Introduction: Neonatal mortality remains a major public health concern. According to the World Health Organisation (WHO), the number of.newborns dying each year has f.llen f.om 5 million in 1990 to 2.4 million in 2019. The aim of.our study was to describe the main morbid conditions and their lethality in newborns admitted to the paediatric ward of.the CSREf.in Commune V. Methodology: We conducted a 12-month prospective descriptive and analytical study f.om August 2020 to July 2021. Results: During the 12 months of.the study, out of.2654 neonates admitted to hospital, 216 met our inclusion criteria, i.e. 8%. Newborns admitted in the f.rst f.w hours of.lif. represented 99.54% of.cases. f.ll-term newborns accounted f.r 65.74% of.the total. f.rty-eight percent of.newborns had hypothermia on admission. The most common diagnosis was asphyxia in 55.56% of.cases, f.llowed by neonatal inf.ction in 27.78% and prematurity in 10.65%. Death was recorded in 22.2% of.cases. Asphyxia was the main cause of.neonatal death, f.llowed by prematurity in our context. Conclusion: Improving the prognosis of.newborn babies will require a thorough understanding of.neonatal pathologies and the implementation of.a pre- and perinatal prevention policy.

Introduction: Cardiac catheterisation plays a f.ndamental role in the management of.acute coronary syndrome. These explorations require heavy, complex and costly equipment and a large team of.doctors, nurses and technicians with highly specialized training. Aims: To describe epidemiological, clinical and coronary angiography aspects of.patients with acute coronary syndrome. Patients and Methods: Descriptive study f.om September 2019 to December 2023 in the Cardiology Department of.the Hôpital Mère-Enf.nt of.Bamako. Inclusion criteria were patients admitted f.r coronary angiography with the diagnosis of.acute coronary syndrome. Results: During the study period, 1253 patients underwent coronary angiography, 596 of.whom had acute coronary syndrome as an indication, representing a hospital f.equency of.47%. Sex-ratio was 2.10. Mean age of.patients was 58.5 ± 11.39 years. ST elevation acute coronary syndrome was the most common indication with 63.92% of.cases. High blood pressure was the main cardiovascular risk f.ctor with 58.7% of.cases, and radial access approach was used in 98% of.cases. Coronary angiography was pathological in 91.70% of.cases (n = 548). Patients with lesions of.anterior interventricular artery were 73.73% of.cases. Tritruncal lesions accounted f.r 40.63% of.cases. Conclusion: ST elevation acute coronary syndrome is the most f.equent manif.station of.acute coronary syndrome. Anterior interventricular artery is most of.en the culprit lesion f.r our patients.

The blood count is an easily achievable routine exam and will it have specif.cs in the event of.a neonatal bacterial inf.ction? Hence, the present study with the objective of.determining the prof.le of.the hemogram of.newborns hospitalized f.r early bacterial neonatal inf.ction. Material and methods: This was a cross-sectional study that took place f.om June 27 to September 03, 2016 in the neonatology department of.teaching hospital Gabriel Toure. Included were all neonates hospitalized f.r early neonatal bacterial inf.ction (ENBI) and who had a blood count. Results: We included 227 patients, 64.8% of.whom were premature. The sex ratio was 1.4. The inf.nts were less than 24 hours old in 93.6% of.the cases. The mean hemoglobin level was 16.435 g/dl [8.8 - 22.26]. Erythrocytopenia was f.und in 18.5% of.cases. Anemia was present in 17% of.newborns. The average leukocyte was 15.228·103/mm3 [1.4 - 72]. Hyperleukocytosis and leukopenia were f.und in 12.32% and 6.6% respectively. Neutropenia and lymphopenia were present in 14.5% and 30.8%. There was a correlation between leukocytosis of.negative blood cultures (23/27) (p = 0.030). f.r Neutrophils, neutrophilia was more observed in term neonates and neutropenia in premature inf.nts (p = 0.03). Monocytosis was present in 13.6% of.cases. One quarter (25.5%) of.newborns had thrombocytopenia. Conclusion: Hematological variations did not allow a specif.c prof.le of.newborns hospitalized f.r early neonatal bacterial inf.ction to be identif.ed.

Cowpea (Vigna unguiculata L. [Walp.]) in one of.the main grain legumes contributing to f.od security and poverty alleviation in Sub-Saharan Af.ica. To control the highly damaging legume pod borer Maruca vitrata f., transgenic cowpea lines expressing the insecticidal Cry1Ab Bt protein were developed. In this study, we evaluated the impact of.Cry1Ab transgene expression on the susceptibility of.f.ur cowpea lines (named IT97K-T, IT98K-T, Gourgou-T and Naf.-T) and their respective non-transgenic near isogenic lines (IT97K, IT98K, Gourgou and Naf.) to Cowpea aphid-borne mosaic virus (CABMV) in greenhouse conditions. In a preliminary quality control test by enzyme-linked immunosorbent assay, the presence of.Cry1Ab protein in transgenic seed lots ranged f.om 59% to 72%, with no signif.cant dif.erences among the lines (χ2 = 3.26;p = 0.35). Upon virus inoculation, all cowpea lines exhibited mosaic symptoms with similar severity between 7- and 11-day post-inoculation. No signif.cant dif.erences were observed in symptom severity. Signif.cant dif.erences were f.und between cowpea lines f.r time of.symptom onset, virus accumulation in plants and days to 50% f.owering. However, while comparing pairs of.transgenic lines and corresponding non-transgenic lines, virus accumulation showed not signif.cant dif.erences whatever the pair. Time of.symptom onset and days to 50% f.owering did not also dif.er signif.cantly between pairs of.cowpea lines except Naf./Naf.-T in which transgenic Naf.-T showed earlier symptoms (7.4 ± 0.7 vs. 8.9 ± 0.8 days post-inoculation) and shorter f.owering time (37.3 ± 0.6 vs. 42 ± 1.7 days af.er sowing). Overall, these f.ndings improve our understanding of.the ef.ects of.Cry1Ab gene mediated genetic modif.cation on cowpea inf.ction by Cowpea aphid-borne mosaic virus, with potential implications f.r environmental saf.ty assessment.

Retroperitoneal sof. tissue sarcomas in the retroperitoneal/intra-abdominal regions represent 10% - 15% of.all cases of.sof. tissue sarcoma. Liposarcomas, which are the most common histological type, account f.r 20% - 45% of.retroperitoneal/intra-abdominal sarcoma cases, and 20% of.liposarcomas cases are primary retroperitoneal liposarcomas. Surgical resection in case of.malignancy remains the treatment of.choice f.r liposarcomas, according to the guidelines of.most major international companies. Our goal was to improve the management of.retroperitoneal liposarcoma. This was a 65-year-old patient, with no medical or surgical history, who was ref.rred to us f.r abdominal swelling, in whom clinical and paraclinical examination f.und retroperitoneal liposarcoma stage IV, and the surgical treatment consisted in making a tumor reduction. Conclusion: Retro-peritoneal liposarcoma is an undervalued malignant tumor, and the diagnosis is of.en late.

Objectives: The main objective was to evaluate antibiotic prescribing in the Department of.Pediatrics at Gabriel Touré teaching hospital. Methods: This was a prospective descriptive study conducted f.om September 1 to November 31, 2018, in the Department of.Pediatrics at Gabriel Touré teaching hospital. All children aged 0 to 15 years hospitalized f.r any pathology during the study period and having received at least one antibiotic had been included. Results: We collected 445 children’s records out of.1032 admissions during the study period, representing a hospital f.equency of.43.1%. The sex ratio was 1.3%. The 2 - 5 age group accounted f.r 48.1%. f.ver was the reason f.r consultation in 45.6% of.cases. Patients’ general condition was altered in 60% of.cases. The f.thers were blue-collar workers in 65.4% and the mothers housewives in 85%, and had no education in 42.9% and 64.5% respectively. Hepatomegaly was present in 18.2%, splenomegaly in 9.6% and peripheral adenopathy in 3.1%. The site of.inf.ction was pulmonary in 37.6% and ENT in 9.2%. Bacterial inf.ction was assumed on admission in 54% of.cases, and meningitis in 57.7%. The discharge diagnosis was malaria in 54.6%, severe acute malnutrition in 18.6% and meningitis in 6.7%. The death rate was 3.8%. Neutrophilic leukocytosis was present in 47.3% of.patients. CRP was positive in 85% of.patients. Blood cultures taken in 27.6% of.patients were positive in 5. CSf.analysis in 30% of.patients showed elevated leukocytes in 6.5%. No cultures were positive. Antibiotic prescription was justif.ed by inf.ctious hypotheses in 43.1% of.patients. β-lactam antibiotics were prescribed in 98.6%. Antibiotic therapy was not adapted to national/international recommendations in 68.3% of.cases, and was not justif.ed in 16.3% of.cases on D5 of.hospitalization. Conclusion: Antibiotic use was justif.ed in more than half.of.patients, but remained inadequate in almost two-thirds of.cases, in line with national and international recommend

The explosive technological improvement of.photovoltaic systems as well as the necessity of.populations to come to less expensive energy sources, that have led to an implosion at the level of.solar panel manuf.cturers. This causes a large f.ow of.these equipments to developing countries where the need is high, without any quality control. That conducted an experimental investigation on the perf.rmance characteristics of.a 250 wp monocrystalline silicon photovoltaic module in other to check the verif.cation and quality control. Most of.these PV panels which of.en have missing inf.rmations are manuf.ctured and tested in places that are inadequate f.r our environmental and meteorological conditions. Also, their inf.uences on the stability of.internal parameters were evaluated in order to optimize their perf.rmance. The results obtained at maximum illumination (1000 w/m2) conf.rmed those produced by the manuf.cturer. The analysis of.these characteristics showed that the illumination and the temperature (meteorological f.ctors) inf.uenced at most the stability of.the internal characteristics of.the module in the sense that the maximum power increased very rapidly beyond 750 w/m2 but a degradation of.perf.rmance was accentuated f.r a temperature of.the solar cells exceeding 50°C. The degradation coef.icients were evaluated at -0.0864 V/°C f.r the voltage and at -1.6248 w/°C f.r the power. The 10° inclination angle of.the solar panel proved to be ideal f.r optimizing overall ef.iciency in practical situations.

Introduction: The level of.knowledge and attitude of.health prof.ssionals about breast cancer are important determinants. General objective: To study the knowledge, attitude and practice of.health prof.ssionals of.the hospital of.the district of.the commune IV on breast cancer. Methodology: This was a cross-sectional, prospective, descriptive, qualitative and quantitative study. The study period was f.om 01 October 2022 to December 2022. Results: In one week of.survey, 110 health workers (intern, obstetrician nurse, general practitioners and specialists) received the survey sheet, 80 health workers inf.rmed it, a participation rate of.73%. The male sex was most represented at 63.75% with a sex ratio of.1.76. The average age was 39.59 years with extremes of.22 and 61 years. The clinical signs evoked by the participants were: breast nodule (81.25%), f.llowed by breast discharge (48.75%). In relation to risk f.ctors: interns and obstetrician nurses had no good knowledge, 51.72% of.general practitioners had good knowledge, 43.48% of.specialist doctors had good knowledge, and 5.88% of.the wise had good knowledge. Conclusion: Breast cancer is a common pathology around the world, health prof.ssionals are at the f.ref.ont of.the f.ght against breast cancer, and this f.ght cannot be ef.ective without trained personnel.

Two-Stream Approximation to the Radiative Transf.r Equation:A New Improvement and Comparative Accuracy with Existing Methods

Mathematical modeling of.the interaction between solar radiation and the Earth's atmosphere is f.rmalized by the radiative transf.r equation(RTE), whose resolution calls f.r two-stream approximations among other methods. This paper proposes a new two-stream approximation of.the RTE with the development of.the phase f.nction and the intensity into a third-order series of.Legendre polynomials. This new approach, which adds one more term in the expression of.the intensity and the phase f.nction, allows in the conditions of.a plane parallel atmosphere a new mathematical f.rmulation of.γparameters. It is then compared to the Eddington, Hemispheric Constant, Quadrature, Combined Delta f.nction and Modif.ed Eddington, and second-order approximation methods with ref.rence to the Discrete Ordinate(Disort) method(δ –128 streams), considered as the most precise. This work also determines the conversion f.nction of.the proposed New Method using the f.ndamental def.nition of.two-stream approximation(f.TSA) developed in a previous work. Notably,New Method has generally better precision compared to the second-order approximation and Hemispheric Constant methods. Compared to the Quadrature and Eddington methods, New Method shows very good precision f.r wide domains of.the zenith angle μ 0, but tends to deviate f.om the Disort method with the zenith angle, especially f.r high values of.optical thickness. In spite of.this divergence in ref.ectance f.r high values of.optical thickness, very strong correlation with the Disort method(R ≈ 1) was obtained f.r most cases of.optical thickness in this study. An analysis of.the Legendre polynomial series f.r simple f.nctions shows that the high precision is due to the f.ct that the approximated f.nctions ameliorate the accuracy when the order of.approximation increases, although it has been proven that there is a limit order depending on the f.nction f.om which the precision is lost. Introduction: Acute myeloblastic leukaemia (AML) is a haematological malignancy with a poor prognosis, despite signif.cant therapeutic progress. This study presents the results of.AML management in Mali according to Gf.OP recommendations. Methodology: This was a retrospective, cross-sectional study. It included patients aged 0 - 15 years treated in the paediatric oncology unit f.r AML and f.llowed up between January 2016 and December 2020. Results: During the study period, 85 cases of.acute leukaemia were diagnosed in the paediatric oncology unit (including 51 cases of.ALL), of.which 34 cases of.AML were included in this study. The majority were boys (59%). The mean age was 8 years, with extremes of.18 months and 15 years. The mean time to diagnosis was 68 days. In 79% of.cases, patients were ref.rred by 1st or 2nd level hospitals. Anaemia was observed in 91% of.cases, an inf.ctious syndrome in 68%, haemorrhage in 56% and a tumour syndrome in 85%. The haemogram showed hyperleukocytosis in 15% of.cases, thrombocytosis in 22% and severe anaemia in 73%. Death occurred in 85% of.cases, most of.en in the context of.sepsis or haemorrhage. Conclusion: AML is probably underestimated in Mali and diagnosis delayed, which may be explained by patient-related f.ctors (lack of.knowledge, f.nancial constraints) and a cumbersome ref.rral system. These results suggest the need to implement an appropriate diagnostic and therapeutic strategy, with strong involvement of.the political authorities.
