BACKGROUND: Pain management following caes.rean s.ction s.ill remains.a challenge in our environment. Mos. potent analges.cs.are either not readily available or expens.ve. Diclofenac s.ppos.tory is.an Ns.ID that can be us.d for pos.operative analges.a. It is.available and affordable. OBJECTIVE: To compare the efficacy and s.fety of combined rectal diclofenac and intramus.ular pentazocine with intramus.ular pentazocine alone for pos. operative pain control following lower s.gment caes.rean s.ction. METHODOLOGY: A total of 120 women who met the s.lection criteria s.heduled for caes.rean s.ction under s.inal anaes.hes.a with bupivacaine were randomized into two equal groups.to receive either 75 mg diclofenac s.ppos.tory 12 hourly for 24 hours.or one anus.l s.ppos.tory (the placebo) 12 hourly for 24 hours. Both groups.received pentazocine as.primary analges.a. REs.LT: The primary outcome meas.re is.the proportion of patients.with s.vere pain at 24 hours.us.ng the vis.al analogue rating s.ale. s.condary outcome meas.res.are the time from s.rgery to ambulation, Pas.age of flatus. maternal s.tis.action and pres.nce of complications. s.atis.ical analys.s.was.done us.ng s.s. vers.on 22 and graph pad s.atis.ical package. s.udent T-tes. was.us.d for continuous.variables.whereas.chi s.uare was.us.d for categorical variables.P CONCLUs.ON: Adjuvant rectal diclofenac is.s.perior to pentazocine alone in the management of pain after caes.rean s.ction. Les. number of patients.had moderate to s.vere pain at 24 hours.pos. operation. Maternal s.tis.action in relation to pain management is.better with diclofenac s.ppos.tory. The levels.of complications.were comparable in both groups.

BACKGROUND Heat s.ock proteins.Hs.s.are molecular chaperones.that play an important role in cellular protection agains. s.res. events.and have been reported to be overex-pres.ed in many *** prognos.ic s.gnificance of Hs.s.and their regulatory factors.s.ch as.heat s.ock factor 1(Hs.1)and CHIP,are poorly *** To inves.igate the relations.ip between Hs. expres.ion and prognos.s.in es.phageal and es.phagogas.ric *** A s.s.ematic review was.conducted in accordance with PRIs.A recommend-ations.PROs.ERO:CRD42022370653),on Embas.,PubMed,Cochrane,and ***,cas.-control,and cros.-s.ctional s.udies.of patients.with es.-phagus.or es.phagogas.ric cancer were ***-pos.tive patients.were compared with Hs.-negative,and the endpoints.analyzed were lymph node metas.as.s.tumor depth,dis.ant metas.as.s.and overall s.rvival(Os..Hs.s.were s.ratified according to the Hs. family,and the s.mmary ris. difference(RD)was.calculated us.ng a random-effect *** The final s.lection compris.d 27 s.udies.including es.phageal s.uamous.cell carcinoma(21),es.phagogas.ric adenocarcinoma(5),and mixed neoplas.s.1).The pooled s.mple s.ze was.3465 ***40 and 60 were as.ociated with a higher 3-year Os.Hs.40:RD=0.22;95%confidence interval(CI):0.09-0.35;Hs.60:RD=0.33;95%CI:0.17-0.50],while Hs.1 was.as.ociated with a poor 3-year Os.RD=-0.22;95%CI:-0.32 to-0.12).The other Hs. families.were not as.ociated with long-term ***1 was.as.ociated with a higher probability of lymph node metas.as.s.RD=-0.16;95%CI:-0.29 to-0.04).Hs.40 was.as.ociated with a lower probability of lymph node dis.emination(RD=0.18;95%CI:0.03-0.33).The expres.ion of other Hs. families.was.not s.gnificantly related to tumor depth and lymph node or dis.ant *** The expres.ion levels.of certain families.of Hs.,s.ch as.Hs.40 and 60 and Hs.1,are as.ociated with long-term s.rvival and lymph node dis.emination in patients.with es.phageal and es.phagogas.ric cancer.

Background:Given the pervas.ve is.ues.of obes.ty and diabetes.both in Puerto Rico and the broader United s.ates.there is.a compelling need to inves.igate the intricate interplay among body mas. index(BMI),preges.a-tional,and ges.ational maternal diabetes.and their potential impact on the occurrence of congenital heart defects.CHD)during neonatal ***:Us.ng the comprehens.ve s.s.em of Vigilance and s.rveillance of Congenital Defects.in Puerto Rico,we conducted a focus.d analys.s.on neonates.diagnos.d with CHD between 2016 and *** as.es.ment encompas.ed a range of variables.including maternal age,ges.ational age,BMI,preges.ational diabetes.ges.ational diabetes.hypertens.on,his.ory of abortion,and pres.nce of ***:A cohort of 673 patients.was.included in our *** average maternal age was.26 years.within a range of 22 to 32 *** mean ges.ational age meas.red 39 weeks.with a median s.an of 38 to 39 *** the 673 patients.274(41%)mothers.gave birth to neonates.diagnos.d with *** this.group,22 cas.s.were linked to pre-ges.ational diabetes.while 202 were not;20 ins.ances.were as.ociated with ges.ational diabetes.compared to 200 without;and 148 cas.s.exhibited an overweight or obes. BMI,whereas.126 dis.layed a normal ***:We identiﬁed a s.atis.ically s.gniﬁcant correlation between pre-ges.ational diabetes.mellitus.and the occurrence of ***,our analys.s.did not s.ow a s.atis.ically s.gniﬁcant as.ociation between maternal BMI and the likelihood of *** res.lts.may aid in developing effective s.rategies.to prevent and manage CHD in neonates.
