Mastocytosis is a rare and heterogeneous disease characterized by various clinical and biological features that affect different prognoses and treatments.The disease is usually divided into 2 principal categories:cutaneous and systemic disease(SM).Clinical features can be related to mast cell(MC)mediator release or pathological MC infiltration.SM is a disease often hard to identify,and the diagnosis is based on clinical,biological,histological,and molecular criteria with different specialists involved in the patient’s clinical work-up.Among all manifestations of the disease,gastrointestinal(GI)symptoms are common,being present in 14%-85% of patients,and can significantly impair the quality of life.Here we review the data regarding GI involvement in SM,in terms of clinical presentations,histological and endoscopic features,the pathogenesis of GI symptoms,and their treatment.

This study presents the formulation,the numerical solution,and the validation of a theoretical framework based on the concept of variable-order mechanics and capable of modeling dynamic fracture in brittle and quasi-brittle solids.More specifically,the reformulation of the elastodynamic problem via variable and fractional-order operators enables a unique and extremely powerful approach to model nucleation and propagation of cracks in solids under dynamic loading.The resulting dynamic fracture formulation is fully evolutionary,hence enabling the analysis of complex crack patterns without requiring any a priori assumption on the damage location and the growth path,and without using any algorithm to numerically track the evolving crack surface.The evolutionary nature of the variable-order formalism also prevents the need for additional partial differential equations to predict the evolution of the damage field,hence suggesting a conspicuous reduction in complexity and computational cost.Remarkably,the variable-order formulation is naturally capable of capturing extremely detailed features characteristic of dynamic crack propagation such as crack surface roughening as well as single and multiple branching.The accuracy and robustness of the proposed variableorder formulation are validated by comparing the results of direct numerical simulations with experimental data of typical benchmark problems available in the literature.

In the last decades,vanadium alloyed coatings have been introduced as potential candidates for self-lubrication due to their perfect tribological properties.In this work,the influence of V incorporation on the wear performance and oxidation resistance of TiSiN/CrN film coatings deposited by direct current(DC)reactive magnetron sputtering is investigated.The results show that vanadium incorporation significantly decreases the oxidation resistance of the coatings.In general,two layers are formed during the oxidation process:i)Ti(V)O_(2) on top,followed by a protective layer,which is subdivided into two layers,Cr_(2)O_(3) and Si-O.ii)The diffusion of V controls the oxidation of V-containing coatings.The addition of vanadium improves the wear resistance of coatings,and the wear rate decreases with increasing V content in the coatings;however,the friction coefficient is independent of the chemical composition of the coatings.The wear of the V-containing coatings is driven by polishing wear.

Despite scientific advances in the Oncology field,cancer remains a leading cause of death worldwide.Molecular and cellular heterogeneity of head and neck squamous cell carcinoma(HNSCC)is a significant contributor to the unpredictability of the clinical response and failure in cancer treatment.Cancer stem cells(CSCs)are recognized as a subpopulation of tumor cells that can drive and maintain tumorigenesis and metastasis,leading to poor prognosis in different types of cancer.CSCs exhibit a high level of plasticity,quickly adapting to the tumor microenvironment changes,and are intrinsically resistant to current chemo and radiotherapies.The mechanisms of CSC-mediated therapy resistance are not fully understood.However,they include different strategies used by CSCs to overcome challenges imposed by treatment,such as activation of DNA repair system,anti-apoptotic mechanisms,acquisition of quiescent state and Epithelial-mesenchymal transition,increased drug efflux capacity,hypoxic environment,protection by the CSC niche,overexpression of stemness related genes,and immune surveillance.Complete elimination of CSCs seems to be the main target for achieving tumor control and improving overall survival for cancer patients.This review will focus on the multi-factorial mechanisms by which CSCs are resistant to radiotherapy and chemotherapy in HNSCC,supporting the use of possible strategies to overcome therapy failure.

BACKGROUND Four-corner fusion(4CF)is a motion sparing salvage procedure that is used to treat osteoarthritis secondary to advanced scapholunate collapse or longstanding scaphoid nonunion advanced collapse.Little is known about the long-term survivorship and outcomes of 4CF.AIM To report on clinical and functional long-term outcomes as well as conversion rates to total wrist fusion or arthroplasty.METHODS The systematic review protocol was registered in the international prospective register of systematic reviews(PROSPERO)and followed the PRISMA guidelines.Original articles were screened using four different databases.Studies with a minimum Level IV of evidence that reported on long-term outcome after 4CF with a minimum follow-up of 5 years were included.Quality assessment was performed using the Methodological Index for Non-Randomized Studies criteria.RESULTS A total of 11 studies including 436 wrists with a mean follow-up of 11±4 years(range:6-18 years)was included.Quality assessment according to Methodological Index for Non-Randomized Studies criteria tool averaged 69%±11%(range:50%-87%).Fusion rate could be extracted from 9/11 studies and averaged 91%.Patient-reported outcomes were extracted at last follow-up from 8 studies with an average visual analog scale of 1±1(range:0-2)and across 9 studies with an average Disabilities of the Arm,Shoulder and Hand score of 21±8(range:8-37).At last follow-up,the cumulative conversion rate to total wrist fusion averaged 6%.There were no conversions to total wrist arthroplasty.CONCLUSION The 4CF of the wrist is a reliable surgical technique,capable of achieving a good long-term patient satisfaction and survivorship with low rates of conversion to total wrist fusion.

Hilar cholangiocarcinoma is a biliary malignancy arising from the perihilar biliary tree,which is associated with poor oncological outcomes due to its aggressive biology,chemo-resistance and insidious onset[1].As stated by Di Martino et al.,the standard of care is radical resection,and during the last decades,there have been great efforts to improve survival of potentially resectable hilar cholangiocarcinoma,with surgery being the treatment associated with longer survival[2].However,radical resection still represents a challenging operation with high risk of intraoperative and postoperative complications.

Following a seismic event that occurred years ago in Central Italy, the public opinion was growing and growing a concern on the adequacy of educational buildings all across Italy. This activated several political decisions and a consequent technical effort is in progress. Technically speaking one has to manage the classical problem of retrofitting existing buildings. However, the legal environment goes across national codes, targeted guidelines and the professional need of achieving pragmatic solutions based on ethical and social acceptation schemes.This paper introduces the topic in its worldwide exception and focuses then on some operative aspects in the Italian situation. It outlines the consolidated steps along this technical process and emphasizes the weak aspects one meets when going across the designers＇ reports.

This contribution focuses on a multidisciplinary research showing the geomorphological evolution and the beach sand composition of the Tyrrhenian shoreline between Capo Suvero promontory and Gizzeria Lido village(Calabria,southern Italy). The aim of the geomorphological analysis was to reconstruct the evolutionary shoreline stages and the present-day sedimentary dynamics along approximately 6 km of coastline. The results show a general trend of beach nourishment during the period 1870–2019. In this period, the maximum shoreline accretion value was estimated equal to + 900 m with an average rate of + 6.5 m/yr. Moreover, although the general evolutionary trend is characterized by a remarkable accretion, the geomorphological analysis highlighted continuous modifications of the beaches including erosion processes. The continuous beach modifications occurred mainly between 1953 and1983 and were caused mainly by human activity in the coastal areas and inside the hydrographic basins. The beach sand composition allowed an assessment of the mainland petrological sedimentary province and its dispersal pattern of the present coastal dynamics. Petrographic analysis of beach sands identified a lithic metamorphi-clastic petrofacies, characterized by abundant fine-grained schists and phyllites sourced from the crystalline terrains of the Coastal Range front and carried by the Savuto River. The sand is also composed of a mineral assemblage comparable to that of the Amato River provenance. In terms of framework detrital constituents of QFL(quartz:feldspars:aphanitic lithic fragments) and of essential extraclasts, such as granitoid:sedimentary:metamorphic phaneritic rock fragments(Rg:Rs:Rm), sand maturity changes moderately from backshore to shoreface, suggesting that transport processes had a little effect on sand maturity. Moreover, the modal composition suggests that the Capo Suvero promontory does not obstruct longshore sand transport from the north. Background: Gradients in local environmental characteristics may favour the abundance of species with particular traits, while other species decline, or favour species with different traits at the same time, without an increase in average species abundances. Therefore, we asked: do variations in species and traits differ along gradients of deadwood variables? Do species abundance and trait occurrence change with species richness within or between functional groups? Thus, we analysed the beetle assemblages of five forest sites located in Italy, along the Apennines mountains.Methods: From 2012 to 2018 we sampled beetles and five deadwood types in 193 plots to characterise the deadwood gradient: standing dead trees, snags, dead downed trees, coarse woody debris, and stumps. We modelled beetle species relative abundances and trophic traits occurrences against the deadwood variables using joint species distribution models.Results: Out of 462 species, only 77 showed significant responses to at least one deadwood type, with a weak mean response across species. Trophic groups showed mostly negative responses to deadwood variables. Species abundance increased with species richness among sites only for phytophagous and saproxylophagous. Trait occurrence did not increase with species richness among sites, except for phytophagous and saproxylophagous.However, trait occurrence changed significantly with species richness of several trophic groups within some sites.We found that increases in species richness do not result in decreases in species abundance of a given trophic group, but rather null or positive relationships were found suggesting low interspecific competition.Conclusions: Our findings suggest that in Mediterranean mountain forests there is still room for increasing the level of naturalness, at least for what concerns deadwood management. On a macroscopic level,an edge computing architecture looks like a distributed and decentralized IT(Information Technology)architecture.More in detail,it could be defined as a mesh network of micro data centers capable of processing and storing critical data locally,and to transmit all data received and/or processed to a central data center or a cloud storage repository.This network topology,also taking advantage of the availability on the market of cost-effective small form factor(SFF)electronic components and systems decreasing,brings the essential components of processing,storage,and networking closer to the sources that generate the data.The typical use case is that of Internet of Things(IoT)devices and implementations,which often face latency problems,lack of bandwidth,reliability,which cannot be addressed through the conventional cloud model.In this context,the edge computing architecture can reduce the size of data to be sent to the cloud,processing critical data,sensitive to latency,at the point of origin,via a smart device,or sending it to an intermediate server,located nearby.The aim of this paper is to report some of the main aspects and significant features of edge computing and analyzing several popular case studies.
