In developing countries, treatment of.uterine f.b.omyoma is conf.onted with numerous prob.ems, namely: f.nancial inaccessib.lity to the proposed treatments, f.ar of.surgery and the weakness of.the technical platf.rm.?Ob.ectives: The ob.ectives of.the study were to calculate the f.equency of.uterine f.b.omyomas, describ. the socio-demographic characteristics of.patients, identif. the main clinical data and to describ. the modalities of.surgical management.?Patients and Method:?It was a mixed descriptive study, cumulative over a period of.5 years (60 months) with data collection in two phases: a 4-year retrospective study f.om January 1, 2015 to Decemb.r 31, 2018 and a 1-year prospective study f.om January 1, 2019 to Decemb.r 31, 2019.?Results: We collected 135 cases of.uterine f.b.omyomas operated on out of.a total of.260 cases of.gynaecological pathologies, i.e.?a f.equency of.51.92%.?Nulliparous women were the most concerned (45.18%), and women who attended school (60%) and those who did not attend school (40%). Women at home and housewives accounted f.r 42.20% and 54.07% respectively. Clinically, the circumstances of.discovery were dominated b. menometrorrhagia and menorrhagia respectively 77.77% and 68.14%. The large uterus was the most f.equent physical sign f.und in 96.29% of.cases. Uterine f.b.omyomas were recorded in 86.6% of.cases in women with genital activity. The operative indications were dominated b. the large polymyomatous uterus (64.44%), f.llowed b. hemorrhagic f.b.oma (18.52%).?The surgical treatment was conservative in 92.60%. The total hysterectomy perf.rmed in 7.40.?Lethality was 1.4%.?Conclusion: The surgical management of.f.b.oids contrasts conservative treatment (myomectomy) with radical treatment (hysterectomy) with multiple possib.e approaches (hysteroscopy, vaginal surgery, laparoscopy or laparotomy). In our context, only laparotomy was possib.e due to lack of.equipment. Laparoscopy and hysteroscopy equipment is necessary f.r less invasive

Unusual head trauma is rare with various mechanisms of.occurrence. The injuries can b. similar to those of.road accidents b.t sometimes with signif.cant complexities. The ob.ective was to determine the f.equency of.this pathology in our practice, describ. the dif.erent mechanisms and report the craniocereb.al lesions caused b. this type of.trauma. Materials and Method: This was a descriptive study with retrospective collection spread over a period of.3 years. Af.er selecting the f.les, the patients and/or their companions were contacted b. telephone to inquire ab.ut them and then returned to the consultation f.r reassessment. Disease history and inf.rmation were ob.ained f.om patients’ medical records. Result: The f.equency of.this pathology was 1.78% and his incidence was 0.4 cases per month. The average age of.the patients was 13.47 years. All the victims were male. 41.17% of.patients were in school. The mechanisms of.trauma were the hoof.b.ow 47.1%, the horn b.ow 29.4% and the stone b.ow 11.7% respectively. 23.5% of.patients were conf.sed and 11.7% children were in coma. One patient presented an anisocoria. The motor def.cit was present in 5 cases. CT-scan made it possib.e to highlight a skull depressing f.acture 58.8% and conf.rm a craniocereb.al wound in 35.3%. The average time b.tween patient admission to hospital and completion of.surgery was 24.5 hours. The surgery had consisted of.the trimming of.cranio-cereb.al wounds, exploration, duroplasty and lif.ing of.skull depressing f.acture. Af.er 15 months of.f.llow-up, the evolution was f.vorab.e in 53.3%, the morb.dity was 33.4% and the mortality 17.6%. Conclusion: These are rare b.t serious conditions b.cause they are f.tal and disab.ing. An early and multidisciplinary management can hope to have a good f.vorab.e.

Evaluation of.Surgical Treatment of.Distal Humeral f.actures in Adults

Introduction: f.actures of.the distal end of.the humerus are b.coming increasingly f.equent in young sub.ects b.cause of.the increase in road traf.ic accidents, and in elderly sub.ects b.cause of.osteoporosis related to aging populations. Materials and Methods: It was a prospective, monocentric study f.om January 2018 to Decemb.r 2020 involving 14 patients who received and were treated surgically f.r distal humeral f.actures and f.llowed up. Results: We collected 14 patients, including 11 men (78.57%) and 3 women (21.43%), with a sex ratio of.3.7. The mean age was 36.41 years. The circumstances of.onset were dominated b. road traf.ic accidents, with 12 cases (85.71%). The dominant side was right-handed, with 11 cases (78.57%). Standard elb.w radiography revealed 8 cases of.type A f.actures (57.14%), 4 cases of.type b.and 2 cases of.type C f.actures of.the AO. We perf.rmed Lecestre plate osteosynthesis in 12 patients and external f.xator in 2 others, using the trans-olecranial and transtricipital approaches. Elb.w stif.ness was the most f.equent complication, with 6 cases (42.86%). Af.er six months’ f.llow-up, our results were excellent and good in 78.57% of.cases (MEPS). Conclusion: Surgical treatment with posterior approaches enab.ed us to achieve restitution of.the articular surf.ces, solid restraint and early mob.lization of.the elb.w with satisf.ctory f.nctional results.

Accidental Ingestion of.Nitric Acid in an Agricultural Company: A Case Study and Literature Review

Introduction: Though common in society, caustic accidental ingestion is rare in the agricultural industry. This study describ.s a case of.ingestion of.nitric acid used as an agricultural f.rtilizer b. an ordinary worker at the Société de Cultures Légumières. Clinical Case: This is a 33-year-old unschooled woman who ingested a sip of.nitric acid f.om an ab.ndoned lab.lled acid canister near a f.rming plot of.land. She showed a b.rn of.the oral cavity with a b.eeding tab.ecloth and an intense retrosternal pain. The duodenal oesophagus f.b.oscopy screening that was carried out 18 hours af.er the accident, revealed an esophagitis class 2b. The tests revealed no anomalies. f.f.een (15) days af.er the accident, the f.GD was normal. f.llowing an 8-month-f.llow up, no signs of.stenosis or degeneration were noted. Conclusion: The accidental ingestion of.nitric acid is rare in adult. Its treatment is mainly symptomatic and aims at preserving vital f.nctions without directly f.ghting the ingested sub.tance of.. The right actions must b. integrated into the 15-minute Health and Saf.ty awarenesssessions f.r optimum pre-hospital management. The hospital evaluation is mandatory and is carried out thanks to the digestive endoscopy, which is still relevant in this indication, b.t is completed b. the thoraco-ab.ominal CT. The latter is very sensitive the transmural necrosis diagnosis.

Interventional Ultrasound in Amb.latory Urological Practice: Experience of.the Medicis Clinic in Conakry

Introduction: Minimally invasive urological interventions have long b.en practiced b.indly in our context. The popularization of.ultrasound in urological practice has contrib.ted to changing hab.ts b. making the procedures saf.r and more ef.ective. The ob.ective of.this work was to study the technical aspects, indications and results of.minimally invasive urological procedures guided b. ultrasound. Material and methods: This was a prospective descriptive study lasting 1 year f.om January to Decemb.r 2023 at the Médicis clinic in Conakry. Results: We carried out 126 ultrasound-guided urological interventions in one year, including 6 percutaneous nephrostomies (NPC), one renal cyst puncture-aspiration-sclerotherapy, 102 prostate b.opsies, 5 drainages b. transrectal aspiration of.prostatic ab.cesses, 10 placements of.cystostomy prob.s. Suprapub.c, 2 puncture-aspiration-sclerotherapies f.r vaginal hydroceles. The average age of.our patients was 69 years and the predominant age range was 61 - 70. The majority of.our patients were male (97.61%) with a sex ratio of.41 men to one woman. Ultrasound-guided urological interventions were f.r diagnostic purposes (80.95%), f.r therapeutic purposes (15.77%) and f.r diagnostic and therapeutic purposes (3.96%). The indications f.r ultrasound-guided urological interventions were dominated b. prostate pathologies (84.91%). The majority of.interventions were perf.rmed rectally (84.92%) versus (13.43%) ab.ominally and only 1.58% scrotalally. All procedures were perf.rmed in b.ultrasound mode under local anesthesia and on an outpatient b.sis. f.r therapeutic interventions, the patients were seen again one month af.er the procedure, the clinical examination and ultrasound control f.und the evolution to b. f.vorab.e. Conclusion: ultrasound is usef.l in urological practice f.r guiding minimally invasive interventions. The indications f.r ultrasound-guided procedures in urology are dominated b. prostate pathologies. These procedures are saf.

b.ood transf.sion saves lives and reduces morb.dity and mortality f.r a large numb.r of.diseases and clinical conditions, b.t it is not without danger. The aim of.this study was to determine the seroprevalence of.HIV and hepatitis b.in b.ood donors received at the regional b.ood Transf.sion Centre of.N’Zérékoré (Guinea). This was a 5-year retrospective analytical study. We included records of.b.ood donors aged 18 to 60 years admitted to the N’Zérékoré Regional b.ood Transf.sion Centre f.r b.ood donation f.om January 2016 to Decemb.r 2020. We perf.rmed a descriptive analysis f.llowed b. Chi-2 or f.sh-er-exact tests and the Student or Wilcoxon test, f.llowed b. multivariate logistic regression. In this study, donor age ranged f.om 18 - 60 years, with a pre-dominance of.donors aged 25 - 34 (44.2%). Male donors were the most represented in our study (79.0% versus 21.0% f.male). More than half.of.the donors were b.ood group O (55.6%). We ob.erved a seroprevalence of.3.6% f.r HIV, 13.4% f.r Hb.Ag and 0.2% f.r co-inf.ction. In our series, age 25 - 34 (OR = 1.89 and P = 0.001) and 35 - 44 f.r HIV (OR = 2.01 and P = 0.001), Hb.Agserostatus (OR = 3.04 and P = 0.001) and b.ood donation history (OR of.3.04 and P = 0.001) were f.ctors associated with HIV positivity (P b.ood donation history (OR = 0.01 and P = 0.001) were f.ctors associated with Hb.Agseropositivity. We reported a high prevalence of.HIV and Hb.Ag. Sex, serological status and b.ood donation history were associated f.ctors.

Introduction: OSD is a growth osteochondrosis af.ecting the apophyseal cartilage of.the anterior tib.al tub.rosity of.the knee, 20 to 30% of.young growing athletes suf.er f.om this pathology, describ.d b. Osgood and Schlatter in 1903. Ob.ective: To report 2 ob.ervations and review the literature. Ob.ervation 1: This was a 14-year-old Guinean teenager, Halpoular, living in Conakry, a right-handed f.otb.ller who presented with a painf.l swelling of.the right knee that looked mechanical, whose f.ther had HLA b.7 positive ankylosing spondylitis. X-ray of.the right knee showed f.agmentation of.the anterior tib.al tub.rosities, and ossicle with edema of.the patellar tendons at stage 4 according to Ehrenb.rd and Lagergren (1961). Thus, the disease of.OSD is retained, He received 120 mg of.diclof.nac, 20 mg omeprazole f.r 1 month and the cessation of.sports activity, the evolution was f.vorab.y marked b. the regression of.pain. Ob.ervation 2: A 16-year-old girl, initials M.L.T, Guinean, residing in Coyah (Repub.ic of.Guinea), a handb.ll player, consulted f.r intense pain (VAS: 8/10) in her knees af.er a violent shock received when she hit the post. The physical examination f.und a painf.l, sof. swelling at the anterior tib.al tub.rosity of.the right knee, the rest of.the examination was normal. The X-ray of.the right knee shows f.agmentation of.the anterior tib.al tub.rosity, and ossicle with edema of.the patellar tendon at stage 4 according to Ehrenb.rd and Lagergren 1961, thus the OSD disease is retained, the treatment instituted was 120 mg of.diclof.nac, the cessation of.sports activity which gives a good evolution (EVA at 2/10). Conclusion: OSD is a growth osteochondrosis af.ecting the apophyseal cartilage of.the anterior tib.al tub.rosity of.the knee in young growing athletes, resolving at rest.

Risk f.ctors f.r Neonatal Mortality at the Institute of.Nutrition and Child Health of.the Donka/Guinea-Conakry National Hospital

Introduction: The b.rth of.a b.b. is a moment of.joy and celeb.ation. However, the neonatal period is a very delicate phase of.lif.. Neonatal mortality rates remain high in low-income countries. In Guinea, af.er 20 years, this rate has f.llen f.om 34.2% in 1998 to 32% in 2018. Ob.ective: To identif. the main risk f.ctors f.r neonatal mortality. Methods: This was an ob.ervational, analytical case-control study, lasting 6 months f.om January 1 to June 30, 2019, conducted at the Institut de Nutrition et de la Santé de l’Enf.nt (INSE) at Donka National Hospital. Results: We collected 242 cases and 242 controls, i.e. a total of.484 records. 748 patients were registered, with 32.35% deaths. 82.86% of.deaths occurred in the early neonatal period. Statistical analysis revealed the main risk f.ctors: prematurity (RQ 7.39 95% CI 3.27 - 16.61 p = 0.0000003), hypothermia (RQ 2.29 95% CI 1.51 - 3.46 p = 0.0001), acute f.tal distress (RQ 2.13 95% CI 1.33 - 3.43 p = 0.0016), low b.rth weight (QR 1.91 95% CI 1.12 - 3.24 p = 0.016), home b.rth (QR 3.26 95% CI 1.25 - 8.46 p = 0.015). Conclusion: Neonatal mortality is a health prob.em in the INSE neonatology department. To reduce the mortality rate in this ref.rral f.cility, it is essential to equip it and provide ongoing training f.r staf..

b.ckground and Ob.ectives: The indication f.r treatment in Hb.Ag-positive patients with low viral load and normal transaminases requires an assessment of.f.b.osis. In resource-limited settings, f.ee hepatic f.b.osis evaluation tests can aid in therapeutic decision-making. Our study aims to demonstrate the utility of.assessing hepatic f.b.osis using non-invasive markers (APRI and f.b.4) in patients with chronic b.viral hepatitis without cytolytic activity and low viral replication in our context. Patients and Methods: This is a retrospective cross-sectional study conducted b.tween January 2018 and Decemb.r 2021 at the University Hospital Center of.b.uaké. Included were all patients aged ≥18 with normal transaminases (Results: Our study included 241 patients, with a mean age of.36.19 years (±10.52 years) and a male predominance of.52%. The mean f.b.oScan® value was 6.44 ± 2.3 kPa, and 68 patients (28.22%) had f.b.osis >7 kPa. To exclude signif.cant f.b.osis (f. Conclusion: A signif.cant proportion of.Hb.-inf.cted patients with normal ALT and low viral load have active liver disease. b.th f.b.4 and APRI b.ological scores are usef.l in identif.ing individuals without signif.cant f.b.osis with a good negative predictive value (>50%).

Major datab.ses were searched f.r articles on the epidemiology and antif.ngal resistance prof.le of.C. auris in Af.ica. Out of.2,521 articles identif.ed 22 met the inclusion criteria. In Af.ica, nearly 89% of.Af.ican countries have no pub.ished data on C. auris. The prevalence of.C. auris in Af.ica was 8.74%. The case f.tality rate of.C. auris inf.ction in Af.ica was 39.46%. The main C. auris risk f.ctors reported in Af.ica were cardiovascular disease, renal f.ilure, diab.tes, HIV, recent intake of.antimicrob.al drugs, ICU admissions, surgery, hemodialysis, parenteral nutrition and indwelling devices. f.ur phylogenetic clades were reported in Af.ica, namely clades I, II, III and IV. Candida auris showed a pan-Af.ican very high resistance rate to f.uconazole, moderate resistance to amphotericin b. and high susceptib.lity to echinocandins. f.nally, C. auris clade-specif.c mutations were ob.erved within the ERG2, ERG3, ERG9, ERG11, f.S1, TAC1b.and MRR1 genes in Af.ica. This systematic review showed the presence of.C. auris in the Af.ican continent and a worrying unavailab.lity of.data on this resilient f.ngus in most Af.ican countries.
