Laparoscopic approach to Meckel's diverticulum

AIM: To retrospective review the laparoscopic management of Meckel Diverticulum(MD) in two Italian Pediatric Surgery ***: Between January 2002 and December 2012, 19 trans-umbilical laparoscopic-assisted(TULA) procedures were performed for suspected MD. The children were hospitalized for gastrointestinal bleeding and/or recurrent abdominal pain. Median age at diagnosis was 5.4 years(range 6 mo-15 years). The study included 15 boys and 4 girls. All patients underwent clinical examination, routine laboratory tests, abdominal ultrasound and technetium-99m pertechnetate scan, and patients with bleeding underwent gastrointestinal endoscopy. The abdominal exploration was performed with a 10 mm operative laparoscope. Pneumoperitoneum was establishedbased on the body weight. Systematic overview of the peritoneal cavity allowed the ileum to be grasped with an atraumatic instrument. The complete exploration and surgical treatment of MD were performed extracorporeally, after intestinal exteriorization through the umbilicus. All patients' demographics, main clinical features, diagnostic investigations, operative time, histopathology reports, conversion rate, hospital stay and complications were registered and ***: MD was identified in 17 patients, while 1 had an ileal duplication and 1 a jejunal hemangioma. Fifteen patients had painless intestinal bleeding, while 4 had recurrent abdominal pain and exhibited cyst like structures in an ultrasound study. Eleven patients had a positive technetium-99m pertechnetate scan. In the patients with bleeding, gastrointestinal endoscopy did not name the source of hemorrhage. All patients were subjected to a TULA surgical procedure. An intestinal resection/anastomosis was performed in 14 patients, while 4 had a wedge resection of the diverticulum and 1 underwent stapling diverticulectomy. Morphologic changes due to human chorionic gonadotropin in the rat testis: Role of vascular endothelial growth factor

Introduction: Human chorionic gonadotropin (hCG) has an important role in the regulation of reproductive system and a high capacity to stimulate angiogenesis by secretion of vascular endothelial growth factor (VEGF). It could be also used for medical treatment of cryptorchidism, as it stimulates Leydig cells to produce testosterone, but its use has been discussed. Aim of the study was to evaluate the testicular morphological changes due to cyclic administration of hCG possibly related to VEGF effect. Methods: 80 pre-puberal male Sprague-Dawley rats were randomly divided in two groups hCG (50 U/I/ Kg) (n = 55/S) and placebo (n = 25/C) treated. The animals were sacrificed respectively after one (S1 n = 10;C1 n = 5), two (S2 n = 10;C2 n = 5) and three weeks of hCG or placebo treatment (S3 n = 35;C3 n = 15);a blood serum was obtained for evaluation of plasmatic testosterone. The testis, prostates, epididymides and seminal vesicles were drawn and weighted. Specimens were fixed in formalin and coloured by hematoxylin-eosin. Morphometric analysis of Leydig cell number, nuclear area and perimeter was carried out. Immunoistochemistry for VEGF has been performed with the avidin-biotin peroxidase technique. Statistical differences between groups, each of different age, were calculating using Student’s T-test. Results: After the first week of hCG treatment it has been observed an increased testicular weight and volume (+/﹣6%) in experimental respect to control group. This result was not observed after the second and third week of treatment. Significant variations were also observed in the mean prostate weight, whereas mean seminal vesicles weight was significant in the S2 and S3 groups respect to control. In the hCG treated rats it has been observed a poor differentiation of the seminiferous epithelium, with high Leydig cell evidence and increased intertubular eosinophilic material. Expression of Subjectivity in Media Genres:Linguistic and Semiotic Analysis of Media Texts on the Example of the English and Georgian Language Quality Newspapers

The study explores linguistic and media genre characteristics of the British, U.S. and Georgian print media. Theoretical apparatus of media studies and other interdisciplinary linguistic fields were employed for comparative analysis of genre characteristics. The paper is part of a longitudinal study of print media genres over the period of 2002-2010. The aim of the research is to(a) define and compare genre characteristics of the British, U.S. and Georgian print media,(b) examine and define structural and linguistic(semantic, pragmatic, semiotic) characteristics of the British, U.S. and Georgian quality newspaper genres,(c) define deictic composition of newspaper articles and(d) study expression of coded meanings in media texts. In this paper, I will focus on two major genres of quality print media: news and features. The media genres are analysed within the theoretical framework of pragmatics, semantics, semiotics and media studies.

Perception of Myth in Print and Online Media

While analyzing media texts, linguists usually distinguish between denotation and connotation of linguistic signs. Barthes(Rayner, Wall & Kruger, 2001) has added another term "myth". This term is used by a number of scholars and was first mentioned in Barthes’ essay "myth today"(Bignell, 1997;Bignell, 2002;Turner, 2005). Myth is a system of signifiers, a semiological system, where social, historical and cultural meanings are connected with a sign(Turner, 2005;Rayner, Wall & Kruger, 2001;Barthes in Bignell, 1997;Gaines, 2001). In this paper, we will discuss a meaning of myth in media, its perception through media in print and online journalism. The paper provides an analysis of the case of myth creation and the characteristics of narration in print and online news. One of the main features of online newspapers is non-linear model of narration and nonlinear perception of information by readers.

Application of laser scanning for rock mass characterization and discrete fracture network generation in an underground limestone mine

Terrestrial laser scanning(TLS) is a useful technology for rock mass characterization. A laser scanner produces a massive point cloud of a scanned area, such as an exposed rock surface in an underground tunnel,with millimeter precision. The density of the point cloud depends on several parameters from both the TLS operational conditions and the specifications of the project, such as the resolution and the quality of the laser scan, the section of the tunnel, the distance between scanning stations, and the purpose of the scans. One purpose of the scan can be to characterize the rock mass and statistically analyze the discontinuities that compose it for further discontinuous modeling. In these instances, additional data processing and a detailed analysis should be performed on the point cloud to extract the parameters to define a discrete fracture network(DFN) for each discontinuity set. I-site studio is a point cloud processing software that allows users to edit and process laser scans. This software contains a set of geotechnical analysis tools that assist engineers during the structural mapping process, allowing for greater and more representative data regarding the structural information of the rock mass, which may be used for generating DFNs. This paper presents the procedures used during a laser scan for characterizing discontinuities in an underground limestone mine and the results of the scan as applied to the generation of DFNs for further discontinuous modeling.

Power Spectrum in the Conductive Terrestrial Ionosphere

Stochastic differential equation of the phase fluctuations is derived for the collision conductive magnetized plasma in the polar ionosphere applying the complex geometrical optics *** second order statistical moments it was shown that the contribution of the longitudinal conductivity substantially exceeds both Pedersen and Hall’s *** observing the broadening of the spatial power spectrum of scattered electromagnetic waves which equivalent to the brightness is analyzed for the elongated ionospheric *** was shown that the broadening of the spectrum and shift of its maximum in the plane of the location of an external magnetic field(main plane)less than in perpendicular plane for plasmonic structures having linear scale tenth of kilometer;and substantially depends on the penetration angle of an incident wave in the conductive collision turbulent magnetized ionospheric *** angle-of-arrival(AOA)in the main plane has the asymmetric Gaussian form while in the perpendicular plane increases at small anisotropy factors and then tends to the saturation for the power-low spectrum characterizing electron density *** conductivity fluctuations increase the AOAs of scattered radiation than in magnetized plasma with permittivity *** of the temporal spectrum containing the drift velocity of elongated ionospheric irregularities in the polar ionosphere allows to solve the reverse problem restoring experimentally measured velocity of the plasma streams and characteristic linear scales of anisotropic irregularities in the terrestrial ionosphere.

Morphodynamic processes of the Elbe River estuary, Germany： the Coriolis effect, tidal asymmetry and human dredging

The Digital Elevation Model （DEM） based on the historical sea-charts and on-site hydrological records were used to examine the morphological change of the Elbe River estuary. The results show that siltation predominated in the tidal flat in the northern estuary, with a net siltation rate of 1.8 cm· a^-1 during 1927-2006. In contrast, a continuous erosion prevailed in the main river channel, south of the estuary, with a net erosion rate of 2.5 cm· a^-1 in the same time. In addition, a seaward shift of the estuarine island has happened with the old island coalescing to the northern tidal flat and new one emerging through siltation process. The tidal asymmetry via ebbing flow （maximum at 140 cm· s^-1, and average at 76 cm· s^-1） prevailed in the tidal flat, meaning continuous aggradation northwestward, while flooding flow （maximum at 100 cm · s^-1, and average at 67 cm· s^-1） dominated in the main fiver channel with deepening thaweg at south, showing a landward sedimentation via the tidal pumping processes. This dextral extension of the estuarine morphology is due to the Coriolis force, leading to the inconsistent directions of in-out flows, which enables to facilitate the estuarine siltation. Human dredging prevailing in the estuary has dramatically altered the nature of the silted river channel to erosional since the last century net erosion rate of 3.2 cm· a^-1 This is characterized by a derived from the DEMs mapping, but only partially accounting for the dredging amount of 1994-2006, when the total dredging volume was 67 × 10^6m^3, equal to 5.9 cm·a^-1.

It is widely known that very low frequency/low frequency(VLF/LF)radiation has been recorded prior to large earthquakes in several seismically active countries of the *** networks employed to detect this radiation consist of stationary transmitters and ***,there are reported cases of existing networks being unable to detect any electromagnetic radiation prior to large *** this study,we determined the optimal arrangement of a mobile VLF/LF electromagnetic radiation network to ensure the detection of an upcoming earthquake *** considered the possible arrangements of the VLF/LF mobile network based on certain physical considerations,and developed a relatively simple arrangement that is completely different from the existing stationary *** suggested design will significantly increase the number of detected/predicted earthquakes by using the relevant electromagnetic radiation receivers strategically placed in regions of increased tectonic and seismic activity.
