本文是《PLoS Medicine》关于卫生系统知证政策指导三篇系列文章的第一篇,阐述了应如何将证据转化为指导,以形成卫生系统政策,并促进临床和公共卫生干预的实施。其要点如下:①薄弱的卫生系统阻碍了有效干预的实施,加强此系统的政策需要基于当前可得的最佳证据;②卫生系统证据最好以政策制定过程参考的指导的形式呈现,但目前卫生系统指导制定质量较差;③将问题、干预及实施相关的研究转化为影响卫生系统组织方式的决策和政策是卫生系统指导制定面临的一个挑战;④制定及时且适用于广大卫生系统利益相关者的指导,并确定评价卫生系统指导质量的方法是另一个挑战;⑤改编现有方法(如用于临床指南的那些方法)制定有意义的建议来应对卫生系统、政策系统及背景的复杂性,尚需进一步研究。

Prostate-specific antigen doubling time and response to cabazitaxel in a hormone-resistant metastatic prostate cancer patient

We report a case of metastatic castration-resistant prostate cancer, who received prior treatment with docetaxel and was then given cabazitaxel as salvage therapy. The patient was monitored by prostate-specific antigen doubling time and prostate-specific antigen absolute value. The prostate-specific antigen doubling time was found to be a good response predictor in the patient.

Optimum chemotherapy for the management of advanced biliary tract cancer

Biliary tract cancers(BTCs) are highly fatal malignancies, which are often diagnosed at an advanced stage and have relatively poor prognosis.The treatment of patients with advanced BTC is systemic, based on chemotherapy or best supportive care, depending on their performance status.Despite clinical trials studyingmany chemotherapeutic regimens and targeted therapies for the treatment of BTC, the standard of care for advanced BTC remains the combination of gemcitabine with cisplatin.Many new molecules targeting proliferation and survival pathways, the immune response and angiogenesis are currently undergoing phase Ⅰ and Ⅱ trials for the treatment of advanced BTC with promising results.

Where does chemotherapy stands in the treatment of ampullary carcinoma? A review of literature

Ampullary carcinoma(AC) is a rare gastrointestinal tumor without clear treatment recommendations.The management of this tumor is usually extrapolated from the treatment of pancreatic,biliary duct and intestinal cancers.Few papers have studied the AC as an independent entity and yet succombs to several limitations.These studies were retrospective single institutional experiences with limited sample sizes recruited over a long period of time.Unlike metastatic ACs where chemotherapy is the only recommended option,localized AC once excised may be approached by either chemotherapy alone or concomitant chemoradiation therapy.In this review,we report the overall survival and recurrence factors of more than 1000 patients from all the studies treating exclusively ACs.We also review the medical treatment of this tumor and conclude to the necessity of multi-institutional randomized controlled studies for AC exclusively.

Management of cataract in keratoconus: early visual outcomes of different treatment modalities

A review of 31 eyes with keratoconus who developed cataract and underwent phacoemulsification. Visual acuities were measured 1 mo postoperatively. Six eyes with a history of good corrected distance visual acuity(CDVA) and a similar refractive and topographic astigmatic axis were implanted with toric intraocular lenses(IOLs). The mean postoperative uncorrected distance visual acuity(UDVA) was 0.2 logMAR with a spherical equivalent(SE): 0.75 D. eleven eyes with a history of good CDVA and different refractive and topographic axis were implanted with monofocal IOL+/-Toric implantable collamer lenses to treat anisometropia and ametropia;mean UDVA was 0.25 logMAR with a mean SE:-0.51 D postoperatively. Six eyes with poor CDVA were first treated with intra-corneal ring segments, followed by phacoemulsification, the mean postoperative UDVA was 0.82 logMAR with an SE: 0.22 D. Eight eyes had advanced ectesia and received combined phacoemulsification and penetrating keratoplasty. Our approach is efficient in addressing ametropia after cataract surgery in keratoconic eyes.

Repair of an aberrant subclavian arterioesophageal fistula following esophageal stent placement

A fistula formation between the esophagus and an aberrant right subclavian artery is a rare but fatal com-plication that has been mostly described in the setting of prolonged nasogastric intubation and foreign body erosion. We report a case of a young morbidly obese patient who underwent sleeve gastrectomy that was complicated by a postoperative leak at the level of the gastroesophageal junction. A covered esophageal stent was placed endoscopically to treat the leak. The pa-tient developed massive upper gastrointestinal bleeding secondary to the erosion of the stent into an aberrant retroesophageal right subclavian artery twelve days after stent placement. She was ultimately treated by endovascular stenting of the aberrant right subclavian artery followed by thoracotomy and esophageal repair over a T-tube. This case report highlights the multidis-ciplinary approach needed to diagnose and managesuch a devastating complication. It also emphasizes the need for imaging studies prior to stent deployment to delineate the vascular anatomy and rule out the possi-bility of such an anomaly in view of the growing popu-larity of esophageal stents, especially in the setting of a leak.

A Multidisciplinary Approach to the Treatment of Gastric Cancer: What Is the Role of the Surgeon?

Despite recent declining incidence, gastric cancer remains one of the most common cancers worldwide. Its treatment involves a multidisciplinary team to ensure implementation of investigated pre-operative and post-operative treatment strategies to improve survival. Our aim is to discuss the current role of the surgeon in improving survival of gastric cancer patients.

Microstructure and texture evolution of AZ31 Mg alloy after uniaxial compression and annealing

The effect of initial texture on the microstructure and texture evolution of as extruded AZ31 alloy after hot uniaxial compression at 430°C and subsequent annealing at 450°C for 72h has been investigated using electron backscatter diffraction.Samples were machined from the extruded block in such orientations that the compression axis,CD,was parallel(CD0ED)or perpendicular(CD90ED)to the initial extrusion direction,ED,respectively.Results show that the deformation microstructure for both samples was characterized by equiaxed grains that resulted from dynamic recrystallization.Annealing at 450°C for 72 h led to the increase of grains size to 28 and 25μm for CD0ED and CD90ED samples,respectively.A random deformation texture was developed in CD0ED sample while the deformation of CD90ED developed typical(0002)basal texture.The deformation textures of both samples were retained after subsequent annealing at 450°C for 72 h.The flow behavior was affected by the initial texture.The CD0ED sample showed the highest work hardening.These evolutions were explained in term of the activation and suppression of basal,non-basal slip and mechanical twinning.©2018 Published by elsevier B.V.on behalf of Chongqing University.This is an open access article under the CC BY-NC-ND license.

Optimum chemotherapy in the management of metastatic pancreatic cancer

Pancreatic cancer is one of the most devastating solid tumors,and it remains one of the most difficult to treat.The treatment of metastatic pancreatic cancer(MPC)is systemic,based on chemotherapy or best supportive care,depending on the performance status of the patient.Two chemotherapeutical regimens have produced substantial benefits in the treatment of MPC:gemcitabine in 1997;and FOLFIRIONOX in 2011.FOLFIRINOX improved the natural history of MPC,with overall survival(OS)of 11.1 mo.Nab-paclitaxel associated with gemcitabine is a newly approved regimen for MPC,with a median OS of 8.6 mo.Despite multiple trials,this targeted therapy was not efficient in the treatment of MPC.Many new molecules targeting the proliferation and survival pathways,immune response,oncofetal signaling and the epigenetic changes are currently undergoing phaseⅠandⅡtrials for the treatment of MPC,with many promising results.

Although gastrointestinal stromal tumors(GISTs)are rare,with an incidence of 1/100000 per year,they are the most common sarcomas in the peritoneal cavity.Despite considerable progress in the diagnosis and treatment of GIST,about half of all patients are estimated to experience recurrence.With only two drugs,sunitinib and regorafenib,approved by the Food and Drug Administration,selecting treatment options after imatinib failure and coordinating multidisciplinary care remain challenging.In addition,physicians across the Middle East face some additional and unique challenges such as lack of published local data from clinical trials,national disease registries and regional scientific research,limited access to treatment,lack of standardization of care,and limited access to mutational analysis.Although global guidelines set a framework for the management of GIST,there are no standard local guidelines to guide clinical practice in a resource-limited environment.Therefore,a group of 11 experienced medical oncologists from across the Gulf and Levant region,part of the Rare Tumors Gastrointestinal Group,met over a period of one year to conduct a narrative review of the management of GIST and to describe regional challenges and gaps in patient management as an essential step to proposing local clinical practice recommendations.
