Percutaneous devices for left atrial appendage occlusion: A contemporary review

Patient with atrial fibrillation(AF) are at risk of developing stroke with the left atrial appendage(LAA) being the most common site for thrombus formation. If left untreated, AF is associated with 4 to 5 folds increase in the risk of ischemic stroke in all age groups. About 5% to 15% of AF patients have atrial thrombi on transesophageal echocardiography, and 91% of those thrombi are located in the LAA in patient with nonrheumatic AF. Although oral anticoagulants are the gold-standard treatment for stroke prevention in patients with non-valvular AF,some patients are at high risk of bleeding and deemed not candidates for anticoagulation. Therefore, LAA occlusion(LAAO) has emerged as alternative approach for stroke prevention in those patients. Surgical LAAO is associated with high rate of unsuccessful closure and recommended only in patients with AF and undergoing cardiac surgery. Percutaneous LAAO uses transvenous access with trans-septal puncture and was first tested using the PLAATO *** is the most common and only Food and Drug Administration(FDA)approved device for LAAO. LAAO using Watchman device is non-inferior to warfarin therapy in preventing ischemic stroke/systemic ***, it is associated with lower rates of hemorrhagic stroke, bleeding and death. Amplatzer is another successful LAAO device that has CE mark and is waiting for FDA approval. Optimal antithrombotic therapy post LAAO is still under debate and highly patient-specific. The aim of this paper is to systematically review the current literature to evaluate the efficacy and safety of different LAAO devices.

Deposition of In2-xFexO3 Films by Ultrafast Microwave Heating Technique

In this paper, we present an ultra fast deposition of oxide films using microwave heating. Using SiC as a susceptor, InFeO powder was placed on top of it in a microwave heating chamber. By heating the SiC substrate to 2,100 °C using microwaves and within 3-4 seconds, the InFeO film was deposited on Si substrate placed right at the top of the chamber. The X-ray diffraction scans taken of the target and the film indicated a one-to-one correspondence with the target material and the deposited film in terms of stoichiometry.

Suppression Effect of Superoxide Dismutase (SOD)-Like Activity Protein Partially Purified from Raphnus sativus Leaves against Liver Metastasis in Mice Intraperitoneally Infected with Ehrlich Ascites Carcinoma Cell

Liver metastasis is a normal result in Ehrlich Ascites Carcinoma Cell (EAC) infected mice. A partially purified protein with Superoxide Dismutase (SOD)-like activity extracted from leaves of Raphnus sativus was tested for its ability to down regulate liver metastasis of Ehrlich Ascites Carcinoma Cell (EAC) intraperitoneally implanted in albino mice. Sixty albino mice were divided equally into 4 groups. Group I mice (G1 control) were treated with physiologic saline solution for 7 days. Group 2 mice (G2) were injected with the partially purified protein. Group 3 mice (G3) were intraperitoneally inoculated with EAC cells and treated with the partially purified protein. Group 4 mice (G4) were inoculated EAC cells and treated with physiologic saline solution. The results showed a disturbance in liver functions. This disturbance was observed as a reduction in the values of serum albumin, hemoglobin, total counts of erythrocytes and platelets, total lipids in liver tissues, erythrocytes SOD activity, serum glucose, reduced glutathione (GSH) and the trace elements (Fe, Mn, Zn, and Cu) contents in liver and spleen tissues as well as the elevation of serum transaminases activities, the levels of malondialdehyde in blood, DNA and RNA of liver tissues in the infected mice (G4). On the other hand, in G3, after treatment with the partial purified protein all these parameters restored their normal values and became near from the normal group (GI). Moreover, the protein showed no toxicity towards the mice as shown in G2. It could be concluded that a partially purified protein with Superoxide Dismutase (SOD)-like activity extracted from leaves of Raphnus sativus as a nature source has a promised future in controlling liver metastasis.

Frequency of Epidermal Growth Factor Mutation Status and Its Effect on Outcome of Patients with Adenocarcinoma of the Lung

he frequency of EGFR mutations is ethnicity-dependent, with a higher proportion in Asian population than in whites. The prevalence of these mutations among Arab patients is unknown. The objective of this study was to report the frequency and spectrum of EGFR mutations in our population with lung adenocarcinoma, and the effect of this mutation on treatment outcome. Tumor specimens from 81 patients histological diagnosed as NSCLC adenocarcinoma were reviewed by our local pathologist and then sent to Lab 21 London, UK. Samples were tested for 28 mutations in EGFR gene. Positive patients received TKI and negative patients received chemotherapy. Data were collected and retrospectively analyzed to determine frequency and spectrum of EGFR positivity. Patients were followed for time to progression (TTP) and overall survival (OS). Overall frequency of EGFR mutation was 37%;Del 19 (46.6%), L858R (40%), D719x and insertion 20 (6.7%) for each. Females and nonsmokers exhibited a statistically significant higher EGFR positivity (P = 0.003, 0.059) respectively. Overall response rate (ORR) was 76.6% and 43% in EGFR positive and negative cases respectively (P = 0.002). There was a statistically significant difference in TTP and OS between EGFR positive and negative patients (P = 0.035 and 0.039 respectively). Arab patients exhibit an EGFR mutation pattern that is closer to Asian population. EGFR gene mutation subtypes are the same as that reported worldwide. A statistically significant TTP and OS benefit was noticed in EGFR positive patients compared to EGFR negative cases.
