BACKGROUND The impact of type 2 diabetes mellitus(T2DM)on acute respiratory distress syndrome(ARDS)is debatable.T2DM was suspected to reduce the risk and complications of ***,during coronavirus disease 2019(COVID-19),T2DM predisposed patients to ARDS,especially those who were on insulin at *** evaluate the impact of outpatient insulin use in T2DM patients on non-COVID-19 ARDS *** We conducted a retrospective cohort analysis using the Nationwide Inpatient Sample *** patients diagnosed with ARDS were stratified into insulin-dependent diabetes mellitus(DM)(IDDM)and non-insulindependent DM(NIDDM)*** applying exclusion criteria and matching over 20 variables,we compared cohorts for mortality,duration of mechanical ventilation,incidence of acute kidney injury(AKI),length of stay(LOS),hospitalization costs,and other clinical *** Following 1:1 propensity score matching,the analysis included 274 patients in each ***,no statistically significant differences emerged between the IDDM and NIDDM groups in terms of mortality rates(32.8%vs 31.0%,P=0.520),median hospital LOS(10 d,P=0.537),requirement for mechanical ventilation,incidence rates of sepsis,pneumonia or AKI,median total hospitalization costs,or patient disposition upon *** Compared to alternative anti-diabetic medications,outpatient insulin treatment does not appear to exert an independent influence on in-hospital morbidity or mortality in diabetic patients with non-COVID-19 ARDS.

Assessment of Gaseous and Particulate Pollutants in the Ambient Air in al Mirfa City, United Arab Emirates

From 2007-2009, National Energy and Water Research Center (NEWRC) on behalf of Abu Dhabi Water and Electricity Authority (ADWEA) conducted a long term baseline study of nitrogen dioxide (NO2), carbon monoxide (CO), sulfur dioxide (SO2), ozone (O3) and particulate matter 2, SO2, and 8 hour average of CO were within the acceptable levels of 400, 300 and 30,000 μg/m3 respectively, whereas the levels of O3 as 8 hour average (3) and PM10 (3) occasionally exceeded the FEA permissible limits during the study period. Seasonal variation based on three years data reveals that the highest concentration of NO2 and SO2 were during winter and for CO and O3 during summer months. Results indicate that the levels of SO2 and CO were significantly controlled and improved while the fuel combustion of al Mirfa power plant had increased from 2007 to 2009. Dust has significantly impact on the air quality by elevated levels of PM10 exceed in several instances associated with regional sand- storm during the monitoring period.

Pollution Scenarios through Atmospheric Dispersion Modelling Based on Real Measurements of Selected Urban Areas in Abu Dhabi, UAE

Power requirements in the city of al Mirfa in western Abu Dhabi are covered by the al Mirfa Power and Distillation Plant. Comprehensive emission inventories for 2007-2008 were used to execute an ENVIMAN (OPSIS AB Company, Sweden) Gaussian dispersion model to predict ambient ground level concentrations of nitrogen oxides (NOx), carbon monoxide (CO), particulate matter (PM10) and sulfur dioxide (SO2) at selected receptors considering all emission sources located in the area. Two years of meteorological data was used in conjunction with the dispersion model to compute NOx and SO2 levels in and around the power plant. To validate the model, computed results were compared with the average values measured at a fixed Air Quality Station in al Mirfa city. The highest hourly, daily and annual ground level concentrations under exiting meteorological conditions were then analyzed. The computed results for the study area revealed that daily, hourly and annual concentration values did not exceed the Federal Environment Agency (FEA) standard, and the contribution of plant emissions to the ground levels pollutants in the surrounded area range from 3.1 to 109 μg/m3 for NO2, and 1.1 to 41.4 μg/m3 for CO. This study can be considered a baseline study for any future expansion in the plant. Based on these results, mitigation strategies are not required.

Modeling discrete-time analytical models based on random early detection: Exponential and linear

Congestion control is among primary topics in computer network in which random early detection(RED)method is one of its common ***,RED suffers from drawbacks in particular when its“average queue length”is set below the buffer’s“minimum threshold”position which makes the router buffer quickly *** deal with this issue,this paper proposes two discrete-time queue analytical models that aim to utilize an instant queue length parameter as a congestion *** assigns mean queue length(mql)and average queueing delay smaller values than those for RED and eventually reduces buffers overflow.A comparison between RED and the proposed analytical models was conducted to identify the model that offers better *** proposed models outperform the classic RED in regards to mql and average queueing delay measures when congestion *** work also compares one of the proposed models(RED-Linear)with another analytical model named threshold-based linear reduction of arrival rate(TLRAR).The results of the mql,average queueing delay and the probability of packet loss for TLRAR are deteriorated when heavy congestion occurs,whereas,the results of our RED-Linear were not impacted and this shows superiority of our model.

although gastrointestinal stromal tumors(GISTs)are rare,with an incidence of 1/100000 per year,they are the most common sarcomas in the peritoneal *** considerable progress in the diagnosis and treatment of GIST,about half of all patients are estimated to experience *** only two drugs,sunitinib and regorafenib,approved by the Food and Drug Administration,selecting treatment options after imatinib failure and coordinating multidisciplinary care remain *** addition,physicians across the Middle East face some additional and unique challenges such as lack of published local data from clinical trials,national disease registries and regional scientific research,limited access to treatment,lack of standardization of care,and limited access to mutational *** global guidelines set a framework for the management of GIST,there are no standard local guidelines to guide clinical practice in a resource-limited ***,a group of 11 experienced medical oncologists from across the Gulf and Levant region,part of the Rare Tumors Gastrointestinal Group,met over a period of one year to conduct a narrative review of the management of GIST and to describe regional challenges and gaps in patient management as an essential step to proposing local clinical practice recommendations.
