Misdiagnosis of an elevated lesion in the esophagus:A case report

BACKGROUND Esophageal carcinosarcoma(ECS) is a rare biphasic tumor and a type of esophageal malignancy,which presents as protruding or elevated *** patients are often not hospitalized until they have severe *** is easily misdiagnosed as a benign tumor due to its atypical characteristics under *** the popularization of endoscopic treatment,these patients are often referred to endoscopic treatment,such as endoscopic submucosal dissection(ESD).However,there is a lack of consensus on the endoscopic features and therapies for ***,we report a case of ECS and discuss the value of endoscopic diagnosis and therapeutic *** SUMMARY A 63-year-old man was admitted to the hospital with *** the endoscopic examination,an elevated lesion was found with an erosive and hyperemic surface covered with white pseudomembranous *** ultrasonography(EUS),biopsies,and enhanced thoracic computed tomography were performed,suggesting that it was a benign lesion and located within the submucosal *** lesion was diagnosed as a fibrovascular polyp with a Paris classification of *** patient was then referred to ESD ***,the post-ESD pathological and immunohistochemical study showed that this lesion was ECS with a vertical positive margin(T1b stage),indicating that we made a misdiagnosis and achieved a noncurative *** to the potential tumor residue,additional open surgery was performed at the patient’s *** the postoperative pathological study,no tumor remnants or metastases were *** patient was followed for 1 year and had no *** ECS can be misdiagnosed at the initial *** can help to identify the tumor *** with T1b stage ECS cannot be routinely referred to ESD treatment due to the high risk of metastasis and recurrence rate.

Randomized controlled trial of uncut Roux-en-Y vs Billroth Ⅱ reconstruction after distal gastrectomy for gastric cancer: Which technique is better for avoiding biliary reflux and gastritis?

AIM To identify which technique is better for avoiding biliary reflux and gastritis between uncut Roux-en-Y and Billroth Ⅱ reconstruction. METHODS A total of 158 patients who underwent laparoscopyassisted distal gastrectomy for gastric cancer at the First Hospital of Jilin University(Changchun, China) between February 2015 and February 2016 were randomized into two groups: uncut Roux-en-Y(group U) and Billroth II group(group B). Postoperative complications and relevant clinical data were compared between the two groups. RESULTS According to the randomization table, each group included 79 patients. There was no significant difference in postoperative complications between groups U and B(7.6% vs 10.1%, P = 0.576). During the postoperative period, group U stomach p H values were lower than 7 and group B p H values were higher than 7. After 1 year of follow-up, group B presented a higher incidence of biliary reflux and alkaline gastritis. However, histopathology did not show a significant difference in gastritis diagnosis(P = 0.278), and the amount of residual food and gain of weight between the groups were also not significantly different. At 3 mo there was no evidence of partial recanalization of uncut staple line, but at 1 year the incidence was 13%. CONCLUSION Compared with Billroth II reconstruction, uncut Rouxen-Y reconstruction is secure and feasible, and can effectively reduce the incidence of alkaline reflux, residual gastritis, and heartburn. Despite the incidence of recanalization, uncut Roux-en-Y should be widely applied.

Let F be a field with char F = 2, l a maximal nilpotent subalgebra of the symplectic algebra sp（2m,F）. In this paper, we characterize linear maps of l which preserve zero Lie brackets in both directions. It is shown that for m ≥ 4, a map φ of l preserves zero Lie brackets in both directions if and only if φ = ψcσT0λαφdηf, where ψc,σT0,λα,φd,ηf are the standard maps preserving zero Lie brackets in both directions.

Role of nesfatin-1 in a rat model of visceral hypersensitivity

AIM: To explore the role of nesfatin-1 on irritable bowel syndrome (IBS)-like visceral hypersensitivity. METHODS: The animal model of IBS-like visceral hypersensitivity was induced by intracolonic infusion of 0.5% acetic acid (AA) in saline once daily from postnatal days 8-21. Experiments were performed when rats became adults. The visceral sensitivity of rats was evaluated by abdominal withdrawal reflex (AWR) and electromyographic (EMG) activity of the external oblique muscle to graded colorectal distension. The content of nesfatin-1 in serum was determined using enzyme-linked immunosorbent assay. After implantation of an intracerebroventricular (ICV) cannula and two electrodes into the external oblique muscle, model rats were randomly divided into four groups. Animals then received ICV injection of 8 μg of anti-nesfatin-1/ nucleobindin-2 (NUCB2), 50 μg of α-helical cortico-tropin releasing factor (CRF) 9-41 (non-selective CRF receptor antagonist), 50 μg of NBI-27914 (selective CRF1 receptor antagonist) or 5 μL of vehicle. After 1 h of ICV administration, visceral sensitivity of each group was measured again, and comparisons between groups were made. RESULTS: Rats treated with AA showed higher mean AWR scores and EMG activity at all distension pressures compared with controls (P < 0.05). On histopathologic examination, no evidence of inflammation or abnormalities in structure were noted in the colon of either control or AA-treated groups. Myeloperoxidase values were not significantly different between the two groups. The level of nesfatin-1 in serum was significantly higher in the AA-treated group than in the control group (5.34 ± 0.37 ng/mL vs 4.81 ± 0.42 ng/mL, P < 0.01). Compared with rats injected with vehicle, rats which received ICV anti-nesfatin-1/NUCB2, α-helical CRF9-41 or NBI-27914 showed decreased mean AWR scores and EMG activity at all distension pressures (P < 0.05). Cardioembolic occlusion of the internal carotid artery presented with infarction in the posterior cerebral artery territory

Posterior circulation stroke may rarely be associated with occlusive disease in the anterior circulation, such as in the context of a direct （fetal） origin of the posterior cerebral artery （PCA） from the internal carotid artery （ICA）, or in the presence of a persistent trigeminal artery （PTA） or persistent hypoglossal artery （PHA）. Usually, they happened as infarction involving both ipsilateral carotid and posterior cerebral artery territories with carotid atherosclerosis etiology.3 Here, we reported a rare case with cadioembolic occlusion of the ICA presented with infarction mainly in the PCA territory.

Polymorphisms of UGT1A7 and XRCC1 are Associated with an Increased Risk of Hepatocellular Carcinoma in Northeast China

Objective： Hepatocellular carcinoma （HCC） is a complex disease which associates with both environmental and genetic factors. The purpose of this study was to investigate whether the genetic polymorphisms of UDP-glucuronosyltransferase（UGT1A7）, an important phase II biotransformation enzyme, and X-ray repair cross-complementing group 1（XRCC1）, a pivotal DNA-repair gene, were related to the risk of HCC in Northeast China. Methods： One hundred and thirty six HCC patients and one hundred and thirty six frequency-matched controls were included in this hospital-based case-control study. Genotypes of UGT1A7 and XRCC1 were determined using allele-specific polymerase chain reaction （AS-PCR） and PCR-restriction fragment length polymorphism （RFLP）, and for which the odds ratio （OR） with 95% confidence interval （95% CI） were calculated. Results： The proportion of UGT1A7 low enzymatic allele （＊2 or ＊3） was higher in HCC patients than those in controls. The UGT1A7＊1/＊2 and ＊3/＊3 genotypes were associated with higher HCC risk （OR=2.09, 95%CI： 1.10-3.97; OR=5.67, 95%CI： 1.76-18.30, respectively）. The XRCC1 codon 399 Arg/Gln genotype could also elevate HCC risk （OR=2.16, 95% CI 1.29-3.61）. In addition to polymorphisms of UGT1A7 and XRCC1, multivariate logistic regression analysis demonstrated that other significant independent factors associated with HCC were HBV infection （OR=68.07, 95%CI： 28.03-165.26）, HCV infection （OR=30.97, 95%CI： 8.06-118.94） and family history of HCC （OR=10.62, 95%CI： 2.22-50.77）. Conclusion： The study shows that the polymorphisms of UGT1A7 and XRCC1 are associated with HCC risk. Determination of the polymorphisms of UGT1A7 and XRCC1 may provide an important clue to preventive measure against HCC.

Serum pepsinogen Ⅱ is a better diagnostic marker in gastric cancer

AIM:To investigate screening makers for gastric cancer,we assessed the association between gastric cancer and serum pepsinogens(PGs).METHODS:The subjects comprised 450 patients with gastric cancer,111 individuals with gastric atrophy,and 961 healthy *** anti-Helicobacter pylori(***) immunoglobulin G(IgG),PGⅠand PG Ⅱ were detected by enzyme-linked immunosorbent *** atrophy and gastric cancer were diagnosed by endoscopy and histopathological *** ratios and 95%CIs were calculated using multivariate logistic ***:Rates of *** infection remained high in Northeastern *** of *** IgG positivity were greater in the gastric cancer and gastric atrophy groups compared to the control group(69.1% and 75.7% vs 49.7%,P < 0.001).Higher levels of PG Ⅱ(15.9 μg/L and 13.9 μg/L vs 11.5 μg/L,P < 0.001) and lower PGⅠ/PG Ⅱ ratio(5.4 and 4.6 vs 8.4,P < 0.001) were found in patients with gastric cancer or gastric atrophy compared to healthy controls,whereas no correlation was found between the plasma PGⅠconcentration and risk of gastric cancer(P = 0.537).In addition,multivariate logistic analysis indicated that *** infection and atrophic gastritis were independent risk factors for gastric *** plasma PGⅠ/PG Ⅱ ratio was associated with higher risks of atrophy and gastric ***,plasma PG Ⅱ?level?significantly?correlated?with?*** gastric ***:Serum PG Ⅱ concentration and PG Ⅰ/PG Ⅱ ratio are potential biomarkers for *** gastric *** Ⅱ is independently associated with risk of gastric cancer.

Two Bisligand-Coordinated Luminescent Zn(Ⅱ)-Coordination Polymers for Sensing of Ions and Pesticides in Aqueous Solutions

Two coordination polymers(CPs) [Zn(PTA)(DTP)(HO)]·(DMF)(CP-1) and [Zn(BTC)(DTP)]·(CHCN)·(HO)(CP-2) with one-and two-dimensional architectures were synthesized from Zn(Ⅱ) ion and different organic linkers like terephthalic acid(HPTA), benzene-1,3,5-tricarboxylic acid(HBTC), and 3,5-di(1,2,4-triazol-1-yl) pyridine(DTP). The fluorescent sensing experiments showed that the two CPs displayed effective, sensitive, and selective abilities towards Feand CrO. For sensing the pesticides, CP-1 outperforms in sensing of metamitron(MMT) and CP-2 is ultrasensitive towards imidacloprid(IMI). The possible mechanisms involved in the quenching of the fluorescence intensity include the inner filter effect(IFE) and the fluorescence resonance energy transfer(FRET) effect.

Risk Analysis of Hepatocellular Carcinoma in Northeast China

Objective： It is known that chronic hepatitis B virus （HBV） infection is a main risk factor for hepatocellular carcinoma （HCC）. To assess the effect of HBV infection and its interaction with other factors on the risk for HCC, a hospital-based case-control study was carried out in Northeast China. Methods： A total of 384 cases with hepatocellular carcinoma and 432 controls without evidence of liver diseases were enrolled in the study. Blood samples were collected to detect the serum markers of hepatitis B virus （HBV） and hepatitis C virus （HCV） and questionnaires about lifestyle and family tumor history were performed in all subjects. Results： The total infection rate of HBV in hepatocellular carcinoma cases was 70.8% and 10.0% in non-liver disease controls. There was a statistically significant difference （P〈0.0001） between cases and controls （OR= 22.0; 95%CI：15.0-32.3）. Interaction analysis indicated that in HBV chronic carriers with HCV infection or alcohol consumption or family HCC history, the risk for HCC increased （OR=41.1, 95%CI： 20.2-83.9, OR=125.0, 95%CI： 66.5-235.2; OR=56.9, 95%CI： 27.2-119.3 respectively）. In addition, hepatitis B history, HCV infection, hepatic cirrhosis and family history of HCC were also potential HCC independent risk factors. Conclusion： We confirmed that HBV is a chief risk factor for hepatocellular carcinoma and accounts for 67.7% of all hepatocellular carcinoma in Northeast China. HCV infection, alcohol intake and family history could enhance the risk for HCC in chronic HBV carriers.
