Clinical retrospective analysis of 15 cases of choanal atresia-Our experience

Background and objectives:An auricular pseudocyst is not uncommon in routine ENT clinical practice,it occurs when fluid accumulates between the intracartilaginous spaces of the *** treatment modalities have been proposed such as incision and drainage of the cyst,simple needle aspiration,tight bandaging with dental rolls,however recurrence and cosmetic problems are still noted in some *** aim of this article was to discuss our experience of surgical treatment of intractable auricular pseudocysts with marsupialisation,deroofing and anterior cartilage leaflet removal along with compression suture *** and methods:Twenty patients were included in the study conducted at ENT department,Sur Ministry of Health Hospital between January 2012 and January 2014 after prior consent and ethical *** following trauma and other pinna conditions like relapsing polychondritis were excluded from our *** clinical appearances were noted and all patients underwent surgical deroofing with removal of anterior cartilage leaflet and compression suture therapy using buttons for two *** and observations:There were 8 males and 12 females out of the 20 and right sided pinna(n Z 14)involvement in the region of the scaphoid fossa(n Z 12)was more than the triangular fossa(n Z 3)or conchal bowl involvement(n Z 5).Mostly patients between 30 and 40 years of age were affected(Mean age of 37 years and standard deviation of 8).The overall success rate with deroofing and compression suture therapy was 98%.Conclusions:Auricular pseudocysts are not an uncommon condition affecting middle aged patients without identifiable *** modalities may be the first choice of treatment for auricular pseudocysts although varied recurrence and failure rates have been published in the ***,the deroofing surgical technique with anterior cartilage leaflet removal with compression suture therapy is a reliable and easy procedure which can achie

Tracheostomy colonisation and microbiological isolates of patients in intensive care units-a retrospective study

Objective: To find out the type of bacteria colonising the tracheostomy tube and to determine the antibiotic sensitivity pattern and resistance in patients who have had tracheostomy in intensive care unit (ICU) set up and to initiate proper empirical treatment in such patients. Methods: The study was a retrospective review of patients who underwent tracheostomy at Ministry of Health, Sur Hospital, Oman January 2005 and December 2015. The Hospital has 4 bedded pediatric intensive care unit (PICU) and 10 bedded adult ICU which is headed by consultant anaesthetists, consultant physicians, ICU trained nurses and respiratory therapists. All patients who required mechanical ventilation and were therefore subject to an orotracheal intubation and those who underwent a conventional tracheostomy were considered for inclusion. Patients who had been intubated in other hospitals or ICUs, other airways infection issues were excluded from this study. Data’s were collected from computer based hospital management system, operation theatre and registers in Medical records department and entered in a preformed questionnaire before thorough analysis. The specimens for swab was obtained from the cut tracheostomy tube tips and the samples were sent to microbiology laboratory for isolation of the organism and obtain an antibiogram to know the susceptibility and resistance to antibiotics. Data were analyzed by Statistical Package for Social Sciences (SPSS, version 16, Chicago, Inc) and the values are reported as number (%). The commonest isolated organism wasPseudomonas followed byAcinetobacter. Results: During the ten year retrospective study, there were 108 patients included in our study with 56 males and 51 females. Fourteen different microorganisms were isolated during our study which includedPseudomonas aeruginosa (n= 39),Acinetobacter baumanii (n= 28),Klebsiella (n= 10) and coagulase negative staphylococcus (n= 6). BACKGROUND In severe cases of coronary artery disease,percutaneous coronary intervention provide promising *** stent used could be a drug-eluting stent(DES)or a titanium-nitride-oxide coated stent(TiNOS).AIM To compare the 5-year effectiveness and safety of the two stent *** The following systematic review and meta-analysis was conducted in accordance with the preferred reporting items for systematic reviews and meta-analysis guidelines,and PubMed/MEDLINE,Scopus,and Cochrane Central were searched from inception till August *** outcomes were major adverse cardiac events(MACE),cardiac death,myocardial infarction(MI),cardiac death or MI,and ischemia-driven total lesion revascularization(ID-TLR).RESULTS Four randomized controlled trials(RCT),which analyzed a sum total of 3045 patients with acute coronary syndrome(ACS)after a median follow-up time of 5 years were *** statistically insignificant,an increase in the ID-TLR was observed in patients receiving TiNOSs vs *** addition,MI,cardiac death and MI,and definite stent thrombosis(DST)were significantly decreased in the TiNOS *** analysis revealed no significant results with meta-regression presenting non-ST elevated MI(NSTEMI)as a statistically significant covariate in the outcome of *** TiNOS was found to be superior to DES in terms of MI,cardiac death or MI,and DST outcomes,however,the effect of the two stent types on ID-TLR and MACE was not significant.A greater number of studies are required to establish an accurate comparison of patient outcomes in TiNOS and DES.

Adaptive Density-Based Spatial Clustering of Applications with Noise(ADBSCAN)for Clusters of Different Densities

Finding clusters based on density represents a significant class of clustering *** methods can discover clusters of various shapes and *** most studied algorithm in this class is theDensity-Based Spatial Clustering of Applications with Noise(DBSCAN).It identifies clusters by grouping the densely connected objects into one group and discarding the noise *** requires two input parameters:epsilon(fixed neighborhood radius)and MinPts(the lowest number of objects in epsilon).However,it can’t handle clusters of various densities since it uses a global value for *** article proposes an adaptation of the DBSCAN method so it can discover clusters of varied densities besides reducing the required number of input parameters to only *** user input in the proposed method is the *** on the other hand,is computed automatically based on statistical information of the *** proposed method finds the core distance for each object in the dataset,takes the average of these distances as the first value of epsilon,and finds the clusters satisfying this density *** remaining unclustered objects will be clustered using a new value of epsilon that equals the average core distances of unclustered *** process continues until all objects have been clustered or the remaining unclustered objects are less than 0.006 of the dataset’s *** proposed method requires MinPts only as an input parameter because epsilon is computed from *** datasets were used to evaluate the effectiveness of the proposed method that produced promising *** experiments demonstrate that the outstanding ability of the proposed method to detect clusters of different densities even if there is no separation between *** accuracy of the method ranges from 92%to 100%for the experimented datasets.

Alzheimer’s disease(AD)is a neurodevelopmental impairment that results in a person’s behavior,thinking,and memory *** common symptoms ofADare losingmemory and early *** addition to these,there are several serious impacts ***,the impact ofADcanbemitigatedby early-stagedetection though it cannot be cured ***-stage detection is the most challenging task for controlling and mitigating the impact of *** study proposes a predictive model to detect AD in the initial phase based on machine learning and a deep learning approach to address the *** build a predictive model,open-source data was collected where five stages of images of AD were available as Cognitive Normal(CN),Early Mild Cognitive Impairment(EMCI),Mild Cognitive Impairment(MCI),Late Mild Cognitive Impairment(LMCI),and *** stage of AD is considered as a class,and then the dataset was divided into three parts binary class,three class,and five *** this research,we applied different preprocessing steps with augmentation techniques to efficiently *** integrates a random oversampling technique to handle the imbalance problem from target classes,mitigating the model overfitting and *** three machine learning classifiers,such as random forest(RF),K-Nearest neighbor(KNN),and support vector machine(SVM),and two deep learning methods,such as convolutional neuronal network(CNN)and artificial neural network(ANN)were applied on these *** analyzing the performance of the used models and the datasets,it is found that CNN with binary class outperformed 88.20%*** result of the study indicates that the model is highly potential to detect AD in the initial phase.
