Objective:Thermal ablation poses challenges in the surgical resection(SR)of small hepatocellular carcinoma(HCC),and its therapeutic outcomes for larger lesions remain debated.Methods:This retrospective study evaluated 729 patients with HCC meeting the Milan criteria,who were treated with curative SR or microwave ablation(MWA)between 2008 and 2014.Overall survival(OS),cancer-specific survival(CSS),disease-free survival(DFS),and local tumor progression(LTP)were compared after propensity score matching(PSM).Co-variates associated with OS,CSS,LTP,and DFS were identified.The risk of death and tumor progression were compared.Results:During the median follow-up of 78.6 months,253 patients were included in each group after PSM.For tumors≤3.0 cm and 3.1–4.0 cm,MWA achieved comparable results in terms of OS,CSS,DFS,and LTP.For tumors 4.1–5.0 cm,MWA had lower OS,CSS,and DFS rates(all P<0.05)than SR.Higher LTP rates were observed in the MWA group for tumors 4.1–5.0 cm,although the difference was not significant(P=0.18).Complication rates(P=0.41)were similar,but MWA led to less estimated blood loss(P<0.01)and shorter postoperative hospitalization times(P<0.01).Conclusions:MWA achieved comparable long-term oncologic outcomes with SR for≤4 cm HCC,with lower complication rates and faster recovery.

Objective:To construct a nomogram based on the albumin-bilirubin(ALBI)grade to provide prognostic value for hepatitis C virus(HCV)-related hepatocellular carcinoma(HCC)patients who underwent ultrasound-guided percutaneous microwave ablation(US-PMWA).Methods:From April 2005 to January 2018,183 treatment-naIve patients with 251 HCV-related HCCs according to the Milan criteria received US-PMWA subsequently.The overall survival(OS)and recurrence-free survival(RFS)were compared between groups classified by ALBI grade.Cox proportional hazard regression model based on risk factors for survival and recurrence was used to construct the nomogram.Results:The cumulative OS rates at 1-,3-,5-and 10-year were 97.7%,73.6%,54.5%and 34.5%,respectively.Stratified according to ALBI grade,the 1-,3-,and 5-year OS in the ALBI grade 1 group and grade 2 group were 99.2%,92.4%,77.9% and 97.7%,52.3%,38.6%,respectively,with significant statistical difference(P<0.001).No significant statistical difference was detected in the1-,3-,and 5-year RFS rates in the ALBI grade 1 group and grade 2 group(P=0.220).The major complication rate was 1.6%.Multivariate analysis results showed age,α-fetoprotein level,tumor number,platelet count,location,Child-Turcotte-Pugh(CTP)and ALBI grade were associated with OS,which generated the nomograms.Internal validation with 1000 bootstrapped sample sets had good concordance index of 0.769(95%CI 0.699-0.839)in OS.Conclusions:This nomogram based on ALBI grade was a visualization risk model,which could provide personalized prediction of long-term outcomes for HCV-related HCC patients after US-PMWA.

Objective:To explore the association between cholecystectomy and the prognostic outcomes of patients with hepatocellular carcinoma(H CC)who underwent microwave ablation(MWA).Methods:Patients with HCC(«=921)who underwent MWA were included and divided into cholecystectomy(n=114)and non-cholecystectomy groups(n=807).After propensity score matching(PSM)at a 1:2 ratio,overall survival(OS)and disease-free survival(DFS)rates were analyzed to compare prognostic outcomes between the cholecystectomy(«=114)and non-cholecystectomy groups(n=228).Univariate and multivariate Cox analyses were performed to assess potential risk factors for OS and DFS.Major complications were also compared between the groups.Results:After matching,no significant differences between groups were observed in baseline characteristics.The 1-,3-,and 5-year OS rates were 96.5%,82.1%,and 67.1%in the cholecystectomy group,and 97.4%,85.2%,and 74.4%in the non-cholecystectomy group(P=0.396);the 1-,3-,and 5-year DFS rates were 58.4%,34.5%,and 26.6%in the cholecystectomy group,and 73.6%,44.7%,and 32.2%in the non-cholecystectomy group(P=0.026),respectively.The intrahepatic distant recurrence rate in the cholecystectomy group was significantly higher than that in the non-cholecystectomy group(P=0.026),and the local tumor recurrence and extrahepatic recurrence rates did not significantly differ between the groups(P=0.609 and P=0.879).Multivariate analysis revealed that cholecystectomy(HR=1.364,95%Cl 1.023-1.819,P=0.035),number of tumors(2 vs.1:HR=2.744,95%Cl 1.925-3.912,P

目的探索≤5 cm肝细胞癌(HCC)微波消融后局部进展(LTP)的空间分布规律。方法回顾性分析2009年12月至2019年12月169枚具有消融前后匹配MRI的HCC。运用三维可视化技术重建肿瘤MRI,根据LTP是否与消融区边缘接触及发生时间(24个月),将LTP分类为接触型或非接触型LTP,早期或晚期LTP,利用八象限地图法将肿瘤周围空间划分8个象限,分析消融最短安全边界(AM)所在象限与不同类型LTP发生象限空间关系。计量资料用t检验或者秩和检验;计数资料用χ^(2)检验比较两组的差异。结果54.4%LTP分布于AM所在象限,64.2%早期LTP及69.1%接触型LTP分布于AM所在象限,较其他象限显著集中(P<0.001);仅15.2%非接触型LTP及17.1%晚期LTP分布于AM所在象限,与八象限平均分布概率12.5%差异无统计学意义(P=0.667,0.743)。46.6%早期接触型LTP位于消融针尖端方位,25.2%位于体部方位,28.1%位于尾部方位,而非早期接触型LTP的位置分布概率分别为34.8%,31.8%和33.3%。结论LTP多数发生于消融最短安全边界所在区域,但非接触型LTP及晚期LTP呈均匀分布特点,这类LTP可能为非消融安全边界不足导致。

In 21st century, more and more people see medical services of sport therapists as a promising industry due to global aging population and high biotechnology development. Hence, understanding medical word-of-mouth (WOM) process among patients in medical services of sport therapists could be interesting and useful. This study would find out the role of WOM on patients psychology when choosing the sport therapists, understand how patients look for sport therapists’ information and which information source is important to them. As a pioneer, this study would like to improve medical WOM marketing of sport therapists. The authors designed a questionnaire based on how patients choosing sport therapists, including questions search importance of different information sources of sport therapists with medical service decision, and respondents’ psychological demographics. We released 253 questionnaires of judo therapy clinics inTaiwan. More than half of the respondents see sport therapists due to others’ recommendations (referral of family, friends and professionals). Women are more likely to seek medical word-of-mouth information than men;and respondents who think choosing sport therapists are important tend to seek medical word-of-mouth information. Further, patients focus on “therapist’s behavior”, “therapist’s skills”, and “therapist’s ethic”.

Aromatic hydrocarbons generally refer to compounds containing benzene rings. Many types of isomers can be formed by replacing hydrogen atoms on the benzene ring. In this paper, an aromatic-hydrocarbon-inspired modular robot(AHIMR) is proposed.The robot can be reassembled into different configurations suitable for various task requirements. A vision-based docking system is designed for the AHIMR. The system primarily consists of two stages: a remote guidance stage and a precise docking stage.During the remote guidance stage, an object module is identified using an illumination adaptive target recognition algorithm,and then the active module moves to the docking area through communication with ZigBee. In the precise docking stage, the active module calculates the relative pose with the object module using a perspective-n-point method and dynamically adjusts its posture to dock. In this process, a Kalman filter is used to reduce target occlusion and jitter interference. In addition, the docking system feasibility is verified via several simulation experiments. The module docking accuracy is controlled within 0.01 m, which meets the reconfiguration task requirements of the AHIMR. In the AHIMR submodule docking experiment, the active module accurately moves to the expected position with a docking success rate of 95%.

目的对比观察微波消融(MWA)治疗乙型(HB)与丙型肝炎(HC)背景下肝细胞癌(HCC)的预后。方法回顾性分析HCC伴HB(HB-HCC)及HCC伴HC(HC-HCC)各159例患者资料,比较组间肿瘤学结局,分析患者死因,观察HCC患者MWA后总生存期(OS)的危险因素。结果HC-HCC组OS率低于HB-HCC组(P=0.045),组间无病生存率(P=0.095)及癌症特异性生存率(P=0.189)差异均无统计学意义。相比HB-HCC组,HC-HCC组患者死于肝硬化并发症的风险更高(HR=2.339,P=0.043)。Child-Pugh B级(HR=3.082,P500 IU/ml(HR=1.654,P=0.006)及病灶最大径≥3.0 cm(HR=1.541,P=0.017)均为HCC患者MWA后OS的独立危险因素。结论相比HB-HCC患者,HC-HCC患者MWA后OS较短。

目的评估经皮微波消融(MWA)治疗肝细胞癌的长期疗效。方法回顾性收集2006年1月至2020年9月中国人民解放军总医院第五医学中心巴塞罗那临床肝癌(BCLC)0~B期患者2054例,所有患者至少随访2年。分析患者超声引导经皮MWA治疗后的主要终点总生存率和次要终点(肿瘤相关生存率、无瘤生存率和术后并发症)。生存率使用Kaplan-Meier法进行分层分析。总生存使用Fine-and Gray竞争风险模型进行分析。结果2006年1月至2020年9月共有5503个肝癌结节[平均肿瘤长径(2.6±1.6)cm]接受3908次MWA,中位随访时间为45.6(24.0~79.2)个月,总队列的技术成功率为97.6%(5376/5503)。5、10、15年总生存率分别为61.6%、38.8%、27.0%;肿瘤特异性生存率分别为67.1%、47.2%和37.7%;无瘤生存率分别为25.8%、15.7%和9.9%;严重并发症发生率2.8%(108/3908)。进一步分析显示,在2006-2010年、2011-2015年和2016年1月-2020年9月的3个时间段内,随时间推移,技术有效率和生存率明显增高,P值均<0.001,特别是对于肿瘤长径为3.1~5.0 cm的肝癌结节(P<0.001)。结论微波消融治疗对于BCLC 0~B期肝癌是一种安全有效的方法,技术有效率和生存率随时间显著提升。

Tourmaline guiding the electric field and dechlorination pathway of 2,3-dichlorophenol by Desulfitobacterium hafniense

The dehalogenation of organohalides has been a research hotspot in bioremediation field;however,the influence of tourmaline,a natural ore that can generate spontaneous electric field,on organohalide-respiring bacteria(OHRB)and their dechlorination process is not well known.In this study,the effect and mechanism of tourmaline on the reductive dechlorination of 2,3-dichlorophenol(2,3-DCP)by Desulfitobacterium hafniense DCB-2Twere explored.The characterization results confirmed that tourmaline had good stability and the optimal dosage of tourmaline was 2.5 g/L,which shortened the total time required for dechlorination reaction to 72 hr.Besides,tourmaline amendment also increased the proportion of 2-chlorophenol(2-CP)from 18%to 30%of end products,while that of 3-CP decreased correspondingly.The theoretical calculations showed that the bond charge of the orthosubstituted chlorine declined from-0.179 to-0.067,and that of meta-substituted chlorine increased from-0.111 to-0.129,which indicated that the spontaneous electric field of tourmaline affected the charge distribution of 2,3-DCP and was more conducive to the generation of 2-CP.Overall,tourmaline with the spontaneous electric field affected the reductive dechlorination pathway of Desulfitobacterium,and the tourmaline-OHRB combining system might serve as a novel strategy for the bioremediation of environments polluted with chlorinated phenols.

In time-variant reliability problems,there are a lot of uncertain variables from different sources.Therefore,it is important to consider these uncertainties in engineering.In addition,time-variant reliability problems typically involve a complexmultilevel nested optimization problem,which can result in an enormous amount of computation.To this end,this paper studies the time-variant reliability evaluation of structures with stochastic and bounded uncertainties using a mixed probability and convex set model.In this method,the stochastic process of a limit-state function with mixed uncertain parameters is first discretized and then converted into a timeindependent reliability problem.Further,to solve the double nested optimization problem in hybrid reliability calculation,an efficient iterative scheme is designed in standard uncertainty space to determine the most probable point(MPP).The limit state function is linearized at these points,and an innovative random variable is defined to solve the equivalent static reliability analysis model.The effectiveness of the proposed method is verified by two benchmark numerical examples and a practical engineering problem.
