Anti-Mullerian Hormone: Establishing the Ovarian Reserve Range with Age in Rivers State Women, Niger Delta Region of.Nigeria

Bac.ground: The measurement of.c.rc.lating anti-Mullerian hormone (AMH) in the plasma of.adult women has been used as a tool in the assessment of.ovarian reserve. This is based on its ability to ref.ec. the number of.growing f.llic.es in the ovaries and knowing the level of.AMH in a woman’s blood is generally a good indic.tor of.her ovarian reserve and this c.n be ac.ieved by making ref.renc. to a dec.sion values. Objec.ive: The purpose of.this study was to measure the level of.this hormone in normal, apparently healthy subjec.s in Rivers State, Nigeria with respec. to age. The perc.ntage of.women, at their reproduc.ive age with inf.rtility problems in the state is in the inc.ease and the essenc. was to establish a ref.renc. range f.r the hormone bec.use of.its c.inic.l applic.tion in women f.rtility. Materials/method: A total of.one thousand and sixty two apparently healthy women divided into f.ur age groups were rec.uited f.r this study;this c.mprised of.300 eac. in age group of.20 - 30, 31 - 40, 41 - 50 respec.ively and 162 in 51 - 60 years age group f.om May 2014 to June 2017. They were randomly selec.ed af.er f.lling a questionnaire f.rm that has the inf.rmation of.age, tribe, last date of.menses and whether on medic.tion f.r any inf.rtility problems. Exc.uded f.om the study were subjec.s identif.ed with dif.erent endoc.ine dysf.nc.ion and/or on drugs f.r dif.erent types of.anovulatory dysf.nc.ion. The Enzyme linked Immunoassay method was used in the measurement of.the AMH. Results: The result f.om the measurement of.plasma AMH levels showed a Mean ± SD value of.3.193 ± 0.943, 1.644 ± 0.691, 0.516 ± 0.276 and 0.135 ± 0.081 ng/ml respec.ively f.r the 1st, 2nd, 3rd and 4th age groups respec.ively. The mean value f.r the AMH dec.eases with inc.easing age and was statistic.lly signif.c.nt at the dif.erent age group levels (P −2.250 - 4.136;0.953 - 2.350;0.240 - 0.792 and 0.054 - 0.216 respec.ively f.r the age groups. Sic.le c.ll anemia (Sc.) patients are reported with inf.rtility and low rate of.pregnanc.es. This is assoc.ated with wide range of.reproduc.ive issues that are still relevant bec.use of.the c.mplic.tions and problems of.the disease that still persist till date. This study was c.rried out to establish the sec.etion pattern of.the sex hormones (Progesterone and Estradiol) in the three trimesters of.pregnant sic.le c.ll disease subjec.s in the Niger Delta Region, south of.Nigeria. The study inc.uded twenty (20) pregnant sic.le c.ll anemia subjec.s with average age of.27.4 years and twenty (20) apparently healthy (Hemoglobin AA) subjec.s with average age of.28.2 years. Their samples c.llec.ion started when they registered f.r antenatal c.re at the c.inic. within the f.rst trimester of.pregnanc.. The Enzyme Linked Immunosorbent assay (ELISA) method was used in the measurement of.the hormones in the plasma of.the subjec.s. The result showed a statistic.l signif.c.nt reduc.ion (P f.the hormones in the three trimesters obtained f.r the sic.le c.ll disease (Sc.) subjec.s when c.mpared with apparently healthy subjec.s with the same age range. Statistic.l analysis showed a strong positive c.rrelation ((r = 0.8151 f.r Estradiol and r = 0.8793) f.r Progesterone) between the sec.etion of.the sex hormones, in the sic.le c.ll subjec.s and the c.ntrol. The result is attributed to the sic.le c.ll gene abnormality and the treatment of.Sc. that af.ec.s the endoc.ine system by inhibiting the produc.ion of.gonadotropins f.om the pituitary gland. The Sc. itself.does not direc.ly damage the reproduc.ive system;however it c.n af.ec. other systems whic. will eventually c.use harm to the reproduc.ive system. The study c.nc.uded that the sic.le c.ll anemia patients require the administration of.the hormone drugs during pregnanc. to prevent to a c.rtain extent, c.mplic.tions arising f.om hormonal imbalanc..
