Treatment of multisystem inflammatory syndrome in children

Background Multisystem inflammatory syndrome in children(MIS-C),a relatively uncommon but severe pediatric complication,is associated with coronavirus disease 2019(COVID-19).A variety of treatment approaches,including intravenous immunoglobulins(IVIGs),glucocorticoids(GCs)and biologic agents,such as anakinra and infliximab,have been described for the management of COVID-19-related MIS-C.Anticoagulant therapy is also important.However,a well-developed treatment system has not been established,and many issues remain controversial.Several recently published articles related to the treatment of MIS-C have been released.Hence,in this review,we identified relevant articles published recently and summarized the treatment of MIS-C more comprehensively and systematically.Data sources We reviewed the literature on the treatment of MIS-C through 20 September 2023.The PubMed/Medline,Web of Science,EMBASE,and Cochrane Library databases were searched with the combination of the terms"multisystem inflammatory syndrome","MIS-C","PIMS-TS","therapy","treatment","drug","IVIG","GCs","intravenous immunoglobulin","corticosteroids","biological agent",and"aspirin".Results The severity of MIS-C varies,and different treatment schemes should be used according to the specific condition.Ongoing research and data collection are vital to better understand the pathophysiology and optimal management of MIS-C.Conclusions MIS-Cis a disease involving multiple systems and has great heterogeneity.With the accumulation of additional experience,we have garnered fresh insights into its treatment strategies.However,there remains a critical need for greater standardization in treatment protocols,alongside the pressing necessity for more robust and meticulously conducted studies to deepen our understanding of these protocols.

Prospects for the transformation and development of carbon storage in abandoned mines of coal enterprises from the perspective ofcarbon neutrality

Under the carbon neutrality goal,coal enterprises must seek breakthroughs from abandoned mines,develop new resources in the new era,turn problems into countermeasures,and participate in the carbon emissions market,for contributing to the accomplishment of the national strategic goal of carbon neutrality.To this end,we investigated the relevant national policies and regulations to clarify the boundaries disclosed by the carbon information of enterprises,understood the development direction of carbon storage in abandoned mines,and clarified the transformation and development of carbon storage in aban-doned mines.We made a few suggestions:(1)China should learn from its past experience and other countries to develop the energy industry with Chinese characteristics and reform the economic system.(2)Coal enterprises must actively respond to the national carbon information disclosure policy,clarify their own responsibilities and carbon emission boundaries.(3)It is necessary to proactively obtain advanced knowledge and plan carbon storage pathways for abandoned mines.(4)Devel-opment problems of coal enterprises should be deduced using cases.The'dual carbon'goals should be achieved steadily step-by-step.(5)Three measures,i.***.improving the existing resource structure,coordinating the information of abandoned mines,and promoting the cultivation of scientific and technological talents.

Equilibrium thallium isotope fractionation and its constraint on Earth’s late veneer

Equilibrium isotope fractionation of thallium(Tl) includes the traditional mass-dependent isotope fractionation effect and the nuclear volume effect(NVE). The NVE dominates the overall isotope fractionation, especially at high temperatures. Heavy Tl isotopes tend to be enriched in oxidized Tl^3+-bearing species. Our NVE fractionation results of oxidizing Tl^+ to Tl^3+ can explain the positive enrichments observed in ferromanganese sediments. Experimental results indicate that there could be0.2–0.3 e-unit fractionation between sulfides and silicates at 1650 ℃. It is consistent with our calculation results,which are in the range of 0.17–0.38 e-unit. Importantly,Tl’s concentration in the bulk silicate Earth(BSE) can be used to constrain the amount of materials delivered to Earth during the late veneer accretion stage. Because the Tl concentration in BSE is very low and its Tl isotope composition is similar with that of chondrites, suggesting either no Tl isotope fractionation occurred during numerous evaporation events, or the Tl in current BSE was totally delivered by late veneer. If it is the latter, the Tl-contentbased estimation could challenge the magnitude of late veneer which had been constrained by the amount of highly siderophile elements in BSE. Our results show that the lateaccreted mass is at least five-times larger than the previously suggested magnitude, i.e., 0.5 wt% of current Earth’s mass. The slightly lighter 205 Tl composition of BSE relative to chondrites is probable a sign of occurrence of Tlbearing sulfides, which probably were removed from the mantle in the last accretion stage of the Earth.

Non-asymptotic Error Bound for Optimal Prediction of Function-on-Function Regression by RKHS Approach

In this paper,we study and analyze the regularized least squares for function-on-function regression model.In our model,both the predictors(input data)and responses(output data)are multivariate functions(with d variables andd variables respectively),and the model coefficient lies in a reproducing kernel Hilbert space(RKHS).We show under mild condition on the reproducing kernel and input data statistics that the convergence rate of excess prediction risk by the regularized least squares is minimax optimal.Numerical examples based on medical image analysis and atmospheric point spread function estimation are considered and tested,and the results demonstrate that the performance of the proposed model is comparable with that of other testing methods.

Analytical modeling of complex contact behavior between rock mass and lining structure

Based on the nondestructive test data of operating railway tunnels in China, this paper summarizes the basic characteristics of the complex contact behavior between the rock mass and lining structure. The contact modes are classified into dense contact, local non-contact, and loose contact. Subsequently, the corresponding mechanical model for each contact mode is developed according to its mechanical characteristics using the complex variable method. In the proposed mechanical model, a special algorithm is introduced to detect whether the local non-contact zone is re-contacted. Besides, a novel conformal mapping method is also proposed to accurately calculate the mechanical response of the concrete lining. Finally, the accuracy of the proposed method is verified by comparing it with the finite element method(FEM). Several parameter investigations are conducted to analyze the effects of different contact modes on the rock-lining interaction. The results show that:(i) the height of the local noncontact area does not have a significant effect on the contact stress distribution if no re-contact occurs;(ii) backfill grouting can reduce the local stress concentration caused by poor contact modes;and(iii) reducing the friction coefficient of the interface can lead to a more uniform distribution of internal forces in the concrete lining.

Effectiveness of acupuncture-related therapies on schizophrenia:a Bayesian network Meta-analysis

OBJECTIVE:To create the hierarchical model for the comparison of efficacy of different ATs for schizophrenia.METHODS:PubM ed,Web of Science,Embase,The Cochrane Library,ClinicalTrials,China National Knowledge Infrastructure Database,China Science and Technology Journal Database,Wanfang Database,and SinoM ed were searched using a specified search strategy to identify relevant studies up to December 2021.The data were extracted independently by two reviewers.The quality of included trials was evaluated based on the guidelines of“Cochrane Handbook for Systematic Reviews of Interventions”.Bayesian network metaanalysis was conducted by statistical analysis software Addis 1.16.6 and Stata 15.1.RESULTS:In total,60 randomized controlled trials covering 4810 patients were enrolled.The network metaanalysis result showed that Body Acupuncture(BA),BA+Electro-acupuncture(EA),Scalp Acupuncture(SA)+EA,Auricular Acupuncture(AA),Low-dose medication and Acupuncture(LA),Acupoint Injection(AI),and Acupoint Catgut Embedding(ACE),when combined with Western Medications(WM),demonstrated a better clinical effect at improving the symptoms of schizophrenia,compared to WM alone.Results of rank probability showed that BA,when combined with WM,was the most optimal AT for schizophrenia at decreasing three aspects of PANSS scale score.CONCLUSIONS:Acupuncture-related therapies help improve the symptoms of schizophrenia,and BA combined with WM may be a better therapy for schizophrenia.

Relationship of nocturnal concentrations of melatonin, gamma-aminobutyric acid and total antioxidants in peripheral blood with insomnia after stroke： study protocol for a prospective non-randomized controlled trial

Melatonin and gamma-aminobutyric acid（GABA） have been shown to regulate sleep. The nocturnal concentrations of melatonin, GABA and total antioxidants may relate to insomnia in stroke patients. In this prospective single-center non-randomized controlled clinical trial performed in the China Rehabilitation Research Center, we analyzed the relationship of nocturnal concentrations of melatonin, GABA and total antioxidants with insomnia after stroke. Patients during rehabilitation of stroke were recruited and assigned to the insomnia group or non-insomnia group. Simultaneously, persons without stroke or insomnia served as normal controls. Each group contained 25 cases. The primary outcome was nocturnal concentrations of melatonin, GABA and total antioxidants in peripheral blood. The secondary outcomes were Pittsburgh Sleep Quality Index, Insomnia Severity Index, Epworth Sleepiness Scale, Fatigue Severity Scale, Morningness-Eveningness Questionnaire（Chinese version）, and National Institute of Health Stroke Scale. The relationship of nocturnal concentrations of melatonin, GABA and total antioxidants with insomnia after stroke was analyzed and showed that they were lower in the insomnia group than in the non-insomnia group. The severity of stroke was higher in the insomnia group than in the non-insomnia group. Correlation analysis demonstrated that the nocturnal concentrations of melatonin and GABA were associated with insomnia after stroke. Output Tracking Control of a Hydrogen-air PEM Fuel Cell

Hydrogen-air proton exchange membrane(PEM)fuel cell is a promising clean energy. However, the stack output tracking control is still a challenging problem due to the soft characteristic of the stack. Both over-and less-control will cause the stack flooding or oxygen lacking which dramatically decreases the life of stacks. Traditional control methods rely on the accurate model of the fuel cell system, which is a high-order nonlinear system, and involve a complex controller design process. This paper combines the data-based fuzzy cluster modeling technology with the sliding mode control and the integral actions. The sliding mode controller tracks the dynamic changes of the fuel cell system and the integral controller eliminates the steady-state errors. Simulation results demonstrate good performance of the proposed control method.

Efficacy and Duration of Electro-Acupuncture Combined with Conventional Antipsychotics for Schizophrenia:A Meta-Analysis

Introduction:This study aimed to evaluate the effectiveness and optimal course time of electro-acupuncture(EA)combined with conventional antipsychotics in the treatment of schizophrenia(SZ)and to provide a basis for its clinical application.Methods:Relevant databases and authoritative websites were retrieved,and references were screened up to May 2021.Literature quality evaluation and data extraction were carried out according to the requirements of Cochrane version 5.1.0,and meta-analysis was conducted using Rav Man5.4 software.Results:In total,16 randomized controlled trails comprising 1352 patients were selected.Meta-analysis revealed that the observation group showed greater improvements than the control group in total clinical efficacy(P0.05).In the 4-6 weeks following treatment,a significant improvement in all the four components of the PANSS scores was observed in the observation group.Conclusion:The efficacy of EA combined with conventional antipsychotics for SZ is better than that of psychiatric drugs alone.In addition,the effect of a 4-to 6-week treatment course on each outcome index is more significant,and the efficacy is higher.
