Since the outbreak of coronavirus disease 2019(COVID-19)in December 2019 in China,various measures have been adopted in order to attenuate the impact of the virus on the population.With regard to spine surgery,French physicians are devoted to take place in the national plan against COVID-19,the French Spine Surgery Society therefore decided to elaborate specific guidelines for management of spinal disorders during COVID-19 pandemic in order to prioritize management of patients.A three levels stratification was elaborated with Level I:Urgent surgical indications,Level II:Surgical indications associated to a potential loss of chance for the patient and Level III:Non-urgent surgical indications.We also report French experience in a COVID-19 cluster region illustrated by two clinical cases.We hope that the guidelines formulated by the French Spine Surgery Society and the experience of spine surgeons from a cluster region will be helpful in order optimizing the management of patients with urgent spinal conditions during the pandemic.

Types of Irrigation Water and Soil Amendment Affect the Growth and Flowering of Petunia x alkinsiana ‘Bravo Pinc’

Water insufficiency is the hampering feature of crop sustainability,especially in arid and semi-arid regions.So,the effectual usage of all water resources especially underground brackish water represents the core priority in Saudi Arabia.The present study aimed to recognize the influence of different types of water irrigation(tap water as a control,salinized well water,and magnetized salinized well water)with or without soil amendments(soil without any amendment as a control,peat-moss,ferrous sulfate,and peat-moss plus ferrous sulfate)on petunia plant growth and flowering as well as ion content.Irrigating Petunia plants with saline well water adversely affected growth and flowering as compared to tap water and magnetized saline well water.Additionally,plants irrigated with magnetized water showed a significant enhancement in all the studied vegetative and flowering growth parameters as compared to those irrigated with salinized well water.Furthermore,mineral contents and survival of Petunia plants irrigated with magnetized well water were higher than those irrigated with tap water.Irrigation with magnetized well water significantly reduced levels of Na+and Cl−ions in leaves of Petunia plants indicating the role of magnetization in alleviating harmful effects of salinity.In conclusion,we recommend the utilization of magnetized saline well water for irrigating Petunia plants either alone or in combination with soil amendments(peat moss plus ferrous sulfate).

Effect of Spraying with GA<sub>3</sub>on the Quality and Oil Content of Syrian Okra Seeds <i>Abelmoschus esculentus</i>(L.)

This study aimed to estimate the effect of spraying of okra plants, Abelmoschus esculentus (L.) with GA3 on the quality and oil content of the seeds. For this, the local okra variety was selected and used, which was growing widespread in the coastal region of Syria. This study was carried out in Al-Haffa area, Lattakia governorate, Syria (with 105 MSL elevation) for two seasons 2019-2020. The results showed that the effect of spraying with GA3 was more evident in the quality of the seeds as well as oil content at 75 ppm concentration with 68.7 g weight of 1000 seeds and 5.31% seed oil percentage. This study also showed that oil of local okra variety seed was rich in the essential fatty acids namely, linoleic, palmitic, oleic, stearic and linolenic, treatment with 75 ppm concentration of GA3 was more effective in the seed content of essential fatty acids with 42.30%, 32.55%, 9.20%, 6.73% and 5.72%, respectively. Finally, our results were indicated that treatment of okra plants at 75 ppm of GA3 enhanced the quality and oil content of seeds compared to the other concentrations.

Predicting long-term outcomes in acute intracerebral haemorrhage using delayed prognostication scores

Objective The concept of the‘self-fulfilling prophecy’is well established in intracerebral haemorrhage(ICH).The ability to improve prognostication and prediction of long-term outcomes during the first days of hospitalisation is important in guiding conversations around goals of care.We previously demonstrated that incorporating delayed imaging into various prognostication scores for ICH improves the predictive accuracy of 90-day mortality.However,delayed prognostication scores have not been used to predict long-term functional outcomes beyond 90 days.Design,setting and participants We analysed data from the ICH Deferoxamine trial to see if delaying the use of prognostication scores to 96 hours after ICH onset will improve performance to predict outcomes at 180 days.276 patients were included.Interventions and measurements We calculated the original ICH score(oICH),modified-ICH score(MICH),max-ICH score and the FUNC score on presentation(baseline),and on day 4(delayed).Outcomes assessed were mortality and poor functional outcome in survivors(defined as modified Rankin Scale of 4-5)at 180 days.We generated receiver operating characteristic curves,and measured the area under the curve values(AUC)for mortality and functional outcome.We compared baseline and delayed AUCs with non-parametric methods.Results At 180 days,21 of 276(7.6%)died.Out of the survivors,54 of 255 had poor functional outcome(21.2%).The oICH,MICH and max-ICH performed significantly better at predicting 180-day mortality when calculated 4 days later compared with their baseline equivalents((0.74 vs 0.83,p=0.005),(0.73 vs 0.80,p=0.036),(0.74 vs 0.83,p=0.008),respectively).The delayed calculation of these scores did not significantly improve our accuracy for predicting poor functional outcomes.Conclusion Delaying the calculation of prognostication scores in acute ICH until day 4 improved prediction of 6-month mortality but not functional outcomes.Trial registration number ClinicalTrials.gov Registry(NCT02175225).

Human Detection by Robotic Urban Search and Rescue Using Image Processing and Neural Networks

This paper proposes a new approach for detecting human survivors in destructed environments using an autonomous robot. The proposed system uses a passive infrared sensor to detect the existence of living humans and a low-cost camera to acquire snapshots of the scene. The images are fed into a feed-forward neural network, trained to detect the existence of a human body or part of it within an obstructed environment. This approach requires a relatively small number of images to be acquired and processed during the rescue operation, which considerably reduces the cost of image processing, data transmission, and power consumption. The results of the conducted experiments demonstrated that this system has the potential to achieve high performance in detecting living humans in obstructed environments relatively quickly and cost-effectively. The detection accuracy ranged between 79% and 91% depending on a number of factors such as the body position, the light intensity, and the relative color matching between the body and the surrounding environment.

Diagnosis and management challenges of in-stent restenosis in coronary arteries

Over the course of the 3 decades, percutaneous coronary intervention(PCI) with stent implantation transformed the practice of cardiology. PCI with stenting is currently the most widely performed procedure for the treatment of symptomatic coronary disease. In large trials, drugeluting stents(DES) have led to a significant reduction in in-stent restenosis(ISR) rates, one of the major limitations of bare-metal stents. Due to these favorable findings, DES was rapidly and widely adopted enabling more complex coronary interventions. Nevertheless, ISR remains a serious concern as late stent complications. ISR mainly results from aggressive neointimal proliferation and neoatherosclerosis. DES-ISR treatment continues to be challenging complications for interventional cardiologists.

Energy-Efficient and Coverage-Aware Clustering in Wireless Sensor Networks

Energy efficiency and sensing coverage are essential metrics for enhancing the lifetime and the utilization of wireless sensor networks. Many protocols have been developed to address these issues, among which, clustering is considered a key technique in minimizing the consumed energy. However, few clustering protocols address the sensing coverage metric. This paper proposes a general framework that addresses both metrics for clustering algorithms in wireless sensor networks. The proposed framework is based on applying the principles of Virtual Field Force on each cluster within the network in order to move the sensor nodes towards proper locations that maximize the sensing coverage and minimize the transmitted energy. Two types of virtual forces are used: an attractive force that moves the nodes towards the cluster head in order to reduce the energy used for communication and a repulsive force that moves the overlapping nodes away from each other such that their sensing coverage is maximized. The performance of the proposed mechanism was evaluated by applying it to the well-known LEACH clustering algorithm. The simulation results demonstrate that the proposed mechanism improves the performance of the LEACH protocol considerably in terms of the achieved sensing coverage, and the network lifetime.

Percutaneous devices for left atrial appendage occlusion: A contemporary review

Patient with atrial fibrillation(AF) are at risk of developing stroke with the left atrial appendage(LAA) being the most common site for thrombus formation. If left untreated, AF is associated with 4 to 5 folds increase in the risk of ischemic stroke in all age groups. About 5% to 15% of AF patients have atrial thrombi on transesophageal echocardiography, and 91% of those thrombi are located in the LAA in patient with nonrheumatic AF. Although oral anticoagulants are the gold-standard treatment for stroke prevention in patients with non-valvular AF,some patients are at high risk of bleeding and deemed not candidates for anticoagulation. Therefore, LAA occlusion(LAAO) has emerged as alternative approach for stroke prevention in those patients. Surgical LAAO is associated with high rate of unsuccessful closure and recommended only in patients with AF and undergoing cardiac surgery. Percutaneous LAAO uses transvenous access with trans-septal puncture and was first tested using the PLAATO device.Watchman is the most common and only Food and Drug Administration(FDA)approved device for LAAO. LAAO using Watchman device is non-inferior to warfarin therapy in preventing ischemic stroke/systemic thromboembolism.However, it is associated with lower rates of hemorrhagic stroke, bleeding and death. Amplatzer is another successful LAAO device that has CE mark and is waiting for FDA approval. Optimal antithrombotic therapy post LAAO is still under debate and highly patient-specific. The aim of this paper is to systematically review the current literature to evaluate the efficacy and safety of different LAAO devices.

Hepatitis C virus NS5A promotes insulin resistance through IRS-1 serine phosphorylation and increased gluconeogenesis

AIM: To investigate the mechanisms of insulin resistance in human hepatoma cells expressing hepatitis C virus(HCV) nonstructural protein 5A(NS5A).METHODS: The human hepatoma cell lines,Huh7 and Huh7.5,were infected with HCV or transientlytransfected with a vector expressing HCV NS5 A. The effect of HCV NS5 A on the status of the critical players involved in insulin signaling was analyzed using realtime quantitative polymerase chain reaction and Western blot assays. Data were analyzed using Graph Pad Prism version 5.0.RESULTS: To investigate the effect of insulin treatment on the players involved in insulin signaling pathway,we analyzed the status of insulin receptor substrate-1(IRS-1) phosphorylation in HCV infected cells or Huh7.5 cells transfected with an HCV NS5 A expression vector. Our results indicated that there was an increased phosphorylation of IRS-1(Ser307) in HCV infected or NS5 A transfected Huh7.5 cells compared to their respective controls. Furthermore,an increased phosphorylation of Akt(Ser473) was observed in HCV infected and NS5 A transfected cells compared to their mock infected cells. In contrast,we observed decreased phosphorylation of Akt Thr308 phosphorylation in HCV NS5 A transfected cells. These results suggest that Huh7.5 cells either infected with HCV or ectopically expressing HCV NS5 A alone have the potential to induce insulin resistance by the phosphorylation of IRS-1 at serine residue(Ser307) followed by decreased phosphorylation of Akt Thr308,Fox01 Ser256 and GSK3β Ser9,the downstream players of the insulin signalingpathway. Furthermore,increased expression of PECK and glucose-6-phosphatase,the molecules involved in gluconeogenesis,in HCV NS5 A transfected cells was observed.CONCLUSION: Taken together,our results suggest the role of HCV NS5 A in the induction of insulin resistance by modulating various cellular targets involved in the insulin signaling pathway.
