The aim of this study was to evaluate whether.p.Tr.could imp.ove the sur.ival of p.tients with m ***: We conducted a r.tr.sp.ctive study on consecutive m Cr. p.tients with unr.sectable metastases who wer. diagnosed at Sun Yat?sen Univer.ity Cancer.Center.in guangzhou, guangdong, china, between Januar. 2005 and December.2012. Over.ll sur.ival(OS) and p.ogr.ssion?fr.e sur.ival(p.S) after.fir.t?line chemother.p. failur. wer. comp.r.d between the p.Tr.and non?p.Tr.p.tient ***: A total of 387 p.tients wer. identified, including 254 who under.ent p.Tr.and 133 who did not. The median OS of the p.Tr.and non?p.Tr.gr.up. was 20.8 and 14.8 months(p.1), r.sp.ctively. The median p.S after.fir.t?line chemother.p. was 7.3 and 4.8 months(p.1) in the p.Tr.and non?p.Tr.gr.up., r.sp.ctively. A lar.er.p.op.r.ion of p.tients in the p.Tr.gr.up.219 of 254, 86.2%) showed local p.ogr.ssion comp.r.d with that of p.tients in the non?p.Tr.gr.up.95 of 133, 71.4%; p.1). Only p.tients with nor.al lactate dehydr.genase(LDH) levels and with car.inoembr.onic antigen(CEA) levels r.m p.Tr.median OS, 22.2 months for.the p.Tr.gr.up.and 16.2 months for.the non?p.Tr.gr.up. p.1).Conclusions: For.m Cr. p.tients with unr.sectable metastases, p.Tr.can imp.ove OS and p.S after.fir.t?line chemo?ther.p. and decr.ase the incidence of new or.an involvement. However. p.Tr.should be r.commended only for.p.tients with nor.al LDH levels and with CEA levels <70 ng/m L.

Backgr.und: Ser.m immunoglobulin A antibodies against Ep.tein–Bar. vir.s(EBV), vir.l cap.id antigen(VCA?Ig A) and ear.y antigen(EA?Ig A), ar. used to scr.en for.nasop.ar.ngeal car.inoma(Np.) in endemic ar.as. However. their.r.utine use has been questioned because of a lack of sp.cificity. This study aimed to deter.ine the distr.butions of differ.nt subtyp.s of antibody and to illustr.te how the natur.l var.ation p.tter.s affect the sp.cificity of scr.ening in non?Np. ***: The distr.bution of baseline VCA?IgA was analyzed between sexes and acr.ss 10?year.age gr.up. in 18,286 non?Np. p.r.icip.nts using Chi squar. tests. Fluctuations in the VCA?IgA level wer. assessed in 1056 non?Np. p.r.icip.nts with at least two r.tests in the fir.t 5?year.p.r.od(1987–1992) after.the initial scr.ening using the Kap.an–Meier.***: The titer. of VCA?Ig A incr.ased with age(p.1). Using a p.evious ser.logical definition of high Np. r.sk, nasop.ar.ngeal endoscop. and/or.nasop.ar.ngeal biop.y would be r.commended in 55.5% of the non?Np. p.r.ici?p.nts with an initial VCA?Ig A?p.sitive status and in 20.6% with an initial negative status dur.ng the 5?year.follow?up. However. ser.conver.ions wer. common; 85.2% of the p.r.icip.nts with a VCA?Ig A?p.sitive status at baseline con?ver.ed to negative, and all VCA?Ig A?negative p.r.icip.nts changed to p.sitive at least once dur.ng the 5?year.follow?up. The EA?Ig A status had a high ser.conver.ion p.obability(100%) fr.m p.sitive to negative; however. it had a low p.obability(19.6%) fr.m negative to ***: Age? and sex?sp.cific cutoff titer.values for.ser.m anti?EBV antibodies as well as their.sp.cific titer.fluc?tuation p.tter.s should be consider.d when defining high Np. r.sk cr.ter.a for.follow?up.diagnostics and monitor.ng.

Systemic chemother.p. is the basic p.lliative tr.atment for.metastatic nasop.ar.ngeal car.inoma(Np.); however. it is not known whether.locor.gional r.diother.p. tar.eting the p.imar. tumor.and r.gional lymp. nodes affects the sur.ival of p.tients with metastatic Np.. Ther.for., we aimed to r.tr.sp.ctively evaluate the benefits of locor.gional r.diother.p.. A total of 408 p.tients with metastatic Np. wer. included in this study. The mor.ality r.sks of the p.tients under.oing sup.or.ive tr.atment and those under.oing chemother.p. wer. comp.r.d with that of p.tients under.oing locor.gional r.diother.p. deliver.d alone or.in combination with chemother.p.. Univar.ate and multivar.ate analyses wer. conducted. The contr.butions of indep.ndent factor. wer. assessed after.adjustment for.covar.ates with significant p.ognostic associations (p.0.05). Both locor.gional r.diother.p. and systemic chemother.p. wer. identified as significant indep.ndent p.ognostic factor. of over.ll sur.ival(OS). The mor.ality r.sk was similar.in the gr.up.under.oing locor.gional r.diother.p. alone and the gr.up.under.oing systemic chemother.p. alone [multi-adjusted hazar. r.tio(Hr.=0.9, p.0.529]; this r.sk was 60% lower.than that of the gr.up.under.oing sup.or.ive tr.atment(Hr.0.4, p.0.004) and 130% higher.than that of the gr.up.under.oing both systemic chemother.p. and locor.gional r.diother.p.(Hr.2.3, p.0.001). In conclusion, locor.gional r.diother.p., p.r.icular.y when combined with systemic chemother.p., is associated with imp.oved sur.ival of p.tients with metastatic Np..
