Comparing shoulder maneuvers to magnetic resonance imaging and arthroscopic findings in patients with supraspinatus tears

BACKGROUND Shoulder maneuvers and magnetic resonance imaging(MRI)are performed to diagnose supraspinatus tendon tears regardless of arthroscopy exam.Although there are many studies on this subject,there is a lack of studies comparing the sensitivity(Se)and specificity(Sp)of shoulder maneuvers and MRI to arthroscopic findings(intact,partial,or full thickness supraspinatus tendon tear).AIM To compare the diagnostic values of shoulder maneuvers with MRI for supraspinatus tendon tears in patients undergoing shoulder arthroscopy.METHODS A total of 199 consecutive patients from four orthopedic centers met the eligibility criteria of shoulder pain persisting for at least four weeks.They were prospectively enrolled in this study from April 2017 to April 2019.Seven clinical tests(full can,empty can,drop arm,Hawkins’,painful arc,Neer’s sign and resisted external rotation)and MRI were performed,and all were compared with surgical findings.Full can,empty can and resisted external rotation tests were interpreted as positive in the case of pain and/or weakness.We assessed the Se,Sp,accuracy,positive predictive value(PPV)and negative predictive value(NPV),positive and negative likelihood ratio and diagnostic odds ratio for overall,partial and fullthickness supraspinatus tears.RESULTS MRI had the highest Se for overall(0.97),partial(0.91)and full-thickness(0.99)tears;moreover,MRI had the highest NPV:0.90,0.88 and 0.98 for overall,partial and full-thickness tears,respectively.For overall supraspinatus tears,the Se and PPV were:Painful arc(Se=0.85/PPV=0.91),empty can(pain)(Se=0.80/PPV=0.89),full can(pain)(Se=0.78/PPV=0.90),resisted external rotation(pain)(Se=0.48/PPV=0.87),drop arm(Se=0.19/PPV=0.97),Neer’s sign(Se=0.78/PPV=0.93)and Hawkins’(Se=0.80/PPV=0.88).MRI had the highest PPV(0.99).The Hawkin’s test had the highest false positive rate in patients with intact tendons(0.36).The Sp of the empty can and full can(both tests positive for pain and weakness),drop arm and MRI were:0.93,0.91,0.98 and 0.96,respectively.For partial and

Failure of levees induced by toe uplift:Investigation of post-failure behavior using material point method

Levees are essential structures in flood defense systems,and their failures can lead to devastating consequences on the surrounding territories.One of the failure mechanisms mostly controlled by the foundation soil stratigraphy is the instability of the land side slope,triggered by the development of high uplift pressures in the foundation.This complex phenomenon has been investigated experimentally with centrifuge tests or large-scale tests and numerically with the limit equilibrium method(LEM)and the finite element method(FEM).In this work,we applied a multiphase formulation of the material point method(MPM)to analyze the development of toe uplift instability mechanism,from the onset of failure to large displacements.The numerical model is inspired by an experiment carried out in a geotechnical centrifuge test by Allersma and Rohe(2003).The comparison with the experiment allows for understanding critical pore pressure triggering large displacements in the foundation soils.Moreover,we numerically evaluated the impact of different values of foundation soils’hydraulic conductivity on the failure mechanism.The results show that hydraulic conductivity mainly influences the time of failure onset and the extension of shear localization at depth.Finally,the advantages of using large displacement approaches in the safety assessment of earth structures are discussed.Unlike FEM,there are no issues with element distortions generating difficulties with numerical convergence,allowing for full postfailure reproduction.This capability permits precise quantification of earth structure damages and post-failure displacements.The ensuing reinforcement systems’design is no longer over-conservative,with a significant reduction in associated costs.

Safety,immunogenicity,efficacy,and acceptability of COVID-19 vaccination in people with multiple sclerosis:a narrative review

In the last two years,a new seve re acute res piratory syndrome coronavirus(SARS-CoV)infection has spread worldwide leading to the death of millions.Va ccination represents the key factor in the global strategy against this pandemic,but it also poses several problems,especially for vulnerable people such as patients with multiple scle rosis.In this review,we have briefly summarized the main findings of the safety,efficacy,and acceptability of Coronavirus Disease 2019(COVID-19)vaccination fo r multiple sclerosis patients.Although the acceptability of COVID-19 vaccines has progressively increased in the last year,a small but significant part of patients with multiple sclerosis still has relevant concerns about vaccination that make them hesitant about receiving the COVID-19 vaccine.Overall,available data suggest that the COVID-19 vaccination is safe and effective in multiple scle rosis patients,even though some pharmacological treatments such as anti-CD20 therapies or sphingosine I-phosphate receptor modulato rs can reduce the immune response to vaccination.Accordingly,COVID-19 vaccination should be strongly recommended for people with multiple scle rosis and,in patients treated with anti-CD20 therapies and sphingosine I-phosphate receptor modulato rs,and clinicians should evaluate the appropriate timing for vaccine administration.Further studies are necessary to understand the role of cellular immunity in COVID-19 vaccination and the possible usefulness of booster jabs.On the other hand,it is mandatory to learn more about the reasons why people refuse vaccination.This would help to design a more effective communication campaign aimed at increasing vaccination coverage among vulnerable people.

Aspects of Indigenous Participation in Brazilian Literature

The article both presents how,from the end of the 1980s onwards,native peoples have begun to occupy certain spaces of textual production and circulation that they had not previously occupied in the Brazilian cultural scene(for social,linguistic,and cultural reasons,but also political and juridical)and discusses how this process has provoked a vigorous movement of dilation of traditional textual and discursive borders in Western culture.Texts deriving from indigenous peoples in the sphere of academic discourse are,in general,bilingual,and are structured in ways that combine aspects of intellectual production with those of artistic creation.Moreover,they are also structured around a rather complex conception of the notion of authorship(considering that they are written by an author but represent the voice of their people).As examples,the article analyzes the case of Os cantos tradicionais Ye'kwana[Traditional Ye'kwana Chants],by the indigenous teacher and researcher Fernando Ye'kwana Gimenes,winner of the 2020/2021 edition of the Dirce Cortes Riedel Masters Dissertation Award by the Brazilian Association of Comparative Literature,as a typical example of the cultural phenomenon discussed.The traditional Ye'kwana chants present significant transgressions in relation to the traditional notions of narrative logic and the dominant forms of narration in the fields of literature and history.The awarding of this academic prize to an indigenous inhabitant of the forest,on the border between Brazil and Venezuela,by the largest association of comparative literature in Latin America,in addition to being an important act in political terms,demonstrates how urgent it is to rethink processes of global literary dissemination beyond the restricted frameworks configured by the logic of hegemonic cultures,which are based on closed divisions and hierarchies.With this,we intend to contribute to the process of including Amerindian texts in the repertoire of Comparative Literature and World Literature.

Air quality nearby road traffic tunnel portals: BTEX monitoring

A monitoring campaign of BTEX （benzene, toluene, ethylbenzene, o- m- and p-xylene） was carried out nearby two tunnel portals in the urban area of Naples with the aim to verify air quality in this kind of urban sites. Sampling was carried out using the active adsorption technique. Sampling time was 1 h. Ambient temperature and traffic flow measurements were carried out during each sampling operation. The results indicate that average benzene concentrations at both sites exceed the limit value of 10 μg/Nm^3 established by the European Community （EC）（Dir. 2000/69）. Concentration levels of other BTEX are relatively high as well. A correlation between BTEX concentration and two wheeler vehicle flow was observed.

Use of topical vancomycin powder in total joint arthroplasty:Why the current literature is inconsistent?

Periprosthetic joint infection(PJI)is a rare but terrible complication in hip and knee arthroplasty,and the use of topical vancomycin powder(VP)has been investigated as a tool to potentially reduce its incidence.However,there remains no consensus on its efficacy.Therefore,the aim of this review is to provide an overview on the application of topical vancomycin in orthopaedic surgery focusing on the recent evidence and results in total joint arthroplasty.Several systematic reviews and meta-analyses on topical VP in hip and knee arthroplasty have been recently published reporting sometimes conflicting results.Apart from all being limited by the quality of the included studies(mostly level III and IV),confounding variables are often included potentially leading to biased conclusions.If taken into consideration the exclusive use of VP in isolation,the available data,although very limited,suggest that it does not reduce the infection rate in routine primary hip and knee arthroplasty.Therefore,we still cannot advise for a routinary application.A properly powered randomized-controlled trial would be necessary to clarify the role of VP in hip and knee arthroplasty.Based on the analysis of the current evidence,the use of topical VP appears to be safe when used locally in terms of systemic adverse reactions,hence,if proven to be effective,it could bring great benefits due to its low cost and accessibility.

Delayed-release oral mesalamine tablet mimicking a small jejunal gastrointestinal stromal tumor:A case report

BACKGROUND Enteric-coated medications are supposed to pass intact through the gastric environment and to release the drug content into the small intestine or the colon.Before dissolution of the enteric coating,they may appear hyperdense on computed tomography(CT).Unfortunately,few reports have been published on this topic so far.In this case report,the hyperdense appearance on contrastenhanced CT of an enteric-coated mesalamine tablet was initially misinterpreted as a jejunal gastrointestinal stromal tumor(GIST).CASE SUMMARY An asymptomatic 81-year-old male patient,who had undergone laparoscopic right nephrectomy four years earlier for stage 1 renal carcinoma,was diagnosed with a jejunal GIST at the 4-year follow-up thoraco-abdominal CT scan.He was referred to our hub hospital for gastroenterological evaluation,and subsequently underwent 18-fluorodeoxyglucose positron emission tomography,abdominal magnetic resonance imaging,and video capsule endoscopy.None of these examinations detected any lesion of the small intestine.After reviewing all the CT images in a multidisciplinary setting,the panel estimated that the hyperdense jejunal image was consistent with a tablet rather than a GIST.The tablet was an 800 mg delayed-release enteric-coated oral mesalamine tablet(Asacol®),which had been prescribed for non-specific colitis,while not informing the hospital physicians.CONCLUSION Delayed-release oral mesalamine(Asacol®),like other enteric-coated medications,can appear as a hyperdense image on a CT scan,mimicking a small intestinal GIST.Therefore,adetailed knowledge of the patients’medications and a multidisciplinary review of the images areessential.

Energy analysis of a wood or pellet stove in a single-family house equipped with gas boiler and radiators

In the residential sector,biomass appliances are widely used for space heating and often combined with other systems.This work aims at comparing the final and primary energy consumption of different configurations,including a conventional and a ducted pellet stove and a wood log stove using air as heat transfer fluid.A dynamic analysis of the interaction between biomass stoves and conventional heating systems,such as gas boilers and radiators,is carried out within a typical single-family house in a mild climate,using TRNSYS software.In addition,natural ventilation of the building is considered using CONTAM,with a focus on external infiltrations and internal air circulation due to the buoyancy effect.Results show that the biomass device in one room promotes the airflows between adjacent thermal zones,enhancing the heat distribution through door openings,in particular when an air ducted stove is present.The final energy consumption resulting from simulations with wood-burning stoves is 21%higher than pellet stoves.The pellet stove results in similar final energy and a 30%increase in overall primary energy,while the wood stove increases the final energy by 22%and approximately 40%of overall primary energy compared to the case of a traditional gas system coupled to radiators which is considered as reference.Nevertheless,non-renewable primary energy savings are higher than 50%with pellet stoves and 60%with wood-log stoves.

Hard and soft tissue augmentation with occlusive titanium barriers in jaw vertical defects:a novel approach

Aim:The aim of this study was to evaluate hard and soft tissue changes following guided bone regeneration(GBR),with occlusive titanium barriers(OTB),in the rehabilitation of partially edentulous atrophic jaws.Vertical bone gain(VBG),horizontal bone width(HBW),and flap thickness(FT)changes,observed between the first and the second surgical stages,were evaluated.Methods:The study included 35 patients(9 men and 26 women;mean age 60±10.53 years)in need of vertical bone augmentation for implant placement.Seventy implants were placed,44 with a one-stage approach(Group A)and 26 with a two-stage approach(Group B).VBG,HBW,and FT were measured and statistically compared.Results:VBG for implants placed in Group B was significantly higher than those placed in Group A(P=0.006).The increased HBW in Group B was highly significant compared to that exhibited in Group A(P=0.000).A highly significant difference was found in FT before and after the GBR in the two groups considered together,for both the upper and lower jaws(P=0.000 for both).Conclusions:OTBs are reliable devices in GBR,yielding predictable results in terms of bone augmentation.In almost all cases(94.3%),a spontaneous increase of the FT,at the second surgical phase,was observed.This could be due to the titanium surface features which increases spontaneously the thickness of soft tissues over the OTB.

Direct oral anticoagulants for the treatment of splanchnic vein thrombosis:A state of art

Splanchnic vein thrombosis(SVT)is a manifestation of venous thromboembolism in an unusual site.Portal,mesenteric,and splenic veins are the most common vessels involved in SVT which occurs mainly in patients with liver cirrhosis,although non-cirrhotic patients could be affected as well.Thrombosis of hepatic veins,also known as Budd-Chiari syndrome,is another manifestation of SVT.Prompt diagnosis and intervention are mandatory in order to increase the recalization rate and reduce the risk of thrombus progression and hypertensive complications.Traditional anticoagulation with heparin and vitamin-K antagonists is the treatment of choice in these cases.However,recent studies have shown promising results on the efficacy and safety of direct oral anticoagulants(DOACs)in this setting.Available results are mainly based on retrospective studies with small sample size,but first clinical trials have been published in the last years.This manuscript aims to provide an updated overview of the current evidence regarding the role of DOACs for SVT in both cirrhotic and non-cirrhotic patients.
