采用有机硅柔软剂对国产聚乙烯醇(PVA)纤维进行表面改性,并制备了纤维增强水泥基复合材料(PVA-ECC)。采用扫描电子显微镜研究了有机硅柔软剂改性对PVA纤维表面结构的影响,用三点弯曲试验研究了有机硅柔软剂改性的PVA纤维对PVA-ECC复合材料弯曲性能的影响。研究结果表明:随着有机硅柔软剂含量的增加,PVA-ECC的极限弯曲强度和极限跨中挠度均先增加再减小,当有机硅柔软剂质量分数为7%时,极限弯曲强度和极限跨中挠度达到最大值,分别为5.627 MPa和2.123 mm;用ASTM C1609标准分析PVA-ECC三点弯曲韧性,当有机硅柔软剂质量分数为7%时,弯曲韧性达到最大值。

Trichobezoar is a rare intriguing disorder in which swallowed hairs accumulates in the stomach. Being indigestible and slippery, it could not be propulsed and becomes entrapped within the stomach. Large amounts can thus accumulate over the years forming a hair ball. Rapunzel syndrome is a variant where hair accumulation reaches the small gut and beyond in some cases. Although the syndrome has been known for many years, only 24 cases have been reported in the literature and the discovery of a new case is always surprising. In this report, we present two cases discovered within a period of three months. One of them was pregnant and had small bowel intussusception and perforation, a very rare combination. We hereby add two more cases to the literature. To our knowledge, this is the first report on two cases of Rapunzel syndrome, the diagnosis of which demands a high index of suspicion.

Background: Blood groups system has key importance for transfusion safety and clinical practice. Therefore, the objective of the present study was to evaluate the distribution of ABO blood groups and its association with blood donation in Saudi Arabia. Methodology: In this study, we collected information regarding blood donation from 715 Saudi volunteers living in the city of Hail, Northern Saudi Arabia. Data about blood donation were obtained from 715 Saudi volunteers living in the city of Hail. Results: The majority of the study population was found with group B+ representing 174 participants, followed by A+, O+, AB+, B-ve, A-ve, AB-ve and O-ve, representing 165, 153, 47, 29, 20, 9, and 7 participants, in this order. Conclusion: The major blood group among Saudi population is group B+ followed by group A+ and O+. Individuals with rare blood groups were less likely to participate in blood donation programs, which necessitate the need for educational programs using motivating factors.

Dear Dr. Martin Hovland, We learned from the literature that premixed insulins are short-acting insulin or rapid-acting insulin analogue mixed with intermediate-acting insulin in a fixed ratio, addressing FBG and PPBG in one injection. There are two categories;high-mix and low-mix premixed insulins. We, a Saudi task force, gathered to develop an explicit, evidence-based consensus for the use of the low-mix premixed insulin for better glycemic control. The treatment with premixed aspart 30 was non-inferior to treatment with premixed insulin lispro 25. In addition, Self-monitored blood glucose levels were comparable. Safety profiles were similar between both treatments, as was the incidence of hypoglycemic episodes. The switch between both products of the low-mix family can be carried out without any problem. Both products of the low-mix premixed insulin analogues aspart 30/70 and premixed insulin lispro 25/75 have comparable efficacy and safety as shown from the medical literature. Therefore, we can change from one to another safely as demonstrated by the US FDA statement. In addition, the ergonomic features of KwikPen’s design and function may offer important advantages for the user during insulin administration.

Glucagon-like peptide-1 receptor agonists (GLP-1RAs) provide adequate glycemic control, weight reduction, low risk of hypoglycemia, and CV risk reduction. Their usage for type 2 DM (T2DM) is recommended mainly when hypoglycemia or weight gain should be considered, also, whenever initial therapy is failed. There are many recent updates in the treatment paradigm of T2DM. There are many types of GLP-1RAs, with a knowledge gap regarding switching between the different types. A Saudi task force gathered to develop an explicit, evidence-based consensus for switching between GLP-1RAs, when, why, and how? This article contains the expert panel’s recommendations as a contribution to complement the knowledge gap in this area from the national perspective. As an alternative to intensifying therapy, switching from one GLP-1RA to another has various advantages. Improvements in glycemic control, weight loss, adherence, and medications with established cardiovascular benefits are among them. Also, switching needs to be individualized upon many discussed factors like the dose of the previous GLP1-RA and gastrointestinal adverse effects. Discussion with patients about the why and how to switch is critical.

上半规管裂(SSCD)综合征是由于颅底上半规管骨质缺损所导致,特别是在弓状隆起处。骨迷路缺损导致在硬脑膜和膜迷路之间形成直接连通,形成可移动的第三窗,导致各种听觉和前庭症状。耳鸣和自听增强是最常见的听力学表现,头晕和不平衡是最常见的前庭表现。听力检查的结果根据疾病的严重程度而有所不同,低频传导性听力下降是常见的表现,骨传导阈值可能为负值。SSCD综合征患者患耳的前庭诱发肌源性电位(VEMP)反应阈值通常降低,也可能具有比正常VEMP更大的振幅。颞骨的高分辨率计算机断层(CT)扫描在确诊SSCD中起着重要作用。通常建议使用P9schl和Stenver重新格式化的视图。外科治疗适用于前庭症状和听力症状严重影响生活质量的患者,有两种主要的手术径路(颅中窝,乳突)和几种技术(封堵、加帽、重铺表面和组合法)。目前,没有足够的证据确切表明哪种手术方法或技术更好。通过颅中窝径路或经乳突径路的手术修复SSCD对听力和前庭症状的改善非常有效且并发症风险较低。

Antiurolithiatic activity of aqueous extract of bark of moringa oleifera (lam.) in rats

In the present study, aqueous extract of bark of Moringa oleifera administered orally, was eva- luated for its antiurolithiatic potential in albino rats of Wistar strains. The stones were produced in this study by zinc disc foreign body insertion in the bladder supplemented with 1% ethylene glycol in drinking water. The reduction in weight of the stones was used as criteria for assessing the preventive or curative antiurolithiatic effect of the bark of this plant. Two doses of extract for prophylactic and curative groups were used. In both groups the oral administration of the extract of bark of Moringa oleifera has resulted in significant reduction in the weight of bladder stones compared to the control group.

Applicability of Dimedone Assays for the Development of Online Aldehyde Sensor in Seawater Flooding Systems

Biocides are oilfield chemicals that are widely used to control bacterial activity throughout the oil industry. A feasibility study has been explored to develop detection techniques for biocide batch treatments, preferably on-line and in real-time, for their potential use in seawater flooding system. Several methods to measure key components of the biocide formulation were investigated and reported in previous study [1]. The enzymatic activity of an immobilized acetylcholine esterase (AChE) on the column material was successfully inhibited by some model compounds, but not by the actual biocides commonly used in Saudi Aramco seawater flooding system. In this paper, an alternative assay for biocide detection in the Saudi Aramco seawater flooding system was investigated for its applicability for the development of on-line biocide sensor. The assay was based on the detection of aldehyde functionality in the biocide mixture through measurement of a fluorescent derivative formed in the reaction of aldehyde groups and dimedone in the presence of ammonium acetate. The reaction of aldehyde groups with dimedone was demonstrated in seawater matrix, and the formed fluorescent product was successfully measured. The results showed that the dimedone-based assay was very sensitive, and relatively straightforward to perform. The ruggedness test also indicated that the assay is sensitive to minor changes of various specific conditions of the method. It is concluded that the dimedone assay is suitable for further development of a real-time biocide monitoring system to detect the presence of biocide slugs in seawater flooding system. The development of an automated on-line biocide sensor based on dimedone assay is underway.

The Clinical Presentations and Patterns of Management of Bowel Obstruction in Northern Saudi Arabia

Background: The prompt management of patients with bowel obstruction is essential and requires initial rapid diagnosis and therapeutic intervention. Therefore, the objective of the present study was to explore the clinical presentation and pattern of management of bowel obstruction in Northern Saudi Arabia. Methodology: About 92 files referring to patients presented with obstructive bowel conditions during the period from 2013 to 2017 were retrieved from department of surgery at King Khalid Hospital, Hail, Northern Saudi Arabia. Results: Most of cases of bowel obstruction were found in 2014, followed by 2013, 2016, 2015, and 2017 constituting 22, 20, 20, 15, and 15 cases, respectively. About 76, 52, 69, 58, 13, and 28 of the patients presented with pain, constipation, vomiting, abdominal distention, bowel sound and accompanied disease, respectively. Conclusion: Laparotomy was the most common methods of bowel obstruction management in Northern Saudi Arabia. Many patients with bowel obstruction symptoms can improve without surgical intervention.
