Clinical Analysis of a Coaxial Dilator Set Attached to Needle Puncture for Percutaneous Nephrolithotomy

Introduction: Percutaneous approach to the kidney is a very useful alternative in renal surgery which minimizes the morbidity of a variety of procedures. Objectives: To evaluate the efficacy, practicity of a reusable system of metallic coaxial dilator coupled to a special puncture needle to perform lumbar puncture and dilation of the percutaneous tract. Methods: A randomized experimental study was carried out involving 50 individuals. These patients were randomized as follows: Group 1 had the procedure performed with the conventional disposable Amplatz set traditionally used for this procedure. Patients in Group 2 were operated utilizing the new reusable coaxial set specially designed for this purpose. The following parameters were measured to compare the 2 groups: sex Corporeal Mass Index (CMI), Pre and postoperative hematocrit and hemoglobin. Time interval between the puncture, dilation and access to the renal pelvis and insertion of the nephroscope were also analyzed. Results: There was no statistical difference between the 2 groups concerning hematocrit and hemoglobin changes when comparing prewith postoperative period. Patients in Group 2 required a significant lower time between puncture and final access to the upper collecting system. Conclusion: The new coaxial dilator set showed to be as safe as the conventional Amplatz set with the advantage of reusability and decrease of tract dilation time.

The Aging Bladder in Females Evaluated by Urodynamics

Aim: To determine whether bladder functions deteriorate with age. Methods: Data contained in electronic medical record (INFOMED?) were used in this institutional retrospective review. Analysis was done on the urodynamic studies in women over 18 years old conducted between May 2011 and November 2015. Patients with previous history of pelvic surgery or radiotherapy, neurological disease, vaginal prolapse greater than grade I, congenital urogenital malformations, urinary obstructive disease, diabetes, or the use of any medication that could interfere with bladder function were excluded from the analysis. The urodynamic parameters analyzed were the Maximum Cystometric Capacity (MCC), Voiding Volume (VV), Maximum Flow (Qmax), Bladder Compliance (BC), detrusor Pressure at Maximum Flow (PdetQmax), Bladder Contractility Index (BCI), Bladder Voiding Efficiency (BVE) and Post-Void Residual Urine Volume (PVR). Patients were further stratified in five groups according to age (A—18 to 40;B—41 to 50;C—51 to 60;D—61 to 70;E—over 70 years old). Results: Out of 3103 urodynamic studies analyzed, 719 were eligible for the study. The average age of patients was 49.3 (+13.2) years old and in all evaluated parameters, statistically significant correlation between age and decline of bladder function was obtained (p Conclusions: This study showed a decline in bladder storage function (reduction in MCC and BC) and in bladder emptying function (reduction in Qmax, PdetQmax, VV, BCI and BVE with an increase in PVR) with age.

AIM： To evaluate the potential of S-nitroso-N-acetylcysteine （SNAC） in inhibition of lipid peroxidation and the effect of oral SNAC administration in the prevention of nonalcoholic fatty liver disease （NAFLD） in an animal model.METHODS： NAFLD was induced in Wistar male rats by choline-deficient diet for 4 wk. SNAC-treated animals （n=6）（1.4 mg/kg/day of SNAC, orally） were compared to 2 control groups： one （n=6） received PBS solution and the other （n=6） received NAC solution （7 mg/kg/d）. Histological variables were semiquantitated with respect to macro and microvacuolar fat changes, its zonal distribution, foci of necrosis, portal and perivenular fibrosis, and inflammatory infiltrate with zonal distribution. LOOHs from samples of liver homogenates were quantified by HPLC. Nitrate levels in plasma of portal vein were assessed by chemiluminescence. Aqueous low-density lipoprotein （LDL） suspensions （200 pg protein/mL） were incubated with CuCl2 （300 μmol/L） in the absence and presence of SNAC （300 μmol/L） for 15 h at 37 ℃ Extent of LDL oxidation was assessed by fluorimetry. Linoleic acid （LA）（18.8 μmol/L） oxidation was induced by soybean lipoxygenase （SLO）（0.056 μmol/L） at 37 ℃ in the presence and absence of N-acetylcysteine （NAC） and SNAC （56 and 560 pmol/L） and monitored at 234 nm. RESULTS： Animals in the control group developed moderate macro and microvesicular fatty changes in periportal area. SNAC-treated animals displayed only discrete histological alterations with absence of fatty changes and did not develop liver steatosis. The absence of NAFLD in the SNAC-treated group was positively correlated with a decrease in the concentration of LOOH in liver homogenate, compared to the control group （0.7±0.2 nmol/mg vs 3.2±0.4 nmol/mg protein, respectively, P〈0.05）, while serum levels of aminotransferases were unaltered. Exploring overweight and obesity beyond body mass index:A body composition analysis in people with and without patellofemoral pain

Background:We compared body mass index(BMI),body fat,and skeletal muscle mass between(1) a mixed-sex nonathletic cohort of people with patellofemoral pain(PFP) and pain-free people,and(2) a nonathletic cohort of people with PFP and pain-free people subgrouped by sex(i.e.,men and women with PFP ***-free men and women).Methods:This cross-sectional study included 114 people with PFP(71 women,43 men) and 54 pain-free controls(32 women,22 men).All participants attended a single testing session to assess body composition measures,which included BMI,percentage of body fat(%BFBioimpedance),and skeletal muscle mass(both assessed by bioelectrical impedance analysis),and percentage of body fat(%BFskinfoid)(assessed by skinfold caliper analysis).A one-way univariate analysis of covariance(age and physical activity levels as covariates) was used to compare body composition measures between groups(i.e.,PFP ***-free group;women with PFP ***-free women;men with PFP ***-free men).Results:Women with PFP presented significantly higher BMI,%BFBiompedance,and %BFSkinfold,and lower skeletal muscle mass compared to pain-free women(p≤0.04;effect size:-0.47 to 0.85).Men with PFP and men and women combined had no differences in BMI,%BFBioimpedance,%BFSkinfold,and skeletal muscle mass compared to their respective pain-free groups(p> 0.05).Conclusion:Our findings indicate that BMI and body composition measures should be considered as part of the evaluation and management of people with PFP,especially in women,who have demonstrated higher BMI and body fat and lower skeletal muscle mass compared to pain-free *** studies should not assess body composition measures in a mixed-sex population without distinguishing men participants from women participants.

High-throughput phenotyping of two plant-size traits of Eucalyptus species using neural networks

In forest modeling to estimate the volume of wood,artificial intelligence has been shown to be quite effi-cient,especially using artificial neural networks(ANNs).Here we tested whether diameter at breast height(DBH)and the total plant height(Ht)of eucalyptus can be pre-dicted at the stand level using spectral bands measured by an unmanned aerial vehicle(UAV)multispectral sensor and vegetation *** do so,using the data obtained by the UAV as input variables,we tested different configurations(number of hidden layers and number of neurons in each layer)of ANNs for predicting DBH and Ht at stand level for different Eucalyptus *** experimental design was randomized blocks with four replicates,with 20 trees in each experimental *** treatments comprised five Eucalyptus species(***,***,***,***-dis,and ***)and Corymbria *** and Ht for each plot at the stand level were measured seven times in separate overflights by the UAV,so that the multispectral sensor could obtain spectral bands to calculate vegetation indices(VIs).ANNs were then constructed using spectral bands and VIs as input layers,in addition to the categorical variable (species), to predict DBH and Ht at the stand level simultaneously. This report represents one of the first appli-cations of high-throughput phenotyping for plant size traits in Eucalyptus species. In general, ANNs containing three hidden layers gave better statistical performance (higher esti-mated r, lower estimated root mean squared error-RMSE) due to their greater capacity for self-learning. Among these ANNs, the best contained eight neurons in the first layer, seven in the second, and five in the third (8 − 7 − 5). The results reported here reveal the potential of using the gener-ated models to perform accurate forest inventories based on spectral bands and VIs obtained with a UAV multispectral sensor and ANNs, reducing labor and time.

Main visual symptoms associated with refractive errors and spectacle need in a Brazilian population

AIM： To determine the main visual symptoms in a Brazilian population sample, associated to refractive errors（REs） and spectacle need to suggest priorities in preventive ***： A cross-sectional study was conducted in nine counties of the southeast region of Brazil, using a systematic sampling of households, between March 2004 and July 2005. The population was defined as individuals aged between 1 and 96 y, inhabitants of 3600 residences to be evaluated and 3012 households were included,corresponding to 8010 subjects considered for participation in the survey, of whom 7654 underwent ophthalmic examinations. The individuals were evaluated according their demographic data, eye complaints and eye examination including the RE and the need to prescribe spectacles according to age. Statistical analysis was performed using SPSS software package and descriptive analysis using 95% confidence intervals（P〈0.05）. RESULTS： The main symptom detected was asthenopia, most frequent in the 2ndand 3rddecades of life, with a significant decline after the *** was the RE most associated with asthenopia. Reduced near vision sight was more frequent in those ≥40y with a progressive decline *** were most frequently required in subjects of≥40 years of ***： The main symptom related to the vision was asthenopia and was associated to astigmatism. The greatest need for spectacles prescription occurred after40＇s, mainly to correct near vision. Subjects of ≥40years old were determined to be at high risk of uncorrected REs. These observations can guide intervention programs for the Brazilian population.

Fungal endophytic species inhabiting the leaves of eucalypts are capable of utilising leaf sugars and can influence both plant growth and health. Endophytic fungal symbionts can use simple soluble sugars in leaves as their main carbon source. This study set out to determine the diversity and distribution of the endophytic filamentous fungal leaf community in the hybrid Eucalyptus urograndis due to its economic importance. The fungal leaf community was characterised using denaturing gradient electrophoresis (DGGE) and correlated with levels of leaf nutrients and sugars throughout plant development. Sequencing of DGGE bands revealed the presence of Basidiomycota and Ascomycota phyla. Fourteen species and three genera of filamentous fungi were identified, and the population structure was affected by the plant developmental stage. Levels of K, Cu, N and Mn influenced communities from the clonal garden, whereas leaves in the field had higher glucose, fructose and sucrose. Many fungi were found to be specific to a certain development stages: Diplomitoporus crustulinus, Podosphaera tridactyla and Aspergillus restrictus to the clonal garden stage;Chaetomella acutiseta and Ascotricha chartarum to the shading stage;Erratomyces patelii and Saxomyces sp. to the shading output stage;Lepidostroma sp. and Saxomyces sp. to the dispatch stage;and Mycosphaerella populicola to the field stage. Teratosphaeria toledana and Teratosphaeria acidotherma were found at more than one developmental stage. Cladosporium sp. and Rhodosporidium fluviale colonized and persisted in plants at the dispatch and field stages. This is the first report of P. tridactyla, A. restrictus, E. patelii, Saxomyces and Lepidostroma sp. as endophytes in eucalipt.
