Effects of Helicobacter pylori eradication on atrophic gastritis and intestinal metaplasia: A 3-year follow-up study

AIM: To investigate the effect of H pylori eradication on atrophic gastritis and intestinal metaplasia (IM).METHODS: Two hundred and fifty-nine patients with atrophic gastritis in the antrum were included in the study, 154 patients were selected for H pylori eradication therapy and the remaining 105 patients served as untreated group. Gastroscopy and biopsies were performed both at the beginning and at the end of a 3-year follow-up study. Gastritis was graded according to the updated Sydney ***: One hundred and seventy-nine patients completed the follow-up, 92 of them received H pylori eradication therapy and the remaining 87 H pyloriinfected patients were in the untreated group. Chronic gastritis, active gastritis and the grade of atrophy significantly decreased in H pylori eradication group (P＜0.01). However, the grade of IM increased in H pylori -infected group (P＜0.05).CONCLUSION: H pylori eradication may improve gastric mucosal inflammation, atrophy and prevent the progression of IM.

Background A virtual reality simulator provides a novel training model for improving surgical skills in a variety of fields. They can simulate a variety of surgical scenarios to improve the overall skills required for endoscopic operations, and also record the operative process of trainees in real-time and allow for objective evaluation. At present, some simulators for transurethral resection of the prostate （TURP） are available. The utility of virtual reality simulators in training of transurethral prostatectomy was investigated. Methods Thirty-eight urologists were randomly selected to take part in a simulation based training of TURP using the TURPSimTM system. Pre and post-training global rate scale （GRS） scores and objective parameters recorded by the simulator were assessed. Then, questionnaires were filled out. Results Compared with baseline levels, the GRS scores of trainees increased （18.0±4.0 vs. 12.4±4.2, P 〈0.001）, while the rate of capsule resection （26.3%±0.6% vs. 21.2%±0.4%, P 〈0.001）, amount of blood loss （（125.8±86.3） ml vs. （83.7±41.6） ml, P 〈0.001）, external sphincter injury （3.6±2.9 vs. 2.0±2.0, P 〈0.001 ） decreased significantly after training. Most trainees were satisfied with the simulator based training and believed that the simulator accurately mimicked actual surgical procedures and could help improve their surgical skills. Conclusions As a new method of training on transurethral prostatectomy skills, training of TURP using a virtual simulator can help urologists improve their surgical skills and safety. Therefore, the application of the TURPSimTM system in education and training of urologic surgery is warranted.

Synthetic control of chiral pore surfaces in covalent organic frameworks for enantioselective separation of chiral drugs

Enantioselective separations of racemates using porous materials are highly attractive and significant in both fundamental research and technological applications. However, achieving controlled chiral pore surfaces to create the necessary driving force for enantiomer discrimination poses a fundamental challenge. Herein, we demonstrate that chiral covalent organic frameworks(COFs) possess the essential characteristics for effective enantioseparations through synthetic control over pore surfaces. We synthesized three 3D chiral COFs (CCOFs) featuring abundant chiral recognition sites on the pore walls via the condensation of enantiopure dialdehyde of 2,2′-dihydroxy-1,1′-binaphthyl and tetraamine. All CCOFs with high stability exhibit an 11-fold interpenetrated framework with tubular open channels, decorated with dihydroxyl or diethoxyl groups. The CCOFs with dihydroxyl groups serve as solid adsorbents for the adsorptive separation and solid-phase extraction of various chiral 1-phenylethanol-derived enantiomers, particularly racemic drugs, achieving up to 98%enantiomeric excess. In contrast, the CCOF with diethoxy groups is unable to resolve the racemates. These CCOF materials can be recycled and reused without any apparent loss of performance. Density functional theory (DFT) calculations reveal that the enantiospecific recognition arises from the well-organized chiral inner sphere and multiple chiral host-guest interactions.

Surveillance recommendations for gastric cancer（GC） in current guidelines focused on advanced precancerous lesions and were based on precise diagnosis of severity/extent of baseline lesions. We aimed to develop a less endoscopy-related equipment-dependent risk-stratification tool, and assessed whether mild-precursor-lesion patients can be safely exempt from surveillance. In the multicenter communitybased cohort, 75,051 participants receiving baseline endoscopy were enrolled during 2015–2017 and followed-up until 2021. Cumulative incidence rates（CIRs） of GC for precancerous-conditions were calculated by Kaplan-Meier method and compared by Log-rank tests. Mixedeffects Cox regression models were used to detect potential factors for progression towards GC. A risk score was calculated as counts of selected factors. An independent cohort, including 26,586 participants was used for external validation. During a median follow-up of 6.25 years, CIRs of GC were 0.302%, 0.436%, and 4.756% for normal group, non-neoplastic（atrophic gastritis/intestinal metaplasia） and neoplastic lesions（low-grade/high-grade dysplasia）, respectively(Ptrend<0.001). Four predictors, including male, ≥60 years, smoking, and limited vegetable consumption, were selected for risk-stratification. High-risk patients（≥3 risk factors） with non-neoplastic lesions showed higher GC risks（adjusted HR=7.73, 95%CI: 4.29–13.92）, and their four-year CIR reached the one-year CIR of neoplastic lesions. Further categorizing non-neoplastic lesions by histological grade, both patients with moderate-to-severe lesions（aHR=3.07, 95%CI: 1.67–5.64） and high-risk patients with mild lesions（aHR=7.29, 95%CI: 3.58–14.86） showed higher risks. Consistent trends were observed in validation cohort. High-risk mild-precursor-lesion patients should receive surveillance within 3–5 years after baseline screening. Our study provides evidence on supplementing current guideline recommendations.

了解裕固族4～13岁儿童身高、体重发育现状,分析该儿童4～7岁组及7～13岁组生长发育随年龄的变化关系,为儿童保健工作提供参考依据和合理化建议。采用随机整群抽样的方法抽取484名儿童,测量身高和体重。以全国身高体重值为参比值,计算年龄别身高(HAZ)和年龄别体重(WAZ)值评价4～7岁儿童体格生长迟缓和低体重状况;身体质量指数(BMI)评价7～13岁儿童超重和消瘦情况。结果显示,调查对象身高均高于对比值,4～7岁儿童超重检出率为3.45%,偏瘦检出率2.30%;7～13岁裕固族儿童营养不良人数占裕固族儿童总人数的36.90%,严重消瘦的人数占13.10%,其中女生的消瘦程度较男生差。所以,儿童身高、体重生长发育符合一般生长规律。7～13岁裕固族男女生身体偏瘦人数较多的问题值得关注,建议改善该人群营养膳食,加强各项体育锻炼。

目的:探讨心理干预对胸腰椎压缩性骨折伴脊髓损伤患者疗效的影响。方法:将确诊为胸腰椎压缩性骨折伴脊髓损伤并行保守治疗的患者随机分为心理干预组和对照组。对照组采用常规综合治疗;心理干预组在采用常规综合治疗的基础上加用心理护理干预。治疗前后对两组临床疗效进行比较。结果:治疗后对照组病例脊髓功能恢复程度低,而心理干预组病例脊髓功能恢复程度较高,二者比较差异有统计学意义(P<0.05)。结论:心理干预能显著提高胸腰椎压缩性骨折伴脊髓损伤患者的治疗效果。
