Once the ascending aorta gets involved and however farther the dissection extends, it is named type Adissection （TAD）, in which the entry tear usually locates in the ascending aorta. But there exists a small subset with primary entry tear in the descending aorta and the dissection process extends back into the ascending aorta, which here we name primary retrograde type A dissection （PRTAD）. Operative procedures are far more difficult and extensive for PRTAD and usually result in poor prognosis in the aspects of surgical mortality and survival rate during follow-up.1-3 Kato et al4 and Chang et al5 showed successful method of endovascular repair for PRTAD and recommended it as an alternative to the operative treatment. During March 2008 to August 2010, five continuous patients with PRTAD were admitted into our institution. We here introduce our preliminary experience with medical therapy and thoracic endovascular aortic repair （TEVAR） for treating PRTAD.

With improvements in the technology of clinical diagnosis of aortic diseases, more and more cases have been found on such vascular lesions as aortic dissection and abdominal aortic aneurysm, which lead to aortic biomechanical changes. Aortic biomechanics research will become a hot spot in the near future. At the same time, stent grafts （SGs） are widely used in the endovascular treatment of vascular diseases. The biomechanical properties of SGs will also become a focus of research. Understanding the relation between the aorta and SGs will better cater to clinical services by improving the success rate of surgery for aortic diseases and reducing intraoperative and postoperative complications. This article reviews researches on the biomechanics of the aorta and SGs.

Background Acute kidney injury （AKI） is considered as a common and significant complication following abdominal aortic aneurysm （AAA） repair. This study aimed to assess the associated risk factors of AKI in the critically ill patients undergoing AAA repair and to evaluate the appropriate AKI management in the specific population. Methods We retrospectively examined data from all critically ill patients undergoing AAA repairs at our institution from April 2007 to March 2012. Multivariable analysis was used to identify factors associated with postoperative AKI, which was defined by risk, injury, failure, loss and end-stage （RIFLE） kidney disease criteria. The goal-directed hemodynamic optimization （maintenance of optimal hemodynamics and neutral or negative fluid balance） and renal outcomes were also reviewed. Results Of the 71 patients enrolled, 32 （45.1%） developed AKI, with 30 （93.8%） cases diagnosed on admission to surgical intensive care unit （SICU）. Risk factors for AKI were ruptured AAA （odds ratio （OR）=5.846, 95% confidence interval （CI）： 1.346-25.390）, intraoperative hypotension （OR=6.008, 95% CI： 1.176 to 30.683）, and perioperative blood transfusion （OR=4.611, 95% CI： 1.307-16.276）. Goal-directed hemodynamic optimization resulted in 75.0% complete and 18.8% partial renal recovery. Overall in-hospital mortality was 2.8%. AKI was associated with significantly increased length of stay （（136.9±24.5） hours vs. （70.4±11.3） hours） in Surgical Intensive Care Unit. Conclusions Critically ill patients undergoing AAA repair have a high incidence of AKI, which can be early recognized by RIFLE criteria. Rupture, hypotension, and blood transfusion are the significant associated risk factors. Application of goal-directed hemodynamic optimization in this cohort appeared to be effective in improving renal outcome.

Background The development of regenerative therapies using derivatives of embryonic stem （ES） cells would be facilitated by a non-invasive method to monitor transplanted cells in vivo, for example, magnetic resonance imaging of cells labeled with superparamagnetic iron oxide （SPIO） nanoparticles. Although ES cells have been labeled with SPIO particles, the potential adverse effects of the label have not been fully examined. The objective of this study was to determine whether SPIO labeling affects routine ES cell viability, proliferation, or ability to differentiate into functional endothelial cells （ECs）. Methods Cross-linked iron oxide （CLIO, an SPIO） was conjugated with human immunodeficiency virus transactivator of transcription （HIV-Tat） peptides, and murine ES cells were labeled with either CLIO-Tat, CLIO, or HIV-Tat. After labeling ES cells were cultured for 4 days and FIk-1^＋ ES cells identified and sorted by immunocytochemistry and fluorescence activated cell sorting （FACS）. FIk-1^＋ cells were replated on fibronectin-coated dishes, and ECs were obtained by culturing these for 4 weeks in endothelial cell growth medium supplemented with vascular endothelial growth factor （VEGF）. ES cell viability was determined using trypan blue exclusion, and the proportion of SPIO^＋ cells was evaluated using Prussian blue staining and transmission electron microscopy. After differentiation, the behavior and phenotype of ECs were analyzed by reverse transcription-polymerase chain reaction, flow cytometry, immunocytochemistry, Dil-labeled acetylated low-density lipoprotein （AcLDL） uptake, and Matrigel tube formation assay. Results CLIO-Tat was a highly effective label for ES cells, with 〉96% of cells incorporating the particles, and it did not alter the viability of the labeled cells. ECs derived from CLIO-Tat^＋ ES cells were very similar to murine aortic ECs in their morphology, expression of endothelial cell markers, ability to form vascular-like channels, and scavenging of AcLDL from the culture medium. Background Although Multi-planar reconstruction （MPR） has been considered a diagnostic imaging technique that observes more perspectives for diseases,few people have applied it surgically.In fact,MPR is also very useful to clinical operation,especially for patients with type B aortic dissection.It helps the surgeon to locate accurately with more information about aortic dissection,so that the safety and effectiveness of operation can be improved.This study examined the application of the MPR in intraoperative DSA imaging for precise positioning by accurately obtaining a crosssection,a spin angle of the coronal plane,and a tilt angle of the sagittal plane in treatment of type B aortic dissection.Methods The conventional and the MPR approaches were compared on positioning the aortic arch for surgery.A group of 40 patients （group A） and another group of 42 patients （group B） was sampled.About the comparison of baseline characteristics,a fourfold table X2 test was conducted on gender,and two independent samples t-test was applied to age between group A and group B.Spin as well as tilt angles for group A were obtained from the patients using both approaches,and their effectiveness was compared with pair t-tests; The MPR data guided stent-grafting in this group.Stent graft placement of group B was based on the conventional approach.Percentages of proximal distributed markers as well as incidences of complications were collected from both groups after stent graft placement.They were also compared with a fourfold table X2 test.Results Gender difference was not found between group A and group B （X2=0.80,P ＞0.05）,and age difference was not statistically significant （F=2.55,homogeneity of variance,t=-1.46,P ＞0.05）.A significant difference was found between the conventional and the MPR approaches for spin angle （t=9.17） as well as tilt angle （t=-2.07）,P ＜0.05.Percentage of proximal distributed markers （5.0％） of group A was significantly lower than that of group B （42.9％）,X2=15.92,P ＜0.05;and incidence of complications （5.0％）

Internal photoemission is a prominent branch of the photoelectric effect and has emerged as a viable method for detecting photons with energies below the semiconductor bandgap.This breakthrough has played a significant role in accelerating the development of infrared imaging in one chip with state-of-the-art silicon techniques.However,the performance of these Schottky infrared detectors is currently hindered by the limit of internal photoemission;specifically,a low Schottky barrier height is inevitable for the detection of low-energy infrared photons.Herein,a distinct paradigm of Schottky infrared detectors is proposed to overcome the internal photoemission limit by introducing an optically tunable barrier.This device uses an infrared absorbing material-sensitized Schottky diode,assisted by the highly adjustable Fermi level of graphene,which subtly decouples the photon energy from the Schottky barrier height.Correspondingly,a broadband photoresponse spanning from ultraviolet to mid-wave infrared is achieved,with a high specific detectivity of 9.83×1010 cm Hz1/2 W−1 at 2,700 nm and an excellent specific detectivity of 7.2×109 cm Hz1/2 W−1 at room temperature under blackbody radiation.These results address a key challenge in internal photoemission and hold great promise for the development of the Schottky infrared detector with high sensitivity and room temperature operation.

Natural gas hydrate(NGH)can cause pipeline blockages during the transportation of oil and gas under high pressures and low temperatures.Reducing hydrate adhesion on pipelines is viewed as an efficient way to prevent NGH blockages.Previous studies suggested the water film can greatly increase hydrate adhesion in gas-dominant system.Herein,by performing the molecular dynamics simulations,we find in water-dominant system,the water film plays different roles in hydrate deposition on Fe and its corrosion surfaces.Specifically,due to the strong affinity of water on Fe surface,the deposited hydrate cannot convert the adsorbed water into hydrate,thus,a water film exists.As water affinities decrease(Fe>Fe_(2)O_(3)>FeO>Fe_(3)O_(4)),adsorbed water would convert to amorphous hydrate on Fe_(2)O_(3)and form the ordered hydrate on FeO and Fe_(3)O_(4)after hydrate deposition.While absorbed water film converts to amorphous or to hydrate,the adhesion strength of hydrate continuously increases(Fe

The global spread of severe acute respiratory syndrome coronavirus 2 has resulted in a significant number of individuals developing pulmonary fibrosis(PF),an irreversible lung injury.This condition can manifest within a short inter-val following the onset of pneumonia symptoms,sometimes even within a few days.While lung transplantation is a potentially lifesaving procedure,its limited availability,high costs,intricate surgeries,and risk of immunological rejection present significant drawbacks.The optimal timing of medication administration for coronavirus disease 2019(COVID-19)-induced PF remains controversial.Despite this,it is crucial to explore pharmacotherapy interventions,involving early and preventative treatment as well as pharmacotherapy options for advanced-stage PF.Additionally,studies have demonstrated disparities in anti-fibrotic treatment based on race and gender factors.Genetic mutations may also impact therapeutic efficacy.Enhancing research efforts on pharmacotherapy interventions,while considering relevant pharmacological factors and optimizing the timing and dosage of medication administration,will lead to enhanced,personalized,and fair treatment for individuals impacted by COVID-19-related PF.These measures are crucial in lessening the burden of the disease on healthcare systems and improving patients'quality of life.

Lignocellulose has the potential to become a bio-based adsorbent due to its biodegradability and renewability.In this study,a novel polydopamine functionalized-lignin(lignin@PDA),prepared via self-polymerization of dopamine(PDA)on lignin,was used as a bio-based adsorbent for rapid scavenging of hexavalent chromium(Cr(VI)).The morphology,functional groups,crystalline structure,and chemical composition of lignin@PDA were characterized with a scanning electron microscope-energy dispersive spectrometer,Fourier transform infrared spectroscopy,X-ray diffraction,and X-ray photoelectron spectroscopy.The Cr(VI)adsorption process of lignin@PDA was studied using batch experiments as a function of pH,ionic strength,adsorbent dose,and contact time at room temperature.The adsorption rate of lignin@PDA was five times greater than that of the unmodified lignin,with a maximum adsorption capacity of 102.6 mg/g in an acidic medium.The adsorption of Cr(VI)on lignin@PDA fit the pseudo-secondorder equation and the Freundlich model,indicating that the adsorption process was mainly dominated by chemisorption and surface complexation.The thermodynamic parameters showed that adsorption of Cr(VI)on lignin@PDA was an endothermic and spontaneous process.The X-ray absorption fine structure results showed that sorption and reduction of Cr(VI)into Cr(II)occurred simultaneously on lignin.Moreover,PDA coating not only improved the reactivity of lignin but also promoted the complete reduction of Cr(VI)by lignin.According to these results,polydopamine functionalized-lignin is a promising bio-based adsorbent for immobilization of Cr(VI)from wastewater.

Background： Intravascular ultrasound （IVUS） examination can provide useful information during endovascular stent graft repair. However, its actual clinical utility in thoracic endovascular aortic repair （TEVAR） for type B aortic dissection （type B-AD） remains unclear, especially in complicated aortic dissection. We evaluated the effect of IVUS as a complementary tool during TEVAR. Methods： From September 2011 to April 2012, we conducted a prospective cohort study of 47 consecutive patients with ＂complicated＂ type B-AD diagnosed. We divided the patients into two groups： 1VUS-assisted TEVAR group and TEVAR using angiography alone group. The general procedure of TEVAR was performed. We evaluated the per±operative and follow-up events. Patient demographics, comorbidities, preoperative images, dissection morphology, details of operative strategy, intraoperative events, and postoperative course were recorded. Results： A total of 47 patients receiving TEVAR were enrolled. Among them （females, 8.51%; mean age, 57.38 i 13.02 years）, 13 cases （27.66%） were selected in the IVUS-assisted TEVAR group, and 34 were selected in the TEVAR group. All patients were symptomatic. The average diameter values of IVUS measurements in the landing zone were greater than those estimated by computed tomography angiography （31.82 ± 4.21 mm vs. 30.64 ± 4.13 ram, P 〈 0.001）. The technique success rate was 100%. Among the postoperative outcomes, statistical differences were only observed between the IVUS-assisted TEVAR group and TEVAR group for total operative time and the amount of contrast used （P = 0.013 and P 〈 0.001, respectively）. The follow-up ranged from 15 to 36 months for the IVUS-assisted TEVAR group and from 10 to 35 months for the TEVAR group （P = 0.646）. The primary endpoints were no statistical difference in the two groups. Conclusions： Intraoperative IVUS-assisted TEVAR is clinically feasible and safe.
