Intracytoplasmic sperm injection （ICSI） is the recommended treatment in many cases of male-factor infertility. Several studies have demonstrated a positive correlation between optimal sperm morphology and positive ICSI outcomes. In fact, spermatozoa with severe abnormalities of the head are well documented to be associated with low fertilisation, implantation and pregnancy rates. However, a spermatozoon which is classified as ＇normal＇ by microscopic observation at low magnification could contain ultrastructural defects that impair both the fertilisation process and embryonic development. The intracytoplasmic morphologically selected sperm injection （IMSI） procedure changed the perception of how a spermatozoon suitable for injection should appear. Sperm selection is carried out at x 6000 magnification, allowing improved assessment of the sperm nucleus. Currently, standardized clinical indications for I MSI are lacking and the candidates are selected on the grounds of their medical history or of a careful analysis of the sperm suspension. further prospective randomized studies are needed to confirm the advantages of IMSI in specific groups of patients. In addition to providing a brief overview of the IMSI procedure, this study aims to review the literature, which explains the theoretical basis and the clinical outcomes of this technique. Several reports show that IMSI is associated with improved implantation and clinical pregnancy rates as well as lower abortion rates when compared to ICSI. Although a possible correlation between the sperm＇s abnormal nucleus shape, increased DNA fragmentation and negative laboratory and clinical outcomes has been long investigated, the results are conflicting.

BACKGROUND Pancreatic ductal adenocarcinoma(PDAC)is the fourth leading cause of death among cancers,it is characterized by poor prognosis and strong chemoresistance.In the PDAC microenvironment,stromal cells release different extracellular components,including CXCL12.The CXCL12 is a chemokine promoting the communication between tumour and stromal cells.Six different splicing isoforms of CXCL12 are known(α,β,γ,δ,ε,θ)but their role in PDAC has not yet been characterized.AIM To investigate the specific role ofα,β,andγCXCL12 isoforms in PDAC onset.METHODS We used hTERT-HPNE E6/E7/KRasG12D(Human Pancreatic Nestin-Expressing)cell line as a pancreatic pre-tumour model and exposed it to theα,β,andγCXCL12 isoforms.The altered expression profiles were assessed by microarray analyses and confirmed by Real-Time polymerase chain reaction.The functional enrichment analyses have been performed by Enrichr tool to highlight Gene Ontology enriched terms.In addition,wound healing assays have been carried out to assess the phenotypic changes,in terms of migration ability,induced by theα,β,andγCXCL12 isoforms.RESULTS Microarray analysis of hTERT-HPNE cells treated with the three different CXCL12 isoforms highlighted that the expression of only a few genes was altered.Moreover,theαandβisoforms showed an alteration in expression of different genes,whereasγisoform affected the expression of genes also common withαandβisoforms.Theβisoform altered the expression of genes mainly involved in cell cycle regulation.In addition,all isoforms affected the expression of genes assay confirmed that CXCL12 enhanced the migration ability of hTERT-HPNE cells.Among the CXCL12 splicing isoforms,theγisoform showed higher induction of migration thanαandβisoforms.CONCLUSION Our data suggests an involvement and different roles of CXCL12 isoforms in PDAC onset.However,more investigations are needed to confirm these preliminary observations.

Strategies targeting intracellular negative regulators such as immune checkpoint inhibitors(ICPIs) have demonstrated significant antitumor activity across a wide range of solid tumors. In the clinical practice, the radiological effect of immunotherapeutic agents has raised several more relevant and complex challenges for the determination of their imaging-based response at single patient level. Accordingly, it has been suggested that the conventional Response Evaluation Criteria in Solid Tumors assessment alone, based on dimensional evaluation provided by computed tomography(CT), tends to underestimate the benefit of ICPIs at least in a subset of patients, supporting the need of immunerelated response criteria. Different from CT, very few data are available for the evaluation of immunotherapy by means of 18f-fluoro-2-deoxy-D-glucose positron emission tomography(fDG-PET). Moreover, since the antineoplastic activity of ICPIs is highly related to the activation of T cells against cancer cells, fDG accumulation might cause false-positive findings. Yet, discrimination between benign and malignant processes represents a huge challenge for fDG-PET in this clinical setting. Consequently, it might be of high interest to test the complex and variegated response to ICPIs by means of PET and thus it is worthwhile to ask if a similar introduction of immune-related PET-based criteria could be proposed in the future. finally, PET might offer a new insight into the biology and pathophysiology of ICPIs thanks to a growing number of non-invasive immunediagnostic approaches based on non-fDG tracers.

试验通过在仔猪体内使用山梨酸和香草醛作为指示剂,检验有机酸(OA)及风味相同的天然酸(NIf)包被混合物在仔猪体内浓度的变化,以及与非包被的相同酸化剂混合物作用效果的比较,观察肠道微生物可能会产生的后果。试验将15头断奶仔猪随机分成3个日粮处理组,在体重达到29.5±0.27kg左右时屠宰。日粮处理组分为对照组、添加经过氢化植物油脂质微胶囊包被的OA和NIf混合物组(包被混合物,PB组)、添加与PB组相同的未经活性包被的OA和NIf混合物组(无保护混合物,NPB组)。对胃、空肠前段、空肠后段、回肠、盲肠,以及结肠采样,用于测定肠道内容物内山梨酸以及香草醛示踪剂的浓度。在对照组仔猪体内并没有检测出山梨酸和香草醛,且这两种物质在PB组和NPB组仔猪的胃内并无显著差异。PB组仔猪肠道内的示踪剂含量逐渐减少,而NPB组仔猪的空肠前段中的示踪剂含量与胃中相比迅速下降。PB组仔猪肠道内的山梨酸和香草醛含量显著高于NPB组(P=0.02)。PB组的仔猪后段空肠和盲肠内的大肠杆菌含量显著低于对照组和NPB组(P=0.03)。对照组和PB组仔猪盲肠乳酸杆菌数量要显著高于NPB组(P=0.03),且NPB组的乳酸杆菌含量呈现出显著的下降趋势(P=0.02),而后段空肠内的pH值则要显著高于对照组和PB组(P=0.02)。利用固态脂质微胶囊将OA和NIf包被使用,能够使其中的活性成分缓慢释放,并且能够防止进入到胃内的有效成分迅速失活。

Background:Children with congenital heart disease(CHD),even after surgical approaches,and especially those who undergo staged procedures in theﬁrst months of life,remain vulnerable to readmissions and complications,requiring very close monitoring and differentiated intervention strategies.Methods:Descriptive and exploratory study,of the experience report type,which presents the process of building the high-risk outpatient clinic for complex congenital heart diseases(AAR)at the Instituto do Coração(InCor).Results:Report of the path taken to structure the AAR,demonstrating the organization,interface with the multidisciplinary team,admission and discharge criteria,training,and patient proﬁle.In theseﬁve years of care,275 patients were treated,59.65%with biventricular interstage physiology,followed by univentricular interstage physiology(34.55%),residual defects after surgical procedures(3.63%),tumors with risk of mechanical obstruction(1.45%)and patients with an intrauterine approach(0.72%).The signiﬁcant number of critical patients who were successfully discharged from hospital(44.72%),the low mortality rate due to sudden deaths at home and the high adherence to follow-up cor-roborate the impact of this specialized assistance.Conclusions:Despite the limitations of the study,this experi-ence report showed that with few resources,there is the possibility of organizing an AAR with differentiated care,with the objective of early detection and treatment of residual injuries,identiﬁcation of early interventions,edu-cation of parents for follow-up of their children,resulting in individualized treatment,promoting a better quality of life for this population.

This study highlights introducing the rare earth metal cerium(Ce(Ⅳ))into the structure of magnetite(fe_(3)O_(4))to achieve enhanced adsorptive properties for the removal of chromium(Cr(VI))from an aqueous medium.Different ratios of Ce(Ⅳ)were introduced into the iron oxide matrix,termed feCe-5,feCe-10,and feCe-20.Their numerical values correspond to the nominal content of the dopant element added to the synthesis medium.The solid materials were characterized for morphology and chemical structure,and N_(2)physical adsorption/desorption measurements were performed.The solid materials doped with Ce(IV)have a high surface area compared to fe_(3)O_(4),and the solid material with the highest content of the dopant ion(Ce(IV))has a 4-fold greater surface area.This increase in the dopant content in the iron oxide structure leads to a total chromium removal of 93.3%.Isotherms studies on the solid materials show that chromium adsorption follows the Langmuir model.The adsorption capacity to fe_(3)O_(4)is 12.59 mg/g and feCe-10 is 22.49 mg/g at 35℃.By fitting the kinetic and isothermal models,it is found that for the fe_(3)O_(4)and feCe-10 materials Cr(VI)removal occurs in very different ways,attributed to the different surface areas and compositions of the oxide,with the formation of the goethite(α-feOOH)phase.The feCe-10reuse process was performed and the removal capacity the Cr(VI)is reduced after the first cycle.This result is attributed to a strong and irreversible adsorption of Cr(VI)on the feCe-10.
