BACKGROUND Extra-intestinal manifestations in inflammatory bowel diseases(IBD)are frequent and involve virtually all ***,the clinical characteristics and course of inflammatory myopathies in IBD remain poorly described and mostly related to orbital ***,alternative therapeutic strategies in non-responder patients to corticosteroid therapy must still be *** SUMMARY A 33-year-old woman with a history of unclassified colitis presented with acute bilateral calf *** admission,her clinical and biological examinations were ***,magnetic resonance imaging showed bilateral inflammatory changes in gastrocnemius muscles suggestive of *** biopsy confirmed the diagnosis of myositis and demonstrated an inflammatory infiltrate mainly located in the perimysial compartment including lympho-plasmocytic cells with the formation of several granulomatous structures while the endomysium was relatively *** combined clinical,biological and histomyopathological findings were concordant with the diagnosis of‘gastrocnemius myalgia syndrome’(GMS),a rare disorder associated with Crohn’s disease(CD).Ileocolonoscopy confirmed CD diagnosis and systemic corticosteroids(CS)therapy was started,resulting in a rapid clinical *** CS tapering,however,she experienced a relapse of GMS together with a severe active *** was started and allowed a sustained remission of both conditions at the latest follow-up(20 mo).CONCLUSION The GMS represent a rare CD-associated inflammatory myopathy for which anti-tumour necrosis factor-αtherapy might be considered as an effective therapeutic option.

The progressive stacking of chalcogenide single layers gives rise to two- dimensional semiconducting materials with tunable properties that can be exploited for new field-effect transistors and photonic devices. Yet the properties of some members of the chalcogenide family remain unexplored. Indium selenide （InSe） is attractive for applications due to its direct bandgap in the near infrared, controllable p- and n-type doping and high chemical stability. Here, we reveal the lattice dynamics, optical and electronic properties of atomically thin InSe flakes prepared by micromechanical cleavage. Raman active modes stiffen or soften in the flakes depending on which electronic bonds are excited. A progressive blue-shift of the photoluminescence peaks is observed for decreasing flake thickness （as large as 0.2 eV for three single layers）. first-principles calculations predict an even larger increase in the bandgap, 0.40 eV, for three single layers, and as much as 1.1 eV for a single layer. These results are promising from the point of view of the versatility of this material for optoelectronic applications at the nanometer scale and compatible with Si and III-V technologies.

Acoustical Design of an Electrical Emergency Plant Using Sea Method

The statistical energy analysis (SEA) was used in the acoustical design of an electrical emergency plant to reduce the outdoor noise level. In the past, when the plant was working, a high annoying noise was heard all over the university camp. At a first glance the principal ways of noise propagation were the open door of the plant which was used for the suction of fresh air and a vast hole in the ceiling which was used for gases outlet. Also, a spectral analysis of the noise inside the plant showed that the dominant frequencies of the noise were in the range of 120 - 270 Hz. This frequency range is near the critical frequency of the brick walls that is 129 Hz, at which the walls are transparent for noise. A two-block diagram is used for the statistical energy analysis. Two ways of sound transmission are considered through the inlet and outlet holes and through the walls and ceiling. This analysis shows that the exclusion of holes wouldn’t be sufficient to reduce noise to an acceptable level in a low frequency range but increase the noise absorption by the wall coating material. The transmission loss is calculated for different wall coatings and hole areas. A layer of fiberglass of two-inch width is selected to increase the wall absorption coefficient. Special silencers are designed and put at the suction of air and at the outlet of engine gases to reduce the noise propagation through the holes. The noise measurement shows that the noise level is considerably reduced after implementation of these measures. The reduction of noise is 7 - 8 dB (A), 19 dB (A) and 23 dB (A), inside the plant, 10 m and 15 m away from the plant, respectively.

An analytical methodology based on an O-[2-(methacryloyloxy)-ethylcarbamoyl]-10,11-dihydroquinidine(MQD)-silica hybrid monolithic column was developed for the enantioseparation of 9-fluorenylmethoxycarbonyl(fMOC)derivatized amino acids by nano-liquid *** mobile phase was optimized including the apparent pH,content of ACN,and concentration of the buffer to obtain a satisfactory enantioresolution performance.27 fMOC derivatized amino acids including 19 protein and 8 non-protein amino acids were tested,and 19 out of them were enantiomerically discriminated obtaining baseline separation for 11 of *** characteristics of the method were evaluated for norvaline and tryptophan in terms of linearity,precision,accuracy,limits of detection(LOD)and quantitation(LOQ)showing good performance to be applied to the enantiomeric determination of these amino acids in dietary *** and LOQ values were 9.3 and 31 mM for norvaline enantiomers and 7.5 and 25 mM for tryptophan enantiomers,*** contents of D-norvaline and D-tryptophan were below their respective LODs in all the analyzed *** of L-tryptophan and L-norvaline showed good agreement with the labeled contents except for one sample which did not show presence of L-norvaline,contrary to the label indication.

AIM: To investigate the protein expression of phosphatase and tensin homolog (PTEN) in human liver biopsies of patients with alcoholic and non-alcoholic liver ***: PTEN protein expression was assessed by immunohistochemistry in formalin-fixed, paraffin-embedded liver sections of patients with non-alcoholic fatty liver disease (NAfLD) (n = 44) or alcoholic liver disease (ALD) (n = 25). Liver resections obtained from 3 healthy subjects candidate for partial liver donation served as controls. Histological evaluations were performed by two experienced pathologists, and diagnoses established based on international criteria. The intensity of the PTEN staining in nuclei was compared between steatotic and non-steatotic areas of each liver fragment analyzed. for each liver specimen, the antibody-stained sections were examined and scored blindly by three independent observers, who were unaware of the patients’ clinical ***: In healthy individuals, PTEN immunostaining was intense in both the cytoplasm and nuclei of all hepatocytes. However, PTEN was strongly downregulated in both the nucleus and the cytoplasm of hepatocytes from steatotic areas in patients with NAfLD, independently of the disease stage. In contrast, no changes in PTEN protein expression were observed in patients with ALD, regardless of the presence of steatosis or the stage of the disease. The degree of PTEN downregulation in hepatocytes of patients with NAfLD correlated with the percentage of steatosis (r = 0.3061, P = 0.0459) and the BMI (r = 0.4268, P = 0.0043). Hovewer, in patients with ALD, PTEN expression was not correlated with the percentage of steatosis with or without obesity as a confounding factor (P = 0.5574). finally, PTEN expression level in steatotic areas of ALD patients was significantly different from that seen in steatotic areas of NAfLD patients (P fLD, but not in ALD. Resistive Cerebral Blood flow as a Potential Marker of Subclinical Brain Damage in Essential Hypertension

Introduction: Subclinical brain damage in essential hypertension is more prevalent than cardiovascular or renal impairment;nevertheless, screening for nervous system involvement is difficult due to the low accessibility and high costs of these techniques. Objective: To assess the frequency of silent target organ damage in a cohort of asymptomatic hypertensive patients and to evaluate the potential usefulness of carotid ultrasonographic (US) variables as predictors of subclinical brain damage. Patients and Methods: Thirty four neurologically asymptomatic subjects (mean age 59 years) with essential hypertension were included. Target organ damage was evaluated: degree of hypertensive retinopathy, heart, kidney and brain. Structural and hemodynamical carotid Doppler US parameters were also investigated. Results: The brain was the most frequently affected target organ (70.6%), followed by the heart (67.9%) and kidney (58.6%). Carotid US parameters showed no association of intima media thickness with brain MRI results;nevertheless, decreased diastolic flow velocity and increased resistive index pointed to a resistive carotid flow pattern in patients with classical brain MRI lesions and predicted subclinical lesions with a sensitivity of 70% and 74% and a specificity of 72% and 80% respectively. Conclusions: This study supports previous findings that place the brain as the most frequently affected target organ in essential hypertensive patients and sheds more light on the potential usefulness of carotid structure and hemodynamics as imaging biomarkers of subclinical brain lesions.

＜正＞ The effect of halothane’ anaesthesia in the memory and in the rat brain 5 - HT1A and galanin （GAL） receptors were determined. Rats （n = 12. adult male Sprague- Dawley） were anaesthetized with halothane （HAL）（3-5 mmHg in 50% O2- 50 % O2N,30 min）. The memory capacity （8 - arm labyrinth） and brain 5 -HT1A and GAL receptors （ autoradiography techniques with 3H - 8OH - DPAT and 125I - human - porcine galanin） were determined 24 h after. HAL increased the time spent in: the first choice （ + 50 % ）,in the first 1 and 8 accuracy choices （ + 53%,+ 67% ）, and the error choices total number （ + 136% ）. HAL diminished the time spent eating （ - 26% ）. and the accuracy choices total number （ - 44 %） and the first 8 choices accuracy choices number （ -42%）（p＜0. 05）. HAL increased the affinity of GAL receptors in hippocampus and amygdala （Ki increment 82 % and 151 % ）（p ＜ 0.01）. HAL increased the 5 - HT1A receptors affinity of fronto - parietal cortex, hippocampus CA1 and amygdala （Kd reduction - 66% , - 88% and - 63% ） while reduced receptors affinity of dorsal raphe （Kd increment +224%）（p＜0.0l）. Amnesia induced by halothane in rats are linked to an increased neurotransmission of the galanin and the 5 - HT1A receptors in limbic areas.

Purpose:The aim of the present study was to assess both the credibility and strength of evidence arising from systematic reviews with meta-analyses of observational studies on handgrip strength and health ***:An umbrella review of systematic reviews with meta-analyses of observational studies was *** assessed meta-analyses of observational studies based on random-effect summary effect sizes and their p values,95%prediction intervals,heterogeneity,small-study effects,and excess *** graded the evidence from convincing(Class I)to weak(Class IV).Results:from 504 articles returned in a search of the literature,8 systematic reviews were included in our review,with a total of 11 ***,nine of the 11 of the outcomes reported nominally significant summary results(pf the more stringent p value(pfidence interval(95%CI):0.670.78),(2)cardiovascular death risk in mixed populations(n=15 studies;relative risk=0.84,95%CI:0.780.91),and(3)incidence of disability(n=7 studies;relative risk=0.76,95%CI:0.660.87).Conclusion:The present results show that handgrip strength is a useful indicator for general health status and specifically for early all-cause and cardiovascular mortality,as well as *** further inform intervention strategies,future research is now required to fully understand mechanisms linking handgrip strength scores to these health outcomes.

EGfr （Epidermal growth factor receptor） overexpression has been detected in many tumors of epithelial origin, specifically in breast cancer and it is often associated with tumor growth advantages and poor prognosis. The nimotuzumab is a genetically engineered humanized MAb （monoclonal antibody） that recognizes an epitope located in the extracellular domain of human EGfr. The aim of this study was to assess the pharmacokinetics of nimotuzumab in patients with locally advanced breast cancer who are receiving neoadyuvant therapy combined with the AC chemotherapy regimen （i.e., 60 mg/m2 of Doxorubicin and 600 mg/m2 of Cyclophosphamide in 4 cycles every 21 days）. A single center, non-controlled, open Phase I clinical trial, with histopathological diagnosis of locally advanced stage III breast cancer, was conducted in 12 female patients. Three patients were enrolled at each of the following fixed dose levels： 50, 100, 200 and 400 mg/week. Multiple intermittent short-term intravenous infusions of nimotuzumab were administered weekly, except on weeks 1 and 10, when blood samples were drawn for pharmacokinetic assessments. Nimotuzumab showed dose-dependent kinetics. No anti-idiotypic response against nimotuzumab was detected in blood samples of participants. There was not interaction between the administration of nimotuzumab and chemotherapy at the dose levels studied. The optimal biological doses ranging were estimated to be 200 mg/weekly to 400 mg/weekly.
