Ischemic colitis and large bowel infarction:A case report

Ischemic bowel disease results from an acute or chronic drop in the blood supply to the bowel and may have various clinical presentations, such as intestinal angina, ischemic colitis or intestinal infarction. Elderly patients with systemic atherosclerosis who are symptomatic for the disease in two or more vascular beds have multiple comorbidities and are particularly at risk. The clinical evolution and outcome of this disease are difficult to predict because of its pleomorphic aspects and the general lack of statistical data. In this paper, we present the case of a patient who was monitored in our unit for six years. For this patient, we encountered iterative changes in the clinical pattern, beginning with chronic "intestinal angina" and finishing with signs of acute mesenteric ischemia after an episode of ischemic colitis. This evolution is particularly rare in clinical practice, and the case is instructive because it raises discussions about the natural history of the condition and the thera-peutic decisions that should be made at every stage of the disease. An important lesson is that ischemic bowel disease should always be considered in patients who have multiple risk factors for atherosclerosis and have experienced recurrent "indistinct" abdominal *** these cases, aggressive investigation and therapeutic decisions must be taken whenever possible. Despite an absence of standardized protocols, angiographic evaluation and revascularization procedures have beneficial outcomes. Current advances in endovascular therapy, such as percutaneous transluminal angioplasty with stenting, should be increasingly used in patients with chronic mesenteric ischemia. Such therapy can avoid the risks that are associated with open repair. However, technical difficulties, especially in severe stenotic lesions, frequently occur.

Soil respiration under three different land use types in a tropical mountain region of China

Soil respiration (SR) Wis one of the largest contributors of terrestrial CO_2 to the *** as well as physicochemical parameters influence SR and thus, different land use practices impact the emissions of soil CO_2. In this study, we measured SR, bi-monthly, over a one-year period in a terrace tea plantation, a forest tea plantation and a secondary forest, in a subtropical mountain area in Xishuangbanna, China. Along with the measurement of SR rates, soil characteristics for each of the land use systems were investigated. Soil respiration rates in the different land use systems did not differ significantly during the dry season, ranging from2.7±0.2 μmol m^(-2) s^(-1) to 2.8±0.2 μmol m^(-2) s^(-1). During the wet season, however, SR rates were significantly larger in the terrace tea plantation(5.4±0.5 μmol m^(-2)s^(-1)) and secondary forest(4.9±0.4 μmol m^(-2)s^(-1)) than in the forest tea plantation(3.7±0.2 μmol m^(-2) s^(-1)).This resulted in significantly larger annual soil CO_2 emissions from the terrace tea and secondary forest,than from the forest tea plantation. It is likely that these differences in the SR rates are due to the 0.5times lower soil organic carbon concentrations in thetop mineral soil in the forest tea plantation, compared to the terrace tea plantation and secondary ***, we suggest that the lower sensitivity to temperature variation in the forest tea soil is a result of the lower soil organic carbon concentrations. The higher SR rates in the terrace tea plantation were partly due to weeding events, which caused CO_2 emission peaks that contributed almost 10% to the annual CO_2 flux. Our findings suggest that moving away from heavily managed tea plantations towards low-input forest tea can reduce the soil CO_2 emissions from these systems. However, our study is a casestudy and further investigations and upscaling are necessary to show if these findings hold true at a landscape level.

Inflammatory model in patients with primary open angle glaucoma and diabetes

AIM: To assess the inflammatory cytokines expression in aqueous humor in diabetic primary open angle glaucoma(POAG) patients. METHODS: A cross-sectional study on 87 eyes, distributed as following: 26 eyes from diabetic patients, 16 eyes with POAG and 21 eyes from diabetic POAG patients; healthy controls(24 eyes) were recruited from patients undergoing conventional cataract surgery. A volume of 100 μL of aqueous humor(AH) was collected during phacoemulsification and 21 inflammatory markers were quantified using a Luminex~? cytometric bead assay: IL-1 Ra, IL-1α, IL-1β, IL-5, IL-6, IL-10, IL-17, GM-CSF, IFNγ, CCL2, CCL3, CCL4, CXCL5, CXCL8, bFGF, VEGF, TNFα. Main changes in cytokine profile were analyzed and compared between groups. Data on demographics, duration of glaucoma, intraocular pressure(IOP), number of anti-glaucoma substances were recorded for correlation analysis and prediction models. RESULTS: Significant differences in cytokine expression between groups were detected for CXCL5(P<0.001), CXCL8(P=0.004), IL-1α(P<0.001), IL-2(P<0.001), CCL4(P=0.003), CCL5(P<0.001) and TNFα(P=0.05). Post-hoc analysis identified IL-2(P=0.009) and CXCL5(P<0.001) as "separation markers" between POAG and diabetic POAG eyes. In POAG patients, the "separation markers" could highly predict the TNFα levels F(1, 16)=14.639, P<0.001, whereas in diabetic patients F(1, 24)=4.844, P=0.006 and diabetic POAG patients F(1, 19)=2.358, P=0.05 the level of prediction was ***: Our results reveal an inflammatory model based on increased TNFα levels in POAG *** co-stimulatory molecules and additional inflammatory pathways need to be further explored in diabetic POAG cases, since the prediction model could only partially explain the increased TNFα level in this category of patients.

In light of the rising evidence of the association between viral and bacterial infections and neurodegeneration,we aimed at revisiting the infectious hypothesis of Alzheimer’s disease and analyzing the possible implications of COVID-19 neurological sequelae in long-term *** wondered how SARS-CoV-2 could be related to the amyloid-βcascade and how it could lead to the pathological hallmarks of the *** also predict a paradigm change in clinical medicine,which now has a great opportunity to conduct prospective surveillance of cognitive sequelae and progression to dementia in people who suffered severe infections together with other risk factors for Alzheimer’s disease.

Value of histomorphometric tumour thickness and smoothelin for conventional m-classification in early oesophageal adenocarcinoma

AIM To test the validity of tumour thickness measurement in distinguishing between the different infiltration depths, especially when the duplication of muscularis mucosae cannot be demarcated clearly. METHODS We re-evaluated 100 completely embedded Barrett's adenocarcinomas regarding m-classification, maximum tumour thickness, and muscularis mucosae duplication. For validation, smoothelin staining was performed on a subset of cases. RESULTS The m1-, m2-and m3-classified adenocarcinomasshowed a significant lower tumour thickness compared to the m4-and sm1-classified lesions(P < 0.001). Smoothelin staining determined a clear muscularis mucosae duplication in 64% of the tested samples and enabled the differentiation of the two layers in diffuse and merged splits. CONCLUSION Tumour thickness in early oesophageal adenocarcinoma significantly correlates with the depth of infiltration and demonstrates its worth as an accurate p T classification in non-polypoid lesions. We created a new algorithm, which combines histomorphology with morphometric analyses. It is noteworthy that it facilitates the assessment of mucosal vs submucosal infiltration depth. The smoothelin staining strengthened our results of the tumour thickness evaluation and can be used in cases of doubt.

Molecular signaling involving intrinsically disordered proteins in prostate cancer

Investigations on cellular protein interaction networks （PINs） reveal that proteins that constitute hubs in a PIN are notably enriched in Intrinsically Disordered Proteins （IDPs） compared to proteins that constitute edges, highlighting the role of IDPs in signaling pathways. Most IDPs rapidly undergo disorder-to-order transitions upon binding to their biological targets to perform their function. Conformational dynamics enables IDPs to be versatile and to interact with a broad range of interactors under normal physiological conditions where their expression is tightly modulated. IDPs are involved in many cellular processes such as cellular signaling, transcriptional regulation, and splicing; thus, their high-specificity/low-affinity interactions play crucial roles in many human diseases including cancer. Prostate cancer （PCa） is one of the leading causes of cancer-related mortality in men worldwide. Therefore, identifying molecular mechanisms of the oncogenic signaling pathways that are involved in prostate carcinogenesis is crucial. In this review, we focus on the aspects of cellular pathways leading to PCa in which IDPs exert a Iorimary role.

Design and Simulation of a Novel Pneumotronic System Aimed to the Investigation of Vascular Phenomena Induced by Limb Compression

Intermittent Pneumatic Compression (IPC) devices can be used to analyze the mechanisms underlying several vascular phenomena, such as hyperaemia. Commercial devices have limited dynamics and do not allow the delivery of customizable compressive pressure patterns, making the analysis of such phenomena difficult, which may require the application of long stimulations with low amplitude as well as fast compressions with higher pressure level. To overcome these issues, a novel pneumotronic device aimed to the investigation of the physiological effects induced by limb compressions is conceived and presented in this work. The design requirements of the system, capable of delivering customizable compressive patterns in the range 0 mmHg-200 mmHg, are outlined. The final prototype architecture is described, and a mathematical model of the entire system, also including the interaction between the device and the limb tissues, is proposed. The performance of the device was evaluated in several conditions by means of simulations, whose results were compared to the data collected from experimental trials in order to validate the model. The outcomes of both experimentation and simulation trials proved the effectiveness of the solution proposed. A possible employment of this device for the investigation of the rapid compression-induced hyperaemia is presented. Other potential applications concern the wide range of intermittent-pneumatic compression treatments.

Assays for predicting and monitoring responses to lung cancer immunotherapy

Immunotherapy has become a key strategy for cancer treatment, and two immune checkpoints, namely, programmed cell death 1 （PD-1） and its ligand （PD-L1）, have recently emerged as important targets. The interaction blockade of PD-1 and PD-L1 demonstrated promising activity and antitumor efficacy in early phase clinical trials for advanced solid tumors such as non-small cell lung cancer （NSCLC）. Many cell types in multiple tissues express PD-L1 as well as several tumor types, thereby suggesting that the ligand may play important roles in inhibiting immune responses throughout the body. Therefore, PD-L1 is a critical immunomodulating component within the lung microenvironment, but the correlation between PD-L1 expression and prognosis is controversial. More evidence is required to support the use of PD-L1 as a potential predictive biomarker. Clinical trials have measured PD-L1 in tumor tissues by immunohistochemistry （IHC） with different antibodies, but the assessment of PD-L1 is not yet standardized. Some commercial antibodies lack specificity and their reproducibility has not been fully evaluated. Further studies are required to clarify the optimal IHC assay as well as to predict and monitor the immune responses of the PD-I/PD-L1 pathway.
