Surgical Outcomes and Local Recurrence Following Total or Subtotal Gastrectomy for Early Adenocarcinoma of Antrum

Objective： The choice between total gastrectomy （TG） and subtotal gastrectomy （STG） for early adenocarcinoma of the lower third of the stomach is still a matter of debate and controversy amongst surgeons. The aim of this study was to compare the surgical outcomes, quality of life （QoL）, and local recurrence rates as well as other results of curative resection of early distal adenocarcinoma of the stomach using TG or STG performed in Iran. Methods： Hospital records of 66 patients with early distal adenocarcinoma of stomach that underwent even total or subtotal gastrectomy with perigastric （D2） lymphadenectomy between October 2001 and February 2006 in Taleghani Hospital, Iran were reviewed retrospectively. Demographic data as well as clinicopathological factors including number of involved lymph nodes, tumor grading, depth of invation, tumor size, and tumor type were recorded. Post-operative outcomes including mortality and morbidity, tumor recurrence and quality of life were assessed. Univariate analyses using Fisher＇s exact test, the Student t-test, and the Pearson χ^2 test were used. Results： Local recurrence of tumor was found in 8 （23%） TG group patients and in 19 （61%） patients of STG group, the Pearson χ^2 test showed a significant higher recurrence rate in STG （P value=0.002, Relative Risk=2.68, Confidence Interval： 1.37-5.24）.The mean time interval between gastrectomy and tumor recurrence was not different between TG and STG. （19.75±5.1 vs. 18.0±7.8 respectively; P value=0.507） Tumor size 〉5 cm （P value=0.004）, diffuse type （P value=0.034）, poor differentiation （P value=0.000） and serosal invasion （P value=0.012） were found to be significantly related to tumor recurrence in patients undergone gastrectomy. In none of the QoL measures a significant difference was found between TG and STG Conclusion： Our results show that subtotal and total gastrectomy methods with perigastric lymphadenectomy have a similar postoperative complication rate and surgical outcomes a

Progress in the development of vaccines for hepatitis C virus infection

The hepatitis C virus(HCV), first described in 1989, is now a leading cause of liver cirrhosis and hepatocellular carcinoma. With more than 170 million people infected globally, this virus is a major public health issue. The current standard therapy is based on interferon in combination with ribavirin. This costly therapy often fails to completely clear the infection and is associated with adverse side effects. Recent anti-HCV therapies are interferon-free direct-acting antiviral(DAA) regimens for HCV, including simeprevir, sofosbuvir, and ledipasvir, which have effects on non-structural proteins. DAA regimens have several advantages, such as specifically targeting HCV viral replication, accompanied by very high sustained virological response rates and lower side effects like flu-like syndrome. These facts plus the fact that most HCV cases progress to chronic infection suggest the potential need for an efficient HCV vaccine. Different innovative methods, including methods based on peptide, recombinant protein, DNA, vector-based, and virus-like particles, have been introduced for the development of HCV vaccines. An extensive number of studies have been published on these vaccines, and some vaccines were even tested in clinical trials. In the current review, progress in the development of preventive and therapeutic vaccines against the HCV is reviewed in the context of peptide vaccines, recombinant protein vaccines, HCV-like particle, DNA vaccines and viral vectors expressing HCV genes.

Background:Genetic and environmental factors are both responsible for the etiology of autism spectrum disorders(ASD).Although epidemiological studies have been conducted to clarify the association between restriction diets and ASD,the conclusion remains *** study was undertaken to investigate the effect of gluten free diet(GFD)on gastrointestinal symptoms and behavioral indices in children with ***:In this randomized clinical trial,80 children diagnosed with ASD by the Autism Diagnostic Interview-Revised(ADI-R)were assigned into GFD(n=40)and regular diet(RD)(n=40)groups for 6 *** the beginning and end of the intervention,the ROMEШquestionnaire for evaluating gastrointestinal symptoms and Gilliam Autism Rating Scale 2 questionnaire(GARS-2)for assessing psychometric properties were ***:Of the 80 children,53.9%had gastrointestinal *** the GFD group,the prevalence of gastrointestinal symptoms decreased significantly(P<0.05)after intake of GFD(40.57%vs.17.10%)but increased insignificantly in the RD group(42.45%vs.44.05%).GFD intervention resulted in a significant decrease in behavioral disorders(80.03±14.07 vs.75.82±15.37,P<0.05)but an insignificant increase in the RD group(79.92±15.49 vs.80.92±16.24).Conclusion:This study suggested that GFD may be effective in controlling gastrointestinal symptoms and ASD behaviors.

VOSO4 immobilized within nanoreactors of Al-MCM-41 (VO2+/Al-MCM-41) was synthesized and characterized by X-ray diffraction, Fourier transform infrared spectroscopy, nitrogen adsorption-desorption, and chemical analysis techniques. The prepared VO2+/Al-MCM-41 successfully catalyzes the oxidation of aryl alkyl sulfides and up to 99% conversion and 90% selectivity for the corresponding sulfoxides were obtained with H2O2 as oxidant in acetonitrile at room temperature in 30 min.

Cutaneous leishmaniasis in Iran: Results from an epidemiological study in urban and rural provinces

Objective: To examine the prevalence and clinical manifestations of cutaneous leishmaniasis(CL) in ***: This study was conducted in Iran between 2011 and 2013. Sampling, preparing, developing, and fixing of suspicious skin lesions were completed in healthcare centers in 31 Iranian provinces as well as in the Academic Reference Laboratory and the National Reference Laboratory. The information was then analyzed at the Ministry of Health's Information Management Center of Contagious ***: Over a three-year period, the number of people identified with CL was 56 *** highest incidence was reported in 2011(27.5 per 100 000). Wet CL accounted for 43.7% of cases while 43.3% resulted from sporotrichoid leishmaniasis. The results showed that there was a higher incidence of CL due to Leishmania major(50.2%) than to Leishmania tropica. The results of this study found that the highest incidence of CL had happened respectively in Ilam, Fars and, Khorasan Razavi Provinces between 2011 and ***: Although the incidence of the disease is declining, CL is still a public health concern and disease control protocols need to be established. Therefore, further studies are needed to identify the vectors, reservoirs, and disease species as well as to develop appropriate disease control strategies.

A combination study of magnetic and magnetostrictive properties in directionally cast(DC)(un-annealed)and differently heat-treated Fe-19 at%Ga samples was carried out at room *** cooling leads to an increase in the occupation of[200]easy magnetic ***,structural ordering of Ga atoms into a metastable D03 phase decreases the saturation magnetostriction(λs)and saturation magnetization(Ms),while increases coercivity(Hc).Water quenching suppresses the formation of stable fee ordered phase(L12)and largely preserves disordered bcc single-phase(A2)structure down to room temperature,leading to enhanced magnetostriction and *** cooling promotes the ordering of metastable bcc ordered phase(D03).Magnetic force microscopy(MFM)study exhibits that the water-quenched(WQ)sample consists of a well-aligned stripe domain structure,while irregular maze-like domain structure is observed in furnace-cooled(FC)*** results confirm that in addition to an inhibitory effect of D03 ordering on magnetic domain wall motions,irregular magnetic domains also contribute to decrease in magnetic and magnetostrictive properties of FC sample.

由溶胶-凝胶法制得的Ni-La化合物经热分解制备了纳米尺度NiLa2O4尖晶石,在750o C焙烧后形成了结晶良好的尖晶石结构.采用差热分析、X射线衍射、透射电镜、扫描电镜和粒度分布分析等手段表征了该尖晶石的物理化学性质.结果表明,该纳米颗粒有规则的外形和确定的晶面,由平均粒径为40 nm的规整半球晶粒组成.精修的晶胞参数a=3.861205和c=12.6793.在NaBH4选择还原亚胺制相应仲胺的反应中,该新型纳米NiLa2O4尖晶石可用作高效多相催化剂,得到了较高的产物产率.所有反应可在室温和相对较短的时间内完成.在优化的反应条件下,均可得到带有不同芳基的,包括带有吸电子和供电子基团的仲胺.该催化剂回收简便,重复使用4次,其催化活性未见明显下降.

Effects of humidity on shear behavior of bamboo

Bamboo is a naturally occurring biological composite, however its microstructure and hence its properties are very complex compared to the manmade composites. Due to optimization, it can be assumed that the variation in properties along the thickness of the culm be a smooth transition for better bonding strength between layers and to prevent non uniformity in stress concentration. As a consequence, biological structures are complicated and functionally graded. Hence, a realistic model that can capture the mechanical performance of bamboo is valuable in future design of robust multifunctional composites. This paper presents the results of experimental and numerical studies on the torsional （shear） properties of bamboo. The hierarchical and multi-scale structure of bamboo and the distribution of micro-scale fibers are revealed via laser scanning and atomic force microscopy. This information was incorporated into a finite element model to analyze the mechanical behavior of bamboo under torsion and to estimate the shear modulus of bamboo along the fibers. Moreover, the effects of humidity and therefore water content on the mechanical properties of bamboo were evaluated by performing torsion tests on samples maintained in environments with different humidities. Increasing the humidity does not cause a drop in the shear modulus, however, a jump in the shear modulus did occur at around 60% humidity. Results of this study indicate that the highest strength values in samples occurred in environments with humidity levels between 60% and 80% and undergo a significant drop after that. In higher humidities, the samples behave more ductile.

Therapeutic effects of acupuncture on blood glucose level among patients with type-2 diabetes mellitus: A randomized clinical trial

Objective:To compare the therapeutic effects of acupuncture with sham acupuncture on levels of fasting plasma glucose (FPG) and glycated hemoglobin (HbA1c) among patients with type-2 diabetes mellitus (T2DM) under treatment with ***:Eighty participants with T2DM were allocated randomly to treatment (n =40) and control (n =40) *** addition to metformin therapy,patients in the acupuncture group received acupoint stimulation with even manipulation at 15 acupoints,and needles were retained for 20 *** sham-acupuncture group was treated with very small needles inserted at a very shallow depth (3-5 mm) without manipulation at non-acupoints 1 cm around the actual acupuncture *** groups were treated for 14 sessions during 10 weeks of *** FPG level was measured before treatment as well as after 1,2,4,6 and 12 weeks of *** HbA1c percentage was assessed once before treatment and once 12 weeks after treatment ***:Levels of FPG and HbA1c in both groups were reduced significantly in comparison with baseline measurements (both P <.01).However,acupuncture treatment led to a significant reduction in the FPG level as compared with sham acupuncture after 4 weeks or 10 sessions of treatment [mean difference (MD) =10.18,95% confidence interval (CI) =1.74 to 18.6,P =.019],after 6 weeks or 12 sessions of treatment (MD =10.53,95% CI =2.22 to 18.83,P =.014) and after 12-week follow-up (MD =8.8,95% Cl =0.66 to 16.94,P =.034).Acupuncture treatment reduced the HbA1c level significantly in comparison with that in the shamacupuncture group (MD =0.27,95%Cl =0.08 to 0.47,P =.007).Conclusion:Our study revealed that acupuncture (as a type of Chinese therapy) combined with metformin (as a conventional hypoglycemic medication) had a greater effect on glycemic control than that elicited in the placebo control ***,acupuncture could be used as adjuvant therapy among patients with T2DM.
