibrium method is utilized to calculate the stability of the wedges in the abutments.Since the finite element method is time-consuming,the neural network is used as an alternative for calculating the wedge safety factor.For training the neural network,1000 random samples are generated and the dam response is calculated.The direction of applied acceleration is changed within 5-degree intervals to reveal the critical direction corresponding to the minimum safety factor.The Latin hypercube sampling(LHS)is employed for sample generation,and the safety level is determined with reliability analysis.Three sample numbers of 1000,2000 and 4000 are used to examine the average and standard deviation of the results.The global sensitivity analysis is used to identify the effects of random variables on the abutment stability.It is shown that friction,cohesion and uplift pressure have the most significant effects on the wedge stability variance.

Optimization of synchronized frequency and voltage control for a distributed generation system using the Black Widow Optimization algorithm

A distributed generation network could be a hybrid power system that includes wind-diesel power generation based on induction generators(IGs)and synchronous generators(SGs).The main advantage of these systems is the possibility of using renewable energy in their structures.The most important challenge is to design the voltage-control loop with the frequency-control loop to obtain optimal responses for voltage and frequency deviations.In this work,the voltage-control loop is designed by an automatic voltage regulator.A linear model of the hybrid system has also been developed with coordinated voltage and frequency control.Dynamic frequency response and voltage deviations are compared for different load disturbances and different reactive loads.The gains of the SG and the static volt-ampere reactive compensator(SVC)controllers in the IG terminal are calculated using the Black Widow Optimization(BWO)algorithm to insure low frequency and voltage deviations.The BWO optimization algorithm is one of the newest and most powerful optimization methods to have been introduced so far.The results showed that the BWO algorithm has a good speed in solving the proposed objective function.A 22%improvement in time adjustment was observed in the use of an optimal SVC.Also,an 18%improvement was observed in the transitory values.

Development and validation of a simple UV spectrophotometric method for the determination of levofloxacin both in bulk and marketed dosage formulations

A rapid, specific and economic UV spectrophotometric method has been developed using a solvent composed of water：methanol：acetonitrile （9：0.5：0.5） to determine the levofloxacin content in bulk and pharmaceutical dosage formulations. At a pre-determined 2 of 292 nm, it was proved linear in the range of 1.0 12.0 μg/mL, and exhibited good correlation coefficient （R2=0.9998） and excellent mean recovery （99.0-100.07%）. This method was successfully applied to the determination of levofloxacin content in five marketed brands from Bangladesh and the results were in good agreement with the label claims. The method was validated statistically and by recovery studies for linearity, precision, repeatability, and reproducibility. The obtained results proved that the method can be employed for the routine analysis of levofloxacin in bulks as well as in the commercial formulations.

Gas Blowout Impacts on Ground Water Environs around the Tengratila Gas Field, Chattak, Bangladesh

Gas blowout is one of the major hazard in petroleum field which normally damages the gas bearing geologic formation, structure, local tectonic setting, environment and so on. In Bangladesh, there have been three well known gas blowouts. Among them, the most dangerous gas well blowout took place on 8 January 2005 in Sunamganj district when chattak-2 (also known as Tengratila) gas field was drilled. As a result, the surrounding area is facing various problems among them water is the top of the list. From this point of view, the present study has been considered to find the impact of blowout on water around the gas blowout area. In this regard, the water samples (some are very near and some are away from the well) are collected and analyzed in the laboratory following the standard method. Some physical and chemical parameters of water such as pH, turbidity, EC, total solids, dissolved solids, suspended solids, manganese ion, calcium ion, magnesium, iron, chloride and total hardness have measured where without turbidity, manganese and iron, all are still in tolerable state for all purposes and ranging within standard limit based on WHO, EU and EQS for Bangladesh. The quality of the near well tube wells water is much decreased than far away tube wells water which might be the direct or indirect influence of the blowout incident around the area.
