AIM:To evaluate the safety and effectiveness of our new cytoreductive surgery(CRS)and hyperthermic intraperitoneal chemoperfusion(HIPEC)***:Retrospective review of patients with gastrointestinal malignancies who were suitable candidates for CRS and HIPEC between 12/1/2009 and 10/1/*** clinicopathologic data were reviewed with a special focus on the surgical outcome and the postoperative morbidity and ***:Fourteen patients were *** age was 64 years;seven were *** primary tumors were:colonic(29%),appendiceal(36%),peritoneal mesothelioma(14%),gastric(7%),adenocarcinoma of unknown primary(7%),and gastrointestinal stromal tumor(7%).Eleven patients(79%)received CRS/HIPEC,three for *** patients that did not undergo CRS/HIPEC had an average peritoneal cancer index(PCI)of *** eight patients that underwent curative CRS/HIPEC had an average PCI of 10 and a completeness of cytoreduction score of 0(87.5%) or 1(12.5%).Postoperative morbidity was 36%;the worst adverse event was Grade 3 *** rate was 0%.COnCLUSIOn:CRS with HIPEC is safe and feasible at tertiary institutions with fledgling *** is an accurate predictor of surgical outcomes.

Objective To evaluate the predictive value of SYnTAX Score II （SS-II） for percutaneous coronary intervention （PCI） in octogenarian （≥ 80 years old） undergoing PCI. Methods ＆ Results Data from three consecutive years of octogenarian undergoing PCI from Ruijin Hospital （Shanghai, China） was retrospectively collected （n = 308）. Follow up clinical data at one year including all cause mortality, cardiac mortality and main adverse cardiovascular and cerebrovascular events （MACCE） were collected. Patients were stratified according to tertiles of SS-II for PCI： SS-II 〈 26 （n = 104）, SS-II： 27-31 （n = 102）, SS-II 〉 31 （n = 102）. After adjustment for confounding factors, SS-II for PCI was an independent risk factors for all cause mortality （odds ratio： 2.77, 95% CI： 1.13-8.06; P = 0.04）. Kaplan-Meier curves showed higher event rates for all cause mortality and cardiac mortality in higher tertile of SS-II for PCI （Log-Rank test P = 0.002 and P = 0.001, respectively）. SSdl for PCI predicted one year mortality in octogenarian population undergoing PCI. Conclusions In octogenarian, SS-II which incorporated clinical variables with angiographic anatomy variable was suitable in risk stratifying and predicting clinical outcomes at one year.

Over 95% of plastid proteins are nuclear-encoded as their precursors containing an n-terminal extension known as the transit peptide （TP）. Although highly variable, TPs direct the precursors through a conserved, posttranslational mechanism involving translocons in the outer （TOC） and inner envelope （TOC）. The organelle import specificity is mediated by one or more components of the Toc complex. However, the high TP diversity creates a paradox on how the sequences can be specifically recognized. An emerging model of TP design is that they contain multiple loosely conserved motifs that are recognized at different steps in the targeting and transport process. Bioinformatics has demonstrated that many TPs contain semiconserved physicochemical motifs, termed FGLK. In order to characterize FGLK motifs in TP recognition and import, we have analyzed two well-studied TPs from the precursor of RuBisCO small subunit （SStp） and ferredoxin （Fdtp）. Both SStp and Fdtp contain two FGLK motifs. Analysis of large set mutations （-85） in these two motifs using in vitro, in organello, and in vivo approaches support a model in which the FGLK domains mediate interaction with TOC34 and possibly other TOC components. In vivo import analysis suggests that multiple FGLK motifs are functionally redundant. Furthermore, we discuss how FGLK motifs are required for efficient precursor protein import and how these elements may permit a convergent function of this highly variable class of targeting sequences.

BACKGROUnD The Black/African Ancestry(AA)population has a higher prevalence of type 2 diabetes mellitus(T2DM)and a higher incidence and mortality rate for colorectal cancer(CRC)than all other races in the United States.T2DM has been shown to increase adenoma risk in predominantly white/European ancestry(EA)populations,but the effect of T2DM on adenoma risk in Black/AA individuals is less *** hypothesize that T2DM has a significant effect on adenoma risk in a predominantly Black/AA *** To investigate the effect of T2DM and race on the adenoma detection rate(ADR)in screening colonoscopies in two disparate *** A retrospective cohort study was conducted on ADR during index screening colonoscopies(age 45-75)performed at an urban public hospital serving a predominantly Black/AA population(92%)(2017-2018,n=1606).Clinical metadata collected included basic demographics,insurance,body mass index(BMI),family history of CRC,smoking,diabetes diagnosis,and aspirin *** dataset was combined with a recently reported parallel retrospective cohort data set collected at a suburban university hospital serving a predominantly White/EA population(87%)(2012-2015,n=2882).RESULTS The ADR was higher in T2DM patients than in patients without T2DM or prediabetes(35.2%vs 27.9%,P=0.0166,n=981)at the urban public *** analysis of the combined datasets showed that T2DM[odds ratio(OR)=1.29,95%confidence interval(CI):1.08-1.55,P=0.0049],smoking(current vs never OR=1.47,95%CI:1.18-1.82,current vs past OR=1.32,95%CI:1.02-1.70,P=0.0026),older age(OR=1.05 per year,95%CI:1.04-1.06,Pnit,95%CI:1.01-1.03,P=0.0003),and male sex(OR=1.87,95%CI:1.62-2.15,Pncreased ADR in the combined datasets,but race,aspirin use and insurance were *** T2DM,but not race,is significantly associated with increased ADR on index screening colonoscopy while controlling for other factors.

A simulation study of formation permeability as a function of methane hydrate concentration

We modeled and studied the permeability of methane hydrate bearing formations as a function of methane hydrate concentration by artificially varying the T2 distribution as well as using a tube-sphere *** varied the proportion of irreducible and movable water as well as the total porosity associated with the T2 distribution and found the normalized permeability as a function of methane hydrate concentration is dependent of these *** a tube-sphere model,we increased the methane hydrate concentration by randomly placing methane hydrate crystals in the pore spaces and computed the permeability using either the Schlumberger T2 relaxation time formula or a direct calculation based on Darcy＇s law assuming Poiseuille *** experimental measurements reported in the literature show there is a methane hydrate concentration range where the permeability remains relatively *** found that when the Schlumberger T2 relaxation time formula is used the simulation results show a curve of normalized permeability versus methane hydrate concentration quite close to that predicted by the Masuda model with n = *** the permeability was directly calculated based on Darcy＇s law,the simulation results show a much higher normalized permeability and only show a trend consistent with the experimental results,i.e.,with a permeability plateau,when the methane hydrate crystals are preferentially placed in the tubes,and the higher the preferential probability,the larger the range where the permeability has a plateau.

赤道太平洋岩芯R C11—209的第四纪碳酸盐记录的离散几乎是同一地区DSDP158孔晚中新世岩芯记录离散的两倍。碳酸盐时序的频谱分析表明,中新世的碳酸盐记录主要由频率极低的组分组成,其离散密集在相当于周期大约400千年的频率上,第四纪记录也以低频组分为主,但其离散密集在周期相当于100千年和40千年的频率上。互频谱分析表明,两种记录都与计算出的地球轨道偏心率的变化相关,并且表明,第四纪记录与计算出的地轴斜度记录相关。因而地球轨道参数的这些离散可以用来“调准”这两项记录的时标。根据400千年周期附近的两种记录中的离散的相似性以及观测到的碳酸盐波动与全球冰川体积变化之间的联系(如氧同位素测定所指出的)认为,占优势的400千年周期自从中新世形成时起就与北极冰帽的长期变动直接或间接地联系在一起。第四纪碳酸盐频谱的主要差别在于与100千年周期有关的频率处的离散值明显增大,这一增大被认为与北半球冰帽的冰期和间冰期变动直接或间接有关。
