Intraoperative Periprosthetic f.moral f.actures Related to Austin Moore Hemiarthroplasty—A Retrospective Review of.365 Patients

Intraoperative periprosthetic f.moral f.actures (IPf.) have been studied extensively f.r total hip arthroplasties, but not f.r hemiarthroplasties. Recent series in the literature show an IPf. rate f.r hemiarthroplasties ranging f.om 0% to 14%. The present study was designed to determine the prevalence and outcome af.er IPf. during non-cemented hemiarthroplasty. In addition, the surgical step at higher risk to produce these f.actures was evaluated in an attempt to identif. strategies that could minimize the prevalence of.this complication. We perf.rmed an observational study of.365 consecutive patients undergoing and Austin-Moore hemiarthroplasty f.om 2005 to 2006 at our institution. The institutional IPf. rate was 6.8% (twenty-f.ve out of.365). The moment at which the f.acture was detected was collected: 1) intraoperatively and 2) in the postoperative radiological control. The surgical step in which the f.acture occurred was collected: 1) neck osteotomy, 2) broaching, 3) prosthesis introduction, and 4) reduction. Results were compared to a control group according to blood transf.sion rate, mortality rate and revision surgery rate. The f.actures were detected during the surgery in twenty cases (80%);f.r the f.ve remaining cases the f.acture was only detected in the postoperative radiology. f.r those detected during the surgery, the two most common manouvers in which the f.acture occurred was hip reduction (10 cases) and prosthesis introduction (7 cases). The blood transf.sion rate, f.rst-month mortality rate and revision surgery rate showed no statistical dif.erence between the two groups (p = 0.3). In the present series, most of.IPf. during Austin-Moore hemiarthroplasty implantation, occurred during arthroplasty reduction. Dif.iculties during this step should lead the surgeon to reconsider if.technical mistakes are present and can be solved. However, if.f.acture occurs, adequate treatment of.IPf. should provide satisf.ctory results without increasing blood transf.si

A Direct Link between Abscisic Acid Sensing and the Chromatin-Remodeling ATPase BRAHMA via Core ABA Signaling Pathway ComDonents

Optimal response to drought is critical f.r plant survival and will af.ect biodiversity and crop perf.rmance during climate change. Mitotically heritable epigenetic or dynamic chromatin state changes have been implicated in the plant response to the drought stress hormone abscisic acid （ABA）. The Arabidopsis SWI/SNf.chromatin-remodeling ATPase BRAHMA （BRM） modulates response to ABA by preventing pre- mature activation of.stress response pathways during germination. We show that core ABA signaling pathway components physically interact with BRM and post-translationally modif. BRM by phosphoryla- tion/dephosphorylation. Genetic evidence suggests that BRM acts downstream of.SnRK2.2/2.3 kinases, and biochemical studies identif.ed phosphorylation sites in the C-terminal region of.BRM at SnRK2 target sites that are evolutionarUy conserved. f.nally, the phosphomimetic BRMs17s^D S1762D mutant displays ABA hypersensitivity. Prior studies showed that BRM resides at target loci in the ABA pathway in the presence and absence of.the stimulus, but is only active in the absence of.ABA. Our data suggest that SnRK2- dependent phosphorylation of.BRM leads to its inhibition, and PP2CA-mediated dephosphorylation of.BRM restores the ability of.BRM to repress ABA response. These f.ndings point to the presence of.a rapid phosphorylation-based switch to control BRM activity; this property could be potentially harnessed to improve drought tolerance in plants.
