BACKGROUND Cervical necrotizing fasciitis(CNF)is a rare,aggressive form of deep neck space infection with significant morbidity and mortality *** surgical debridement acts as the cornerstone of CNF treatment;however,it often results in defects requiring complex *** SUMMARY We report two cases in which the keystone flap(KF)was used for CNF defect coverage:Case 1,an 85-year-old patient with CNF in the anterior neck,and Case 2,a 54-year-old patient with CNF in the posterior *** patients received empirical intravenous antibiotic therapy and underwent serial debridement,enabling adequate wound preparation and *** final defect size measured 5.5 cm×12 cm in Case 1 and 6 cm×11 cm in Case *** defect coverage,we employed an 8 cm×19 cm type II KF based on perforators from the superior thyroid artery in Case 1 and a 9 cm×18 cm type II KF based on perforators from the transverse cervical artery in Case *** flaps showed complete *** postoperative complications occurred in both cases,and favorable outcomes were observed at 7-and 6-month follow-ups in case 1 and 2,*** We effectively treated CNF-associated defects using the KF technique;KF is viable for covering CNF defects in carefully selected cases.

Background:The optimal surgical timing and clinical outcomes of ventricular septal defect(VSD)closure in neo-nates remain *** aimed to evaluate the clinical outcomes of VSD closure in neonates(age≤30 days).Methods:We retrospectively reviewed 50 consecutive neonates who underwent VSD closure for isolated VSDs between August 2003 and June *** for the procedure included congestive heart failure/failure to thrive and pulmonary *** adverse events(MAEs)were deﬁned as the composite of all-cause mortality,reoperation,persistent atrioventricular block,and signiﬁcant(≥grade 2)valvular ***:The median age and body weight at operation were 26.0 days(interquartile range[IQR],18.8–28.3)and 3.7 kg(IQR,3.3–4.2),*** median follow-up duration was 110.4 months(IQR,56.8–165.0).Seven patients required preoperative respiratory support,andﬁve had signiﬁcant(≥grade 2)preoperative valvular *** early mortality occurred due to irreversible cardiogenic shock;no late mortality was *** reopera-tion was due to hemodynamically signiﬁcant residual VSD at 103.8 months *** overall survival,freedom from reoperation,and freedom from MAE at 15-years were 98.0%,96.3%,and 94.4%,***-operative mechanical ventilation was associated with a longer duration of postoperative mechanical ventilation(pmight be indicated earlier in neonates with respiratory compromise.

This study proposes a hybridization of two efficient algorithm’s Multi-objective Ant Lion Optimizer Algorithm(MOALO)which is a multi-objective enhanced version of the Ant Lion Optimizer Algorithm(ALO)and the Genetic Algorithm(GA).MOALO version has been employed to address those problems containing many objectives and an archive has been employed for retaining the non-dominated *** uniqueness of the hybrid is that the operators like mutation and crossover of GA are employed in the archive to update the solutions and later those solutions go through the process of MOALO.A first-time hybrid of these algorithms is employed to solve multi-objective *** hybrid algorithm overcomes the limitation of ALO of getting caught in the local optimum and the requirement of more computational effort to converge *** evaluate the hybridized algorithm’s performance,a set of constrained,unconstrained test problems and engineering design problems were employed and compared with five well-known computational algorithms-MOALO,Multi-objective Crystal Structure Algorithm(MOCryStAl),Multi-objective Particle Swarm Optimization(MOPSO),Multi-objective Multiverse Optimization Algorithm(MOMVO),Multi-objective Salp Swarm Algorithm(MSSA).The outcomes of five performance metrics are statistically analyzed and the most efficient Pareto fronts comparison has been *** proposed hybrid surpasses MOALO based on the results of hypervolume(HV),Spread,and *** primary objective of developing this hybrid approach has been achieved *** proposed approach demonstrates superior performance on the test functions,showcasing robust convergence and comprehensive coverage that surpasses other existing algorithms.

背景与目的奥沙利铂、伊立替康、氟尿嘧啶和亚叶酸的联合化疗方案(FOLFIRINOX)极大提高了晚期胰腺癌患者的生存期。然而,FOLFIRINOX方案作为吉西他滨失败后二线治疗的疗效尚未进行前瞻性的检测。我们研究了减量FOLFIRINOX方案治疗吉西他滨抵抗的晚期胰腺癌患者的可行性和安全性。方法在14家医院进行了这一多中心II期前瞻、开放标签的单臂研究。经组织学确诊为胰腺浸润性导管腺癌、有可测量或可评价的病灶、东部肿瘤协作组体能状况评分0或1分、器官功能良好、年龄19岁或以上的患者符合入组条件。减量FOLFIRINOX方案的组成:奥沙利铂65 mg/m2、伊立替康135 mg/m2和亚叶酸400 mg/m2,第1天静脉注射;5?氟尿嘧啶2000 mg/m2,第1–2天持续静脉输注46 h;每2周重复。主要终点为FOLIFLIOX治疗起始后的无进展生存期。次要终点为客观缓解率、疾病控制率、总生存期、安全性和耐受性。采用Kaplan?Meier法评估了总生存期和无进展生存期。结果我们招募了来自14个中心的39例患者。客观缓解率为10.3%,疾病控制率为64.1%。6个月和1年总生存率分别为59.0%和15.4%。中位无进展生存期和总生存期分别为3.8个月[95%置信区间(confidence interval,CI)=1.5–6.0个月]和8.5个月(95%CI=5.6–11.4个月)。3或4级不良事件为中性粒细胞减少(41%)、恶心(10.3%)、食欲减退(10.3%)、贫血(7.7%)、黏膜炎(7.7%)、肺炎/胸膜融合(5.1%)和乏力(5.1%)。发生1例由感染性休克引起的治疗相关死亡。结论减量FOLFIRINOX方案有望作为二线方案治疗吉西他滨抵抗的胰腺癌。
