Enhance extemal counterpulsation （ECP） procedure has exhbited itself to be an effective therapy for the m anagem entof ischem ic card iovascu lar diseases, However, considering that EECP significantly increases the acute diastolic pressure, whether it will intervene in the chronic progression of advanced plaque causing great concern in clilical applkation, but yet rein ains elusive presently. In the current paper, a flu id-structure interface （FSI） num erical model of artery with p iaque corn ponent w as developed based on in vivo hem odynam ic m easurem entperfotm ed h a porcine model, to caku late the m echanical stresses of the plaque before and during EECP, and h lum to assess the potential effects of long-term EECP treatm ent on plaque progression. The resu Its show that E E C P augm ented the wall shear stress （WSS） and plaque w all stress （PWS） over the card lac cycles, aswell as the spacial oscillatory of W SS （WSSG ）. Durhg EECP treatm ent, the PW S level respectively raised 6.82. and 6.07% in two simulation cases. The currentpilot study suggests that E E C P treatm entre ay p lay a positive effect on inh biting the conthued plaque progression by hcreashg the PW S level over the card iac cycles. Meanwhile, the plaque morphology should be taken into consideration while m aking patient- specific plan for Ion g- term E E C P treatment in clinic.

目的:探讨婴儿运动表现测试(TIMP)联合全身运动评估(GMs)及头颅磁共振成像(MRI)对早产儿神经运动发育结局的预测价值。方法:选取12.例早产高危儿作为研究对象。对所有纳入对象在纠正胎龄40周时进行TIMP测试,在纠正胎龄46~56周时进行GMs测试,并在期间进行头颅MRI检测。对所有研究对象随访1年,并记录神经运动发育不良结局发生情况。比较三种检测方式的结果及预测准确度,采用Kappa法进行一致性分析,并采用受试者工作特征曲线(ROC)分析各检测方式对早产儿神经运动发育结局的预测价值。结果:共发生17例不良结局,其中4例脑瘫,13例神经运动发育迟缓,不良结局发生率为13.60%。TIMP测试结果、GMs测试、MRI检测结果均与神经发育结局相比一致性一般(0.4≤Kappa0.05)。经ROC曲线分析得知,TIMP测试的AUC为0.872.敏感度为0.882.特异度为0.861;GMs测试的AUC为0.755,敏感度为0.52.,特异度为0.981;MRI的AUC为0.839,敏感度为0.706,特异度为0.972.三者联合检测的AUC为0.931,敏感度为1.000,特异度为0.861。结论:TIMP、GMs及头颅MRI检测在预测早产儿神经运动发育结局方面均具有较高的预测价值,三者联合应用可提高对早产儿并发神经运动不良结局的预测价值。

Performance of Simplified Acute Physiology Score 3 in Predicting Hospital Mortality in Emergency Intensive Care Unit

Background： Since the 1980s, severity of illness scoring systems has gained increasing popularity in Intensive Care Units （ICUs）. Physicians used them for predicting mortality and assessing illness severity in clinical trials. The objective of this study was to assess the performance of Simplified Acute Physiology Score 3 （SAPS 3） and its customized equation for Australasia （Australasia SAPS 3, SAPS 3 [AUS]） in predicting clinical prognosis and hospital mortality in emergency ICU （EICU）. Methods： A retrospective analysis of the EICU including 463 patients was conducted between January 2.13.and December 2.15 in the EICU of peking university Third Hospital. The worst physiological data of enrolled patients were collected within 2. h after admission to calculate SAPS 3 score and predicted mortality by regression equation. Discrimination between survivals and deaths was assessed by the area under the receiver operator characteristic curve （AUC）. Calibration was evaluated by Hosmer-Lemeshow goodness-of fit test through calculating the ratio of observed-to-expected numbers of deaths which is known as the standardized mortality ratio （SMR）. Results： A total of 463 patients were enrolled in the study, and the observed hospital mortality was 2..1% （12./463）. The patients enrolled were divided into survivors and nonsurvivors. Age, SAPS 3 score, Acute Physiology and Chronic Health Evaluation Score 11 （APACHE 11）, and predicted mortality were significantly higher in nonsurvivors than survivors （P 〈 0.05 or P 〈 0.01 ）. The AUC （95% confidence intervals [C/s]） for SAPS 3 score was 0.836 （0.796-0.876）. The maximum of Youden＇s index, cutoff, sensitivity, and specificity of SAPS 3 score were 0.52.%, 70.5 points, 66.9%, and 85.7%, respectively. The Hosmer-Lemeshow goodness-of-fit test for SAPS 3 demonstrated a Chi-square test score of 10.2., P = 0.33, SMR （95% CI） = 0.63 （0.52.0.76）. We report the discovery of a peculiar X-ray transient,EP2.0408a,by Einstein Probe （EP） and follow-up studies made with EP,Swift,NICER,GROND,ATCA and other ground-based multiwavelength *** new transient was first detected with Wide-field X-ray Telescope （WXT） on board EP on April 8th,2.2.,manifested in an intense yet brief X-ray flare lasting for *** flare reached a peak flux of 3.9×10-9erg cm-2.-1in 0.5-4 keV,300 times brighter than the underlying X-ray emission detected throughout the *** and more precise follow-up observations by EP/FXT,Swift and NICER confirmed the finding of this new *** X-ray spectrum is non-thermal in 0.5-10 keV,with a power-law photon index varying *** X-ray light curve shows a plateau lasting for4d,followed by a steep decay till becoming undetectable10 d after the initial *** on its temporal property and constraints from previous EP observations,an unusual timescale in the range of 7-2. d is found for EP2.0408a,which is intermediate between the commonly found fast and long-term *** counterparts have been found in optical and near-infrared,with the earliest observation at 17 h after the initial X-ray detection,suggestive of intrinsically weak emission in these *** demonstrate that the remarkable properties of EP2.0408a are inconsistent with any of the transient types known so far,by comparison with,in particular,jetted tidal disruption events,gamma-ray bursts,X-ray binaries and fast blue optical *** nature of EP2.0408a thus remains an *** suggest that EP2.0408a may represent a new type of transients with intermediate timescales of the order of10 *** detection and follow-ups of more of such objects are essential for revealing their origin.

A green fabrication process(organic solvent-free)of artificial scaffolds is required in tissue engineering *** this work,a series of aligned three-dimensional(3D)scaffolds are made from biodegradable waterborne polyurethane(PU)emulsion via directional freeze–drying method to ensure no organic *** optimizing the concentration of polymer in the emulsion and investigating different freezing temperatures,an aligned PUs scaffold(PU14)generated from 14wt%polymer content and processed at196C was selected based on the desired oriented porous structure(pore size of 32.569.3 lm,porosity of 92.)and balanced mechanical properties both in the horizontal direction(strength of 41.3 kPa,modulus of 72.3 kPa)and in the vertical direction(strength of 45.5 kPa,modulus of 139.3 kPa).The response of L92. cells and the regeneration of muscle tissue demonstrated that such pure material-based aligned 3D scaffold can facilitate the development of orientated cells and anisotropic tissue regeneration both in vitro and in ***,these pure material-based scaffolds with ordered architecture have great potentials in tissue engineering for biological anisotropic tissue regeneration,such as muscle,nerve,spinal cord and so on.
