The role of echocardiography and CT angiography in transcatheter aortic valve implantation patients

The transcatheter aortic valve implantation （TAVI） consist an altemative treatment in patients with severe aortic stenosis. Multimodality imaging using transthoracic echocardiography （TTE） or transesophageal echocardiography （TOE） and multislice CT （MSCT） constitute cornerstone techniques for the pre-operative management, peri-procedural guidance, follow up and recognition of possible transcatheter valve related complications. CT angiography is much more accurate regarding the total definition of aortic annulus diameter and circumferential area. Two-dimensional （2D） echocardiography, underestimates the aortic valve annulus diameter compared to 3D imaging techniques （MSCT, MRI and 3D TOE）. Three-dimensional TOE imaging provides measurements of the aortic valve annulus similar to those delivered by MSCT. The pre-procedural MSCT constitutes the gold standard modality minimizing the presence of paravalvular aortic regurgitation, one of the most frequent complications. TOE/TTE and MSCT performance could predict the possibility of pacemaker implantation post-procedural. The presence of a new transient or persisting MR can be assessed well by TOE. Both TTE and TOE, consist initially the basic examination for post TAVI evaluation. In case of transcatheter heart valve failure, the MSCT could be used as additional imaging technique.

The objective of the present work is to provide an overview of the general circulation features in the Black Sea basin. In order to achieve this, 18 years (1993-2010) of satellite data coming from the Aviso website were analyzed. A description of the general circulation patterns in the Black Sea is first presented. This is followed by statistical analyses of the satellite data in 20 points covering the entire area of the sea. The reference points were chosen as follows: 12 points along the Rim cyclonic current, 3 points inside the Rim cyclonic current, 4 points on the edge of two of the biggest anticyclonic gyres outside the Rim current and one point in the northwestern shelf area of the basin. Rose graphics were drawn for the reference points for winter and summer time. Finally, 9 years of in situ data obtained from the Gloria drilling platform were analyzed and compared with the satellite data. The present study shows that most of the reference points are sensitive to seasonal changes. The current velocities depend mostly on the points location: the points located on the Rim current and on the nearshore anticyclonic eddies present higher values than the ones located in or outside the general circulation features.

As is well known, hand-arm vibration syndrome (HAVS), or vibration-induced white finger (VWF), which is a secondary form of Raynaud’s syndrome, is an industrial injury triggered by regular use of vibrating hand-held tools. According to the related biopsy tests, the main vibration-caused lesion is an increase in the thickness of the artery walls of the small arteries and arterioles resulted from enlarged vascular smooth muscle cells (VSMCs) in the wall layer known as tunica media. The present work develops a mechanobiological picture for the cell enlargement. The work deals with acoustic variables in solid materials, i.e., the non-equilibrium components of mechanical variables in the materials in the case where these components are weakly non-equilibrium. The work derives an explicit expression for the infinite-time cell-volume relative enlargement. This enlargement is directly affected by the acoustic pressure in the soft living tissue (SLT). In order to reduce the enlargement, one can reduce either the ratio of the acoustic pressure in the SLT to the cell bulk modulus or the relaxation time induced by the cell osmosis, or both the characteristics. Also, a mechanoprotective role of the above relaxation time in the cell-volume maintenance is noted. The above mechanobiological picture focuses attention on the pressure in an SLT and, thus, modeling of propagation of acoustic waves caused by the acceleration of a vibrating hand-held tool. The present work analyzes the propagation along the thickness of an infinite planar layer of an SLT. The work considers acoustic modeling. As a general viscoelastic acoustic model, the work suggests linear non-stationary partial integro-differential equation (PIDE) for the weakly non-equilibrium component of the average normal stress (ANS) or, briefly, the acoustic ANS. Cystic fibrosis is a life-threatening, wide spread genetic disease diagnosed in 1 to 3000 livebirths of the Caucasian population. Here a mouse model for this disease is described and optimized using the CFTR-channel selective inhibitor CFTR(inh 172). The target parameter was mucociliary clearance measured using microdialysis of the transported fluorescent dye rhodamine in the mouse trachea in situ. The impact of Ap4A (diadenosine tetraphosphate) as a potential drug was investigated. Its inhalation was effective at low concentrations;established compounds such as Salbutamol and UTP increased mucociliary clearance as well. Our data show a functioning model of cystic fibrosis and the effectiveness of the newly tested Ap4A.

The surface antigen CD14 plays an important role in innate immunity, serving as a pattern recognition receptor for lipopolysaccharides (LPS). The aim of this study was to investigate the proliferation, NFκB activation, and chemokine secretion of BEAS-2B cells, a human bronchial epithelial cell line, after LPS stimulation, and some details of inVolved signaling. The presence of CD14 was investigated by flow cytometry. Cell proliferation was measured with a [3H]-thymidine incorporation assay. sCD14, RANTES, and IL-8 concentrations in cell supernatants were measured by ELISA. BEAS-2B cells express CD14 on their surface and secrete soluble CD14 into the supernatant. Cells react on LPS with increased proliferation, activation of NFκB, and the secretion of the pro-inflammatory chemotactic cytokines IL-8 and RANTES, which proves the functionality of the CD14 receptor. Neither CD14 nor sCD14 are regulated by LPS. Specific inhibitors of various intracellular signaling pathways diminish the LPS-induced proliferation and IL-8 secretion: Thus MAP-Kinases p38 and JNK, tyrosine kinases, and PI3-kinase are involved in the signaling cascade from the LPS-CD14-complex on the cell surface to the increased cell proliferation and expression of IL-8;furthermore, ERK 1/2, IRAK 1/4, and the NFκB pathway are inVolved in the latter. The data show the existence and functionality of CD14 receptors on BEAS-2B cells and elucidate the signaling pathways inVolved. LPS is able to increase cell prolife-ration, various cytokines which are dependent on endogenous CD14. Three MAPK pathways, PI3 kinase and tyrosine kinase may be involved. Also CD14 is present/involved which was controversial.

The role of the ipsilaterally descending motor pathways in the recovery mechanisms after unilateral hemispheric damage is still poorly understood. Motor output reorganization was investigated in a 56-year-old male patient with acquired unilateral hemispheric atrophy due to Rasmussen encephalitis. In particular,the ipsilateral corticospinal pathways were explored using focal transcranial magnetic stimulation. In the first dorsal interosseous and wrist extensors muscles, the median amplitudes of the ipsilateral motor evoked potentials induced by transcranial magnetic stimulation in the patient were higher than those of 10 age-matched healthy control subjects. In the biceps brachii muscle, the median amplitudes of the ipsilateral motor evoked potentials were the second largest in the patient compared to the controls. This study demonstrated a reinforcement of ipsilateral motor projections from the unaffected motor cortex to the hemiparetic hand in a subject with acquired unihemispheric cortical damage.

The precise micromanufacturing of complex dies with small structures for sheet-bulk metal forming is a challenge due to the high hardness of the materials to be machined. Experiments have shown that micromilling of these difficult-to-machine materials is possible despite of their high hardness. Nevertheless, the higher wear of the tools plays a decisive role. When implementing the machining task as five-axis process, it is possible to control the wear distribution by tilting the milling tools. In this paper, a simulation system is presented which determines the loads acting on the cut- ting edge with regard to different criteria, e.g., the machined material or the effective impulse. Based on this knowledge, it is possible to design the milling process to minimize the tool wear and thereby to increase the lifetime of the milling tools. In order to show the applicability of the simulation system, test workpieces were machined and the experimental results are compared to the simulation data.

The main objective of the present work is to evaluate the impact on the nearshore waves and coastal processes of a generic wave farm. The target area considered is in the Romanian nearshore, in the vicinity of the Mamaia sector, coastal environment usually subjected to a strong erosion process. A picture of the wave conditions in this coastal environment is first provided by analyzing some in situ data registered at the Gloria drilling unit, which operates offshore the area of interest at about 50 meters water depth. A high resolution numerical modeling framework was implemented in the target area. This is based on the SWAN spectral model (Simulating Waves Nearshore) for waves and the 1D circulation model SURF (or the Navy Standard Surf Model) to assess the nearshore currents. The presence of the farm in the computational domain was represented by using the command obstacle, which is available in SWAN, and considering various transmission and reflection coefficients. Different wave farm configurations have been considered by adjusting the transmission and the reflection coefficients associated with the wave farm, between a no farm scenario and a fully developed project (corresponding to the case of total absorption). The influence of the farm on the wave field was quantified by performing analyses in the geographical space concerning the variability of the significant wave height. The results look interesting and they indicate that besides the production of the electric power, the presence of the wave farms may have a positive influence on controlling the coastal processes, reducing the erosion and giving in general more stability to the coastal environment, especially during the extreme storm conditions.
