AIM:To investigate the postprandial response of bone turnover markers in patients with Crohn’s disease(CD).METHODs:Fifty nine patients with CD aged 38±14years,and 45 healthy individuals matched for age and body mass index were included in the *** participants underwent an oral glucose tolerance test(OGTT)after an overnight fast and serum levels of the bone resorption marker C-terminal crosslinking telopeptide of type?Ⅰ?collagen(CTX-Ⅰ)and the bone formation marker procollagen type?Ⅰ?N propeptide were *** of the disease was assessed by calculation of the Crohn’s disease activity index(CDAI).REsULTs:serum CTX-I was significantly higher in patients compared to controls(CTX-I:453±21 pg/mL vs 365±25 pg/mL,P=0.008),and values were significantly correlated with the activity of the disease(r=0.435,P=0.001).Results from OGTT-induced suppression of CTX-I showed two different *** with more active disease(assessed as CDAI>150)had a more excessive suppression of CTX-I compared to controls(55%vs 43%Ps on remission(assessed as CDAIstrated an attenuated CTX-I suppression(30%vs 43%Ps,CTX-I suppression after oral glucose load was significantly correlated with the activity of the disease(r=0.913,PsION:The physiological skeletal response of postprandial suppression of bone resorption is maintained in patients with CD and is strongly dependent to the activity of the disease.

Is verumontanum resection needed in transurethral resection of the prostate?

Transurethral resection of the prostate is the mainstay for treatment of bladder outflow obstruction. It is a procedure that involves various complications and has a high success rate. In view of a recent publication presenting the effect of verumontanum resection on functional outcome and possible complications after TURP, the present manuscript presents the available evidence on the subject as well as the possible criticism about the technique suggested by the authors. The results available do not confirm that by resecting the verumontanum there is a clinically significant improvement in the functional outcome, however confirm that continence is not affected. The criticism probably lies in the fact that resecting such a small amount of tissue like the verumontanum(its size probably remains the same with few changes during lifetime) probably does not affect outcome, yet the resection of hyperplastic apical tissue around it may play a role in functional improvement.

New nucleos(t)ide analogues(NAs) with high genetic barrier to hepatitis B virus(HBV) resistance(such as entecavir, tenofovir) have improved the prognosis of patients with HBV decompensated cirrhosis and have prevented HBV recurrence after liver transplantation(LT). NAs are considered the most proper approach for HBV infection in patients under renal replacement therapy but their doses should be adjusted according to the patient's creatinine clearance. In addition, physi-cians should be aware of the potential nephrotoxicity. However, patients with chronic hepatitis C and decom-pensated cirrhosis can receive only one therapeutic option before LT, as well as for Hepatitis C virus(HCV) recurrence after LT, which is the combination of sub-cutaneous Peg-IFN and ribavirin. Generally, therapy for HCV after renal transplantation should be avoided. Although the optimal antiviral therapy for HCV infec-tion has not been established, attention has turned to a new, oral direct acting antiviral treatment which marks a promising strategy in prognosis and in amelioration of these diseases.

The current paper outlines the evaluation methodology developed in the framework of the neighbourhood oriented brokerage electricity and monitoring system (NOBEL) project. NOBEL is an EU funded project of the 7th Framework Programme that developed a pilot monitoring and energy brokerage system, within a local electricity network, aiming to the optimization of energy usage. A holistic evaluation approach was followed encompassing all significant aspects, focusing on the enhancement of energy usage and efficiency due to NOBEL system. More particularly, indicators related to energy and monetary impacts were measured, analysed and will be finally assessed. The preliminary analysis of the first round of pilots results is currently performed while the pilot second phase is going to take place during July and August. The final results will assess the project"s overall performance and will be used as a baseline for the wider deployment of the NOBEL services in different levels of magnitude.

Anastomotic complications occur more frequently in patients with Crohn's disease leading to postoperative intra-abdominal septic complications(IAsC). Patients with IAsC often require re-operation or drainage to controlthe sepsis and have an increased frequency of disease recurrence. The aim of this article was to examine the factors affecting postoperative IAsC in Crohn's disease after anastomoses, since some risk factors remain controversial. studies investigating IAsC in Crohn's operations were included, and all risk factors associated with IAsC were evaluated: nutritional status, presence of abdominal sepsis, medication use, Crohn's disease type, duration of disease, prior operations for Crohn's, anastomotic technique, extent of resection, operative timing, operative length, and perioperative bleeding. In this review, the factors associated with an increased risk of IAsC are preoperative weight loss, abdominal abscess present at time of surgery, prior operation, and steroid use. To prevent IAsC in Crohn's patients, preoperative optimization with nutritional supplementation or drainage of abscess should be performed, or a diverting stoma should be considered for patients with multiple risk factors.
