Spontaneous Infection of Ascites fluid at the National and University Hospital Hubert Koutoukou Maga in cotonou: Prevalence and Associated factors

Background: Spontaneous ascites fluid infection (SAfI) is an extremely serious and frequently encountered complication in cirrhotic patients. We aimed to determine the prevalence of SAfI and the factors associated with it in the largest hospital in cotonou. Methods: This was a retrospective descriptive and analytical study conducted from January 2013 to July 2019, at the National and University Hospital Hubert Koutoukou Maga (cNHU-HKM) in cotonou, Benin. All patients followed in the University clinic of Hepato-Gastroenterology and diagnosed with SAfI were included. results: Eighty-two patients were included, predominantly males (69.5%), with a mean age of 51.5 ± 14.5 years. Among them, 32 had SAfI, i.e., a prevalence of 39%. Of the 32 cases of SAfI, the culture of ascites fluid was positive in 6 cases (18.7%). The most frequent germ found in SAfI was Escherichia coli (5 patients, 83.3%). The factors associated with SAfI in this study were: abdominal pain (p = 0.004), increased bilirubinemia (p = 0.009), decreased prothrombin level 20 (p = 0.001). conclusion: SAfI was common in cirrhotic patients in the department. certain clinical and paraclinical factors were associated with it, as was the severity of cirrhosis. Early diagnosis and aetiological management of cirrhosis could reduce its frequency.

Stability Analysis of Dc Microgrid clusters Based on Step-by-step System Matrix Building Algorithm

Dc microgrids(DcMGs) are made up of a network of sources and loads that are connected by a number of power electronic converters(PEcs). The increase in the number of these PEcs instigates major concerns in system stability. While interconnecting the microgrids to form a cluster, the system stability must be ensured. This paper proposes a novel stepby-step system matrix building(SMB) algorithm to update the system matrix of an existing DcMG cluster when a new microgrid is added to the cluster through a distribution line. The stability of the individual DcMGs and the DcMG cluster is analyzed using the eigenvalue method. further, the particle swarm optimization(PSO) algorithm is used to retune the controller gains if the newly formed cluster is not stable. The simulation of the DcMG cluster is carried out in MATLAB/Simulink environment to test the proposed algorithm. The results are also validated using the op4510 real-time simulator(rTS).

Spatial distribution of archaeal and bacterial ammonia oxidizers in the littoral buffer zone of a nitrogen-rich lake

The spatial distribution and diversity of archaeal and bacterial ammonia oxidizers （AOA and AOB） were evaluated targeting amoA genes in the gradient of a littoral buffer zone which has been identified as a hot spot for N cycling. Here we found high spatial heterogeneity in the nitrification rate and abundance of ammonia oxidizers in the five sampling sites. The bacterial amoA gene was numerically dominant in most of the surface soil but decreased dramatically in deep layers. Higher nitrification potentials were detected in two sites near the land/water interface at 4.4–6.1 μg NO2-N/（g dry weight soil·hr）, while only 1.0–1.7 μg NO2-N/（g dry weight soil·hr） was measured at other sites. The potential nitrification rates were proportional to the amoA gene abundance for AOB, but with no significant correlation with AOA. The NH4＋ concentration was the most determinative parameter for the abundance of AOB and potential nitrification rates in this study. Higher richness in the surface layer was found in the analysis of biodiversity. Phylogenetic analysis revealed that most of the bacterial amoA sequences in surface soil were affiliated with the genus of Nitrosopira while the archaeal sequences were almost equally affiliated with candidatus ‘Nitrososphaera gargensis’ and candidatus ‘Nitrosocaldus yellowstonii’. The spatial distribution of AOA and AOB indicated that bacteria may play a more important role in nitrification in the littoral buffer zone of a N-rich lake.

The TESS light curve(Lc) of the marginal contact binary A A cet was analyzed simultaneously with the radial velocity and the orbital period(op) change of the system was investigated.The physical parameters of the system were obtained by analyzing the Lc of A A cet with the Wilson-Devinney method,and the absolute parameters of the components were calculated using the results obtained.for the components of AA cet,the masses and radii were calculated as M1=1.39±0.04 M⊙,M2=0.48±0.02 M⊙,and r1=1.64±0.03 r⊙,r2=1.01±0.04 r⊙,respectively.AA cet is a marginal contact binary with a temperature difference of 1305 K between its components.A total of 14 eclipse times were obtained from the TESS data and used in the op analysis together with those collected from the literature.It has been observed that the change in the op of AA cet is in the form of a decreasing parabola.conservative mass transfer between the components has been interpreted as the reason for this change.The op decrease amount of A A cet was obtained as dP/dt=0.0062±0.0006 s yr-1.and the reason for this decrease was attributed to a 3.3(9)×10-8M⊙.mass transfer per year from the more massive component to the less massive one.The age of A A cet has been estimated as 7 Gyr,as the age of contact systems helps us to understand their evolution.

Legume rhizobium symbiosis is initiated upon perception of bacterial secreted lipo-chitooligosaccharides （LcOs）. Perception of these signals by the plant initiates a signaling cascade that leads to nodule formation. Several studies have implicated a function for cytokinin in this process. However, whether cytokinin accu- mulation and subsequent signaling are an integral part of rhizobium LcO signaling remains elusive. Here, we show that cytokinin signaling is required for the majority of transcriptional changes induced by rhizo- bium LcOs. In addition, we demonstrate that several cytokinins accumulate in the root susceptible zone 3 h after rhizobium LcO application, including the biologically most active cytokinins, trans-zeatin and iso- pentenyl adenine. These responses are dependent on calcium- and calmodulin-dependent protein kinase （ccaMK）, a key protein in rhizobial LcO-induced signaling. Analysis of the ethylene-insensitive Mtein21 Mtsickle mutant showed that LcO-induced cytokinin accumulation is negatively regulated by ethylene. Together with transcriptional induction of ethylene biosynthesis genes, it suggests a feedback loop negatively regulating LcO signaling and subsequent cytokinin accumulation. We argue that cytokinin accumulation is a key step in the pathway leading to nodule organogenesis and that this is tightly controlled by feedback loops.

Population genetic structure of culex quinquefasciatus in India by ISSr marker

Objective:To characterize the genetic structure of various populations of culex quinquefasciatus (cx.quinquefasciatus) from India representing different geoclimatic locations.Methods:Inter simple sequence repeat(ISSr) markers were used.A set of 20 primers were screened with the laboratory populations of mosquito species.finally the IS 40 primer was chosen based on the scorable banding pattern showing 100 percent polymorphism among the various populations.The statistical analysis was done using PopGENE 1.31 software.The consensus tree was generated based on UPGMA modified from NEIGHBOr procedure of PHYLIP Version 3.5.results:The cluster analysis shows the main cluster which is divided into two sub cluster representing all the populations separated as per their phylogeographic and geoclimatic condition.conclusions: The findings will be helpful in understanding the population variation under different ecological conditions and development of effective vector management strategies.

Aims Our study was conducted in the Nokrek Biosphere reserve(NBr) in the Garo hills districts of Meghalaya,Northeast India.Our aim was to assess the effects of human activities on plant diversity,population structure and regeneration.Methods We selected a representative 1.2 hm2 stand in both the core and buffer zones of NBr.Structure and composition were determined by randomly sampling square quadrats,population structure was assessed by determining age structure,and regeneration was assessed by measuring densities of seedling,sapling and adult trees.Important findings More woody species were recorded from the core zone than the buffer zone(87 vs.81 species),and there were a large number of tropical,temperate,and Sino-Himalayan,Burma-Malaysian and Malayan elements,primitive families and primitive genera.The trees were distributed in three distinct strata,canopy,subcanopy and sapling.Subcanopy and sapling layers had the highest species richness(81%-88%).Lauraceae and Euphorbiaceae were the dominant families in terms of the number of species,and a large number of families were represented by single species.Most woody species(57%-79%) were contagiously distributed and had low frequency(ffer zone,its basal area was low compared to the stand in the core zone.Low similarity and high β-diversity indicate marked differences in species composition of the stands.Shannon diversity index was high in both the stands,while Simpson dominance index was low.The diameter-class distribution for dominant species revealed that the most had a large number of young individuals in their populations.Preponderance of tree seedlings,followed by a steep decline in population density of saplings and adult trees,indicated that the seedling to sapling stage was the most critical in the life cycle of the tree populations.Most species(42%-48%) had no regeneration,25%-35% had good/fair regeneration,and the rest had poor regeneration or reoccurred as immigrants.

combined hepatocellular and cholangiocellular carcinoma presenting with radiological characteristics of focal nodular hyperplasia

combined hepatocellular and cholangiocellular carcinoma (cHcc-cc) is a rare tumor type containing unequivocal elements of both hepatocellular carcinoma and cholangiocarcinoma that are intimately mixed. Although these tumors are usually considered to be more related to hepatocellular carcinoma than to cholangiocarcinoma, they sometimes, in contrast to hepatocellular carcinoma, contain a significant amount of fibrous stroma. This might in some cases explain atypical radiological features. We report a case of a cHcc-cc in a 47-year-old female that resembled focal nodular hyperplasia on Magnetic resonance Imaging. correlation of imaging and serum levels of α-fetoprotein and cA19.9 can help to make the correct diagnosis preoperatively.

Acute necrotizing pancreatitis as fi rst manifestation of primary hyperparathyroidism

We report the case of a female patient with severe acute necrotizing pancreatitis associated with hypercalcemia as first manifestation of primary hyperparathyroidism caused by a benign parathyroid adenoma.Initially the acute pancreatitis was treated conservatively.The patient subsequently underwent surgical resection of the parathyroid adenoma and surgical clearance of a large infected pancreatic pseudocyst.Although the association of parathyroid adenoma-induced hypercalcemia and acute pancreatitis is a known medical entity,it is very uncommon.The pathophysiology of hypercalcemia-induced acute pancreatitis is therefore not well known,although some mechanisms have been proposed.It is important to treat the provoking factor.Therefore,the cause of hypercalcemia should be identif ied early.Surgical resection of the parathyroid adenoma is the ultimate therapy.
