Patient Experience at Endoscopy Centers in Three West African Countries during the COVID-19 Pandemic

Corona virus infection and the risk of contagiousness to medical personnel or patients during endoscopy necessitated further measures in the endoscopy rooms. The objective of this study was to assess the experience of patients in endoscopy rooms during the COVID-19 epidemic in West Africa. Patients and Methods: This was a multicenter cross-sectional study that took place over a period of 3 months (June to August 2020) in endoscopy centers in Burkina Faso, Benin and Niger. An online questionnaire was sent to patients who had performed upper digestive endoscopies, in 8 digestive endoscopy centers including private, public and religious structures. Depending on the choice of patients, questionnaires were either sent electronically or completed in the endoscopy room with the help of field investigators. The choice of centers was random from the list of centers and descriptive analyses were carried out. Results: A total of 294 patients responded to the online questionnaire. There were 37 lower and 257 upper gastrointestinal endoscopies. The female sex represented 52.1%. The wait times for obtaining an endoscopy appointment were considered satisfactory by 281 patients 95.6%. In the endoscopy departments, 112 patients, or 38.1%, were questioned about the risk factors for contamination of SARS-CoV-2. Among the risk factors found, 6 patients (2.0%) would have traveled abroad in the 2 weeks preceding the examination, 4 patients 1.4% had already been in contact with a subject at risk. The most frequent symptoms were chest pain (80 cases;27.2%), flu-like syndrome (29 cases;9.9%), cough (40 cases;13.6%), fever (46 cases;15.6%). In contrast, ageusia (7 cases;2.4%) and anosmia (5 cases;1.7%) were only found in very few patients. Twenty-two patients (7.5%) felt highly exposed to COVID-19 during the endoscopy examination and 144 patients (48.9%) rated the examination as satisfactory. Indications and Findings of Upper Gastrointestinal Endoscopy in Elderly Patients in Parakou, Republic of Benin

Introduction: Elderly people are considered fragile and at greater risk of having malignant gastrointestinal tumors. The objective of this work was to report the reasons for performing gastrointestinal endoscopy and the lesions found during the endoscopy of this target population in Parakou. Patients and Study Methods: This was a descriptive and cross-sectional study with a retrospective collection of data from January 2016 to December 2017, then from January 2020 to December 2021. It took place in the Regional Teaching Hospital of Borgou-Alibori in Parakou and in the private gastrointestinal endoscopy center of Parakou (Northern Gastrointestinal Exploration Center). All patients aged at least 60 years who had undergone an upper gastrointestinal endoscopy during the study period were included. The variables studied were: the sex, age, indications for the examination, endoscopic lesions and data from the anatomo-pathological examination. Results: In total, out of 1540 upper gastrointestinal endoscopies performed during the study period, 249 (16.17%) involved patients aged 60 years and over. The sex ratio was 1.26. The main indication for the examination was epigastric pain (123 cases, i.e. 49.40%) followed by vomiting (53 cases, i.e. 21.29%). In terms of lesions, non-tumorous gastropathy came first in the stomach (206 cases, i.e. 82.73%) while esophageal lesions were dominated by esophageal candidiasis and cardial incompetence (39 cases, i.e. 15.66% in each of the two situations). In the duodenum, ulcer was noted in 30 patients (12.05%). In 38 patients, 12 (31.58%) were tested positive for Helicobacter pylori infection. Cancers of the gastrointestinal tract were confirmed in 11 patients (4.42%). Conclusion: Upper gastrointestinal endoscopy remains an excellent examination for the exploration of the upper gastrointestinal tract. Assessment of Regular Physical Activity in the Care of Hypertensive Patients at the Cardiology Institute of Abidjan and at the National Police Hospital (Ivory Coast)

Aim: Physical activity and other lifestyle changes are an important part of the guidelines for anti-hypertensive therapy. The effect of physical activity on hypertension and heart disease has been well described. No study in our context has been done on this issue. The aim of this study was to evaluate regular physical activity in the management of hypertensive patients at the Cardiology Institute of Abidjan and the National Police Hospital. Material and methods: This was a prospective and descriptive study held in the External Consultations and Rehabilitation Departments of the aforementioned structures. It started on July 20, 2017 and ended on September 7, 2017. The data were analyzed on SPSS version 22. A p-value of less than 0.05 was considered to be statistically significant. Results: Out of the 190 hypertensive patients recruited and educated, only 70 actually participated in the physical activities that took place in the Rehabilitation Department of Heart Institute. The mean age of the patients was 52 years ± 15 years. The most affected age group was between 40 and 49 years of age (47.40%).There was a male predominance with a sex ratio of 1.11. 63.16% of the patients had a superior. Modalities of Prescription of Regular Physical Activity by Cardiologists in the Management of Hypertensive Patients at the Institute of Cardiology of Abidjan

Introduction-Objective: The clinical management of hypertension is well codified. It is based on drug and non-drug therapies, including regular physical activity. The aim was to evaluate the knowledge of physicians on the importance of regular physical activity (RPA) in the care of hypertensive patients at the Institute of Cardiology of Abidjan (ICA). Material and Methods: That was a prospective and descriptive study carried out from July 20, 2019 to September 7, 2019 at the Outpatient Department of the ICA. Our study population consisted of 32 physicians from the Outpatient Department of the ICA. To achieve our goal, we developed a questionnaire for the physicians, which included seventeen items related to the socio-occupational characteristics, to the level of knowledge of hypertension, to the attitudes and the practices in the management of hypertension, and to the factors limiting the accessibility to the practice of a physical activity. Results: The majority of the health professionals in our study were men (62.50%). Among 32 physicians, 56.25% were cardiologists and 75% had a seniority in position of 1 to 4 years. 81.25% of the physicians responded that the hypertensive patients could not only benefit from non-drug means. The suggestion of a complementary physical activity (jogging and brisk walking) to the hypertension treatment was found 137.5 times. Jogging and brisk walking were the main types of physical activity which were suggested to patients. All the physicians responded 3 times per week when they were asked about the frequency of activities proposed for the clinical management of patients with hypertension. Regarding the medical prescription on the type, frequency, and duration of RPA to patients, there was some discrepancy, although the majority of healthcare professionals recommended brisk walking and jogging. On the Patterns of the Occurrence of Diarrheas in the Southern Benin

The present study focused on the analysis of the risk factors of diarrhea in Southern Benin. Data have been collected from 442 peasant households on the basis of a stratified random sampling method in the targeted communities, with information on the source of water provision, the location of the village, the usual defecation place, the site of refusing dumping and the presence of trash dump in the vicinity of the household. Data have been analyzed using descriptive statistics. In addition, analyses of variance and log-linear analyses have been applied to test the various factors in the outbreak of diarrheas. Results have allowed a hierarchical organization of the drinking water sources with regards to their use and to the cases of diarrheas (boreholes: p 0.001;rainwater and territorial water: p 0.001;wells: p = 0.547 and pipe-borne water: p = 0.277. If we rule out people’s insanitary behaviors, the impact of some risk factors such as the sites of waste disposal (2%) and excreta disposal (1%) is of little influence in comparison to the water supply source and the place of residence, which play respectively for 62% and 30% in the outbreak of episodes of diarrheas in households. It follows that, to curb the diarrhea morbidity, there is the need to improve the water supply policy and the environmental sanitation.

Evaluation of the Microbiological and Nutritional Quality of Fermented-Dried Lesser African Threadfin (<i>Galeoides decadactylus</i>) Used as Food Supplement in Southern Benin

Lesser African threadfin (Galeoides decadactylus) is a nutritious marine fish, popular and widely used for drying-fermentation. The present work aims to evaluate the nutritional and microbiological quality of dried-fermented Lesser African Threadfin, currently used in food preparing in southern Benin. Four major sites of drying-fermentation fishes in southern Benin were investigated and dried-fermented Lesser African Threadfin was collected for quality control. Results indicated that the dried-fermented fishes were good sources of nutriments with a moisture content ranging from 44.62% ± 0.68% to 55.33% ± 0.23%. Proteins contents are ranged from 15.26% ± 0.32% to 22.95% ± 0.71%. All samples analyzed were rich in minerals such as magnesium, phosphorus, iron, with a higher content of calcium (1.23% ± 0.52% - 1.69% ± 0.38%). Statistical analyses showed significant difference (p 0.05) between samples analyzed. Microbiological analyses revealed that the total flora count of samples ranged from 1.4 × 102 to 8.0 × 104 ufc/g. The enumeration of total coliforms and fecal coliforms indicated that samples were contaminated with enteric bacteria. The spores of anaerobic sulfite reducer’s and Staphylococcus aureus counts were less than 10 cfu/g with an absence of Salmonella spp. Based on results from the present study, microbiological standards must be put in place during the process of dried-fermented fishes. Then, it’s advisable that relevant quality control unit should be reactivated to assess the quality of ready-to-eat food, and particularly the fermented fishes in other to protect consumer’s health.

Contribution of Computed Tomography in the Diagnosis of Cranioencephalic Trauma

Background: cranioencephalic trauma is an important cause of mortality and morbidity in young subjects. CT scan is the first-line examination in it exploration. In Central African Republic (CAR), CT scan examination profile is poorly known in current practice, reason why we chose this subject. Objective: list traumatic cranioencephalic lesions and describe their CT appearance. Patients and methods: This was a cross-sectional study carried out in the Bangui radiology and medical imaging department from March 1 to December 31, 2021. We included patients of two sexes regardless of age, having performed brain CT for cranioencephalic trauma during the study period. Data analysis was done using Epi info software. Results: During the study period, 593 cerebral CT scans were performed, of which 84 patients met our inclusion criteria, i.e., a hospital frequency of 14.2%. There was a male predominance with a sex ratio of 2.8. The average age was 26 years with extremes of 1 to 84 years. The most represented age group was that of 20 to 30 years old. The clinical signs were dominated by Master’s group 1 with Glasgow between 15 and 9 (52.4%). Road accidents were the predominant cause with 81% of cases. In 61.9% of cases, the CT examination is performed within more than 24 hours after the trauma. The main CT lesions were dominated by skull vault and facial bone fractures (42.8%). Subdural hematoma, oedemato-hemorrhagic contusions and extra dural hematoma were the main pericerebral and intracerebral lesions with 38.5% respectively;24%;and 9.5%. In 42.8% of cases the CT scan was normal. Conclusion: CT is a first-line examination for lesional assessment in the event of cranioencephalic trauma. These lesions are dominated by fractures of the vault of the skull and the facial bone. Subdural hematoma, oedemato-hemorrhagic contusions and extra dural hematoma are the main pericerebral and intracerebral lesions observed.

Contribution of CT Scanning in the Investigation of Cranioencephalic Lesions in Bangui

Introduction: Cranioencephalic exploration has always played a major role in CT scans. In the Central Africa Republic (CAR), the lack of cross-sectional imaging before the year 2020 meant that no study had focused on cranioencephalic lesions. The aim of this study was to contribute to improving the management of cranioencephalic pathologies in CAR. Patients and Method: The study took place at the Bangui National Medical Imaging Centre (CNIMB). It was a retrospective study over a two-year period (March 1, 2021 to February 30, 2023). All patients referred for cranioencephalic CT scans were included, regardless of age or sex. Results: 1745 CT scans were performed, 575 of which were cranioencephalic CT scans. The majority of patients were male (53%). Most lived in the capital Bangui (90.9%). Patients aged 61 and over were the most representative. The distribution of patients by requesting department showed that the reception and emergency department was one of the least requesting departments. The main abnormalities observed were strokes, 82.1% of which were ischaemic strokes and 17.9% haemorrhagic strokes. Strokes were followed by degenerative lesions. Post-traumatic injuries included haemorrhagic contusions (38.3%), subdural haematomas in 20.5% of cases, and extradural haematomas (9.3%). Craniofacial lesions (fractures) were observed in 45.8% of cases. Conclusion: Cranioencephalic scans accounted for 1/3 of CT examinations performed during the study period. It revealed pathologies that could not be detected by conventional means. All in all, CT scans contributed to the diagnosis of cerebral pathologies.

Contribution of CT in the Exploration of Non-Traumatic Pathologies of the Lumbar Spine in Adults in Bangui

Background: In Central African Republic, a study carried out in 2019 highlighted the limits of conventional radiography in the diagnosis of non-traumatic lower back pain in adults. Objective: The objective of this study is to show the value of CT scanning in the exploration of non-traumatic pathologies of the lumbar spine in adults. Patients and Method: Retrospective and descriptive cross-sectional study covering the files of patients aged at least 18 years old, sent for lumbar scanning from March 1 to December 31, 2021 at the medical imaging center. Results: In total, 593 scan examinations were performed including 159 (26.8%) examinations of the lumbar spine. The average age of the patients was 49.84 years. The majority were male (66%). 127 (79.8%) were referred mainly to the neurology and rheumatology departments. Common low back pain (n = 97, 61%) was the main reason for requesting a CT scan. The lumbar scan was performed without contrast product in 88.7% of cases. In total, 148 (93.1%) examinations were pathological and dominated by overall disc overhang (62.2%) and somatic and interapophyseal osteoarthritis (16.9%). The other lesions were spondylodiscitis (7.4%), tumor-like lesions (3.4%) and narrow lumbar canal (1.4%).
