Evaluation of vitamin D level in patients from neurosurgical intensive care unit

Vitamin D plays an important role in maintaining normal bone metabolism. Recent studies have suggested that vitamin D influences many other physiological processes, including muscle function, cardiovascular homeostasis, nerve function, and immune response. Furthermore, accumulated evidence suggests that vitamin D also mediates the immune system response to infection. Critical neurosurgical patients have higher infection and mortality rates. To correlate vitamin D deficiency to the immunological status of neurosurgical intensive care unit patients, we detected serum vitamin D level in 15 patients with clinically suspected infection and 10 patients with confirmed infection. Serum level of 25-hydroxyvitamin D, the primary circulating form of vitamin D, was significantly decreased in patients with suspected or confirmed infection after a 2-week neurosurgical intensive care unit hospitalization, while serum level of 1,25-dihydroxyvitamin D, the active form of vitamin D, was significantly decreased in patients after a 4-week neurosurgical intensive care unit hospitalization. These findings suggest that vitamin D deficiency is linked to the immunological status of neurosurgical intensive care unit patients and vitamin D supplementation can improve patient＇s immunological status.

Several clinical trials have demonstrated the potent antiviral efficacy of entecavir (ETV), and this relatively new nucleoside analogue drug has rapidly become a frequently prescribed therapy for chronic hepatitis B (CHB) worldwide. While the studies have also shown a good overall safety profile for ETV, adverse drug reactions (ADRs) in patients with advanced cirrhosis have been reported and represent a broad spectrum of drug-induced injuries, including lactic acidosis, myalgia, neuropathy, azotemia, hypophosphatemia, muscular weakness, and pancreatitis, as well as immune-mediated responses (i.e., allergic reactions). Cutaneous ADRs associated with ETV are very rare, with only two case reports in the publicly available literature; both of these cases were classified as unspecified hypersensitivity allergic (type I) ADR, but neither were reported as pathologically proven or as evaluated by cytokine release analysis. Here, we report the case of a 45-year-old woman who presented with a generalized maculopapular rash after one week of ETV treatment for lamivudine-resistant CHB. The patient reported having experienced a similar skin eruption during a previous three-month regimen of ETV, for which she had self-discontinued the medication. Histopathological analysis of a skin biopsy showed acanthotic epidermis with focal parakeratosis and a perivascular lymphocytic infiltrate admixed with interstitial eosinophils in the papillary and reticular dermis, consistent with a diagnosis of drug sensitivity. A lymphocyte stimulation test showed significantly enhanced IL-4, indicating a classification of type IVb delayed hypersensitivity. The patient was switched to an adefovir-lamivudine combination regimen and the skin eruption resolved two weeks after the ETV withdrawal. This case represents the first pathologically and immunologically evidenced ETV-induced delayed type hypersensitivity skin reaction reported to date. Objective： Chemoport-related thrombosis （CRT） is a serious complication that causes morbidities and interruptsadministration of intravenous cancer therapy. We investigated the incidence and risk of CRT in colorectal cancer（CRC） patients treated with bevacizumab （BEV）.Methods： We retrospectively reviewed 1,534 CRC patients who received chemotherapy with or without BEVusing a chemoport between 2014 and ***： The participants had a median age of 58 （18-85） years, and 60.3% were male. All participants werestratified into three groups： adjuvant chemotherapy （AC）（n=670）, palliative chemotherapy （PC） without BEV（n=356）, and PC with BEV （n=508）. The median follow-up was 20.19 （interquartile range, 14.07-27.19） *** occurred in 3.8% of all patients; incidence of symptomatic and asymptomatic CRT was 2.9% and 0.9%,respectively. CRT occurred more in patients with BEV （5.7%） than in patients without BEV （2.9%, P=0.008）. Thecumulative incidence of CRT in patients administered PC with BEV was significantly higher than that in thoseadministered AC （P=0.011） and there was a trend toward increased CRY in patients administered PC with BEVcompared with those administered PC without BEV （P=0.044）. Multivariate analysis found that BEV treatment wasthe only variable that was significandy associated with CRT （hazard ratio, 2.06; 95% confidence interval, 1.24-3.43;P=0.006）.Conclusions： BEV treatment was significandy associated with increased incidence of CRT in CRC patients.

The effects of the combination of exercise and TRT on symptoms of late-onset hypogonadism （LOH） and the durability of response after cessation of TRT were investigated. A total of fifty patients with erectile dysfunction （ED） who had a sedentary lifestyle and low serum total testosterone （T） levels were enrolled and followed for 20 weeks. Patients were randomly divided into two groups; all of them received T gel for 12 weeks and it was discontinued for 8 weeks. Patients assigned to Group II were offered a supervised exercise program for 20 weeks. Measurement of serological testing was performed and self-assessment questionnaires and Global Assessment Question （GAQ） were asked. Baseline characteristics and the initial symptom scores showed no significant difference between the two groups. Serum total T levels and the symptom scores were increased at 12 weeks in both groups, and Group II showed better results with statistical significance. There was a decrease in T levels and worsening of symptom scores at week 20 compared to week 12 in both groups, and Group II showed better results with statistical significance. On the GAQ, Group II showed higher ratio of ＂yes＂ at week 12 and the same tendency was sustained at week 20 with significant difference between two groups. The combination of exercise and TRT showed significant improvements in serum T levels and LOH symptoms compared to TRT alone. In addition, these improvements were maintained in the combination group with continuous exercise, even after cessation of TRT.

AIM:To evaluate the applicability of AIMS65 scores in predicting outcomes of peptic ulcer ***:This was a retrospective study in a single center between January 2006 and December *** enrolled 522 patients with upper gastrointestinal haemorrhage who visited the emergency *** patients were regarded as those who had rebleeding within 30 d from the first endoscopy as well as those who died within 30 d of visiting the Emergency room.A total of 149 patients with peptic ulcer bleeding were analysed,and the AIMS65 score was used to retrospectively predict the high-risk ***:A total of 149 patients with peptic ulcer bleeding were *** poor outcome group comprised 28 patients[male:23(82.1%)vs female:5(10.7%)]while the good outcome group included 121patients[male:93(76.9%)vs female:28(23.1%)].The mean age in each group was not significantly *** mean serum albumin levels in the poor outcome group were slightly lower than those in the good outcome group(P=0.072).For the prediction of poor outcome,the AIMS65 score had a sensitivity of35.5%(95%CI:27.0-44.8)and a specificity of 82.1%(95%CI:63.1-93.9)at a score of *** AIMS65 score was insufficient for predicting outcomes in peptic ulcer bleeding(area under curve=0.571;95%CI:0.49-0.65).CONCLUSION:The AIMS65 score may therefore not be suitable for predicting clinical outcomes in peptic ulcer *** albumin levels may be a risk factor associated with high mortality in peptic ulcer bleeding.
