Urothelial bladder carcinoma(UBC)is an intricate malignancy with a variable natural history and clinical *** developments in diagnosis/prognosis refi nement and treatment modalities,the recurrence rate is high,and progression from non-muscle to muscle invasive UBC commonly leads to ***,patients with muscle-invasive or extra-vesical disease often fail the standard chemotherapy treatment,and overall survival rates are ***,UBC remains a challenge in the oncology fi eld,representing an ideal candidate for research on biomarkers that could identify patients at increased risk of recurrence,progression,and ***,progress toward personalized medicine has been hampered by the unique genetic complexity of *** genome-wide expression and sequencing studies have brought new insights into its molecular features,pathogenesis and clinical diversity,revealing a landscape where classical pathology is intersected by the novel and heterogeneous molecular ***,it seems plausible to postulate that only an integrated signature of prognostic/predictive biomarkers inherent in different cancer hallmarks will reach clinical *** this review,we have summarized ours and others’research into novel putative biomarkers of progression and chemoresistance that encompass several hallmarks of cancer:tumor neovascularization,invasion and metastasis,and energy metabolism reprogramming of the tumor microenvironment.

Effect of retained austenite and nonmetallic inclusions on the thermal/electrical properties and resistance spot welding nuggets of Si-containing TRIP steels

five advanced high-strength transformation-induced plasticity(TRIP) steels with different chemical compositions were studied to correlate the retained austenite and nonmetallic inclusion content with their physical properties and the characteristics of the resistance spot welding nuggets. Electrical and thermal properties and equilibrium phases of TRIP steels were predicted using the JmatPro? software. Retained austenite and nonmetallic inclusions were quantified by X-ray diffraction and saturation magnetization techniques. The nonmetallic inclusions were characterized by scanning electron microscopy. The results show that the contents of Si, C, Al, and mn in TRIP steels increase both the retained austenite and the nonmetallic inclusion contents. We found that nonmetallic inclusions affect the thermal and electrical properties of the TRIP steels and that the differences between these properties tend to result in different cooling rates during the welding process. The results are discussed in terms of the electrical and thermal properties determined from the chemical composition and their impact on the resistance spot welding nuggets.

A ternary Ni2feSb shape memory alloy was fabricated by powder metallurgy route. Sintering kinetics was estimated from dilatometry tests; whereas the microstructure and morphology of the powder and consolidated bulk samples were evaluated by XRD and SEm, respectively. microhardness tests were performed on the surface of sintered samples. The results indicated that milling time has an effect on the shape and particle size as well as the homogeneity of the crystalline structures of the powders. Samples with longer milling time presented higher relative densities, better distribution of the elements on the alloy as well as the L21 and martensite phases, which will give the shape memory effect. The estimated activation energy values ranged from 109 to 282 kJ/mol at temperatures between 750 and 1273 K, indicating that sintering is controlled mainly by volume diffusion. microhardness was improved by increasing the milling time and the heating rate.

Experimentation on Tool Wear and Surface Roughness in AISI D2 Steel Turning with WC Insert

The Taguchi method, based on an orthogonal arrangement (L9, 33), the vari-ance analysis, the signal-to-noise ratios and the response surface methodol-ogy have been used to optimize maximum flank wear (VBmax) and surface roughness (Ra) of the cutting tool when turning a hardened steel AISI D2 (65 HRC) with PVD—TiAlN coated WC insert upon dry environment. By em-ploying regression models;cutting speed, cutting depth and feed rate, which optimize maximum flank wear and surface roughness were validated. Results of relation signal-to-noise ratios, showed that with cutting speed of 200 m/min, cutting depth of 0.2 mm and feed rate of 0.20 mm/rev, Ra is opti-mized. With cutting speed of 150 m/min, cutting depth of 0.4 mm and feed rate of 0.3 mm/rev, VBmax is optimized. Through the variance analysis it was concluded that the depth of cut was the main parameter that affected on the surface roughness;whereas, the feed rate was the most influential parameter on the flank wear. Confirmation test results showed that the Taguchi method was very successful in the optimization of machining parameters for mini-mum surface roughness and flank wear in the turning of the D2 steel.

Assessment of Key Players in the Special Rice Value Chain in Nueva Ecija, Philippines

This study traced the special rice value chain, described the various players’ roles, assessed the identified chains’ performance, determined the constraints and opportunities, and identified interventions to improve the special rice value chain in the province. Key informant interviews with government and nongovernment organizations (NGOs) and surveys among different special rice value chain players were done. The special rice industry in Nueva Ecija is relatively limited and consists of aromatic, pigmented, and glutinous rice. The value chain mapping for special rice in Nueva Ecija starts downstream (traders) and moves upstream (input provider). Some of the identified constraints within the market levels of the chain are as follows: farmers’ limited access to affordable yet quality seeds, the volatility of the price of paddy rice in the market, and the rapid changes in the prices of production inputs, such as seeds, fertilizers, pesticides, and others. moreover, natural factors like typhoons, floods, droughts, insect pests, and weeds were noted. One major problem identified by the institutional buyers was the limited demand for special rice. To improve competitiveness, the special rice industry should focus on developing and promoting pest-resistant varieties and cost-reducing technologies. furthermore, promoting special rice among consumers is needed to increase demand and encourage more farmers to engage in special rice production. The government agencies should continue supporting special rice farmers not only in production but also in postharvest and marketing aspects. Provision of seminars, trainings, and machinery related to special rice is recommended. Regression analysis using backward method of analysis was found to be significant in three (3) variables, namely, household size, years in school, and labor.

fatal and non-fatal injuries due to suspension trauma syndrome: A systematic review of definition, pathophysiology, and management controversies

BACKGROUND: Suspension trauma syndrome is a life-threatening event that occurs when a person is "trapped" in a prolonged passive suspension. It is most commonly seen in people who engage in occupational or sport activities that require harness suspension. The aim of this study is to identify the predisposing factors, pathophysiology, and management of suspension ***: A review and analysis of the literature published in English and Spanish from 1972 to 2020 on suspension trauma were performed. Search sources were Pub med, medline, Cochrane Library, meSH, Up To Date, and Google Scholar. Articles referring to suspension trauma associated with other injury mechanisms(traumatic impact injuries, drowning, asphyxiation, or bleeding), case reports, and pediatric population were ***: forty-one articles were identified. Of these, 29 articles related to mechanism, pathophysiology, and management of individuals who suffered prolonged suspension trauma without associated traumatic injuries were included in the study. We encountered several controversies describing the putative pathophysiology, ranging from blood sequestration in the lower extremities versus accumulation of metabolic waste and hyperkalemia to dorsal hook-type harness as a trigger cause of positional asphyxia;to vascular compression of femoral vessels exerted by the harness causing decreased venous return. Pstients suspended in a full-body harness with dorsal hook showed more hemodynamic alterations in response to the compressive effect on the rib cage, causing a reduction in perfusion by presenting a decrease in pulse pressure. management strategies varied across ***: Progress has been made in individualizing the population at risk and in the management of suspension trauma. We recommend the formation of consensus definitions, larger cohort or registry studies to be conducted, and experimental animal models to better understand the mechanisms in order to develop management

Inguinal Hernia Repair with Local Anesthesia in the Outpatient—10 Year Experience

Objective: To demonstrate the feasibility of inguinal hernia repair with local anesthesia in an out-patient regime, with safety, efficacy and short learning curve. methods: We prospectively evaluated 1186 patients undergoing inguinal hernia repair under local anesthesia on an outpatient basis between November 2004 and march 2014. Of the total number of hernias surgically treated in this period, 755 were operated on the right, 394 on the left and 37 bilateral. We used clinical, surgical and psychosocial criteria for inclusion in the procedure. The parameters for exclusion were complex, irreducible or recurrent hernia, obesity (BmI greater than 30 kg/m2), patient’s refusal and psychiatric disorder. All patients underwent elective surgery and were analyzed regarding surgical outcome, complications and hospital stay. Results: All operations were completed successfully. In no case there was a need to change the anesthetic method. Surgical time was similar to that conducted with other methods of anesthesia and there were no cases of adverse effects of local anesthetics. Intra-operative complications amounted to approximately 2.64%. There was no need for hospital admissions greater than 24 hours. Conclusion: The procedure is feasible and causes no perioperative significant pain, is safe, can be performed by residents under supervision, has satisfactory patient acceptance and complications similar to those observed in a conventional herniorrhaphy, allowing lower time and cost of hospitalization and faster access to treatment.
