Effectiviness, Safety and Satisfaction in Teleurology

Introduction: Telemedicine has been used as a tool for improving access to health services worldwide. The aim of the present study is thus to evaluate the effectiveness of urological consultation by videoconference, perceptions regarding safety on the part of medical teams providing video consultation services and patient satisfaction after a urological appointment. Materials and Methods: A cross-sectional, observational, and analytical study was carried out with 50 volunteers referred from the basic health unit for a urology appointment. All patients were evaluated remotely by videoconference and in person by different urology teams. Results: The study revealed that effectiveness in terms of the degree of match between diagnoses performed by video consultation and those performed in person was 92%. The urology team’s perception regarding safety, measured using an in-house safety questionnaire, was high for both videoconferencing and in-person consultations and there was no statistical difference between the two (overall safety score for video consultation compared to in-person consultation was 9.7 ± 0.8 compared to 9.6 ± 0.8 and the p-value was 0.3 for Student’s t-test). Patient satisfaction with the appointment was similarly high for both groups, averaging 9.72 ± 0.4 among patients who attended a video consultation and 9.82 ± 0.4 among those whose consultation was conducted in person, with a p-value of 0.10478 for Student’s t-test. Conclusion: Video consultation in urology is an effective way to perform diagnoses, with high levels of perceived safety among urologists and high satisfaction rates among patients.

Surgical treatment of familial adenomatous polyposis: Dilemmas and current recommendations

familial adenomatous polyposis (fAP) is an autosomal dominant inherited syndrome characterized by multiple adenomatous polyps (predisposing to colorectal cancer development) and numerous extracolonic manifestations. The underlying genetic burden generates variable clinical features that may influence operative management. As a precancerous hereditary condition, the rationale of performing a prophylactic surgery is a mainstay of fAP management. The purpose of the present paper is to bring up many controversial aspects regarding surgical treatment for fAP, and to discuss the results and perspectives of the operative choices and approaches. Preferably, the decision-making process should not be limited to the conventional confrontation of pros and cons of ileorectal anastomosis or restorative proctocolectomy. A wide discussion with the patient may evaluate issues such as age, genotype, family history, sphincter function, the presence or risk of desmoid disease, potential complications of each procedure and chances of postoperative surveillance. Therefore, the definition of the best moment and the choice of appropriate procedure constitute an individual decision that must take into consideration patient’s preferences and full information about the complex nature of the disease. All these facts reinforce the idea that fAP patients should be managed by experienced surgeons working in specialized centers to achieve the best immediate and long-term results.

Arboviral diseases are viral infections transmitted to humans through the bites of arthropods,such as mosquitoes,often causing a variety of pathologies associated with high levels of morbidity and *** the past decades,these infections have proven to be a significant challenge to health systems worldwide,particularly following the considerable geographic expansion of the dengue virus(DENV)and its most recent outbreak in Latin America as well as the difficult-tocontrol outbreaks of yellow fever virus(YfV),chikungunya virus(CHIKV),and Zika virus(ZIKV),leaving behind a substantial portion of the population with complications related to these ***,the world is experiencing a period of intense globalization,which,combined with global warming,directly contributes to wider dissemination of arbovirus vectors across the ***,all continents remain on high alert for potential new ***,this review aims to provide a comprehensive understanding of the pathogenesis of the four main arboviruses today(DENV,ZIKV,YfV,and CHIKV)discussing their viral characteristics,immune responses,and mechanisms of viral evasion,as well as important clinical aspects for patient *** includes associated symptoms,laboratory tests,treatments,existing or developing vaccines and the main associated complications,thus integrating a broad historical,scientific and clinical approach.

In this article,we explored the role of adipose tissue,especially mesenteric adipose tissue and creeping fat,and its association with the gut microbiota in the pathophysiology and progression of Crohn’s disease(CD).CD is a form of inflammatory bowel disease characterized by chronic inflammation of the gastrointestinal tract,influenced by genetic predisposition,gut microbiota dysbiosis,and environmental *** microbiota plays a crucial role in modulating immune response and intestinal inflammation and is associated with the onset and progression of ***,visceral adipose tissue,particularly creeping fat,a mesenteric adipose tissue characterized by hypertrophy and fibrosis,has been implicated in CD pathogenesis,inflammation,and *** bacteria from the gut microbiota may translocate into mesenteric adipose tissue,contributing to the formation of creeping fat and influencing CD *** creeping fat may be a protective barrier against bacterial invasion,its expansion can damage adjacent tissues,leading to *** gut microbiota through interventions such as fecal microbiota transplantation,probiotics,and prebiotics has shown potential in managing ***,more research is needed to clarify the mechanisms linking gut dysbiosis,creeping fat,and CD progression and develop targeted therapies for microbiota modulation and fat-related complications in patients with CD.

Colorectal cancer in young adults: A difficult challenge

Sporadic colorectal cancer(CRC) is traditionally diagnosed after the sixth decade of life,and current recommendations for surveillance include only patients older than 50 years of age. However,an increasing incidence of CRC in patients less than 40 years of age has been reported. This occurrence has been attributed to different molecular features and low suspicion of CRC in young symptomatic individuals. When confronting young-onset CRC with older patients,issues such as biological aggressiveness,stage at diagnosis and clinical outcomes seem to differ in many aspects. In the future,the identification of the molecular profile underlying the early development of sporadic CRC will help to plan tailored screening recommendations and improve management. Besides that,differential diagnosis with CRC linked with hereditary syndromes is necessary to provide adequate patient treatment and family screening. Until we find the answers to some of these doubts,doctors should raise suspicion when evaluating an young adult and be aware of this risk and consequences of a late diagnosis.

Overweight and obesity in young adults with patellofemoral pain:Impact on functional capacity and strength

Purpose:This study aimed to(a)investigate the proportion of overweight/obesity in a cohort of young adults with patellofemoral pain(PfP)and(b)explore the association of body mass index(BMI),body fat,and lean mass with functional capacity and hip and knee strength in people with ***:We included a mixed-sex sample of young adults(18-35 years old)with PfP(n=100).Measurements for BMI,percentage of body fat,and lean mass(assessed by bioelectrical impedance)were *** capacity was assessed by the Anterior Knee Pain Scale,plank test,and single-leg hop *** of the knee extensors,knee flexors,and hip abductors was evaluated isometrically using an isokinetic *** proportion of overweight/obesity was calculated based on *** association between BMI,body fat,and lean mass and functional capacity and strength was investigated using partial correlations,followed by hierarchical regression analysis,adjusted for covariates(sex,bilateral pain,and current pain level).Results:A total of 38%of our cohort had their BMI categorized as overweight/*** BMI was associated with poor functional capacity(ΔR^(2)=0.06-0.12,pflexion strength only(ΔR^(2)=0.04,p=0.030).Higher body fat was associated with poor functional capacity(ΔR^(2)=0.05-0.15,p≤0.015)and reduced strength(ΔR^(2)=0.15-0.23,pfunctional capacity(ΔR^(2)=0.04-0.13,p≤0.032)and reduced strength(ΔR^(2)=0.29-0.31,pfat,and lean mass should be considered in the assessment and management of young people with PfP because it may be detrimental to function and strength.

Considering the expansion of forest plantations over the Brazilian territory and the imminent increase in air temperatures due to global warming,the search for technologies that ensure high levels of productivity and high quality plants in clonal nurseries is *** kaolin-based particle films(KBPf)has been used as a physical barrier to reduce the negative effects of excessive solar radiation and adverse *** effects of KBPf on production and physiological quality of eucalyptus mini-cuttings were evaluated during two different seasons.A randomized block design was used,consisting of five treatments and four *** concentrations(0,3%,5%,7%,and 10%)were tested here and six weekly replicates of each concentration was *** different kaolin concentrations and seasonal variations resulted in a quadratic response of the leaf SPAD reading during both daily evaluation *** applications resulted in a quadratic response of photochemical efficiency in the morning period,and an increasing specific leaf weight linear response,and during the dry season,the different kaolin concentrations resulted in a bud length quadratic *** results indicated that the kaolin application maintained the production and yield index statistically equal between weekly KBPf applications,improving photochemical efficiency,leaf thickness,SPAD reading,and bud *** use of KBPf could reduce the intervals between collections of mini-cuttings and increase nursery yields in environments that present high temperatures.

Global climate changes have increased temperatures,radiation indexes,and consequently,irregularities in rainfall in mainly tropical countries,considerably hindering plant establishment in recovering degraded *** objective of this study was to evaluate the growth and physiological characteristics of one species of each successional group:pioneer,secondary,and climax when subjected to different light intensities and hydrogel as a soil conditioner during rainy and dry *** experiment was conducted in the ecotone between Brazil’s two largest biomes,the Cerrado and the Amazon in the State of Maranh?*** parameters consisted of three species:Guazuma ulmifolia Lam.(pioneer),Astronium fraxinifolium Schott(secondary),and Cariniana rubra Gardner ex Miers(climax).There were two light intensities:70%and 100%,and two planting conditions:with and without soil conditioner(hydrogel).Gas exchanges were higher during the rainy season;the pioneer and secondary species had greater heights and photosynthetic rates in the dry period;the climax species had the lowest gas exchange and lowest recovery as rainfall *** pioneer and secondary species showed higher physiological plasticity,denoting better adaption to environments with high *** improved the photosynthetic performance of these species in the dry season and in areas with 100%sunlight.
