Methicillin Resistant Staphylococcus aureus in Pigs and Pig Breeders in Three Regions of Cameroon

The spread of antibiotic resistance, whether in the community or in the hospital, has recently become a major public health problem. Moreover, livestock seems to be a reservoir of resistant microorganisms such as porcine methicillin resistant Staphylococcus aureus (P-MRSA) whose carriage and transmission was mainly demonstrated in persons with occupational exposure to pigs. Much uncertainty remains about the public health implications of P-MRSA. To address concerns that exist regarding the zoonotic risk that pig carriers pose to breeders, the prevalence of P-MRSA in pigs and pig breeders was determined among 152 pig breeders in three regions of Cameroon. Materials and Method: Participants in this study came from Adamawa, Far North and Littoral regions of Cameroon. A total of 152 pig breeders or farm workers participated in this study. After having collected some social and cultural data with the aid of a questionnaire, nasal swabs were collected from pigs (n = 275) and their breeders using the standard collection procedures and placed in cooler containing cold accumulators. For each sample, microbiological assays were done as well as antibiotic susceptibility tests. Results: MRSA was isolated from 25 out of 275 pigs sampled (9.09%) and from 32 out of 152 (21.05%) pig breeders sampled;33 breeders (21.71%) were carriers of S. aureus including 32 MRSA and one MSSA. The prevalence of MRSA nasal carriage was 21.05% ± 6.48. A significant negative association between body protection and nasal carriage of MRSA (OR = 0.29, 95% CI [0.093;0.93];p = 0.04) was observed. In each region, similar strains of MRSA were isolated both in pigs and their breeders with the same antibiotic resistant profile. Conclusion: The prevalence of MRSA though not high compared to European countries warrants further research as data on this zoonosis is scarce in our context. Hypertensive disorders of pregnancy are among the leading causes of severe maternal morbidity and mortality, particularly in developing countries. The aim was to determine the maternal and perinatal prognosis of patients with hypertensive disorders of pregnancy as a function of serum ionised calcium levels. Material and Methods: A cross-sectional analytical study of 114 patients with arterial hypertension during pregnancy or during pregnancy or in the postpartum period at the HGR/Panzi from 1 January 2021 to 30 June 2022, text was entered using Microsoft Office Word 2010 and the tables were analysed using Excel 2010. The data was analysed using SPSS version 20.0 and Stata 14.0. The associations of the variables were calculated using Pearson's chi-square test, with a significance threshold set at a value of p < 0.05. Study of risk factors, Odds ratios and their confidence intervals were estimated in a univariate analysis. The most determining factors were identified by multivariate analysis using the Forward conditional logistic regression model. Results: The mean gestational age was 34.43 ± 4.327 amenorheas weeks, 46.6% of patients had a vaginal delivery, 66.65% of which were indicated for maternal prognosis, maternal complications were associated with maternal hypocalcaemia in 81, 82% (P = 0.043) and an OR = 3.255 (P = 0.0158) threefold risk that the patient presenting with a complication is likely to be in a state of hypocalcaemia at 95% confidence index, and fetal prognosis was not significantly related to maternal calcaemia. Objective: Bleeding during the first trimester of pregnancy is common and can be a sign of complication that often necessitates obstetric ultrasound for the assessment of the haemorrhage, and of fetal well being. The aim of this study was to determine the causes of first trimester bleeding on obstetrical ultrasound in our area. Method: It was a cross-sectional descriptive study during a six-month period from May to October 2017. All women who referred to the Regional center of medical imaging of Ngaoundere who agreed to participate in the study with first trimester bleeding were evaluated with clinical history and ultrasonography (US). Ultrasonographic exam was done via transabdominal or endovaginal approaches using GE LOGIQ 7 scanner brand. Microsoft Office Excel 2010 and SPHINX V 4.0 were used for data analysis. Results: A total of 121 of pregnant women who presented vaginal bleeding during the first trimester were enrolled into the study. These Ultrasound examinations represented 37.93% of all obstetric ultrasounds in the first trimester. The patients in this study ranged in age from 16 to 47 years with an average of 30 years. 50 (41.32%) were pregnant for the first time and 24 (19.83%) were primiparous. Mean gestational age was 9 weeks with the extremes of 5 and 12 weeks. 97 (80.2%) reported spontaneous bleeding while in 24 (18.8%) bleeding was mostly caused by sexual intercourse 11 (45.8%). Pelvic pain 91 (75.2%) was the most associated sign. 36 (29.8%) patients were diagnosed as threatened abortions. 27 (22.3%) were diagnosed with incomplete abortions. 4 (3.3%) cases showed an ectopic gestation. 9 (7.4%) cases showed complete abortions. Concordance between clinical diagnosis and ultrasound results was 84%. Conclusions: Bleeding in the first trimester of pregnancy is common in our area.
