Objective Previous studies have shown that meteorological factors may increase COVID-19 mortality,likely due to the increased transmission of the ***,this could also be related to an increased infection fatality rate(IFR).We investigated the association between meteorological factors(temperature,humidity,solar irradiance,pressure,wind,precipitation,cloud coverage)and IFR across Spanish provinces(n=52)during the first wave of the pandemic(weeks 10–16 of 2020).Methods We estimated IFR as excess deaths(the gap between observed and expected deaths,considering COVID-19-unrelated deaths prevented by lockdown measures)divided by the number of infections(SARS-CoV-2 seropositive individuals plus excess deaths)and conducted Spearman correlations between meteorological factors and IFR across the *** We estimated 2,418,250 infections and 43,237 *** IFR was 0.03%in<50-year-old,0.22%in 50–59-year-old,0.9%in 60–69-year-old,3.3%in 70–79-year-old,12.6%in 80–89-year-old,and26.5%in≥*** did not find statistically significant relationships between meteorological factors and adjusted ***,we found strong relationships between low temperature and unadjusted IFR,likely due to Spain’s colder provinces’aging *** The association between meteorological factors and adjusted COVID-19 IFR is *** age differences or ignoring COVID-19-unrelated deaths may severely bias COVID-19 epidemiological analyses.

“Time flies over us,but leaves its shadow behind”(Nathaniel Hawthorne,1860,The Marble Faun).The shadow that separates the latest guidance on vascular liver disorders of the American Association for the Study of Liver Diseases(AASLD)from its former counterpart is 12 years long(1,2).Along the course,several guidelines from different societies have been published on this topic(3-7).As acknowledged in the preamble,the lack of high-quality evidence in the field led the AASLD to commission a guidance from an expert panel based on formal review and analysis of the *** thus differs from other guidelines that perform systematic reviews with explicit methods of searching,selection,and rating the quality of evidence and,if appropriate,meta-analysis on certain clinical questions(2).

Modelling the probability of erroneous negative lymph node staging in patients with colon cancer

Background:Patients in who with insufficient number of analysed lymph nodes(LNs)are more likely to receive an incorrect LN *** ability to calculate the overall probability of undiagnosed LN involvement errors in these patients could be very useful for approximating the real patient prognosis and for giving possible indications for adjuvant *** objective of this work was to establish the predictive capacity and prognostic discriminative ability of the final error probability(FEP)among patients with colon cancer and with a potentially incorrectly-staged LN-negative ***:This was a retrospective multicentric population study carried out between January 2004 and December *** used a mathematical model based on Bayes’theorem to calculate the probability of LN involvement given a FEP test *** sum graphs were used to calculate risk groups and the survival rates were calculated,by month,using the Kaplan-Meier ***:A total of 548 patients were analysed and classified into three risk groups according to their FEP score:low-risk(FEP15%).Patients with LN involvement had the lowest overall survival rate when compared to the three risk *** difference was statistically significant for the low-and intermediate-risk groups(P=0.002 and P=0.004,respectively),but high-risk group presented similar survival curves to pN+group(P=0.505).In terms of disease-free survival,the high-risk group presented similar curves to the intermediate-risk group until approximately 60 months’follow-up(P=0.906).After 80 months’follow-up,the curve of high-risk group coincided with that of the pN+group(P=0.172).Finally,we summarized the FEP according to the number of analysed LNs and accompanied by a contour plot which represents its calculation ***:The application of Bayes’theorem in the calculation of FEP is useful to delimit risk subgroups from among patients without LN i

Impact of an acute hemodynamic response-guided protocol for primary prophylaxis of variceal bleeding

AIM To evaluate the long-term outcome of an acute hemodynamic response-guided protocol in which acute responders to intravenous propranolol received traditional nonselective beta-blockers(NSBBs) and acute nonresponders received *** Retrospective review of a protocol for primary prophylaxis of variceal bleeding guided by the acute hemodynamic response to intravenous propranolol. Fifty-two acute responders treated with traditional NSBB(i.e. propranolol or nadolol) were compared with 24 acute nonresponders receiving carvedilol. A second hemodynamic study was performed in 27 and 13 patients, respectively. The primary endpoint was development of first or further decompensation. Secondary endpoints included death from any cause, association between acute and chronic hemodynamic response, and baseline clinical and laboratory variables related to the acute hemodynamic *** Acute responders and acute nonresponders presented similar 1, 2, and 3-year probabilities of first decompensation(NSBB: 0%, 13.7%, 26.1% vs carvedilol: 0%, 20%, 20%, P = 0.968) or further decompensation(21.2%, 26.1%, 40.9% vs 21.2%, 50.0%, 50.0%, P = 0.525). A previous episode of hepatic encephalopathy was the only independent predictor of decompensation [hazard ratio(95% confidence interval): 8.03(2.76-23.37)]. Mortality rates were similar in acute responders and acute nonresponders with compensated(P = 0.428) or decompensated cirrhosis(P = 0.429). No clinical, laboratory, endoscopic or hemodynamic parameter predicted the acute hemodynamic response. In patients receiving traditional NSBB, the acute and chronic changes of hepatic venous pressure gradient were correlated(r = 0.59, P = 0.001). A case about a HIV woman with nasal ulcers is described in this paper. In every inmunodepressed patient who has mu-cosal or cutaneous ulcers, infection by leishmanial parasites needs to be ruled out, especially when ulcers have not re-gression with usual treatments. The Leishmanial nasal disease usually shows swelling and mucosal ulcers, that may progress to necrosis. Delaying in appropriate therapy might cause irreversible damage.

Background:Astrocytes play an essential role in neuroinflammation and are involved in the pathogenesis of neurodenegerative *** of glial fibrillary acidic protein(GFAP),an astrocytic damage marker,may help advance our understanding of different neurodegenerative *** this study,we investigated the diagnostic performance of plasma GFAP(pGFAP),plasma neurofilament light chain(pNfL)and their combination for frontotemporal dementia(FTD)and Alzheimer's disease(AD)and their clinical utility in predicting disease ***:pGFAP and pNfL concentrations were measured in 72 FTD,56 AD and 83 cognitively normal(CN)participants using the Single Molecule Array *** the 211 participants,199 underwent cerebrospinal(CSF)analysis and 122 had magnetic resonance *** compared cross-sectional biomarker levels between groups,studied their diagnostic performance and assessed correlation between CSF biomarkers,cognitive performance and cortical *** prognostic performance was investigated,analyzing cognitive decline through group comparisons by ***:Unlike pNfL,which was increased similarly in both clinical groups,pGFAP was increased in FTD but lower than in AD(all P<0.01).Combination of both plasma markers improved the diagnostic performance to discriminate FTD from AD(area under the curve[AUC]:combination 0.78;pGFAP 0.7;pNfL 0.61,all P<0.05).In FTD,pGFAP correlated with cognition,CSF and plasma NfL,and cortical thickness(all P<0.05).The higher tertile of pGFAP was associated with greater change in MMSE score and poor cognitive outcome during follow-up both in FTD(1.40 points annually,hazard ratio[HR]3.82,P<0.005)and in AD(1.20 points annually,HR 2.26,P<0.005).Conclusions:pGFAP and pNfL levels differ in FTD and AD;and their combination is useful for distinguishing between the two *** could also be used to track disease severity and predict greater cognitive decline during follow-up in patients with FTD.

In recent years the development of chitosan (CH) based materials as useful adsorbent polymeric matrices is an expanding field in the area of adsorption science. Even though CH has been successfully used for dye removal from aqueous solutions due to its low cost, no considerations have been made about, for example, the effect of changing the pH of chitosan hydrogelor about the dehydrating effect of Ethanol (EtOH) treatment of chitosan film on the dyes removal from water. Consequently in our laboratory we carried out a study focusing the attention, mainly, on the potential use of CH films under different conditions, such as reducing the intrinsic pH, increasing the hydrophobic character by means of ethanol treatment and neutralization of CH films to improve their absorption power. Textile anionic dyes named Direct Red 83:1, Direct Yellow 86 and Direct Blue 78 have been studied with the aim of reducing the contact time of CH film in waste water improving the bleaching efficiency. Neutralized acid CH film and longtime dehydrated one result to be the better films in dye removal from water. Also the reduction of the CH solution acidity during the film preparation determines the decreasing of the contact time improving the results. The effect of initial dye concentration has been examined and the amount of dye adsorption in function of time t, qt (mg/cm2), for each analyzed film has been evaluated comparing the long term effect with the decoloration rate. A linear form of pseudo-first-order Lagergren model has been used and described. The best condition for removing all examined dyes from various dye solutions appears to be the dehydration of a novel projected CH film obtained by means of the film immersion in EtOH for 4 days. Also CH films prepared by well-known literature procedure and neutralized with NaOH treatment appear having an excellent behavior, however the film treatment requires a large quantity of water and time.
