Objective:To investigate the relationship between triterpenoid saponin content and antioxidant,antimicrobial,and α-glucosidase inhibitory activities of 70%ethanolic,butanolic,aqueous,supernate and precipitate extracts of Juglans regia ***:Triterpenoid saponins of different Juglans regia leaf extracts were measured by the vanillin *** activity was evaluated against DPPH and ABTS free *** also assessed α-glucosidase inhibitory and antimicrobial activities of the leaf ***’s correlation coefficient was evaluated to determine the correlation between the saponin content and biological ***:The butanolic extract was most effective against DPPH with an IC50of 6.63μg/mL,while the aqueous extract showed the highest scavenging activity against ABTS free radical with an IC50of 42.27μg/***’s correlation analysis indicated a strong negative correlation (r=-0.956) between DPPH radical scavenging activity (IC50) and the saponin content in the samples *** addition,the aqueous extract showed the best α-glucosidase inhibitory activity compared with other *** the extracts had fair antibacterial activity against Bacillus subtilis,Escherichia coli,and Klebsiella pneumoniae except for the aqueous ***:Juglans regia extracts show potent antioxidant,antimicrobial,and α-glucosidase inhibitory *** is a correlation between saponin levels in Juglans regia leaf extracts and the studied ***,additional research is required to establish these relationships by identifying the specific saponin molecules responsible for these activities and elucidating their mechanisms of action.

Severe Anaemia during Natalizumab Treatment: Case Presentation with Literature Review

Progressive multifocal leukoencephalopathy is the most common serious complication related to natalizumab. However, serious haematological complications are very uncommon during treatment with natalizumab. Here we reported the case of an 18-year-old man with a 4-year history of relapsing-re- mitting multiple sclerosis. He was treated firstly by Teriflunomide for 1 year, but he presented relapses and his MRI shows new contrast-enhancing lesions. Therefore, we decided to switch from Teriflunomide to Natalizumab. The patient presented with profound anaemia after the 16th infusions treatment with Natalizumab. The patient’s hemoglobin was 3.2 g/dL with a lower blood reticulocyte value. After red cell transfusions and cessation of Natalizumab, anaemia resolved. Natalizumab was changed by an anti-CD20 monoclonal antibody. The patient had a stable course of multiple sclerosis at 13 months after initiation of Rituximab. We should alert clinicians to be aware of the possibility of anaemia during treatment by Natalizumab.

Rituximab for the Treatment of Multiple Sclerosis: A Retrospective Observational Cohort in Morocco

Background: RITUXIMAB (RTX) is a chimeric anti-CD20 monoclonal antibody that has initially demonstrated efficacy in patients with B-cell lymphoma. Then, over time, it has demonstrated its efficacy in systemic inflammatory diseases and recently in neurological diseases such as multiple sclerosis (MS). Here we describe our experience with rituximab from one MS center from MOROCCO. Objectives: To investigate the safety and efficacy of Rituximab in MS in a Moroccan population. Methods: A retrospective uncontrolled observational single-center study from January 2017 to July 2020, was including all off-label Rituximab-treated patients with MS with at least 6 months of follow-up. Outcome data were collected and evaluated relapse rate, EDSS score, and adverse events (AEs) from the medical charts. Adverse events grade according to the Common Terminology Criteria for Adverse Events. Results: A total of 63 MS patients were treated with RTX, 47 patients were included, while 12 cases had just initiated treatment and 4 cases were lost to follow-up. The mean age of the patients was 39 ± 12 years with a female predominance (F/M: 1.6). All forms of MS were included, 83% of whom had relapsing-remitting MS. The duration of disease progression was 8 ± 5 years. Median EDSS before RTX initiation was 5.5 (0 - 7). 51% of patients were treated with RTX as second-line therapy after failure of other disease-modifying therapies, whereas 34% received it as first-line therapy. The annualized relapse rates decreased from 0.8 to 0.2 after RTX treatment. The Median EDSS remained unchanged at 71%. Radiological stability was noted in 83.7%, while 13.5% had a single new T2 lesion. Infusion-related AEs occurred during 27.6% of infusions and most were mild. Simple infection grades ≤ 2 were noted in 19%. Abortion occurred in only one patient. Conclusion: Our study confirms the usability of rituximab treatment for MS in the MOROCCO healthcare environment.

Upper limb compartment syndrome after an adder bite： a case report

Compartment syndrome after an adder bite is extremely rare, whose effects are only secondary to the cytotoxic and hemorrhagic effects of venom. Here we reported a case of compartment syndrome in the upper limb following an adder bite in the thenar eminence. Elevated compartment pressure was documented and immediate surgical fasciotomy was practiced. The patient achieved complete recovery with a good functional result. We discussed the controversies on fasciotomy and non-invasive measures in such a situation, and recommended intracompartmental pressure monitoring during the management of compartment syndrome following adder bites.

Impact of body mass index on hepatocellular carcinoma recurrence after liver transplantation through long-term follow-up

Background:Obesity is associated with increased oncological risk and outcomes but the evidence surrounding the effect of body mass index(BMI)on increased risk of hepatocellular carcinoma(HCC)recurrence after liver transplantation(LT)is still *** purpose of this retrospective study of a large cohort of adult patients transplanted for HCC was to investigate the effect of BMI on the incidence of HCC recurrence and ***:Data from 427 adult recipients transplanted for HCC between 2000 and 2017 were *** were classified at time of LT according to the World Health Organization BMI classification into 3 groups;group 1:BMI<25(n=166),group 2:BMI 25-29.9(n=150)and group 3:BMI≥30(n=111).Results:There were no significant changes of mean BMI overtime 26.8±5.0 kg/m2 at time of LT and 28.8±23.1 at 5 *** recurrence rates of HCC after LT in the three groups were 19%,16%and 17%*** 5,10 and 15-year recurrence free survival(RFS)rates were respectively 68.6%,47.3%and 40.8%in group 1,73.3%,66.2%and 49.5%in group 2 and 68.8%,57.5%and 47.7%in group 3(log rank P=0.47).Conclusions:Recipient BMI at time of transplant and during follow-up didn’t impact the incidence of HCC recurrence nor long-term patient survival,irrespective to the status of the patients and their tumor characteristic at time of *** present study clearly confirms that obesity should not be considered,when selecting patients with HCC to LT,as a predictive factor of recurrence.

Nonunion of the scaphoid treated by anterior vascularized bone graft： a review of 26 cases

[Abstract] Objective： To evaluate the clinical and radiological outcome of anterior vascularized bone graft in the treatment of primitive nonunion of the scaphoid. Methods： This is a retrospective study of 26 scaphoid nonunion cases treated by this technique between 2004 and 2009. The mean age was 38 years. Nonunion was of types IIA and IIB according to the Alnot＇s classification and only one case showed a proximal pole necrosis （type IV）. The fixation was ensured by K-wire. The mean immobilization period was 9 weeks. Results： At mean 19 months follow-up, the bone union rate was 88.5%. Failure was noted in three cases. No significant complications secondary to this technique were observed. The mobility in extension and flexion had an average recovery of 18°. Nineteen patients （73%） were free of pain, and the others had occasional pain Conclusions： It is a meticulous surgical technique that provides better union rates than those of conventional scaphoid graft. This technique is first adopted in cases of failure of conventional grafts in the treatment of scaphoid nonunion but the encouraging results suggest expanding indications to primitive nonunion.

Predictive Factors of No Reflow during Primary Angioplasty

Introduction: No reflow during primary angioplasty is associated with a poor prognosis despite the reopening of the culprit coronary. The aim of our work was to determine the predictive factors of no reflow. Methodology: Single-center retrospective analytical study from June 2000 to December 2016 that included patients presenting with STEMI took care of by primary angioplasty. No reflow was defined according to angiographic criteria: a TIMI flow Results: The prevalence of no reflow was 24%. In univariate analysis mean age, diabetes,hypertension, tachycardia, hypotension, killip stage 4 left ventricular failure, hyperglycemia > 11, renal failure, left ventricular dysfunction, tritruncal status, common trunk involvement, initial TIMI flow at 0, significant thrombotic load, delay to angioplasty > 6 hours, and predilation were all correlated with no reflow with a p 75 years [OR = 6.02, 95% CI 1.4 - 27, p = 0.014], tachycardia [OR = 4.3, 95% CI 1.6 - 7.4, p = 0.037], delay to angioplasty > 6 hours [OR = 1.3, 95% CI 1.1 - 2.1, p = 0.003] and high thrombotic load [OR = 1.5, 95% CI 1.3 - 3.2, p = 0.02] were independent predictors of no reflow. Conclusion: No reflow is associated with a poor short-term prognosis. Its care requires knowledge of predictive factors, prevention and treatment.

Greenhouse Environmental Control Using Optimized, Modeled and Simulated Fuzzy Logic Controller Technique in MATLAB SIMULINK

The main objective of this study is the control of the agricultural greenhouse in view of the economic interest generated by such an activity. A simulation model is developed, gathering all the external and internal climatic conditions that influence the microclimate of the greenhouse to predict the temporal evolution of the state variables characterizing this microclimate. The fuzzy control is an alternative to the approaches proposed by the automatic for the control of complex systems. The performance objectives of the looped systems and the corresponding actions are summarized in the form of rules of expertise, which are spelled out in plain language. This technique thus makes it possible to dispense with the use of mathematical models which are sometimes difficult to obtain. Our objective is the multivariable strategy synthesis and the fuzzy application to a multivariate system （MIMO ~ such as the agricultural greenhouse.） First, the principles of fuzzy logic and fuzzy control are recalled. The origins of non-Linearitys of the command are explained. One of the practical problems of this technique is the combinatorial explosion of the rule base when the number of variables involved becomes large. A solution to simplify the complexity of the system is presented together with an optimization algorithm to automatically adjust the parameters of the fuzzy controller. The last part is devoted to the synthesis of an optimal control of the greenhouse in order to compare it to the fuzzy control implemented.

Wegener’s Granulomatosis Presenting with Bilateral Excavated Nodules

Wegener’s granulomatosis (WG) is a systemic inflammatory disease whose histopathologic features often include necrosis, granuloma formation, and vasculitis of small-to-medium-sized vessels. We report a new case of Wegener’s granulomatosis occurring in a 41-year-old man who was admitted for respiratory symptoms, fever, asthenia, night sweats and weight loss since one month. Chest X-ray and computed tomography showed multiple bilateral pulmonary excavated lesions. A trransparietal lung biopsy was performed. Histological examination showed an area of necrosis, surrounded by a palisade of histiocytes. Some of the histiocytes were multinucleated and formed true granulomas within the wall of the adjacent pulmonary artery. Serology was strongly positive for ANCA. A diagnosis of Wegener granulomatosis was conducted and the patient was started on cyclophosphamide and methylprednisolone as an initial treatment, with a favorable evolution.
