Time Series and Spatial Epidemiological Analysis of the Prevalence of Iodine Deficiency Disorders in China

Objective To recognize the spatial and temporal characteristics of iodine deficiency disorders(IDD),China national IDD surveillance data for the years of 1995–2018 were analyzed.Methods Time series analysis was used to describe and predict the IDD related indicators,and spatial analysis was used to analyze the spatial distribution of salt iodine levels.Results In China,the median urinary iodine concentration increased in 1995–1997,then decreased to adequate levels,and are expected to remain appropriate in 2019–2022.The goiter rate continually decreased and is expected to be maintained at a low level.Since 2002,the coverage rates of iodized salt and the consumption rates of qualified iodized salt(the percentage of qualified iodized salt in all tested salt) increased and began to decline in 2012;they are expected to continue to decrease.Spatial epidemiological analysis indicated a positive spatial correlation in 2016–2018 and revealed feature regarding the spatial distribution of salt related indicators in coastal areas and areas near iodine-excess areas.Conclusions Iodine nutrition in China showed gradual improvements.However,a recent decline has been observed in some areas following changes in the iodized salt supply in China.In the future,more regulations regarding salt management should be issued to strengthen IDD control and prevention measures,and avoid the recurrence of IDD.

Effect of Reduction in Iodine Content of Edible Salt on the Iodine Status of the Chinese Population

Objective The aim of this study was to evaluate the impact of the revised Chinese National Standard GB26878-2011 ＇Iodine Content in Edible Salt＇ on the iodine status among the Chinese population. Methods In 2011 and 2014, the probability proportionate to size sampling（PPS） was used in each Chinese province to obtain the representative data. In each sampling unit, school children aged 8-10 years and pregnant women were selected. Key indicators included urinary iodine concentration（UIC）, thyroid volume（TV）, and the iodine content in edible household salt. Results The median urinary iodine concentration（MUIC） decreased between 2011 and 2014 from 238.6 to 197.9 μg/L in school-age children. The number of provinces with iodine excess decreased to zero. The proportion of children whose UIC was 〉 300 μg/L was 18.8% and decreased to 11% compared with 29.8% in 2011. There was no significant difference in UIC 〈 50 μg/L between 2014（4.3%） and 2011（3.7%）（P 〉 0.05）. The MUIC among pregnant women in 2014 was more concentrated between 110 and 230 μg/L. The goiter rate among children aged 8-10 years was unchanged, both the goiter rate of 2011 and 2014 remaining below 5%, in view of the sustainable elimination of iodine deficiency disorders. Conclusion The National Standard GB26878-2011 ＇Iodine Content in Edible Salt＇ that was introduced in March 2012 resulted in an overall improvement in iodine status, reducing the risk of excessive iodine intake in the Chinese population.

Quantile Trends in Temperature Extremes in China

A number of recent studies have examined trends in extreme temperature indices using a linear regression model based on ordinary least-squares. In this study, quantile regression was, for the first time, applied to examine the trends not only in the mean but also in all parts of the distribution of several extreme temperature indices in China for the period 1960–2008. For China as a whole, the slopes in almost all the quantiles of the distribution showed a notable increase in the numbers of warm days and warm nights, and a significant decrease in the number of cool nights. These changes became much faster as the quantile increased. However, although the number of cool days exhibited a significant decrease in the mean trend estimated by classical linear regression, there was no obvious trend in the upper and lower quantiles. This finding suggests that examining the trends in different parts of the distribution of the time-series is of great importance. The spatial distribution of the trend in the 90 th quantile indicated that there was a pronounced increase in the numbers of warm days and warm nights, and a decrease in the number of cool nights for most of China, but especially in the northern and western parts of China, while there was no significant change for the number of cool days at almost all the stations.

Serum Thyroglobulin——A Sensitive Biomarker of Iodine Nutrition Status and Affected by Thyroid Abnormalities and Disease in Adult Populations

Objective To evaluate the usefulness of the thyroglobulin （Tg） level in adults as a nutritiona biomarker of iodine status and to identify the factors related to the serum Tg level. Methods A cross-sectional study was conducted in adult populations of areas differing in iodine nutrition from three provinces （Autonomous Region） in China. Serum levels of thyroid hormones and Tg as well as thyroid autoantibodies were measured. The thyroid volume and nodule were measured by ultrasound. A multivariate linear regression analysis was used to assess iodine intake and other indeterminate factors associated with the serum Tg level. Results A total of 573 adults were recruited for this study. The serum Tg levels differed significantly among the three groups （22.27 ~g/L, 9.73 pg/L and 15.77 pg/L in the excess, more-than-adequate, and deficient groups, respectively）. The results of multivariate linear regression analysis indicate that excess and deficient iodine intake, goiter, thyroid nodule, hypothyroidism are significantly related with higher Tg level, and TgAb positivity is significantly related with lower serum Tg. Conclusion The serum Tg level reflects abnormal thyroid function and is a sensitive functiona biomarker of iodine nutrition status.

Statistically Downscaled Temperature Scenarios over China

Monthly mean temperatures at 562 stations in China are estimated using a statistical downscaling tech-nique.The technique used is multiple linear regressions (MLRs) of principal components (PCs).A stepwise screening procedure is used for selecting the skilful PCs as predictors used in the regression equation.The predic-tors include temperature at 850 hPa (T),the combination of sea-level pressure and temperature at 850 hPa (P+T) and the combination of geo-potential height and tempera-ture at 850 hPa (H+T).The downscaling procedure is tested with the three predictors over three predictor do-mains.The optimum statistical model is obtained for each station and month by finding the predictor and predictor domain corresponding to the highest correlation.Finally,the optimum statistical downscaling models are applied to the Hadley Centre Coupled Model,version 3 (HadCM3) outputs under the Special Report on Emission Scenarios (SRES) A2 and B2 scenarios to construct local future temperature change scenarios for each station and month.The results show that (1) statistical downscaling produces less warming than the HadCM3 output itself;(2) the downscaled annual cycles of temperature differ from the HadCM3 output,but are similar to the observation;(3) the downscaled temperature scenarios show more warming in the north than in the south;(4) the downscaled tempera-ture scenarios vary with emission scenarios,and the A2 scenario produces more warming than the B2,especially in the north of China.

Association of stage 1 hypertension defined by the 2017 ACC/AHA guideline with cardiovascular events and mortality in Chinese adults

Background:The 2017 American College of Cardiology/American Heart Association(ACC/AHA)blood pressure(BP)guideline lowered the threshold defining hypertension to 130/80 mmHg.However,how stage 1 hypertension defined using this guideline is associated with cardiovascular events in Chinese adults remains unclear.This study assessed the association between stage 1 hypertension defined by the 2017 ACC/AHA guideline and clinical outcomes in the Chinese population.Methods:Participants with stage 1 hypertension(n=69,509)or normal BP(n=34,142)were followed in this study from 2006/2007 to 2020.Stage 1 hypertension was defined as a systolic blood pressure of 130-139 mmHg or a diastolic blood pressure of 80-89 mmHg.None were taking antihypertensive medication or had a history of myocardial infarction(MI),stroke,or cancer at baseline.The primary outcome was a composite of MI,stroke,and all-cause mortality.The secondary outcomes were individual components of the primary outcome.Cox proportional hazards models were used for the analysis.Results:During a median follow-up of 11.09 years,we observed 10,479 events(MI,n=995;stroke,n=3408;all-cause mortality,n=7094).After multivariable adjustment,the hazard ratios for stage 1 hypertension vs.normal BP were 1.20(95%confidence interval[CI],1.13-1.25)for primary outcome,1.24(95%CI,1.05-1.46)for MI,1.45(95%CI,1.33-1.59)for stroke,and 1.11(95%CI,1.04-1.17)for all-cause mortality.The hazard ratios for participants with stage 1 hypertension who were prescribed antihypertensive medications compared with those without antihypertensive treatment during the follow-up was 0.90(95%CI,0.85-0.96).Conclusions:Using the new definition,Chinese adults with untreated stage 1 hypertension are at higher risk for MI,stroke,and all-cause mortality.This finding may help to validate the new BP classification system in China.
