Pretransplantation fetal-maternal microchimerism in pediatric liver transplantation from mother

AIM To investigate the rates of pretransplantation fetalmaternal microchimerism(MC) and its effect on rejection in children receiving maternal liver grafts. METHODS DNA or blood samples before liver transplantation(LT) were available in 45 pediatric patients and their mothers. The presence of pretransplantation MC to non-inherited maternal antigens(NIMAs)(NIMA-MC) in the peripheral blood was tested using nested PCRsingle-strand conformation polymorphism analysis for the human leukocyte antigen(HLA)-DRB1 alleles. NIMA-MC was successfully evaluated in 26 of the 45 children. Among these 45 pediatric LT recipients,23 children(51.1%) received transplants from maternal donors and the other 22 from non-maternal *** Among these 26 children,pretransplantation NIMAMC was detected in 23.1%(n = 6),6.1(range,0.8-14) years after birth. Among the children with a maternal donor,the rate of biopsy-proven cellular rejection(BPCR) was 0% in patients with NIMA-MC positivity(0/3) and those with HLA-DR identity with the mother(0/4),but it was 50% in those with NIMA-MC negativity(5/10). Patients with NIMA-MC positivity or HLA-DR identity with the mother showed significantly lower BPCR rate compared with NIMA-MC-negative patients(0% vs 50%,P = 0.04). NIMA-MC-positive patients tended to show lower BPCR rate compared with NIMAMC-negative patients(P = 0.23). CONCLUSION The presence of pretransplantation NIMA-MC or HLADR identity with the mother could be associated with BPCR-free survival in pediatric recipients of LT from maternal donors.

The Outcome of the Chemotherapy and Oncothermia for Far Advanced Adenocarcinoma of the Lung: Case Reports of Four Patients

Lung cancer is one of the most aggressive and lethal form of cancers. Patients with far advanced lung cancer are treated by chemotherapy with or without radiotherapy. However, median survival of these patients is less than 6 months. To increase survival and quality of life for these patients, various forms of complementary treatments have been tried in clinical practices, and oncothermia is supposed to be one of the promising candidates. From May 2008 to November 2013, 4 patients with far advanced lung adenocarcinoma (stages IIIB and IV) were treated with oncothermia in addition to conventional chemotherapy at Gangnam Severance Hospital and Bundang CHA Hospital. All these patients have survived for more than 2 years.

Effects of Band Therapy Using Music on Grasping Power, Depression, and Personal Relationships in Nursing-Home-Dwelling Elderly Individuals

The current study examined the effects of band therapy using music on grasping power, depression, and personal relationships among residents of a nursing home. Thirty subjects participated in the study. The band therapy included greetings, warm-up exercises with music, singing with dance, playing instruments, closing speech, and stretching with background music. Band therapy was held for 40 minutes once per week, for a total of four sessions, in the activity room of the nursing home. Findings showed that grasping power, depression, and personal relationships were improved at posttest, but the differences were not statistically significant. A better study design to compare the effects of band therapy with the other group, and a more simple and repeated intervention for the elderly to follow without stress might be necessary.

目的:基于MRI影像测量肾脏区域穴位的针刺危险深度及有效安全深度,为临床应用提供影像学依据。方法:采集61例健康受试者腹部增强的T1横断面MRI影像,分析双侧胃俞、三焦俞、肾俞、胃仓、肓门和志室的解剖结构,比较左右两侧及不同体型受试者直刺、斜刺危险深度和有效安全深度。结果:双侧肾脏区域穴位直刺、斜刺危险深度和有效安全深度比较,差异均无统计学意义(P>0.05)。适中组受试者双侧肾脏区域穴位直刺、斜刺时的危险深度和有效安全深度均大于瘦人组(P<0.05),胖人组受试者均大于适中组和瘦人组(P<0.05)。结论:肾脏区域穴位的针刺安全深度与体型有关,对于中等和稍胖体型者可适当增加针刺深度以加强疗效;对于偏瘦体型者应谨守安全的进针方向和深度,避免发生针刺意外。

Differences in Skin Properties of Korean Women at the Initial Aging Phase

Many studies on aging have focused on evaluating differences between older and younger people, but only a few have focused on differences in skin properties among subjects from the same age group according to their skin aging status. In this study, we evaluated the facial skin condition and life style factors in 110 Korean women aged 25 to 35 in an attempt to evaluate factors which may affect the skin aging status in the initial aging phase. The facial skin condition of 110 healthy Korean women was assessed over two successive 6-month periods, summer and winter. Using clinical assessments including aging, wrinkles and skin’s elasticity values, the subjects were divided into 7 groups. Then, various facial skin conditions and life style factors were examined between a severe aging group and mild aging group. In the severe aging group, the mean value pH was lower and the mean value of water content was slightly lower than that of women in the mild aging group. Also, the seasonal site variation in water content and sebum secretion level were significantly higher in the severe aging group than in the mild aging group. Topical sunscreen using percentage was not significantly different between the two groups. However, the number of cosmetic subject use was slightly higher in the mild aging group than in the severe aging group. The study suggested that there were several differences in skin characteristics between women in the severe aging group and in the mild aging group at the initial aging phase. Seasonal site variation between cheek and forehead was the most dominant differences. We also considered that life style factors such as cosmetic use could affect skin aging status.

Perforation of the colon by invading recurrent gastrointestinal stromal tumors during sunitinib treatment

The molecular targets of sunitinib are receptor tyrosine kinases (RTKs),and this drug has also been known to exert blocking effects on the activation of KIT,which is similar to the mechanism of action of imatinib. Moreover,sunitinib has an additional anti-angiogenic effect through its inhibition of the vascular endothelial growth factor receptor activation. We report here a 70-year-old patient diagnosed with a recurrent gastrointestinal stromal tumor (GIST),which invaded the transverse colon and led to a perforation during sunitinib treatment. A computed tomography scan and 3-dimensional reconstruction showed necrosis of the recurrent hepatic mass and perforation of the invaded transverse colon. After percutaneous drainage of the intraperitoneal abscess,antibiotic treatment and restricted diet,the condition of the patient improved. The present case is the first to report that sunitinib,which is administered to treat GIST resistant to imatinib,can cause unexpected colon perforation and subsequent peritonitis.

Peroxisome proliferator-activated receptor-delta agonist ameliorated inflammasome activation in nonalcoholic fatty liver disease

AIM: To evaluate the inflammasome activation and the effect of peroxisome proliferator-activated receptors(PPAR)-δ agonist treatment in nonalcoholic fatty liver disease(NAFLD) ***: Male C57BL/6J mice were classified according to control or high fat diet(HFD) with or without PPAR-δ agonist(GW) over period of 12 wk [control, HFD, HFD + lipopolysaccharide(LPS), HFD + LPS + GW group]. Hep G2 cells were exposed to palmitic acid(PA) and/or LPS in the absence or presence of ***: HFD caused glucose intolerance and hepatic steatosis. In mice fed an HFD with LPS, caspase-1 and interleukin(IL)-1β in the liver were significantly increased. Treatment with GW ameliorated the steatosis and inhibited overexpression of pro-inflammatory cytokines. In Hep G2 cells, PA and LPS treatment markedly increased m RNA of several nucleotide-binding andoligomerization domain-like receptor family members(NLRP3, NLRP6, and NLRP10), caspase-1 and IL-1β. PA and LPS also exaggerated reactive oxygen species production. All of the above effects of PA and LPS were reduced by GW. GW also enhanced the phosphorylation of AMPK-α.CONCLUSION: PPAR-δ agonist reduces fatty acidinduced inflammation and steatosis by suppressing inflammasome activation. Targeting the inflammasome by the PPAR-δ agonist may have therapeutic implication for NAFLD.

Sugar, Starch, and Proline in Peach Trees Exposed to Freezing Temperatures during Dehardening

Freezing temperatures cause different levels of freezing injury and change biochemical components of trees. In this study, the range of survival temperature and variation in electrolyte leakage, and in concentrations of sugar, starch, and proline were determined in peach twigs (Jinmi, Changhowonhwangdo, and Kawanakajima Hakuto) exposed to artificially controlled freezing temperature. Freezing temperatures at which the plants were damaged by stress were found to be -21°C in Jan. and Feb., -18°C in Mar., and -6°C in Apr. Electrolyte leakage increased as temperature decreased from -15°C to -24°C at each assessment time. Sugar gradually decreased after the endodormancy period in the late part of winter, and sugar concentration was overall lower in freezing temperature treatments, while starch increased as a result of changing seasons from Jan. to Apr. The trend of proline concentration was related to the survival rate of peach trees during winter. This study suggests possible safe temperatures for survival and cultivation of peach trees as well as identifying freezing temperature effects on internal components of freeze-injured peach twigs.
