Percutaneous transhepatic cholangiography vs endoscopic ultrasound-guided biliary drainage:A systematic review

BACKGROUNd Percutaneous transhepatic cholangiography is a diagnostic and therapeutic procedure that involves inserting a needle into the biliary tree,followed by the immediate insertion of a *** ultrasound-guided biliary drainage(EUS-Bd)is a novel technique that allows Bd by echoendoscopy and fluoroscopy using a stent from the biliary tree to the gastrointestinal *** To compare the technical aspects and outcomes of percutaneous transhepatic Bd(PTBd)and *** different databases,including PubMed,Embase,***,the Cochrane library,Scopus,and Google Scholar,were searched according to the guidelines for Preferred Reporting Items for Systematic reviews and Meta-Analyses to obtain studies comparing PTBd and *** Among the six studies that fulfilled the inclusion criteria,PTBd patients underwent significantly more reinterventions(4.9 vs 1.3),experienced more postprocedural pain(4.1 vs 1.9),and experienced more late adverse events(53.8%vs 6.6%)than EUS-Bd *** was a significant reduction in the total bilirubin levels in both the groups(16.4-3.3μmol/L and 17.2-3.8μmol/L for EUSBd and PTBd,respectively;P=0.002)at the 7-d *** were no significant differences observed in the complication rates between PTBd and EUSBd(3.3 vs 3.8).PTBd was associated with a higher adverse event rate than EUSBd in all the procedures,including reinterventions(80.4%vs 15.7%,respectively)and a higher index procedure(39.2%vs 18.2%,respectively).CONCLUSION The findings of this systematic review revealed that EUS-Bd is linked with a higher rate of effective Bd and a more manageable procedure-related adverse event profile than *** findings highlight the evidence for successful EUS-Bd implementation.

Evaluation of Cytotoxicity and Genotoxicity of Insecticide Carbaryl to Flounder Gill Cells and Its Teratogenicity to Zebrafish Embryos

In this study, we determined the cytotoxicity and genotoxicity of carbamate insecticide carbaryl to flounder gill （FG） cells and its teratogenicity to zebrafish embryos. The cytotoxicity of carbaryl to FG cells was determined with methods including MTT and neutral red uptaking （NRU）, lactate dehydrogenase （LdH） releasing and Hoechst 33342 and propidiurn idodide （PI） double staining. Moderate cytotoxicity in a concentration-dependent manner was observed. The 24h-IC50 value of 53.48 ± 1.21, 59.13 ± 1.19 and 46.21 ± 1.24 mgL 1 carbaryl was obtained through MTT, NRU and LdH assays, respectively. double fluorescence staining dem- onstrated that carbaryl induced the death of FG cells mainly through necrosis. There was no significant genotoxicity found in the FG cells exposed to the highest testing concentration of carbaryl （20mgL-1, P〉0.05） as was demonstrated by Comet assay. Zebrafish embryos exposed to carbaryl at concentrations 〉10 mg L-1 displayed moderate toxic effects on the survival, spontaneous movement, hatching, heart rates of the embryos and their development, which were evidenced by yolk and pericardial sac edemas, body length reduction and tail flexure in time- and concentration-dependent manners at specific stages. The 24h-, 48h- and 96h-LC50 values of carbaryl to zebra fish embryos were 41.80 ± 1.10, 17.80 ± 1.04 and 14.46 ± 1.05 mg L-1, respectively. These results suggested that car- baryl is moderately toxic to FG ceils cultured in vitro and zebrafish embryos, and the FG cells were similar to zebrafish embryos in their sensitivity to carbaryl as 24h-IC50 and LC50 indicated.

脓毒症存活患儿健康生活质量研究

目的儿童脓毒症病死率在既往的数十年中稳步下降,但对于存活者的残疾情况所知甚少,本文旨在确定社区获得性脓毒症患儿无法恢复基础健康生活质量的发生率以及风险因素。设计回顾性队列研究。场所西雅图儿童医院。对象2012~2015年间入住普通病房或重症监护病房并在住院4 h内符合2005脓毒症诊断共识的1个月~21岁间患儿,且已被纳入医院预后评估项目并可获得基础、入院和出院时健康相关生活质量数据。干预措施无。测量方法与主要结果本文采用儿童生活质量目录TM评估入院前基础,入院时以及出院后(中位时间31 d)健康相关生活质量,确定随访中评分无法恢复至距基线4.5分内患儿与疾病特点的关系。随访发现790例患儿中,23.8%无法恢复基础健康相关生活质量,无法恢复相关因素包括脓毒性休克、年长儿童、私人保险、复杂慢性疾病、免疫抑制、中枢神经系统感染或菌血症、入住重症监护病房和长期住院。控制随访时间的多因素分析显示恢复失败与脓毒性休克(RR 1.79,95%CI 1.24~2.58),年长儿童(RR 1.02岁,95%CI 1.01~1.05),免疫抑制(RR 1.83,95%CI 1.40~2.40)和长期住院(RR 1.03d,95%CI 1.01~1.04)独立相关。结论近1/4因社区获得性脓毒症住院且存活患儿的健康相关生活质量显著临床恶化。本文确定了脓毒症不良预后的风险因素并强调脓毒症存活者出院后需要初级和专业医疗人员进行持续评估和治疗。

Comparison of cross sectional optical coherence tomography images of elevated optic nerve heads across acquisition devices and scan protocols

Background:Optic nerve head measurements extracted from optical coherence tomography(OCT)show promise for monitoring clinical conditions with elevated optic nerve *** aim of this study is to compare reliability within and between raters and between image acquisition devices of optic nerve measurements derived from OCT scans in eyes with varying degrees of optic nerve ***:Wide angle line scans and narrow angle radial scans through optic nerve heads were obtained using three spectral domain(Sd)OCT devices on 5 subjects(6 swollen optic nerves,4 normal optic nerves).Three raters independently semi-manually segmented the internal limiting membrane(ILM)and Bruch’s membrane(BM)on each scan using customized *** rater segmented each scan *** were qualitatively and quantitatively ***-rater,intra-rater and inter-device reliability was assessed for the optic nerve cross sectional area calculated from the ILM and BM segmentations using intraclass correlation coefficients and graphical ***:Line scans from all devices were qualitatively *** scans for which frame rate could not be adjusted were of lower ***-rater reliability for segmentation and optic nerve cross sectional area was better than inter-rater reliability,which was better than inter-device reliability,though all ICC exceeded *** was not impacted by the degree of optic nerve ***:Sd-OCT devices acquired similar quality scans of the optic nerve head,with choice of scan protocol affecting the *** image derived markers,variability between devices was greater than that attributable to inter and intra-rater differences.

目的:观察丰富环境刺激对缺氧缺血性脑损伤大鼠脑超微结构及神经丝蛋白(NF)的影响。方法:实验于2005-03/2006-06在新桥医院中心实验室完成。取7日龄健康Sd大鼠52只随机分为3组:①丰富环境干预组:20只,采用Rice法建立缺氧缺血性脑损伤模型,予早期抚触(15min/次,2次/d)和丰富环境(2h/次,1次/d)刺激共28d。②缺氧缺血非干预组:20只,同前造模,造模后不干预。③正常对照组:12只,不造模,不干预。饲养至1月龄时各组随机选10只进行Morris水迷宫测试学习记忆功能;行为学测定后处死大鼠取脑,免疫组织化学方法观察海马神经丝蛋白的染色情况,借助自动图像分析系统对其进行定量分析;利用透射电镜观察海马神经元超微结构、神经丝蛋白及突触情况。结果:52只进入结果分析。①隐匿平台逃避潜伏期(学习能力):缺氧缺血非干预组较正常对照组明显延长[(39.98±7.86),(26.12±4.03)s,P<0.001],丰富环境干预组较缺氧缺血非干预组缩短[(29.06±5.11)s,P<0.01],与正常对照组无差异。②跨越平台次数(记忆能力):缺氧缺血非干预组明显少于正常对照组[(2.13±1.33),(4.91±2.01)次,P<0.001],丰富环境干预组多于缺氧缺血非干预组[(4.45±1.59)次,P<0.01],与正常对照组无差异。③缺氧缺血非干预组左侧与右侧脑组织NF-H积分吸光度值之比值明显小于正常对照组和丰富环境干预组(0.398±0.110,0.975±0.011,0.821±0.138,P<0.01),后2组比较无差异。④超微结构显示缺氧缺血非干预组海马神经细胞固缩改变,线粒体肿胀,神经丝数量减少,排列稀疏,突触数量减少;丰富环境干预组海马神经元和突触无明显异常。结论:早期抚触及丰富环境刺激可以促进缺血缺氧的脑损伤恢复,脑组织神经网络重建及脑的可塑性增加是其可能的机制之一。
